1.Clinical research of bilevel non-invasive mechanical ventilation in the treatment of chronic obstructive pulmonary disease with respiratory failure

Objective To explore the application of bilevel non-invasive mechanical ventilation in the treatment of chronic obstructive pulmonary disease(COPD) with respiratory failure.Methods 126 COPD patients with type Ⅱ respiratory failure were selected and divided into control group and research group,63 cases in each group.The control group received regular treatment,and the research group received bilevel non-invasive mechanical ventilation on the basis of regular treatment.,The results of blood gas analysis,respiratory frequency(RR),heart rate(HR) and expiratory flow(PEF) were observed and compared between the two groups.Results The indicators of pH,PaCO2,PaO2 in the research group after treatment were (7.41±0.08),(51.26±12.13)mmHg,(77.28±15.34)mmHg,which in the control group were (7.26±0.07),(67.21±13.05)mmHg,(69.26±14.31)mmHg,compared with before treatment,pH and PaO2 of the two groups were increased,PaCO2 was decreased after treatment,and the three indicators of the research group were changed significantly better than those of the control group,the differences were statistically significant(t=11.2,7.11,3.03,all P<0.01).After treatment,HR and PR of the two groups were decreased,PEF was increased.The three indicators of the research group were also changed significantly better than those of the control group,the differences were statistically significant(t=3.13,8.58,5.45,all P<0.01).Conclusion Bilevel non-invasive mechanical ventilation in the treatment of COPD with respiratory failure has remarkable clinical effects,can significantly improve the patients' blood gas analysis index,HR,RR and PEF,and it is worthy of clinical application.

Lung cancer is one of the most malignant tumors that damage human health and life.Metastasis and recurrence of lung cancer are the leading causes of death.Developing techniques that can detect lung cancer,particularly at an early stage,as well as predict recurrence and metastasis,is an important strategy to improve the outcome.Circulating tumor cell (CTC),regarded as liquid biopsy,open a new field for early diagnosis of lung cancer,as well as for prediction of metastasis,evaluation of prognosis and monitoring of recurrence.
3.The effect of nursing intervention on metabolic syndrome caused by Olanzapine

Objective To study the influence of nursing intervention on metabolic syndrome of schizophrenia patients treated with Olanzapine.Methods 100 acute-episode schizophrenia patients treated with Olanzapine alone were randomized into the intervention group and the control group,each group with 50 cases.The control group received the routine psychiatric nursing,and the intervention group added nursing intervention to prevent metabolic syndrome.The changes of BMI,fasting blood-glucose,triglyceride,cholesterol,high-density lipoprotein of both groups were compared on the day upon admission and on the 4th,8th,12th and 48th weeks after hospitalization respectively.Results On the 4th week BMI and fasting blood-glucose,triglyceride,cholesterol of the intervention group were significantly lower than those of the control group,but high-density lipoprotein was higher than the control group.Conclusions Strengthening nursing intervention measures can effectively control metabolic syndrome caused by Olanzapine treatment,which can improve the medication compliance of patients and prevent schizophrenia recurrence.
4.Clinical effect of pills of six ingredients with rehmannia combined with ginkgo biloba on prevention and treatment of early retinopathy in type 2 diabetes mellitus patients

AIM:To investigate the clinical effect of pills of six ingredients with rehmannia combined with ginkgo biloba leaf table in the prevention and treatment of early retinopathy in patients with type 2 diabetes mellitus.METHODS:A total of 82 patients (82 eyes) with type 2 diabetes mellitus who were treated with endoscopy from January 2013 to January 2015 were randomly divided into observation group and control group according to the random number table method.The control group was given conventional western medicine treatment, the observation group in the control group was given pills of six ingredients with rehmannia combined with ginkgo leaf table treatment besides the control group treatment.The diabetic retinopathy was assessed in both groups, as well as changes in blood glucose, blood pressure, and glycosylated hemoglobin.We recorded the occurrence of adverse reactions during and after treatment.RESULTS: The new diagnosed rate and the progress rate of observation group were lower than those of the control group, remission rate was higher, and the difference was statistically significant (P<0.05).There was no significant difference of the two groups on blood glucose, blood pressure and glycosylated hemoglobin before and after treatment (P>0.05).The incidence of adverse reactions between the two groups was not statistically significant (P>0.05).CONCLUSION: Type 2 diabetes mellitus patients treated with pills of six ingredients with rehmannia combined with ginkgo biloba leaf table can effectively control the rate of retinopathy, the rate of progress, improve the rate of remission with high drug safety, and have the significance of further clinical research.

Objective To solve the conflict between data security and sharing in hospital so that patients can get their clinical data through result query service system.Methods Advanced secure data-exchange technology was used to realize network isolation.Integrated result query service system adopted the embedded system as hardware and the embedded LINUX operation system as software.Results Through LAN query module,internet query module and the secure data-exchange module,the design of HIS multimedia query system was realized.Conclusion Via the application of HIS integrated query platform,the design of the hospital secure data exchange is proved to be feasible,reliable and practical.
9.Effect of Bushen Huatan Recipe on the Akt signal pathway in polycystic ovarian syndrome model rats with insulin resistance: an experimental research.

OBJECTIVETo observe the effect of Bushen Huatan Recipe (BHR) on the Akt signal pathway in polycystic ovarian syndrome (PCOS) model rats with insulin resistance (IR).
METHODSFifty Wistar female PCOS rats were randomly and equally divided into 5 groups, i.e., the control group, the model group, the low dose BHR group (5.406 g/kg), the medium dose BHR group (10.812 g/kg), and the high dose BHR group (21.624 g/kg), 10 in each group. Levels of fasting blood glucose (FBG) and insulin were detected to calculate homeostasis model assessment of insulin resistance (HOMA-IR). The expression of insulin receptor substrate 1 (IRS-1), glycogen synthetase kinase-3beta (GSK-3beta), glucose transporter 4 (GLUT-4), and peroxisome proliferator activated receptor (PPAR-gamma) mRNA were detected by RT-PCR. The expression of insulin signal transduction molecular kinase B (Akt) was detected by Western blot.
RESULTSCompared with the control group, HOMA-IR and the mRNA expression of PPAR-gamma mRNA significantly increased, the mRNA expression of GSK-3beta, GLUT-4, and IRS-1, protein expression of Akt and p-Akt significantly decreased in the model group (P < 0.05). Compared with the model group, HOMA-IR significantly decreased, the mRNA expression of GSK-3beta, GLUT-4, IRS-1, and Akt protein significantly increased in the high dose BHR group (P < 0.05, P < 0.01). The mRNA expression of p-Akt protein increased more obviously (P < 0.05, P < 0.01). mRNA expression of GSK- 3beta and GLUT-4 significantly increased (P < 0.05), while the mRNA expression of PPAR-gamma significantly decreased in the low and middle BHR groups (P < 0.05). The expression of p-Akt significantly increased in the low dose BHR group (P < 0.05).
CONCLUSIONSIR and abnormal insulin signal pathway existed in PCOS model rats. BHR could improve IR of PCOS rats, which was correlated with regulating protein expression of insulin signal transduction molecules.

10.Effect of two different surgeries on corneal endothelial cells in grade Ⅱ - Ⅲ lens nucleus

AIM: To compare the effect of small incision extracapsular cataract extraction and phacoemulsification on corneal endothelial cells and explore the differences between these two most commonly used surgeries for grade Ⅱ-Ⅲ lens nucleus. · METHODS: Retrospective study. We retrospectively evaluated the data of age-related cataract patients who had completed surgery in our hospital between June 2013 and December 2015. There were 84 eyes of 84 patients. The corneal endothelial cell density of all the patients were greater than 2000/mm2 and lens nucleus were gradeⅡ- Ⅲ. The patients were divided into manual small incision cataract surgery ( MSICS ) group and phacoemulsification ( Phaco ) group. MSICS group included 42 cases (42 eyes) and Phaco group included 42 cases ( 42 eyes ) . Postoperative evaluations were performed at 1d, 1wk, 1mo and 6mo and included uncorrected visual acuity ( UCVA ) , corneal endothelial cell density and the proportion of hexagonal cells. · RESULTS: There were no statistically significant differences of the two groups in preoperative basic data, UCVA, corneal endothelial cell density and the proportion of hexagonal cells, nuclear classification. The postoperative UCVA were significantly improved in both groups (x2=148. 01, P<0. 001; x2=165. 97, P<0. 001). The postoperative UCVA were on the rise as time goes on. The postoperative corneal endothelial cell density was obviously reduced in both groups compared with the preoperative (F=37. 74, P<0. 001; F=24. 56, P<0. 001). The proportion of hexagonal cells in Phaco group was declined at 1d (P=0. 002) after the operation and returned to baseline by 1wk (P=0. 894) after surgery. There was no significant difference between the two groups in UCVA, corneal endothelial cell density and the proportion of hexagonal cells at each observation point (P>0. 05). ·CONCLUSION: Manual small incision cataract surgery and phacoemulsification can indiscriminately improve UCVA and decrease the density of corneal endothelial cells. However, the change of hexagonal cells proportion was not obvious. These two surgeries for cataract patients with corneal endothelial cell density greater than 2000/mm2 , grade Ⅱ-Ⅲ lens nucleus have similar efficacy and safety.
