1.The effect and their mechanism of acid pocket in reflux esophagitis

Objective To investigate the function of acid pocket in reflux esophagitis. Methods The 15 healthy controls and 24 reflux esophagitis patients were identified by reflux disease questionnaire (RDQ) and gastric endoscopy. The location of subjects' lower esophageal sphincter (LES) was determined by 4 channel esophageal manometry system. Then a single-channel pH electrode was positioned 1 cm below the distal border of the LES to monitor fasting pH for half an hour. After a standard meal, the pH was continuously measured for two hours. Then the electrode was moved to 5 cm above the proximal border of the LES to monitor the dynamic pH for 24 h.Results Acid pocket was found in 16 cases of reflux esophagitis patients(66.67%) and 10 cases of healthy individuals (10/15). Acid pocket occurred earlier in reflux esophagitis group than healthy controls [11.00(4.25-17.00) min vs 30.00(15.50-54.25) min, P<0.05], and the average pH value was lower [1.84(1.59-2. 19) vs 2.32 (1.96-2.71), P<0.05]. There was no statistic difference in mean pH value of gastroesophageal junction and the duration of acid pocket before the meal.Conclusion There is abnormal acid reflux in reflux esophagitis patients, and acid pocket with earlier occurrence and lower pH value may relevant to esophageal mucosal impairment.
2.Expression of heme oxygenase-1 in peritoneal metastasis of gastric cancer and its significance HUI

Objective To investigate the expression and significance of heme oxygenase-1(HO-1) in gastric adenocarcinoma and its peritoneal metastatic tissues, as well as drug-resistant cell strains. Methods The expression of HO-1 in patients with (n=68) or without (n=46) peritoneal metastasis of gastric adenocarinoma was examined. The expression of HO-1 in cancerous tissue, peritoneal metastatic foci, and normal peritoneum was detected by immunohistochemistry. The expression of HO-1 protein in metastatic foci and drug-resistant cell strains was measured by Western blotting. Results The positive expression of HO-1 was 39.7% (27/68) in gastric adenocarcinoma tissues with metastasis and 41.2% (28/68) in peritoneal metastatic tissues, which was significantly higher than that in normal peritoneum (0%,0/68,P<0.01) and gastric adenocarcinoma tissues without metastasis (21.7%, 10/46, P<0.05). The Western blot study showed that the HO-1 expression in metastatic tissues was higher than that in normal peritoneum (P<0.05). The expression of HO-1 protein was markedly increased in GC9811-P drug-resistant cell strains compared with its parental cell strains (P<0.05). Conclusions The increased expression of HO-1 may be involved in the peritoneal metastasis of gastric adenocarcinoma, and related to the malignant potential. The underlying signal pathways in neopastic epithelium may also be related to the multi-drug resistance.
3.Survival analysis of HIV/AIDS patients with antiretroviral therapy among drug users in Yili Prefecture from 2005 to 2019

Objective:
To understand the survival status and influencing factors of HIV/AIDS patients with highly active antiretroviral therapy ( HAART ) among drug users in Yili Prefecture, Xinjiang from 2005 to 2019, so as to provide references for reducing AIDS mortality.
Methods :
The demographic information, clinical stage, baseline CD4+T lymphocyte ( CD4 ) level and treatment status of HIV/AIDS patients with HAART in Yili Prefecture from 2005 to 2019 were collected through AIDS Antiretroviral Therapy Information System. The survival rate was calculated by the life table method. The influencing factors for survival time were analyzed by Cox proportional hazard regression model.
Results:
Totally 1 935 patients were recruited, the median age receiving HAART was 37 years old and the median CD4 counts was 293/μL. The cumulative survival rates at 1, 5, 7 and 10 years were 97%, 78%, 73%, and 66%, respectively. The multivariate Cox proportional hazards regression analysis showed that the patients with body mass index of 18.5-<28.0 kg/m2 ( HR: 0.391-0.656, 95%CI: 0.234-0.958 ), baseline CD4>200/μL ( HR: 0.354-0.667, 95%CI: 0.232-0.841 ) , or missed medication in the last 7 days ( HR=0.009, 95%CI: 0.001-0.061 ) had lower risk of death; the patients with WHO clinical stage of Ⅱ-Ⅳ ( HR: 1.479-2.311, 95%CI: 1.004-3.288 ) or treatment delay ≥1 years ( HR: 1.287-1.388, 95%CI: 1.029-1.826 ) had higher risk of death.
Conclusions
The 5-year cumulative survival rate of HIV/AIDS patients with HAART in Yili Prefecture is 78%. Body mass index, baseline CD4 level, WHO clinical stage, treatment delay and missed medication in last 7 days were the influencing factors for survival time.
4.Advanced NSCLC first pass perfusion at 64-slice CT: reproducibility of volume-based quantitative measurement.

BACKGROUND AND OBJECTIVEThe aim of this study is to explore the reproducibility of volume-based quantitative measurement of non-small cell lung cancer (NSCLC) perfusion at 64-slice CT.
METHODSFourteen patients with proved advanced NSCLC were enrolled in this dynamic first pass volume-based CT perfusion (CTP) study (8x5 mm collimation), and they underwent the second scan within 24 h. According to the longest diameters, those patients were classified to <3 cm and >3 cm groups, and each group had 7 patients. Intradclass correlation coefficient (ICC) and Bland-Altman statistics were used to evaluate the reproducibility of CTP imaging.
RESULTSIn both groups of advanced NSCLC, the reproducibility with BF, BV, and PS values were good (ICC > 0.75 for all), but mean transit time (MTT) values. For advanced NSCLC (<3 cm), repeatability coefficient (RC) values with blood flow (BF), blood volume (BV), MT and permeability surface area product (PS) values were 56%, 45%, 114%, and 78%, respectively, and the 95% change intervals of RC were -39%-53%, -29%-62%, -83%-145%, and -57%-98%, respectively. For advanced NSCLC (>3 cm), those values were 46%, 30%, 59%, and 33%, respectively, and the 95% change intervals of RC were -48%-45%, -33%-26%, -54%-64%, and -18%-48%.
CONCLUSIONThere is greater reproducibility of tumor size >3 cm than that of < or =3 cm. BF and BV could be addressed for reliable clinical application in antiangiogenesis therapeutic monitoring with advanced NSCLC patients.

5.Inhibitory effects of synthetic PⅢpeptide on invasion and peritoneal metastasis of gastric carcinoma with high peritoneal metastasis potential cell line

Objective To evaluate the potential utility of PⅢpeptide in anti peritoneum metastasis in gastric carcinoma cells with high peritoneal metastasis potential(GC9811 P).Methods The adhesion and invasion inhibitory effects of PⅢpeptide on GC9811-P cells were detected by in vitro matrix adhe- sion and cell invasion experiments.By using nude mice metastatic model of human gastric cancer,the effects of PⅢpeptide on peritoneal metastasis of gastric cancer GC9811 P were evaluated.Mice were randomly divided into the experimental(GC9811-P+peptide PⅢgroup)and control groups(GC9811-P +0.9% NaCl solution group),12 mice in each group.At the exhaustion time after inoculation,mice were sacrificed to observe the incidence of peritoneal metastasis,the number of the metastasis foci and the volume of primary tumor.Results Two hours after 40?g PⅢpeptide incubation,the adhesion in hibitory rate reached 86.30%.The adhesion inhibitory effects were in a time dependent manner.The in- vasion inhibitory effects became apparent(81.4%)48 hours after PⅢpeptide insult.After the GC9811- P cells were orthotopic implanted in nude mice and treated with PⅢpeptide,the number of peritoneal metastatic nodes were significantly reduced as compared with control group(3.2?6.5 vs.26.3?5.2) ( P<0.01).But the mass of primary tumor were (1.9?1.2) g in PⅢpeptide treated group and (2.1?1.0) g in the control group,no difference was noted between two groups(P>0 05 ).Conclusion PⅢpeptide can markedly inhibit the adhesion,invasion and peritoneal metastasis of gastric carcinoma cell line GC29811-P with high peritoneal metastasis potential
6.Generation of streptavidin-tagged human-granulocyte macrophage colony-stimulating factor fusion proteins.

OBJECTIVETo obtain streptavidin-tagged human granulocyte-macrophage colony-stimulating factor (SA/hGM-CSF) fusion protein and evaluate its bioactivity .
METHODSPET24a-6His-SA-L-hGM-CSF and PET24a-hGM-CSF-L-SA-6His plasmids were constructed and expressed in Rosetta (DE3) host bacteria to generate the fusion proteins. The two fusion proteins were refolded by gradient dialysis after Ni-NTA affinity chromatography and finally purified using DEAE-sepharose FF anion exchange chromatography. MTT method was used to evaluate the effect of SA/hGM-CSF fusion proteins in inducing the proliferation of human erythroleukemia cells (TF-1). The efficiency of the fusion proteins for surface modification of biotinylated MB49 tumor cells was evaluated by flow cytometry.
RESULTSThe recombinant fusion proteins SA-hGM-CSF and hGM-CSF-SA were highly expressed in Rosetta (DE3) at about 20% of the total bacterial proteins, with a purity of about 96% after purification. The two fusion proteins exhibited bifunctional activities, namely the pro-proliferation effect on human erythroleukemia cells (TF-1) and SA-mediated high-affinity binding to biotinylated cell surfaces (with an anchoring modified rate of about 99%).
CONCLUSIONSA/hGM-CSF bi-fusion proteins obtained in this study lays the groundwork for the development of cancer cell vaccines with surface modification by hGM-CSF.

7.Comparison of clinical results between an isometric and an anatomic technique for medial patellofemoral ligament reconstruction.

OBJECTIVETo compare the clinical results of single-stranded isometric with double-stranded anatomic reconstruction of medial patellofemoral ligament (MPFL) for recurrent patellar dislocation.
METHODSRetrospective analysis of 60 consecutive patients (72 knees) with recurrent patellar dislocation underwent medial patellofemoral ligament reconstruction between August 2004 and October 2008, 22 cases (27 knees) with single-stranded isometric reconstruction, 38 cases (45 knees) with double-stranded anatomic reconstruction. Postoperatively patellar stability was examined, and recurrent rate was recorded. Patellar lateral shift ratio and tilt were measured on CT scans. Knee function was evaluated with subjective questionnaire and the Kujala score.
RESULTSAll patients were followed up for more than 12 months without recurrent redislocation. (1) Recurrent instability rate was 18.5% in single-stranded group and 2.2% in double-stranded group, with statistical difference (P < 0.05). (2) The patellar tilt and lateral shift ratio returned to normal, without statistical difference between groups (P > 0.05). (3) The Kujala score were 59 ± 9 and 62 ± 9 preoperatively and 87 ± 4 and 94 ± 6 postoperatively in single- and double-stranded group, with statistical difference (P < 0.05). (4) The excellect rate was 85.2% in single-stranded group, 97.8% in double-stranded group, with statistical difference(P < 0.05).
CONCLUSIONBoth reconstruction can improve patellar stability and knee function significantly, with double-stranded anatomic reconstruction better in clinic evaluation.

8.Characteristics of foreign bodies in upper gastrointestinal tract and analysis of its risk factors for complications

Objective To summarize the clinical characteristics of foreign bodies in the upper gastrointestinal tract and analyze its risk factors for complications. Methods We retrospectively analyzed the medical data of the patients who were diagnosed of foreign body impaction in the upper gastrointestinal tract, then analyzed its risk factors of complications. Results The date pits were the most common type of foreign bodies and upper esophagus were the most common locations in upper gastrointestinal tract; In Logistic regression analysis, risk factors for complications were the duration of impaction that longer than 12 hours (OR^ = 9.04, 95%CI: 2.91 ~ 28.04; P = 0.000) and the sharpness of foreign bodies' edge (OR^ = 7.95, 95% CI: 2.09 ~ 30.21; P = 0.002). Conclusion The duration of impaction that longer than 12h and sharpness of foreign bodies' edge are the risk factors for complications.

BACKGROUNDNa + /Ca 2+ exchanger (NCX) plays a crucial role in pentylenetetrazol-induced convulsion. However, it is unclear whether NCX is critically involved in hyperthermia-induced convulsion. In this study, we examined the potential changes in NCX3 in the hippocampus and cerebrocortex of rats with hyperthermia-induced convulsion.
METHODSTwenty-one Sprague Dawley rats were randomly assigned to control group, convulsion-prone group and convulsion-resistant group (n = 7 in each group). Whole-cell patch-clamp method was used to record NCX currents. Both the Western blotting analysis and immunofluorescence labeling techniques were used to examine the expression of NCX3.
RESULTSNCX currents were decreased in rats after febrile convulsion. Compared to the control group, NCX3 expression was decreased by about 40% and 50% in the hippocampus and cerebrocortex of convulsion-prone rats, respectively. Furthermore, the extent of reduction in NCX3 expression seemed to correlate with the number of seizures.
CONCLUSIONSThere is a significant reduction in NCX3 expression in rats with febrile convulsions. Our findings also indicate a potential link between NCX3 expression, febrile convulsion in early childhood, and adult onset of epilepsy.

10.Modified lumbopelvic fixation for sacral and L5 fractures associated with spinopelvic instability: a case report and introduction of the surgical technique.

Posterior lumbopelvic fixation with iliac screws is the most commonly used method for unstable spinopelvic injuries. It has certain limitations including inability to use distraction along the spinopelvic rod as an indirect reduction maneuver, need for complex 3-dimensional rod contouring and complications such as hardware prominence and soft tissue coverage. In the present case report, we described a surgical technique of lumbopelvic fixation with sacral alar screws for traumatic spinopelvic instability resulted from a unilateral Denis-III comminuted sacral fracture and the L5 burst fracture. On the opposite side of the sacral fracture, caudal screws were implanted into the pedicle of the S1, whereas on the side of sacral fracture, two sacral alar screws were placed parallel to the superior sacral endplate as well as the plane of sacroiliac joint. In addition, horizontal stabilization was conducted with cross-link connections to maintain the longitudinal traction. For sacral fracture associated with traumatic spinopelvic instability, this modified lumbopelvic fixation technique using sacral alar screws makes longitudinal reduction easier, requires less rod contouring, and reduces hardware prominence without compromising the stability.
