1.Complications of Visceral and Vascular Injury in Laparoscopic Surgery for Gynecologic Diseases

Objective To study the incidence of complications of visceral and vascular injury in laparoscopic surgery for gynecologic diseases,and to discuss the ways to decrease the incidence. Methods The data of 2684 patients who received laparoscopic surgery from Januray 2003 to December 2005 in the Department of Obstetrics and Gynecology were reviewed retrospectively.The incidence and treatment of complications of visceral and vascular injury were observed. Results The total incidence rate of complications was 2.53%(n=68),and that of the visceral and vascular injury was 0.37%(n=10).Four cases of injury were related with trocar punctures(injury of omental blood vessel,n=2;postperitoneal vessel,n=2),three took place during the operation (severe bleeding,n=1;bladder injury,n=2),and the other three were observed in the postoperative stage(ureter injury,n=2;intestinal fistula,n=1). Conclusion The complications in laparoscopic surgery for gynecologic diseases are increased with the extension and difficulty of operation,and are closely related with the experience of the surgeons.Proper candidates for the surgery and established operative technique are the key factors in decreasing the incidence.
2. Absorption of euphobiasteroid across Caco-2 cell monolayer model

Objective: To study the absorption and transportation of euphobiasteroid in Caco-2 cell monolayer model. Methods: Caco-2 cell monolayer model was used to study the process of bi-direction transport of euphobiasteroid, and the effects of time, drug concentration, and inhibitors on the process were investigated. The concentration of euphobiasteroid was detected by UPLC-MS/MS, and the apparent permeability coefficient (Papp) and apparent permeability (PDR) were calculated. Results: During the transport process of euphobiasteroid in Caco-2 cells, the Papp values of variety concentration were from 1 × 10-6 to 1 × 10-5. The cumulative transshipment volume was increased with time and concentration, which presented concentration-dependent manner. The PDR values of 10, 30, and 50 μmol/L euphobiasteroid were 1.35, 0.83, and 0.65, respectively. Verapamil hydrochloride could promote the transportation of euphobiasteroid from AP side to BL side. Conclusion: The absorption of euphobiasteroid in intestine is moderate and mainly through passive transport. There may be excretion mechanism of intestinal transport protein in the intestine absorption of euphobiasteroid.

4.Study of lesion characteristics and stenting for diseased grafts in older patients

Objective To determine the lesion characteristics and effect of intervention on diseased grafts in older patients. Methods Thirty-eight patients with angina after coronary artery bypass grafts (CABG) were enrolled into Nanjing First Hospital. Interventions were performed after multiple-view angiogram and formal consent from relatives received. Angiographic follow-up was conducted within 6 months. Cardiac events were recorded during the 6-month clinical follow-up. Results Of the 38 patients (36 males and 2 females), the average age was 63.2 years. Angina occurred 11.5 months after CABG. Angipgrams were performed 4.8 years after CABG. Diabetes was complicated in 10 cases and old myocardial infarction in 8, respectively. The mechanism of osteogenesis inducted by SW and its effect of bone repairing are explained emphatically. 6.Study on the relationship between psychological harmony and mental health of health care providers

Objective To study on the relationship between psychological harmony and mental health of health care providers. Methods Surveys on psychological harmony and mental health were conducted in 313 health care providers in Heibei Province by psychological harmony and SCL-90 questionnaire. Results The general psychological harmony,and mental health level of health care providers is lower than the national level; The psychological harmony and its dimensionality of health care providers is affected mainly by many factors such as emotional stability, the level of patients' cooperation, on-duty situation in holiday time, salary, the rate of solving difficult miscellaneous diseases, the number of medical tangle and paranoid symptoms.etc., among which solving difficult miscellaneous diseases effectively is the most important factor. Conclusion The general psychological harmony situation does not affect health care providers' mental health.
7.Study on Aorui Gel in Preventing Esophageal Mucosa from Injury in Reflux Esophagitis Rats

Objective To observe protective and repairing effect of Aorui Gel on esophageal mucosa injury in reflux esophagitis rats. Methods Rat models of reflux esophagitis were established by partial pylori ligation plus cardiomyotomy. Model control group,low-,middle-and high-dose Aorui Gel groups,positive control group,excipient control group and sham operation group were set up. Rats were treated with the corresponding drugs 7 days after operation. After treatment for 14 days,the rats'general state,and macroscopic features and pathological features of esophageal mucosa were compared. Results Macroscopic features and pathological features of esophageal mucosa in Aorui Gel groups differed from those in the model control group,inflammation of esophageal mucosa in Aorui Gel groups was alleviated,and pathological features were relieved and arrived to the normal state. Conclusion Aorui Gel can alleviate esophageal mucosa injury in reflux esophagitis rats,indicating that Aorui Gel has protective and repairing effect on esophageal mucosa injury.

9.Analysis of VMAT dosimetric verifications with different techniques

Objective To investigate the gamma (γ) passing rates for volumetric-modulated arc therapy (VMAT) dosimetric verification with different techniques.Methods A total of 12 VMAT plans for the treatment of different anatomical sites in cancer patients were chosen.The Octavius 4D system was used to measure the dose distributions in two different settings:the gantry was rotating (three-dimensional (3D) and 2D γ-analysis) and the gantry was fixed at 0°(2D γ-analysis).The γ passing rates were analyzed with 3%/3 mm and 2%/2 mm criteria, using the paired t test or Wilcoxon signed-rank test.The 2D γ passing rates for different irradiation methods were calculated.Results For the 3D and 2D dose distributions obtained at a rotating gantry angle as well as the 2D dose distribution obtained at zero gantry angle, the average γ passing rates were 96.03%, 96.98%, and 98.90% for 3%/3 mm (P=0.227, P=0.000, P=0.003);82.08%, 84.04%, and 90.90% for 2%/2 mm (P=0.379, P=0.000, P=0.000).For the 2D dose distributions obtained with different irradiation methods, the average γ passing rate was 98.99% for 3%/3 mm and 93.68% for 2%/2 mm.Conclusions The VMAT dosimetric verification based on a 3D volumetric dosimeter at a rotating gantry position can be clinically useful for delivery quality assurance (QA), and can achieve the most reliable dose calculation for VMAT, which has more referential values.
10.Application of detector array in treatment planning system modeling adjustment

Objective To investigate the feasibility of detector array in Monaco modeling for MLC parameters adjustment.Methods One parameter was fixed, and then the other parameter was changed.The γ pass rates of the test beams, namely 3ABUT, 7SegA, and FOUR L, were assessed to determine the values of leaf transmission and leaf offset.A total of 12 tumor cases from different anatomical sites were randomly selected.Two-dimensional dose verification (rack angle zero) of Step& Shot and dMLC plans as well as three-dimensional dose validation of VMAT plan were performed using Octavius 4D system.The γ pass rates were analyzed at a standard of 3%/3 mm.Meanwhile, the point dose verification for these three plans was analyzed to obtain the dose deviations.Results The values of leaf transmission and leaf offset were 0.0105 and-0.08 mm, respectively.The average γ pass rates (%) of Step& Shot, dMLC, and VMAT plans were 88.59±2.94, 87.81±3.28, and 87.45±2.24 before adjustment and 98.45±1.23, 98.9±1.01, and 96.03±1.66 after adjustment.In addition, the average dose deviations (%) according to the point dose verification were 0.85±0.75, 0.95±0.39, and 0.98±0.40 before adjustment and 0.97±0.57, 1.08±0.76, and 0.86±0.45 after adjustment.Conclusions Octavius detector 729 ionization chamber array is a feasible and reliable device in Monaco modeling for MLC parameters adjustment.
