1.The inhibitory effects of dexamethasone on cisplatin induced apoptosis of human lung adenocarcinoma cell SPC-A1 and its molecular mechanism.

The aim of this study is to investigate the apoptotic inhibition and its molecular mechanism of dexamethasone (DEX) acting on cisplatin (CDDP)-induced apoptosis of human lung adenocarcinoma cell SPC-A1; SPC-A1 cells were pre-cultured in vitro for 24 hours with DEX in different concentrations and then CDDP was added in different concentrations for culturing for further 48 hours. The survival rates of the cells were determined by MTT. The expression of serum/glucocorticoid-induced kinase (SGK-1) and mitogen-activated protein kinase phosphatase-1 (MKP-1) in SPC-A1 cells after being cultured by 1 micromol/l DEX at different time was detected by semi-quantitative RT-PCR technology. The expression of glucocorticoid receptor (GR) in SPC-A1 cells was measured by immunohistochemistry (IHC) with biotin-labeled anti-GR. The results of MTT showed that SPC-A1 cells had resistance to CDDP-induced apoptosis with pre-cultured DEX and the resistance intensity presented DEX concentration-dependent. The expressing quantity of SGK-1 in SPC-A1 cells stimulated by DEX could be elevated and increased with intention of time, but the express of MKP-1 was not detected. Up-regulated expression of GR in SPC-A1 cells stimulated by DEX was detected by IHC. The number of cells expressing GR in SPC-A1 cells was significantly higher than that in the control group. The results showed that DEX inhibited apoptosis of SPC-A1 cells induced by CDDP. The possible molecular mechanism is that elevated expression of GR induced by DEX up-regulates the expression of SGK-1 which locates at the downstream of anti-apoptosis pathway. The apoptosis resistance of SPC-A1 cells may account for all above the factors.

2.Analysis of the effect of empowerment education on self-management of patients with diabetes

Objective To investigate the effect of empowerment education on self-management in patients with diabetes mellitus. Methods A total of 128 patients with diabetes mellitus who met the criteria of WHO in 1999 were selected and divided into the control group(62 cases)and the observation group(66 cases). The control group accepted the traditional nursing methods, and the observation group accepted the empowerment education model. FBG, PBG, HbA1c and Diabetes Management Self-efficacy Scale (C-DMSES) were compared between two groups of patients after 3 months and 6 months. Results After the intervention with the empowerment education model, the patient′s FBG, PBG, HBA1c of the control group were (7.69±4.21) mmol/L, (11.78±3.21) mmol/L, (7.95±1.46)%three months after discharge, and (7.21 ± 3.19) mmol/L, (10.48 ± 4.65) mmol/L, (9.15 ± 2.36)% six months after discharge; while in the observation group, FBG, PBG, HBA1c were (6.23 ± 3.59) mmol/L,(9.32 ± 4.06) mmol/L, (6.69 ± 1.10)%three months after discharge, and (6.15 ± 1.89) mmol/L,(7.33 ± 2.10) mmol/L,(6.02 ± 2.17)%. The differences between two groups were significant (t=2.012-5.196, P<0.05). C-DMSES scores of the control group were 103.48 ± 18.41, 134.38 ± 13.01 on admission and six months after discharge, while in the observation they were 102.83 ± 10.91,157.64 ± 19.56. The differences between two groups were significant (t=3.151, P<0.05). Conclusions Empowerment education model can improve the ability of self-management of diabetes patients, improve self-efficacy to improve their own ability to control blood glucose.
3.Long-Term Outcome of Breast-Conserving Therapy Versus Mastectomy Therapy for Early Stage Invasive Breast Cancer:Result of Meta Analysis

Objective To compare the long-term outcome between breast-conserving therapy with mastectomy therapy for early stage invasive breast cancer through a Meta analysis of the randomized controlled trials published worldwide.Methods Cochrane systematic evaluation was used to search through Cochrane libraries of clinical comparative trials,PubMed,Embase,Cancer Lit,and so on.The quality of literatures was independently evaluated and cross-checked by two evaluators,indicators for assessment including death number at the end of follow-up,locoregional and total recurrence.The results were analyzed with RevMan 4.2.2 software.Results Six articles were involved in the Meta analysis with total 3 933 patients.No statistical difference was found in the death at the end of follow-up between breast-conserving therapy group and mastectomy therapy group(OR=1.05,95% CI=0.93-1.19,P=0.45).Locoregional and total recurrence rate of breast-conserving therapy group were statistical higher than those in mastectomy therapy group(OR=1.64,95% CI:1.10-2.44,P=0.01;OR=1.42,95% CI:1.22-1.64,P
4.Risk factors for surgical site infection in patients with thoracolumbar fracture

Objective To evaluate risk factors for surgical site infection(SSI) in patients with thoracolumbar fracture,and provide basis for making prevention and control measures.Methods Patients with thoracolumbar fractures operation in the department of spinal surgery of a hospital between January 2010 and March 2015 were monitored through information monitoring system,on-site visit,and telephone follow-up,incidence of SSI was investigated,risk factors were performed univariate analysis.Results A total of 326 patients undergoing thoracolumbar fracture operation were monitored,15(4.60%) had SSI.SSI were high in patients≥60 years old,underwent operation at nonlaminar air flow operation room,didn't receive antimicrobial prophylaxis 30 minutes prior to operation,preoperative hospital stay >3 days,operation duration >3 hours,as well as with diabetes mellitus and chronic respiratory disease (all P< 0.01).Conclusion Risk factors for SSI in patients undergoing thoracolumbar fractures operation were patients with older age,receive operation at non-laminar air flow operation room,didn't receive antimicrobial prophylaxis,long length of preoperative hospital stay and operation duration,as well as combined with diabetes mellitus and chronic respiratory disease,preventive measures should be taken to reduce the incidence of infection.
6.Influence of laser positioning system to position repeatability in the use of improved belly board

Objective To investigate influence of laser positioning system to position repeatability of patients treated with radiotherapy for pelvic tumors.Methods Forty patients with pelvic tumors were divided into two groups,using laser positioning system positioning (20 cases) and conventional positioning (20 cases),respectively,CBCT was used to verify and analyze the setup errors of two groups.Results The setup errors on X,Y and Z axes in laser positioning system positioning group were (2.63±1.07) mm,(2.38±1.28) mm and (2.46±1.21) mm,they were (3.04±1.22) mm,(2.55±1.35) mm and (3.09±1.37) mm in conventional positioning group.It has significance reduce on X and Z axes (P =0.040,0.003,respectively) by using laser positioning system,and has no significance on Y axis (P =0.432).Conclusion It can improve patient positioning repeatability effectively by using laser positioning system for patient positioning.
7.Comparison of Lewis lung cancer cells and their derived cells

Objective By comparing the biological characteristics among Lewis lung cancer cells ( LLC) , LLC or-thotopic transplantation derivative cells ( R1-LLC) and R1-LLC orthotopic transplantation derivative cells ( R2-LLC) , we e-valuate their invasion and metastatic abilities in orthotopic transplantation models.Methods In vitro, we applied CCK8, clonogenic assay, and Transwell invasion assay to detect the proliferation ability, invasion ability and morphologic structures of LLC,R1-LLC, R2-LLC cells respectively, meanwhile observing morphologic structures by transmission electron microsco-py.In vivo, we constructed LLC, R1-LLC and R2-LLC orthotopic transplantation models.Herein, we observed their tumor growth and metastasis.The tumor formation rate and tumor-forming time were recorded for statistic analysis.Results In vitro:LLC, R1-LLC and R2-LLC cells showed no significant difference in proliferation ability ( P>0.05 ) , but significant differ-ences in invasion ability:R2-LLC>R1-LLC>LLC(P<0.05).In vivo:In the orthotopic group, the tumor formation rates of LLC, R1-LLC and R2-LLC cells were 66.67%、80%、93.33%(P <0.05).Conclusions In orthotopic implantation mouse models, R2-LLC cells present higher invasion and metastatic ability than R2-LLC and LLC cells.
8.Validation of the revised Atlanta classification and the determinant-based classification for severity of acute pancreatitis

Objective To validate the accuracy of the revised Atlanta classification (RAC) and the determinant-based classification (DBC) for severity of acute pancreatitis.Methods We retrospectively analysed the clinical data from 192 patients with acute pancreatitis who were admitted to our hospital between January 2014 and October 2014.Using the RAC and the DBC,The duration of total parenteral nutrition,length of ICU and hospital stay,operation rate and mortality were assessed in the subgroups,respectively.Results Fluid collections in and around the pancreas was present in 74 patients (38.5%),pancreatic or peripancreatic necrosis in 51 (26.6%),and infection in 18 (9.4%).On the basis of the RAC,106 patients (55.2%),77 patients (40.1%),and 9 patients (4.7%) were classified to have mild,moderately severe,or severe AP,respectively.On the basis of the DBC,11l (57.8%),62 (32.3%),14 (7.3%),and 5 (2.6%) patients were classified to have mild,moderate,severe,or critical AP,respectively.The different categories of severity for each classification system were significantly associated with the duration of total parenteral nutrition,the length of ICU and hospital stay,the operation rate and mortality.Conclusion Both the RAC and DBC accurately classified the severity of AP and differentiated prognosis in the subgroups of patients.

Traditional Chinese medicine (TCM) is a great knowledge base that has been developed through thousands of years and has been accumulated a wealth of experience in diagnosis and treatment.TCM has advanced concepts,but lag behind the use of modern technology and means.With the progress of modern science and technology,the combination of TCM theory and moderm science and technology promotes the development of TCM engineering.TCM has been a breakthrough in diagnostic techniques and clinical treatment,especially modern TCM instruments.At present,diagnostic instruments of TCM mainly depend on pulse and tongue device,and gradually develop to four diagnostic comprehensive analysis platform.Therapeutic instruments of TCM include electroacupuncture device,meridian instrument,laser treatment device and magnetic therapy instrument.This article focuses on the current status,problems and trend on TCM diagnostic and therapeutic instruments,which mainly include diagnostic instruments,clinical treatment instruments,Chinese medical instruments.
10.Treament of fractures of distal humerus with open reduction and anatomical plate internal fixation

[Objective]To investigate the treatment of fractuees of distal end of humerus with open reduction and internal fixation with anatomical plate.[Method]From February 2001 to April 2006,open reduction and anatomical plate internal fixation were performed in 22 cases with distal end of humerus fracture,of which olecranon osteotomy approach was carried out in 11 while musculus triceps brachii linguiform flap approach in other 11.All the patients received combined therapy including fumigation and rinsing with Chinese herbs,CPM(continuous passive motion) and manipulation.[Result]All the cases were followed up for 13 to 22 months.All the fractures got union.The postoperative elbow functions were excellent in 9 cases,good in 7 cases according to Mayo standards.The satisfactory rate was 72.71%.[Conclusion]Open reduction and internal fixation with anatomical plate can provide stable fixation and allow early postoperative elbow functional training in treatment of distal humerua fracture.Good clinical results can be achieved using fumigation and rinsing with Chinese herbs,CPM and manipulation after operation.
