1.Influencing factors of heart failure among patients with continuous ambulatory peritoneal dialysis

Objective To explore the influencing factors of heart failure among patients with continuous ambulatory peritoneal dialysis(CAPD).Method Clinical data were collected from the histories of 158 patients receiving CAPD to investigate the factors inducing heart failure for the purpose of finding out the potential independent influencing factors using t test or chi-square test for univariate analyses and logistic regression in multivariate analyses.Results The incidence of heart failure was 36.71%among the enrolled CAPD patients.Single factor analysis showed that level of volume management,age,hemoglobin,serum pre-albumin, cholesterol and c-reactive protein were related to heart failure.Analysis of multiple variables logistic regression revealed that levels of volume management,serum pre-albumin and blood urea nitrogen were related to heart failure.Conclusions It is an easy,effective and cheaper way for nurses to increase patients’ability of volume control.The strategy including remedying malnutrition and decreasing the level of uremic toxins might be effective for delayed exacerbation of cardiac function among CAPD patients.
2.Recent progress in ERCP for biliary and pancreatic diseases

In recent years,with the continuous development of endoscopic and interventional techniques,many new devices and methods have been used in clinical practice,and the application of endoscopic retrograde cholangiopancreatography (ERCP)in biliary and pancreatic diseases has developed rapidly.This paper reviews and summarizes the recent progress in ERCP among patients with biliary and pancreatic diseases,including those with altered gastrointestinal anatomy,pregnant patients,patients with benign and malignant biliary strictures,and patients with pancreatic pseudocysts,as well as the application of SpyGlass,photodynamic therapy,and radiofrequency ablation,the man-agement of ERCP-related duodenal perforation,and the prevention of post-ERCP pancreatitis.All the progress has made a great contribu-tion to the diagnosis and treatment of biliary and pancreatic diseases.
3.Effect of calcitonin combined with alfacalcidol on clinical outcome, lumbar vertebrae bone mineral density and safety in patients undergoing internal fixation of humeral shaft fracture

Objective To analyze the effect of calcitonin combined with alfacalcidol on clinical outcome,lumbar vertebrae bone mineral density and safety in patients undergoing internal fixation of humeral shaft fracture.Methods 80 patients with humeral shaft fracture treated with internal fixation in Xunwu county people's hospital from June 2010 to June 2016 were selected,and randomly divided into control group and observation group,40 cases in each groups.The control group was given calcium carbonate D3 tablets and functional exercise therapy,the observation group treated calcitonin combined with alfacalcidol on the basis of control group,analysis and comparison the clinical effects,lumbar spine BMD and safety.Results Compared with before treatment,serum calcium level of two groups increased significantly at treatment 2 weeks and 3 weeks(P<0.05);The control group BMD of lumbar spine was significantly increased in treatment 12 weeks(P<0.05),the observation group BMD of lumbar spine was significantly increased in treatment 4 weeks and 12 weeks(P<0.05).Compared with control group,the level of serum calcium in observation group was higher at treatment 2 weeks and 3 weeks (P<0.05),BMD of lumbar spine was higher at treatment 4 weeks and 12 weeks(P<0.05);The total adverse reaction rate of observation group was 10%,the difference was not statistically significant to control group 15%.Conclusion The combination of calcitonin and alfacalcidol treatment of internal fixation of humeral shaft fractures in patients with significant results, can improve serum calcium levels in patients with and improve bone mineral density, and less adverse reactions.
4.Application of laparoscopy in non-traumatic acute abdominal emergency

Objective To explore the application of laparoscopy in non-traumatic acute abdominal emergency. Methods The authors retrospectively analyzed 201 cases of emergency laparoscopy between March 1999 and May 2002. Results All the 201 patients were unequivocally diagnosed during operation. Laparoscopic operations were successfully completed in 190 cases (139 cases of acute appendicitis, 21 cases of acute cholecystitis, 19 cases of upper gastrointestinal tract perforation, 10 cases of intestinal obstruction and 1 case of common bile duct stones accompanied with acute cholecystitis), with a success rate of 94.5% (190/201). The remaining 11 patients underwent a conversion to open surgery. Conclusions Emergency laparoscopic exploration can not only clarify a diagnosis for acute abdominal emergency with unknown causes but also simultaneously offer a therapeutic method in at least 90% of cases.

8. Pharmacokinetics and extended-release characteristics of minocycline hydrochloride extended-release tablets in healthy volunteers

OBJECTIVE: To investigate the pharmacokinetics of single and multiple oral doses of minocycline hydrochloride extended-release tablets and evaluate its extended-release characteristics by comparing with ordinary tablets. METHODS: Twelve healthy volunteers received a single oral dose of 45, 90 and 135 mg minocycline hydrochloride extended-release tablets respectively with a 10-day washout period. After the single-dose study, the volunteers participated in the multiple dose study in which each volunteer received 90 mg per day for 10 consecutive days. The ordinary tablets were administered by single and multiple doses as reference preparation in the end. The concentrations of minocycline in human plasma were determined by LC-MS method. RESULTS: The main pharmacokinetic parameters of a single dose of minocycline hydrochloride extended-release tablets of 45, 90 and 135 mg in 12 healthy volunteers were as follows: pmax (0.4770±0.1280), (1.011±0.191) and (1.500±0.281) μg · mL-1, tmax(3.3±1.1),(3.6±0.8) and (3.4±0.7) h, t1/2 (17.1±5.4), (18.3±4.9) and (17.9±3.4) h, AUC0-t (9.391±3.019), (20.01±3.07) and (31.81±6.80) μg · h · mL-1, respectively. And those of multiple-dose of minocycline hydrochloride extended-release tablets of 90 mg were as follows: pav(0.8440±0.2250) μg · mL-1, DF(1.1±0.2), pmax(1.438±0.383) μg · mL-1, tmax(3.5±0.8) h, t1/2 (19.3±4.4) h, AUC0-t(31.18±9.39) μg · h · mL-1. The main pharmacokinetic parameters of a single dose of ordinary minocycline hydrochloride tablets of 100 mg were as follows: pmax(1.418±0.427) μg · mL-1, tmax (2.6±0.7) h, t1/2 (16.9±3.9) h, AUC0-t(25.35±5.80) μg · h · mL-1, and those of multiple-dose of ordinary minocycline hydrochloride tablets at 100 mg were as follows: pav(1.229±0.377) μg · mL-1, DF(1.3±0.2), pmax(2.188±0.652) μg · mL-1, tmax(2.3±0.7) h, t1/2 (17.7±2.4) h, AUC0-t(44.83±16.29) μg · h · mL-1, respectively. CONCLUSION: In the range of 45-135 mg, the AUC0-t and pmax of minocycline hydrochloride extended-release tablets increased in a dose-dependent manner after single-dose administration, indicating linear pharmacokinetics. No significant gender differences were found. An accumulation of 50% in AUC0-t occurred after multiple-dose administration. The minocycline hydrochloride extended-release tablets have extended-release character comparing with ordinary tablets.
9.Effect of family-of-origin paternal parenting on father involvement of 1-12 years old children: the mediating effect of role perception and moderating effect of depression

Objective:To identify the distal and proximal influencing factors of father involvement, and explore the mediating role of father's role perception and the moderating role of depression in the effect of his family-of-origin paternal parenting on father involvement.Methods:Totally 572 fathers with children aged under 12 years old completed the father involvement questionnaire, role of the father questionnaire (ROFQ), parental bonding instrument (PBI) and the center for epidemiologic studies depression scale (CES-D). The Pearson correlation analysis and non-parametric percentile Bootstrap method were conducted via SPSS 21.0.Results:(1)Father involvement (2.86±0.58) was found to be negatively correlated with his own family-of-origin paternal indifference (7.30±3.86) ( r=-0.175, P<0.01), positively correlated with his family-of-origin paternal care (10.46±3.98) ( r=0.291, P<0.01), negatively correlated with his depression (7.07±5.09) ( r=-0.266, P<0.01) and positively correlated with his role perception (61.52±5.92) ( r=0.522, P<0.01). (2)Father's role perception partially mediated the relationship between his family-of-origin paternal care and his own paternal involvement (the mediating effect was 0.015, which was 35.71% of the total effects) and fully mediated the relationship between his family-of-origin paternal indifference and his own paternal involvement (the mediating effect was -0.016). (3)Father's depression played a moderating role between his family-of-origin paternal care and his own role perception ( β=-0.069, P<0.05). And it also played a moderating role between his family-of-origin paternal indifference and his own role perception ( β=0.121, P<0.001). The results of J-N methods showed that the values and significance of simple slopes varied according to father's depression levels in both two models. Conclusion:The paternal parenting that fathers had experienced in his family of origin is an important factor affects his own paternal involvement.In the relationship between father's family-of-origin paternal parenting and his own paternal involvement, father's role perception plays a mediating role and his depression level plays a moderating role.
