1.Comparison of the clinical effect of laparoscopic and laparotomy D2 gastric cancer radical on advanced gastric cancer

Objective To evaluate the clinical value of laparoscopic and open radical gastrectomy for gastric cancer in advanced stage(D2).Methods A retrospective analysis was conducted in 100 patients with advanced gastric cancer who underwent surgical treatment.The patients were randomly divided into laparotomy group (n=44) and laparoscopic group (n=56).The laparotomy group was treated with D2 radical operation for advanced gastric cancer by the conventional laparotomy, while the laparoscopic group underwent laparoscopic radical resection of advanced gastric cancer with D2.The surgery related indicators, as well as intraoperative and postoperative complications etc.were compared between the two groups.All patients were followed up for 24 months.The recurrence, metastasis and mortality of the patients in the two groups were compared.Results The operation time and the amount of bleeding of the laparoscopic group were less than those of the laparotomy group (P=0.020, 0.001).The postoperative intestinal function recovery and hospitalization time of the laparoscopic group were shorter than those of the laparotomy group (P=0.020,0.001).There was no significant difference between the two groups in the number of lymph node dissection (P=0.650).Various complications occurred in the two groups, such as bleeding, spleen injury, vascular injury of colon injury and pancreatic injury, but the difference between the two groups was not statistically significant (P=1.000,0.570,1.000,1.000).There was no significant differences between the two groups in pancreatic fistula, anastomotic leakage, remnant stomach weakness and anastomotic bleeding (P=0.020,0.005,1.000,1.000,0.570).After 24 months of follow-up, there was no significant difference in recurrence, metastasis and mortality between the two groups (P=0.450,0.290,0.530).Conclusion Endoscopic surgery for D2 resection in the treatment of advanced gastric cancer has advantages, such as less trauma, rapid recovery and so on, can reduce the occurrence of postoperative complications.
2.Tentative Exploration of the Training Plan on Professional Abilities of Young Administrators in a certain Affiliated Hospital of University in Guangzhou

Objective To understand the status qua of professional abilities and training needs of young administrators in a certain Affiliated Hospital of the University in Guangzhou, and to formulate a tentative exploration of the training plan for their specialized abilities on hospital management.Methods A questionnaire was prepared on the basis of literature review, and 58 young administrators for a specified affiliated hospital of a university in Guangzhou were surveyed.Results The respondents consider that the professional abilities were rather important for them, but the scores of the status qua were generally lower than that of importance, especially in terms of the knowledge of medicine, law and psychology, and the skills of scientific research, innovation and Time management.They are also of the view that the training frequency was not enough and the forms were not as ideal as expected.Conclusion A three-stages training plan has been laid forward include Rotation Training, the Youth Management Salon and further education.
3.Effect of adipose stem cells derived conditioned medium on fibrogenesis of dermal fibroblasts co-stimulated by transforming growth factor-β1

Objective · To clarify effect of adipose stem cells derived conditioned medium (ASCs-CM) on fibrogenesis of dermal fibroblasts costimulated by transforming growthfactor-β1 (TGF-β1), and explore the possible paracrine pathway of ASCs in regulating the dermal tissue rehabilitation.Methods · Co-stimulated by TGF-β1, dermal fibroblasts were treated with different concentrations of ASCs-CM. Variouscellular events including cell proliferation, apoptosis, and mRNA and (or) protein levels of α-smooth muscle actin (α-SMA), type Ⅰ collagen (COL-1) , type Ⅲ collagen (COL-3), hepatocyte growth factor (HGF), basic fibroblast growth factor (FGF-2) were explored. Furthermore, the effect of HGF antibody on apoptosis of fibroblasts caused by ASCs-CM and TGF-β1 were also observed. Results · ASCs-CM inhibited fibroblasts proliferation caused by TGF-β1 and lead to cell apoptosis. α-SMA expression in fibroblasts was attenuated by 10% ASCs-CM +TGF-β1. It was demonstrated that 100% ASCs-CM with TGF-β1 had promoted collagens (especially COL-3) expression in fibroblasts, and increased HGF mRNA level as well. HGF antibody inhibited fibroblasts apoptosis produced by 100% ASCs-CM and TGF-β1. Conclusion · ASCs may have an effect on fibrogenesis of dermal fibroblasts co-stimulated by TGF-β1 through a paracrine way.High concentration of ASCs-CM not only increases collagen production and secretion, but also inhibits fibroblasts proliferation and accelerates apoptosis.
4.Establishment of HCV infection model in vitro using a human liver cell line 7721

Objective To establish a cell model that is close to the HCV replication in vivo and can support long-term HCV replication in vitro.Methods A human liver cell line 7721 was inoculated with serum from a chronic hepatitis C patient for 8 hours.After incubation,the presence of HCV RNA,the expression of HCV antigen and the location of HCV RNA in cells and/or supernatant were detected by RTPCR,immunohistochemistry and in situ hybridization respectively.Results It was found that plus-and minus-strand of HCV RNA could be discontinuously detected in both cells and supernatant as late as 66 days after inoculation even if cells had been subcultured for 6 times.HCV NS3、NS5 antigens could be expressed in cells and HCV RNA was mainly located within cytoplasm.Conclusion The above results suggested that 7721 cell line was not only susceptible to HCV but also could support its long-term replication in vitro.This HCV infection model in vitro was proved to be a useful tool for studying the mechanism of HCV infection and replication,as well as evaluating the antiviral compounds and screening the protective antibodies/vaccines.
5.Effect of Probiotics on Gastrointestinal Hormine in Intrahepatic Cholestasis Rats Induced by Alpha-Naphthy-lisothiocyanate and Mechanism of Probiotics on Cholestasis

Objective To investigate the effect and mechanism of probiotic products on alpha-naphthylisothiocyanate(ANIT) induced intrahepatic cholestasis of rats.Methods Seventy-two SD rats were randomly divided into 3 groups: control group(n=8),intoxication group(n=32) and intervention group(n=32).After 3 days adapted feeding,all rats received a single intragastric administration of ANIT(200 mg/kg) to induce acute intrahepatic cholestasis.Probiotics[4.2?108/(kg?d)] was given to intervention group 2 days before ANIT was administrated.After ANIT was given,biliary flow,alanine aminotransferase transaminase(ALT),total bilirubin(TB) and gastrointestinal hormone were recorded in every 48 h till 192 h.SPSS 12.0 software was used to analyze the data.Results 1.Forty-eight hours after ANIT was gavaged,the biliary flow of intoxication and intervention groups were lowered while ALT and TB were increased.But the above observation items of intoxication group were more significantly altered than those of intervention group.Then,they all recovered gradually.Those of intervention group returned to nearly normal levels 144 h after ANIT was gavaged while those of the intoxication group recover at 192 h.2.After ANIT was gavaged,MTL of intoxication and intervention groups was lowered while vasoactive intestinal peptide(VIP) increased.But the above items of intoxication group were more significantly altered than those of intervention group.Then,they all recovered gradually and the intervention group was much more early to returned to normal levels than the intoxication group at 192 h.Conclusions Probiotics can promote the bowel movement through increasing the secretion of gastrointestinal hormone,then relieve the cholestasis.
6.Correlation analysis of SLC29A1 gene mutation with gemcitabine toxicity and clinical prognosis in treating advanced pancreatic cancer

Objective To detect the sequence of SLC29A1 gene rs1288 single nucleotide polymorphism (SNP) in advanced pancreatic cancer patients,and to explore its correlation with gemcitabine toxicity and prognosis.Methods Peripheral blood samples were collected and DNA was extracted.The segment containing SLC29A1 gene SNP (rs1288) was amplified by PCR,and then DNA sequencing was conducted to identify SLC29A1 gene SNP (rs1288).According to the sequencing results,the patients were divided into SLC29A1 gene rs1288 T→A mutation type group and wild type group.Clinical data,toxicity of gemcitabine chemotherapy,progression free survival (PFS) and overall survival (OS) between two groups were compared.Results A total of 83 pancreatic cancer patients were enrolled.Sequencing results showed that SLC29 A1 gene 1288 T→A mutation type was present in 52 patients and wide type was observed in 31 patients,so mutation rate was 62.7%.All the patients in both two groups could tolerate the gemcitabine toxicity,and no chemotherapy related death occurred.There were no statistical differences on the gender,age,CA19-9,tumor site,size and TNM stage between the two groups.There were statistically higher iucidences of leukopenia and thrombocytopenia in the SLC29A1 gene rs1288 T →A mutation type group compared to the wild type group (55.8% vs 32.3%,P<0.05;40.4% vs 19.4%,both P<0.05).Median OS and PFS in mutation type group were shorter than those in wide type group (11 months vs 14 months,P < 0.05;9 months vs 12 months,P < 0.05).Conclusions Advanced pancreatic cancer patients with the SLC29A1 gene rs1288 T→A mutation type had a higher incidence of adverse reaction in gemcitabine chemotherapy and a worse therapeutic effect,and thus detecting the mutation of SLC29A1 gene rs1288 point mutation may serve as a marker for evaluating the toxicity and prognosis of gemcitabine chemotherapy in patients with pancreatic cancer.

Background and purpose:To investigate the clinico- pathological characteristics, prognosis and diagnosis of primary renal sarcoma that is a rare, highly malignant tumor with poor prognosis and survival. Methods:9 cases of renal sarcoma in our hospital were reviewed and analyzed. Nephrectomy was the main treatment for the patients. All the specimens were stained with histochemicial and immunohistochemical method, correlated literatures were also reviewed.Results:The distributions of pathology were malignant fibrous histiocytoma leiomyosarcoma, liposarcoma, myxosarcoma synoviosarcoma. Immunohistiochemical test was essential for the diagnosis of renal sarcoma.Conclusions:Pain and palpable mass were the common symptoms. Early radical surgery remains the optimal treatment, the role of chemotherapy and radiotherapy in the treatment modalities of the disease is still controversial.

9.Protective mechanism of hepatic ischemic postconditioning

lschemic postconditioning(IPo)is a way that after long ischemia on liver graft,animals are given one or several brief reperfusion-ischemia before persistent reperfusion to improve the hepatic tolerance and relieve the ischemie reperfusion injury.It has been proved an effective and controlled method to attenuate the ischemic reperfusion injury.Protective mechanism of ischemic postconditioning on hepatic graft is related with protecting sinus hepaticus endotheliocyte and hepatic microcirculation,relieving hepatic cells injury and inflammatory reaction induced by oxygen free radicals,relieving calcium ovedoad in hepatic cells and mitochondria,regulating apoptosis genes,transforming ion channels condition in mitochondria.This article will makes a brief review on protective mechanism ofhepatic ischemic posteanditioning.
10.Screening of proteins interacting with HSPC016 by yeast two-hybridization technique

Objective To screen and identify proteins interacting with hematopoietic stem/progenitor cell differentiation-related gene HSPC016, and to explore the molecular mechanism involved in the regulation by HSPC016 on the aggregative behavior of dermal papilla cells. Methods By using yeast two-hybridization,HSPC016 gene was sub-cloned into pGBKT7 to construct the bait plasmid (named as pGBKT7-HSPC016) in yeast AH109. The cDNA yeast expression library of human dermal papillae cells in yeast Y187 was screened with the bait plasmid and the proteins interacting with HSPC016 were identified. Yeast two-hybridization retransformation experiment was conducted to exclude the false positive clones and verify the interactions, then,the positive clones were sequenced and analyzed by using bioinformatic methods. Results The bait plasmid pGBKT7-HSPC016 was constructed successfully and there was no self-activation in or toxicity against yeast AH 109. Four proteins,including forkhead family of transcription factors (FOXO 1 ), mitogen-activated protein kinase 11 (MAPK 11 ), phosphoinositide-3-kinase (PIK3R3) and liver X receptor were screened and identified. Bioinformatic analysis revealed that these proteins had close relationship with intracellular energy metabolism and translational regulation. Conclusions HSPC016 may regulate the aggregative behavior of DPCs by regulating the levels of intracellular reactive oxygen species (ROS) and interacting with signaling molecules involved in intracellular energy metabolism and translational regulation.
