Lung cancer is one of the most malignant tumors that damage human health and life.Metastasis and recurrence of lung cancer are the leading causes of death.Developing techniques that can detect lung cancer,particularly at an early stage,as well as predict recurrence and metastasis,is an important strategy to improve the outcome.Circulating tumor cell (CTC),regarded as liquid biopsy,open a new field for early diagnosis of lung cancer,as well as for prediction of metastasis,evaluation of prognosis and monitoring of recurrence.
2.The rapid regulation of glucocorticoid on high-K~+ induced catecholamine secretion in PC12 cell

Objective: To investigate the effect of glucocorticoid on high K + induced catecholamine secretion in PC12 cells. Methods: High performance liquid chromatography with electrochemical detection were used to measure catecholamine secreted by PC12 cells. Results:(1) After PC12 cells were pretreated with various concentration of corticosterone at 37℃ for 5 min and then stimulated with various concentration of high K + for different time periods, the inhibitory effect of corticosterone on catecholamine secretion was found in a dose dependent manner. (2) When the duration of high K + treatment was prolonged, the inhibitory effect of corticosterone on catecholamine secretion tended to be attenuated. Conclusion: Glucocorticoid can rapidly inhibit the catecholamine secretion induced by high K + in PC12 cells. [
3.Effect of Acupuncture plus Medication on Serum 5-HT, CGRP and ET in IBS-D

Objective To observe the efficacy of acupuncture plus medication in treating diarrhea-irritable bowel syndrome (IBS-D) due to liver depression and spleen deficiency, and its effect on serum 5-hydroxytryptamine (5-HT), calcitonin gene-related peptide (CGRP) and endothelin (ET). Method Sixty IBS-D (liver depression and spleen deficiency) were recruited and randomized into an observation group and a control group, 30 cases each. Patients in the observation group were intervened by acupuncture plus modified Bai Zhu Shao Yao powder, while the control group was intervened by acupuncture alone. The total effective rate, change of syndrome score of traditional Chinese medicine (TCM) and diarrhea-irritable bowel syndrome quality of life scale (IBS-QOL), as well as the levels of serum 5-HT, CGRP and ET, were observed. Result The total effective rate was 83.3% in the observation group, significantlyhigher than 63.3% in the control group (P<0.05). There were no significant differences in comparing the global and component TCM symptoms scores and IBS-QOL between the two groups before the treatment (P>0.05). The global and component TCM symptoms scores and IBS-QOL dropped significantly in both groups after the intervention (P<0.05). The global TCM symptoms score and IBS-QOL in the observation group were significantly lower than those in the control group after the intervention and 3 months after the intervention (P<0.05); the component TCM symptoms scores in the observation group were significantly lower than those in the control group after the intervention (P<0.05). There were no significant differences in comparing the serum 5-HT, CGRP and ET levels between the two groups (P>0.05) before the treatment. The levels of 5-HT, CGRP and ET decreased significantly after the treatment in both groups (P<0.05); after the intervention, the levels of 5-HT, CGRP and ET in the observation group were significantly lower than those in the control group (P<0.05). Conclusion Acupuncture plus medication can effectively mitigate the symptoms in IBS-D (liver depression and spleen deficiency) and enhance the quality of life, which is possibly achieved by down-regulating the serum 5-HT, CGRP and ET levels.

5.Effect of duration of carbon dioxide pneumoperitoneum on coagulation, fibrinolysis and endothelial activation in elderly patients

Objective To observe the effect of duration of carbon dioxide pneumoperitoneum on coagulation, fibrinolysis and endothelial activation in elderly patients undergoing laparoscopic cholecystectomy (LC). Methods The 45 elderly patients with cholelithiasis scheduled for LC, aged over 60 yeas, were placed in different groups respectively after surgery according to the duration of pneumoperitoneum. The duration of pneumoperitoneum was ≤60 minutes in group A (n=21),and more than 60 minutes in group B (n=24). Venous blood samples were taken on admission (baseline), at the end of surgery, the 1st, 2nd and 3rd day after surgery for determination of prothrombin time (PT), activated partial thromboplastin time (APTT), prothrombin fragment F1+2 (F1+2), antithrombin 3 (AT-Ⅲ activity), fibrinogen (Fib), tissue plasminogen activator (t-PA), plasminogen activator inhibitor type-1 (PAI-1), D-dimer (D-D), von Willebrand factor (vWF activity). Results Concerning the coagulation activation, at the 3rd postoperative day, the level of F1+2 was significantly higher in group B than in group A [(1.60±0.26) μg/L vs. (1.32±0.24) μg/L, P<0.05]; AT-III was significantly higher in group B than in group A [(84.82%±20.21%) vs. (97.49%±16.87%), P<0.05]. At the 2nd and 3rd postoperative day, the levels of Fib were significantly higher in group B than in group A [(3.87±0.62) g/L vs. (3.42±0.72) g/L, (3.98±0.77) g/L vs. (3.42±0.63) g/L, respectively, P<0.05]. Concerning fibrinolysis, But at the 2nd and 3rd postoperative day, the level of PAI-1 was significantly higher in group B than in group A [(33.93±10.42) μg/L vs. (26.69±9.49) μg/L, (32.90±11.25) μg/L vs. (26.31±7.06) μg/L respectively, P<0.05]. Concerning endothelial activation, at the 2nd and 3rd postoperative day, vWF was significantly higher in group B than in group A [(174.53%±44.03%) vs. (134.37%±37.74%), (176.31%±47.6%) vs. (131.21%±36.34%), respectively, P<0.05]. Conclusions Marked activations of coagulation-fibrinolysis and endothelial activation are observed postoperatively in elderly patients undergoing laparoscopic cholecystectomy. Along with prolonged duration of pneumoperitoneum, more pronounced alterations of increased coagulation, reduced fibrinolysis and endothelial activation are observed, which could constitute an imbalanced situation of coagulation-fibrinolysis and increases the risk of venous thrombosis.
6.Comparison of changes in blood coagulation, fibrinolysis and endothelial damage in patients undergoing laparoscopic cholecystectomy with different durations of carbon dioxide pneumoperitoneum

Objective To compare the changes in blood coagulation, fibrinolysis and endothelial damage in patients undergoing laparoscopic cholecystectomy with different durations of carbon dioxide pneumoperitoneum. Methods Sixty-four ASA Ⅰ orⅡpatients, aged 23-60 yr, weighing 45-82 kg, scheduled for elective laparoscopic cholecystectomy, were randomly divided into 3 groups according to the duration of pneumoperitoneum: duration of pneumoperitoneum ≤30 min group (group Ⅰ, n=21), 30 min < duration of pneumoperitoneum < 60 min (group Ⅱ, n=23) and duration of pneumoperitoneum≥ 60 min (group Ⅲ , n=20).The intra-abdominal pressure was maintained at 12-14 mm Hg. Venous blood samples were taken before surgery (baseline, T0 ),at the end of surgery(T1), and at 1, 2 and 3 d after surgery (T2-4) for determination of prothrombin time, activated partial thromboplastin time, concentrations of prothrombin fragment 1+2(F1+2), fibrinogen (Fib), tissue plasminogen activator and plasminogen activator inhibitor type-1 (PAI-1), and activities of antithrombin Ⅲ(AT-Ⅲ)and von Willebrand factor(vWF).Results Compared with groupⅠ , the vWF activity and PAI-1 concentration at T2 , concentrations of Fib, F1+2, PAI-1 and activity of vWF at T3 and concentrations of Fib and F1+2 at T4 were significantly increased, while the AT-IE activity at T3 was significantly decreased in group Ⅲ(P<0.05) .Conclusion When the duration of pneumoperitoneum is short, no obvious changes in the blood coagulation, fibrinolysis and endothelial damage are observed postoperatively in patients undergoing laparoscopic cholecystectomy, and along with the prolongation of the duration of pneumoperitoneum, increased blood coagulation, reduced fibrinolysisand aggravated endothelial damage are observed postoperatively.
7.The treatment of postoperative local recurrence of periampullary carcinoma

Objective To evaluate the treatment modalities for local recurrent periampullary carcinoma. Methods From January 1997 to May 2007, 23 patients with local recurrent periampullary carcinoma underwent different therapy. The recurrent tumor was resected through laparotomy in 3 cases and the unresectable tumors were treated with radiofrequency ablation(RFA) in 5 patients. In 10 patients and 5 patients respectively high intensity focused ultrasound ( HIFU ) and regional chemotherapy ( RC ) were performed. Results The resection rate for local recurrent periampullary carcinoma was 13% in this group, 2 patients have survived for 10 months and 13 months respectively after reoperation while one patient achieved tumor-free survival for six months so far. The symptoms were alleviated to some extent in 20 patients treated with RFA, HIFU or RC. Conclusions Resection of local recurrent periampullary carcinoma may benefit a longer survival time, and the improvement of the quality of life. RFA, HIFU and RC are the alternative modalities for patients with unresectable tumors.
8.Relationship between Helicobacter pylori Infection and Expression of FOXA1 in Gastric Mucosa

Background:Helicobacter pylori( Hp)is listed as Group 1 human carcinogen. Transcription factor FOX family plays important role in the development of tumor,and FOXA1 is a key target transcription factor of some microRNAs( miRNAs). Aims:To investigate the relationship of Hp infection with expression of FOXA1 in gastric mucosal tissues. Methods:Eighty patients undergone gastroscopy from Jan. 2014 Sept. 2014 were enrolled. Hp infection was detected by 14 C-urease breath test( 14 C-UBT),modified Giemsa staining and bacterial culture. Expression of FOXA1 in gastric mucosal tissues was determined by immunohistochemical SP method. Results:FOXA1 expression in Hp positive tissues was significantly higher than that in Hp negative tissues(73. 8% vs. 47. 4%;χ2 =4. 81,P<0. 05). Conclusions:High expression of FOXA1 in gastric mucosal tissues suggests that miRNAs pathway may be one of the molecular mechanisms of Hp infection leading to gastric intraepithelial neoplasia even gastric cancer.
9.Comparison of Preoperative Screening for Patients with Obstructive Sleep Apnea Hypopnea Syndrome by Cinema Magnetic Resonance and Fiber Optic Laryngoscope

Objective To localize the upper airway obstruction of patients with obstructive sleep apnea hypopnea syndrome (OSAHS)with the Cinema Magnetic Resonance(Cine-MR) and fiber optic laryngoscope with Müller maneuver(FLMM)before operation and discuss the clinical application values.Methods Before operation, FLMM and Cine-MR were applied to 22 patients diagnosed as OSAHS by Polysomnography(PSG).Medical examinations conducted in this study from September 2015 to April 2016 to examine the obstruction of the soft palate region, the lingual region and epiglottis.Results There were complete agreements between the Cine-MR and FLMM at locating obstruction sites of the soft palate (n=22/n=22),and there were moderate agreements between the Cine-MR and FLMM in locating obstruction sites of the retroglottal region(n=13/n=6),epiglottal region (n=4/n=2)and multiple level(n=13/n=6), respectively.Conclusion For those moderate and severe OSAHS patients with multiple sites obstruction , the preoperative application of the Cine-MR and FLMM together will be better in locating the obstruction sites.
10.Comparison of Sedative Effect of Dexmedetomidine and Midazolam on Patients with Comprehensive ICU Me-chanical Ventilation

OBJECTIVE:To compare the sedative effects of dexmedetomidine and midazolam in patients with comprehensive ICU mechanical ventilation. METHODS:74 patients in ICU were randomly divided into dexmedetomidine group (37 cases) and midazolam group (37 cases). Based on mechanical ventilation,dexmedetomidine group received 200 μg Dexmedetomidine injec-tion,dissolving in 48 ml 0.9% Sodium chloride injection,pumped by 1 μg/kg within 30 min,then adjusted to 0.2-0.7 μg/(kg·h) by micropump. Midazolam group received 30 mg Midazolam injection,dissolving in 44 ml 0.9% Sodium chloride injection, pumped by 0.05 mg/kg within 15 min,then adjusted to 0.04-0.20 mg/(kg·h)by micropump. The sedative effects,clinical indica-tors(weaning time,extubation time,time of mechanical ventilation,ICU hospitalization time)and the incidence of adverse reac-tions in 2 groups were observed. RESULTS:There was no significant difference in the sedative effects(P>0.05);weaning time, extubation time,and ICU hospitalization time in dexmedetomidine group were significantly shorter than midazolam group,the dif-ferences were statistically significant(P<0.05);the incidence of total adverse reactions in dexmedetomidine group was significant-ly lower than midazolam group,the difference was statistically significant (P<0.05). CONCLUSIONS:Dexmedetomidine show similar sedative effects with midazolam in patients with comprehensive ICU mechanical ventilation,shorter weaning time,extuba-tion time,ICU hospitalization time,and lower incidence of total adverse reactions than midazolam.
