Nodular-cystic fat necrosis is an unusual, distinctive entity characterized by histologically en-capsulated fat necrosis in the subcutis. We report two cases of nodular-cystic fat necrosis. The lesions were subcutaneous nodules. Histopathologically, the nodules showed strong periodic acid-Schiff -positive necrotic adipose tissue with lipomembranous changes and calcification.

Nodular cystic fat necrosis is a distinct benign subcutaneous nodule characterized by encapsulated necrosis of subcutaneous fat in which non-viable adipose cells are pathologically surrounded by fibrous tissue. We report a case of nodular cystic fat necrosis in a 47-year-old woman who presented with a 2-month history of had multiple highly- movable, subcutaneous nodules on both shins and posterior thigh. There was an absence of any noteworthy trauma history. Each lesion showed a highly-migratory property, and could easily be moved about 10 cm in any direction. Plain film revealed multiple radiopaque nodules on the right shin. Calcification and lipomembranous changes, in addition to typical features, were also found on histopathologic examination.

Membranous fat necrosis is a variant of fat necrosis characterized by the presence of membranocystic foci which is lined by eosinophilic, homogeneous and crenulated membrane that has pseudopapillary projections. Membranous fat necrosis may be idiopathic or has been associated with many local and systemic diseases. The pathogenesis of membranous fat necrosis is uncertain but trauma may be suspected in our case. We describe a case of lipoma with membranous fat necrosis.

Encapsulated fat necrosis is a distinct, benign subcutaneous lesion characterized by histologically non-viable adipose cells surrounded by thin fibrous capsules. We report a case of encapsulated fat necrosis in a 30-year-old woman who had one mobile, subcutaneous nodule on her left shin for 3 years without previous history of trauma. Histopathologically the nodule showed strong PAS positive necrotic adipose tissues encapsulated by fibrous tissue.

Suction-assisted lipectomy has become an increasingly popular procedure, being one of the most frequently performed aesthetic procedures in Korea. Serious complications of suction-assisted lipectomy are uncommon but aesthetic problems are often troublesome. Here we report a patient with multiple abdominal masses composed of fat necrosis with fibrosis after suction- assisted lipectomy that was treated with excision. We also discussed and deduced the cause of its complication because plastic surgeons are liable to overlook this rare complication.

We report a case of subcutaneous fat necrosis associated with pancreatitis that may be the first case in Korean hterature as for we know. The patient was a 41-year-old man wbo presented erythema nodesum-like erythematous subcufaneous nodules on both lower extremities. Histopathologic findings showed subcutaneoms focal fat necrosis and ghoat-like cells. Granular basophilic material was deposited in and around the necrotic fat cells and stained positively with von Kossa stain. The lesioas subsided gradwdly without any specific treatment, as the underlying pancreatitis was ameliorated.

Nodular cystic fat necrosis, first described by Przyjemski and Schuster, is a peculiar form of encapsulated necrosis of subcutaneous fat characterized by totally or near-totally encapsulated necrosis of fatty tissue in which clusters of nonviable adipocytes are surrounded by condensed fibrous tissue. We report two cases of nodular cystic fat necrosis associated with history of trauma about the site of the lesion. Each lesion was a subcutaneous movable nodule on buttock (case 1) and shin (case 2) which has evolved over months. Both cases showed possible relation to multiple intramuscular injection or direct trauma injury. Pathologically, encapsulated nodule showed a characteristic feature of nodular cystic fat necrosis which composed of the ghosts of anucleated adipocytes showing fairly well-preserved outline.

Lipomembranous fat necrosis is a special form of fat necrosis characterized by the presence of cystic cavities lined by crenelated hyaline membranes. Lipomembranous fat necrosis has been observed in the subcutaneous tissue associated with various conditions, mainly with ischemia. The term, nodular-cystic fat necrosis was coined to describe a characteristic form of encapsulated subcutaneous fat necrosis resulting form encapsulated lipoma associated with trauma or ischemia. We herein report a case of lipomembranous fat necrosis associated with vascular insufficiency and possibly associated with nodular-cystic fat necrosis, and discuss the common pathogenesis of the two disorders.

A 32-year-old woman had a brownish hard plaque with localized hypertrichosis. She had fallen down some stairs 1 year previously, and the injury had left a scar on her right shin. Even though pain and the scar had gone, the brown patch had remained. During that time hair had been grown on the site. Histologic examination showed lobular panniculitis, fat necrosis, fibrotic change and lipomembranous change. We diagnosed the patient as a traumatic panniculitis with localized hypertrichosis, based on the trauma history, skin lesion and histologic findings. We report a case of traumatic panniculitis with localized hypertrichosis.
