Objective To observe the application effect of the eucalyptol-limonene-pinene enteric soft capsule in chronic sinusitis during perioperative period.Methods Analyzed 576 cases of nasal endoscopic surgery for chronic sinusitis patients.According to diagnostic criteria (Haikou in 1997),patients with type Ⅰ (without nasal polyps) had 318 cases (group A),patients with type Ⅱ and Ⅲ (with nasal polyps) had 258 cases (group B).Then patients in each group were respectively numbered by hospitalized order.According to the single and double number,they were divided into two groups:treatment group A (159cases) and control group A (159 cases),treatment group B (129 cases) and control group B (129 cases).Patients in the treatment group A and B were treated with the eucalyptol-limonene-pinene enteric soft capsule before operation.Patients in the control group A and B were not taking any mucus cilium scavenger before operation.The follow-up was lasted for 1 year for four groups.Subjective symptoms were assessed by visual analog scale (VAS) score.The changes and outcomes of nasal mucosa were evaluated objectively by LundKennedy endoscopic mucosal morphology score.Then the results were analyzed statistically.Results VAS score:compared treatment group A with control group A,score differences were statistically significant in 4,12 and 24 weeks after operation [(9.56 ± 5.23) scores vs.(16.21 ± 5.78) scores,(6.24 ± 4.05) scores vs.(10.36 ±4.77) scores,(5.08 ± 3.35) scores vs.(6.23 ±4.10) scores](P＜ 0.05),but it was no significant difference in 48 weeks after operation (P ＞ 0.05) ; compared treatment group B with control group B,score differences were statistically significant in 4,12 and 24 weeks after operation [(13.34 ± 6.28) scores vs.(17.62 ±5.98) scores,(8.08 ±5.56) scores vs.(12.59 ±5.70) scores,(6.12 ±4.80) scores vs.(8.79 ± 5.04) scores](P＜ 0.05),but it was no significant difference in 48 weeks after operation (P＞ 0.05).LundKennedy endoscopic mucosal morphology score:the differences between treatment group A and control group A in 4 and 12 weeks after operation were statistically significant [(3.44 ± 1.96) scores vs.(5.82 ± 1.74)scores,(3.03 ± 1.87) scores vs.(5.08 ± 1.96) scores] (P ＜ 0.05),but no statistical significance in 24 and 48 weeks after operation (P ＞ 0.05) ; compared treatment group B with control group B,the scores in 4,12 and 24 weeks after operation were significantly different[(6.15 ± 1.99) scores vs.(7.52 ± 2.04) scores,(4.80 ± 2.12) scores vs.(6.92 ± 1.57) scores,(2.53 ± 2.32) scores vs.(4.12 ± 1.72) scores] (P ＜ 0.05),but no obvious difference in 48 weeks after operation (P ＞ 0.05).The cure rate of treatment group A was 98.11％ (156/159),control group A was 87.42％ (139/159),treatment group B was 94.57％ (122/129) and control group B was 79.84％ (103/129),there was significant difference on cure rate between treatment group and control group (P＜ 0.05).The mucosal healing's time in treatment group A and B were significantly shorter than those in control group A and B [(9.62 ± 1.12) weeks vs.(12.35 ± 2.46) weeks,(13.37 ± 1.84) weeks vs.(17.28 ± 3.83) weeks] (P ＜ 0.05).Conclusions These indicated that the dry scab in surgical cavity of patients with mucus cilium scavenger was less and easy to clean,and postoperative recovery time of patients was short.So the mucus cilium scavenger can obviously improve the secretion of mucosa and epithelial recovery,thus accelerate healing of the disease.It can also improve the success rate of functional endoscope sinus surgery,and may play a promising role in clinical application.

OBJECTIVE: We detected expression of MMP9 to discuss its role in the occurrence and development of sinonasal squamous cell carcinoma. METHOD: The immunohistochemical staining, real-time PCR and Western blot were used to measure the expression of MMP9 in sinonasal squamous cell carcinoma tissues (Experimental group) and corresponding normal mucosa tissues (Control group). Relationship between MMP9 and the main clinical features of patients with sinonasal squamous cell carcinoma was analysed. RESULT: Positive expression rates of MMP9 in sinonasal squamous cell carcinoma tissues and corresponding normal mucosa tissues were 81. 25% and 18. 52% respectively. Positive expression rate of MMP9 was not significantly correlated with patient's age and gender (P>0. 05), but correlated with pathological type (P<0. 05). The expression of MMP9 mRNA in sinonasal squamous carcinoma tissues was 30. 66 times of tissues adjacent to carcinoma (P<0. 05). Western blot analysis also showed that the expression of MMP9 protein in squamous carcinoma tissues was significantly higher than tissues adjacent to carcinoma (P<. 05). CONCLUSION The expression of MMP9 was significantly higher in the sinonasal squamous cell carcinoma and correlated with the degree of differentiation. The results suggest that MMP9 may play a role in the occurrence and development of sinonasal squamous cell carcinoma and degree of malignancy from the protein and cellular and molecular level. Objective To investigate gene expression patterns of atherosclerosis (As) induced by homocysteine (Hcy). Methods Total RNAs were isolated from peripheral blood lymphocytes of 3 inpatients who had no traditional risk factors of coronary heart disease (CHD). They were one with normal coronary angiography and plasma Hcy level , one CHD patient with normal plasma Hcy level, and one CHD patient with high plasma homocysteine level . The RNAs were reversely transcribed to the cDNAs to prepare the hybridization probes. The probes were hybridized to the gene chip with 1152 genes to study gene expression patterns. Results There were 177 differentially expressed genes between the CHD patient with high plasma homocysteine level and the patient with normal coronary angiography and plasma Hcy level (group 1) . There were 151 differentially expressed genes between the CHD patient with high plasma homocysteine level and the CHD patient with normal plasma Hcy level (group 3) . A parallel comparison between group 1 and group 3 gene chips showed that there were 48 differentially expressed genes in common , of which there were 25 down-regulated and 23 up-regulated genes.Conclusions Among the differentially expressed genes, some are involved in the functions of apoptosis, signal transduction, immunology, translation of protein and proto-oncogene, etc. They had potential values in genesis of As induced by Hcy. The study based on gene chip in researching the differentially expressed genes of As induced by Hcy will provide new clues for finding out the pathogenesis of As.

Objective To evaluate the value of ultrasound-guided channel establishment in minimally invasive percutaneous nephrolithotomy(MPCNL).MethodsBetween May 2006 and December 2007,85 patients received MPCNL in our hospital.Puncture points were selected and then channels were established under the guidance of ultrasonography according to the distributions of calculi during MPCNL.ResultsUnder the guidance of ultrasonography the puncture was completed at one session in all of our 85 cases.No massive hemorrhage or other severe complications occurred.In the cases,57 cases were cured after the first operation,and 27 cases received a second surgery.One of the cases was diagnosed as having calcification of the renal papillae without kidney calculi during the operation.Residual calculi(

Tumor metastasis, the main characteristic of malignance tumor, is the primary cause of death for most cancer patients.The initiation of tumor metastasis involves complex signaling pathway within tumor cell and microenvironment, mediating primary tumor metastasis, invasion, survival and arrest in the blood circulation, and progressive growth at the distant site.The most research on the mechanism of tumor metastais will help understand the metastasis process, and identify promising molecular targets for cancer clinical diagnosis and treatment.

Objective To evaluate efficiencies of 4 kinds of γ‐interferon release assay(IGRA) detection kits in diagnosis of pul‐monary tuberculosis .Methods 4 kinds of IGRA reagents produced by Oxford Immunotec Ltd (Oxford) in British ,Shanghai Fuxing Changzheng Medical Science Co .,Ltd(Fuxing) ,Cellectis in Australia(Cellectis) and Haikou VTI Biological Institute(VTI) ,respec‐tively ,were used to determine release levels of peripheral bloodγ‐interferon which had antigenicity of Mycobacterium tuberculosis in 86 cases of patients with tuberculosis and 80 cases of healthy individuals ,and diagnostic efficiencies were evaluated .Results Among the 4 kinds of IGRA reagents ,including Oxford ,Fuxing ,Cellectis and VTI ,the sensitivity was 93 .02% ,88 .37% ,90 .70% and 91 .86% ,respectively ;the specificity was 92 .50% ,75 .00% ,82 .50% and 87 .50% ,respectively ;the positive predictive value was 93 .02% ,79 .17% ,84 .78% and 88 .76% ,respectively ;the negative predictive value was 92 .50% ,85 .71% ,89 .19% and 90 .91% ;the diagnostic accordance rate was 92 .77% ,81 .93% ,86 .75% and 89 .76% ,respectively ;the area under receiver operating charac‐teristic(ROC) curve was 0 .975 ,0 .892 ,0 .958 and 0 .963 .Conclusion There are no significant differences among Oxford ,Fuxing , Cellectis and VTI reagents ,and reagents could be selected according to clinical requirements .

OBJECTIVE To investigate the reasons and preventive measures of recurrent laryngeal nerve injury during operation in patients with benign thyroid neoplasm. METHODS The clinical data of 2243 cases with benign thyroid neoplasm were reviewed. The relations among different neoplasm regions, different procedures and injury of recurrent laryngeal nerve were analyzed. RESULTS Injury incidence of recurrent laryngeal nerve in this study was 3.0 %. In cases with nodular goiter on the dorsum of thyroid and recurrent laryngeal nerve exposed during operation, injury incidence of recurrent laryngeal nerve was 1.0 %. And there was no recurrent laryngeal nerve injury in cases with nodular goiter on other region of the thyroid. In cases used locality protection during operation, injury incidence of recurrent laryngeal nerve was 7.6 % when benign lesions on the dorsum of thyroid and was 0.17 % when benign lesions on other region of the thyroid. CONCLUSION In cases with nodular goiter or adenoma on the dorsum of thyroid, exposing recurrent laryngeal nerve is necessary during operation and in cases with benign lesions on other region of thyroid, locality protection is useful. There are three major routes for recurrent laryngeal nerve exposure.

Objective To investigate the infection rates of mycoplasma penetrans (Mpe),mycoplasma pneumoniae (Mp) and mycoplasma ferments (Mf) in patients with IgA nephropathy (IgAN),chronic kideny disease (CKD),and heathy people,to compare the difference of infection rate,and to analyze the association of mycoplasma infection and clinicopathological features in IgAN.Methods Blood samples were collected from 118 patients in IgAN group,90 patients in CKD group,and 89 cases in health control group.DNA of Mpe,Mp and Mf was detected in plasma by PCR.Positive cases were confirmed by Southern blot.According to mycoplasma infection,IgAN patients were divided into two groups,then analyzed the clinicopatholgical features.Results (1)Genus,Mpe,Mp,and Mf positive rates were 33.05％,16.1％,25.45 ％,and 8.47％ in IgAN group,respectively; 5.56％,2.22％,5.56％,and 2.22％ in CKD group,respectively; and 3.33 ％,1.11％,2.22％,and 0 in health group,respectively.Compared with CKD and health group,patients in IgAN group had a higher infection rate in Genus,Mpe,and Mp (P ＜ 0.05).In IgAN group,10 patients had three kinds of mycoplasmas infection at the same time,and positive rate was 8.47％ much higher than CKD group (positive rate was 2.22％) (P ＜ 0.05).(2) Based on mycoplasm detection results,IgAN patients were divided into two groups,overlapping infection group and mycoplasma negative group.In overlapping infection group,the mean age of onset was much younger than negative group.Compared with negative group,overlapping infection group had higher tonsillitis and urinary tract infection rate,more severe microscopic hematuria and tubulointerstitium lesion (P ＜ 0.05).Conclusions Patients with IgAN had higher infection rate of Genus,Mpe and Mp,compared with CKD patients and health people.Compared with mycoplasma negative group in IgAN patients,more severe microscopic hematuria and tubulointerstitium lesion in overlapping infection group,which suggested that infection of Mpe might have some possible connection with IgAN.

Objective To investigate surgical strategy and outcome for cervical metastasis in different anatomical segments.Methods Data of 31 patients,who had undergone surgical treatment for cervical metastasis from 2001 to 2009,were retrospectively analyzed.Two anatomical regions were defined:upper cervical spine (C1 and C2)and subaxial cervical spine plus cervicothoracic junction (C3-T1).VAS score,Frankel grade,Tomita score and Karnofsky performance scale were used to evaluate pain,neurological status,expected survival time and general health.The improvement of symptoms,survival time and characteristics of surgical strategies for different anatomical regions were analyzed.Results Twenty-four patients were followed up successfully.An effective recovery in neck pain and quality of life was found after operation.The median survival time was 45.0 months.In upper cervical spine group,the surgical strategies included posterior occipitocervical fixation combined with 125I seed brachytherapy and other surgical methods,and the median survival time 48.0 months) of patients who underwent 125I seed brachytherapy was longer than that 22.0 of patients treated with other surgical methods.In C3-T1 metastasis group,patients who underwent combined anterior-posterior approach total spondylectomy had a longer survival time than those treated with anterior corpectomy.Conclusion Surgical treatment can effectively relief neck pain,maintain or improve neurological function and improve quality of life.Doctors should choose different surgical strategies according to anatomical region of cervical metastasis.For upper cervical spine,the main purpose of surgery is to stabilize the cervical spine; the posterior occipitocervical fixation is usually chosen,and the 125I seed brachytherapy is useful in local control of metastasis.For subaxial cervical spine and cervicothoracic junction,the anterior corpectomy is usually chosen,and a combined anterior-posterior approach total spondylectomy can be used for patients with corresponding conditions.

Objective To explore the curative effect of acyclovir combined with pidotimod in treatment of adult varicella.Methods 80 adult patients with varicella in our hospital were randomly divided into two groups.40 cases in group A were intravenously injected acyclovir.40 cases in group B were intravenously injected acyclovir combined with oral pidotimod.The time of herpes blisters drying and completely crusted were observed in two groups.Results The time of herpes blisters drying and completely crusted of B group was shorter than A group.And the differences were statistically significant(P ＜0.01).Conclusion Acyclovir combined with pidotimod in treatment of adult varicella can significantly shorten the healing time,and show better curative effect than single use of acyclovir.
