2.A comparison of two microinvasive biopsy procedures in sonographically highly suspicious breast malignancies

Objective To evaluate the value of fine needle aspiration (FNA) and core needle biopsy (CNB) in highly suspicious breast malignant lesions in terms of diagnostic accuracy,complication rate and cost-effectiveness.Methods We retrospectively reviewed records of patients with imaging diagnosis of BI-RADS 4c or 5 categories who have undergone either FNA or CNB under ultrasound guidance in Peking University People's Hospital from 2012 to 2014.Sensitivity,specificity,diagnostic accuracy,positive predictive,negative predictive,false positive rate,false negative rate,accuracy rate and unsatisfactory rate (non-diagnostic rate) were calculated and compared between FNAC and CNB.The complication of the procedures,operation time and cost were assessed.Results Among 638 consecutive cases,273 of them underwent FNA,and 365 underwent CNB.The accuracy rate of FNA and CNB were 99% and 97.2%.The sensitivity,specificity,diagnostic accuracy,positive predictive,false positive rate and false negative rate were similar between the two groups.The unsatisfactory rate of FNA was significantly higher than that of CNB (25.3% vs.10.9%,x2 =22.59,P =0.000).There was not severe complication in either groups,while subcutaneous ecchymosis and hematoma were more common in FNA group than in CNB group.The operation time of FNA was shorter than that of CNB (4.8 ± 1.3 min vs.15.5-± 1.7 min,P =0.000).The waiting time for final pathological report was shorter in patients undergoing FNA (1 vs.3 days).An estimated cost for FNA was RMB 447.5 Yuan/case,whereas that was 995 Yuan/case for CNB.Conclusions FNA and CNB are both accurate and safe preoperative diagnostic procedures.FNA is a simple and cost-effective method.
3.Value of different biological markers in prediction of response to neoadjuvant chemotherapy in breast cancer

Background and purpose:Neoadjuvant chemotherapy is an excellent model for evaluation of predictive parameters.The goal of predictive parameters is to select patients who would most likely benefit from neoadjuvant chemotherapy.The objective of this study was to investigate the predictive value of biological markers for the patients' response to neoadjuvant anthracycline combined with taxanes chemotherapy.Methods:We investigated 160 patients with breast cancer who underwent 4 cycles of neoadjuvant anthracycline combined with taxanes chemotherapy,retrospectively.The expression of estrogen receptors(ER),progesterone receptors(PgR),Her2,Topo-Ⅱ and Ki-67 proteins were detected by immunohistochemical assay in core-needle biopsy specimens.The associations between biological markers as well as clinical and pathologic responses were analyzed.Results:The overall clinical response was 85%,including 28.8%(46/160) with clinical complete response(cCR) and 56.3%(90/160) with clinical partial response.The pathological complete response(pCR) was 14.4%.In the univariate analysis,absence of ER,PgR expression and over-expression of Her-2 were predictive of the cCR and pCR(P
4.Response and prognosis of taxanes and anthracyclines neoadjuvant chemotherapy in patients with triple negative breast cancer

Background and purpose:Triple-negative breast cancer(TNBC) is a high risk breast cancer that lacks the benefit from molecular targeted therapy.The purpose of this study was to compare the response to neoadjuvant taxanes and anthracyclines chemotherapy and survival in patients with TNBC and non-TNBC.Methods:One hundred and thirty-eight patients treated with 4 cycles of neoadjuvant taxanes,and anthracyclines chemotherapy were included in this retrospective study.TNBC is defined as the lack of estrogen receptor(ER),progesterone receptor(PR),and human epidermal growth factor receptor 2(Her-2) expression by immunohistochemistry method.The clinical and pathologic response and neoadjuvant taxane and anthracycline chemotherapy,and the relationships of response and survival between TNBC and non-TNBC were analyzed.Results:Of the 138 patients,37(26.8%) were TNBC and 101(73.2%) were non-TNBC.The overall response rate(OR)was 85.5%(118/138),including 35.5% clinical complete response(cCR) and 50.0% clinical partial response(cPR).The pathologic complete response(pCR) was 21.7%.The cCR and pCR were 51.4% and 45.9% in patients with TNBC,significantly higher than patients with non-TNBC respectively(29.7% and 12.9%)(P0.05).In contrast,TNBC patients with residual disease after neoadjuvant chemotherapy had worse 5-year DFS and OS compared with non-TNBC(P

ObjectiveTo investigate the effects of ligustrazine on renal tubulointerstitial injury in adriamycin nephrosis rats and its mechanism.MethodsForty male Sprague-Dawley rats were randomly divided into sham-operation group,model group,ligustrazine group and Benazepril group.The rat nephropathy model was established by adriamycin injection and unilateral nephrectomy.The 24-hour urinary protein excretion at the start,2nd,4th,6th weekends was analyzed.All rats were sacrificed at the 6th weekend,and then the renal function and the tubulointerstitial pathological injury were examined.Immunohistochemistry was used to measure the expression of ET-1.ResultsThe 24-hour urinary protein excretion [ (30.07 ±2.12) mg/24 h,(201.83 ± 8.63 ) mg/24 h,( 470.70 ± 58.79 ) mg/24 h ] ( at the 2th,4th,6th weekend),blood urea nitrogen[ BUN( 20.20 ± 2.65 ) mmol/L],serum creatinine[ Scr ( 86.79 ± 2.20 ) μmol/L),tubulointerstitial pathological injury (4.38 ± 0.26) and the expression of ET-1 ( 126.92 ± 3.63 )in model group were significantly higher than those in sham-operation group [ ( 6.75 ± 2.07 ) mg/24 h,( 8.28 ± 0.71 ) mg/24h,( 25.37 ± 4.30) mg/24 h,( 8.93 ± 1.05 ) mmol/L,(49.00 ± 5.34 ) μmol/L,1.06 ± 0.19,32.09 ± 3.71,P < 0.01 ].Compared with model group,the 24-hour urinary protein excretion [ ( 176.93 ± 9.20)mg/24 h,( 270.45 ± 60.21 ) mg/24 h) ( at the 4th,6th weekend),BUN [ ( 13.75 ± 2.60 ) mmol/L ],Scr [ ( 62.49 ±3.29)μmol/ L ],Renal tubulointerstitial pathological injury (2.78 ± 0.10) and the expression of ET-1(57.44 ± 4.98 ) were significantly decreased in ligustrazine group( P < 0.01 ).ConclusionsLigustrazine can downregulate the expression of ET-1 and decreased urinary protein excretion,leading to reduce tubulointerstitial inflammation and fibrosis.
7.Ultrasound-guided fine-needle aspiration for the evaluation of axillary lymph node metastasis in breast cancer

Objective To study the utilization of preoperative ultrasound-guided fine-needle aspiration(USFNA) for axillary lymphnode(ALN) metastasis in breast cancer and to evaluate the feasibility and accuracy of the technique. Methods Forty ALNs in 40 armpit negative palpation highly suspective breast cancer patients underwent USFNA. The cytopathological results were compared with the histopathological results of sentinel lymphnode(SLN) biopsy or axillary lymphnode dissection(ALND). The sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV) and diagnostic accuracy of USFNA for the ALN metastasis were analysed. Results All the 40 patients were confirmed with breast cancer by pathology. Sensitivity of USFNA was 52. 2%, specificity was 100%, PPV was 100%,NPV was 60. 7% and diagnostic accuracy was 72. 5%. 12(30% ) patients with positive cytology proceeded to ALND, avoiding SLN biopsy. Conclusions USFNA is a minimally invasive, highly specified technique for prediction of breast cancer ALN metastasis before operation.
8.Expression of microRNA-21 in myocardial tissue of rats in the early stage of severe scald injury and its mechanism.

OBJECTIVETo explore the molecular mechanism of microRNA-21 in myocardial damage of rats in the early stage of severe scald injury by observing the expression of microRNA-21 and programmed cell death 4 (PDCD4) in myocardial tissue of rat and to validate the relationship between them in cell model.
METHODS(1) Forty SD rats were divided into sham injury group (n =8, sham injured) and scald injury group (n =32, inflicted with 30% TBSA full-thickness scald on the back) according to the random number table. The left ventricular tissue was collected from rats in sham injury group at post injury hour 1 without any fluid infusion. Rats in scald injury group were given an intraperitoneal injection of lactic acid Ringer's solution and 8 rats were respectively sacrificed at post injury hour 3, 6, 12, 24 to harvest left ventricular tissue. The expression of microRNA-21 in myocardial tissue was assessed by real-time fluorescent quantitative RT-PCR. The protein expression of PDCD4 in myocardial tissue was assessed by Western blotting. (2) Rat myocardial cell line H9C2 was divided into microRNA-21 inhibitor group (cells were transfected with microRNA-21 inhibitor) and negative transfection control group (cells were transfected with negative control of microRNA inhibitor) according to the random number table. At post transfection hour 48, real-time fluorescent quantitative RT-PCR and Western blotting were performed respectively to determine the mRNA and protein expression levels of PDCD4 in cells. Data were processed with one-way analysis of variance, LSD-t and two independent samples t test. The relationship between microRNA-21 expression and PDCD4 protein level in myocardial tissue of rats was assessed by linear correlation analysis.
RESULTS(1) The expression levels of microRNA-21 in myocardial tissue of rats in sham injury group at post injury hour 1 and in scald injury group at post injury hour 3, 6, 12, 24 were respectively 0. 96 ± 0. 13, 0. 44 ± 0. 08, 0. 42 ± 0. 10, 0.33 +0.07, and 0.61 0.10 (F = 27.331, P <0.001). Compared with that in myocardial tissue of rats in sham injury group at post injury hour 1, expression level of microRNA-21 was significantly decreased in scald injury group at post injury hour 3, 6, 12, 24 (with t values from 4. 558 to 9.410, P values below 0.01). The protein expression levels of PDCD4 in myocardial tissue of rats in sham injury group at post injury hour 1 and in scald injury group at post injury hour 3, 6, 12, 24 were respectively 0.44 ± 0.05, 0.60 ± 0.09, 0.92 ± 0. 15, 0. 86 ± 0.11, and 0.57 ± 0. 10 (F =8.622, P =0.003). Compared with that in sham injury group at post injury hour 1, protein expression level of PDCD4 was significantly increased in scald injury group at post injury hour 6 and 12 (with t values respectively 4. 968 and 4. 122, P values below 0.01). A significant negative correlation between the expression of microRNA-21 and PDCD4 protein in myocardial tissue of rats of scald injury group was observed at each time point (r = -0. 572, P = 0. 026). (2) The mRNA and protein expression levels of PDCD4 of myocardial cells in microRNA-21 inhibitor group were respectively 1.73 ± 0. 29 and 0. 38 ± 0. 08, which were significantly higher than those in negative transfection control group (0.95 ± 0.14 and 0.23 ± 0.03, with t values respectively 4. 857 and 3.356, P <0.05 or P <0.01).
CONCLUSIONSExpression of microRNA-21 was decreased, while expression of PDCD4 was increased, in myocardial tissue of rats in the early stage of severe scald injury. MicroRNA-21 might participate in myocardial damage in the early stage of scald injury by negatively regulating expression of PDCD4.

9.Preparation of 68Ga-PSMA-617 and its microPET imaging in BGC-823 cell bearing mice

Objective To prepare 68Ga-PSMA-617 and perform its microPET imaging on both normal BALB/c mice and BGC-823 (PSMA expression) tumor bearing mice.Methods 68GaCl3 was eluted from 68Ge-68Ga generator by 0.05 mol/L HCl,then added to the DKFZ-PSMA-617 and heated at 85 ℃ for 5 min.The labeling efficiency and in vitro stability of 68Ga-PSMA-617 in sodium chloride solution and HAS were analyzed by radio-HPLC.Water partition coefficient and plasma protein binding rate were also evaluated.MicroPET imaging was performed in normal female BALB/c mice and human gastric tumor (BGC-823) bearing mice at 60 min post-injection of 68Ga-PSMA-617.18F-FDG was also injected to BGC-823 tumor bearing mice to acquire microPET imaging for contrast.Results The labeling yield of 68Ga-PSMA-617 was 97.9%,and it could be used directly without purification.68Ga-PSMA-617 showed good in vitro stability in sodium chloride solution and 5% HAS,the radiochemical purities were 94.9% and 81.0% respectively at 80 min post-incubation.68Ga-PSMA-617 was water-solubility substance,and it cleared mainly through the kidneys.MicroPET imaging showed that 68Ga-PSMA-617 could be accumulated in tumor (T/NT=2.28),which was better than 18F-FDG.Conclusions Preparation of 68Ga-PSMA-617 is convenient and has a high labeling yield.It can specifically target to PSMA expression tumors and has a promising prospect in clinical application.
10.Clinical analysis on combined medication in patients of acute pancreatitis in real world.

OBJECTIVETo study the characteristics and regular pattern of the medicine in common use and combined medication in patients of acute pancreatitis in real world.
METHODCollect the information of 5 433 acute pancreatitis patients in 19 grade IIIA general hospitals in China, analysis by descriptive statistics and association rule.
RESULTIn the 5 433 patients of acute pancreatitis, the glycyrrhizic acid injection and somatostatin are the frequency top used Chinese traditional and western medication. Glycyrrhizic acid injection, somatostatin and insulin are the frequency top used drug combination pattern.
CONCLUSIONThe Chinese and western integrative medicine drug use pattern are accord with the clinical guideline of acute panceatitis. The hepatic and renal function, blood routine and coagulation function should be monitored when the medicines are used.
