2.The Surveillance and Research of the Cerebrovascular Disease in Dali Inhabited by Bai Nationality in Yunnan Province

We have been trying to find out the way to prevent and cure the cerebrovascular diseases in ethnic minorities. By surveying the cerebrovascular diseases and finding out the risk factors causing the stroke, strengthening knowledge popularization and education among the people and curing the hypertension patients, we check whether the incidence and mortality of the cerebovascuar diseases show a tendency of decrease. The study is carried out by means of ran- dora samping. Our study is based on the 6-year monitoring of the 11 359 individuas living in Zhoucheng Town and Renliyi Village of Dali City. In our study the cerebrovascular diseases are calculated by the complete stroke. The 6-year data from 1984 to 1989 indicate that the in- cidence rate of the complete stroke decreased by an annual 4.8% and the mortality rate of the complete stroke decreased by 4% each year, which suggests that the study can provide impor- tant information for preventing and curing the cerebrovascular diseases.
3.Duration and effect of home-made ganglioside on improving clinical symptoms of patients with Parkinson disease: Multi-regional multi-central retrospective analysis

BACKGROUND:Patients with Parkinson disease (PD) are always supplied exogenous dopamine so as to balance a disproportion of neurotransmitter.However,a long-term administration may easily cause drug resistance;therefore,it is not able to solve nerve injuries at all.A lot of researches suggest that ganglioside can promote neural axon growth,improve neurotrophy,and resist cellular toxicity mediated by glutamic acid (GA).OBJECTIVE:To probe into the duration and effect of oraL administration of ganglioside on improving clinical symptoms of PD patients.DESIGN:Retrospective case analysis.SETTING:Department of Internal Medicine.Shanghai Electric Power Hospital;Departments of Neurosurgery and Internal Medicine,the First Affiliated Hospital of Kunming Medical College.PARTICIPANTS:A total of 6 344 PD patients were selected from Heilongjiang,Hebei,Shandong,Henan, Beijing,Jiangsu,Gansu,Qinghai.Guizhou,Fujian,Hunan,Shanxi, Hubei,Sichuan,Chongqing,Jiangxi, Shanghai and Yunnan from January 2001 to January 2005.All patients met the diagnostic and typing criteria of PD in Practical Neurology.They orally took≥10 mg/d ganglioside for more than 1 month,and all of them were provided the consent.METHODS:Patients who were treated with oral ganglioside accepted neural functional rehabilitation. Every oral solution of ganglioside contained 0.25 g/L ganglioside [including monosialoteterahexosyl ganglioside 1 (21%),diplo-sialoteterahexosyl ganglioside 1a(40%),diplo-sialoteterahexosyl ganglioside 1 b(16%),and tri-sialoteterahexosyl ganglioside 1b(19%)],and patients fastingly took two ampoules in every morning and 2 in every evening before sleeping, or took 4 ampoules in every evening before sleeping. Every 3 months was regarded as a course, and patients were always treated for two or three courses;especially,patients having a long-term duration were treated for above 4 courses.Regular follow-up was performed during the administration to observe the improvements of PD patients on the aspects of chatter, myotone, bradypraxia,posture irregularity, etc. Effective evaluation: Iilness improvements of PD patients who orally took ganglioside were observed within 3,6,9 months and 1 year.Excellent effective: Chatter and myotone disappeared after gradually stopping administration of dopamine preparation;improvement: Symptoms mentioned above were relieved after gradually reducing administration of dopamine preparation (effect=apparent effect+improvement);inefficacy:Symptoms mentioned above were not changed.MAIN OUTCOME MEASURES:Effect at various time Points of administration.RESULTS:Among 5 937 patients who met the inclusion criteria,all of them were involved in the final analysis without any loss.① Improvement of clinical symptoms at various time points of administration:There were 669 PD patients with the effective improvements within 1-3 months.941 within 4-6 months and 1713 within 7-12 months.The total effective rate was 56%.② Therapeutic effective rate of PD patients at various administrating times:The therapeutic effective rates of patients who received administration from 1 to 3 months,from 4 to 6 months and from 7 to 12 months were 30%,57%and 84%respectively.The rate was higher of patients administrated from 4 to 6 months than.of those from 1 co 3 months(P<0.05),and also higher of those from 7 to 12 months than of those from 4 tp 6 months and from 1 to 3 months (P<0.05,P<0.01).CONCLUSION:Ganglioside can effectively improve clinical symptoms,such as chatter,myotone,bradypraxia,posture irregularity,etc.In addition,it has a better effect during a long-term administration.
4.Clinical observation of collagen sponge for reducing volume of drainage after surgery for lumbar spinal stenosis

ObjectiveTo observe the effectiveness of collagen sponge for reducing volume of drainage after surgery for lumbar spinal stenosis. MethodsOne hundred and eighty-six patients who suffered from lumbar spinal stenosis were divided into two groups by random digits table method. The test group(96 cases) used collagen sponge to cover dura mater before placing drainage tube,the control group (90 cases) was treated without collagen sponge. The volume of drainage at 1,12,24 h after surgery were observed, the blood routine test was carried out at before and 48 h after surgery and the volume and ratio of blood transfusion after surgery was also measured and compared between the two groups. ResultsThe volume of drainage were significantly decreased in the test group compared with the control group at 1,12,24h after surgery [( 106.11 ± 20.02 ) ml vs. ( 127.02 ± 25.09) ml, (236.12 ± 34.06) ml vs. (327.31 ± 51.21 )ml, (355.16 ± 49.03 ) ml vs.( 506.36 ± 85.29 ) ml](P < 0.05 ). The volume and the ratio of blood transfusion in the test group were ( 176.27 ± 21.37) ml and 10.42%(10/96) ,which were greatly lower than those in the control group[(445.94 ±24.56) ml and 32.22% (29/90)](P <0.05). The number of RBC and the concentration of Hb were (2.96 ± 0.45 ) × 1012/L and ( 106.75 ± 7.30) g/L, differently in the test group at 48 h after surgery,which were increased significantly compared with the control group[(2.35 + 0.57) × 1012/Land (90.45 ± 5.10) g/L](P < 0.05 ). ConclusionsCollagen sponge provides rapid ,effective and durable hemostasis and decreases the leak of cerebrospinal fluid after surgery for lumbar spinal stenosis. It can be used as an effective and economic method to reduce the volume of drainage after surgery.
5.Effects of structured group psychotherapy on symptoms and social function of patients with mild to moderate depression

Objective:To investigate the effects of structured group psychotherapy on symptoms and social function of patients with mild to moderate depression.Methods:Sixty patients with mild to moderate depression who received treatment in Department of Psychiatry of the Third People's Hospital of Yongkang from June 2019 to May 2020 were included in this study. They were randomly assigned to receive either conventional antidepressants ( n = 30, control group) or conventional antidepressants combined with structured group psychotherapy ( n = 30, observation group). The Self-rating Anxiety Scale, Self-rating Depression Scale Hamilton Anxiety Scale, Hamilton Depression Scale and the Simplified Coping Style Questionnaire were used to compare the anxiety state, depression state and coping style between the two groups. Results:Self-rating Anxiety Scale, Self-rating Depression Scale, Hamilton Anxiety Scale, Hamilton Depression Scale and Simplified Coping Style Questionnaire scores in the observation group were (44.21 ± 4.15) points, (45.28 ± 5.16) points, (12.41 ± 2.16) points, (9.75 ± 2.83) points, and (10.35 ± 2.23) points, respectively, which were significantly lower than those in the control group [(51.14 ± 4.39) points, (53.64 ± 5.31) points, (16.73 ± 2.54) points, (14.18 ± 2.72) points, (14.73 ± 2.54) points, t = 6.283, 6.184, 7.097, 6.182, 7.375, all P < 0.001]. Simplified Coping Style Questionnaire score in the observation group was significantly higher than that in the control group [(31.42 ± 4.43) points vs. (24.16 ± 4.27) points, t = 7.432, P < 0.001]. Conclusion:Structured group psychotherapy combined with conventional antidepressant therapy for treatment of mild to moderate depression can greatly reduce the symptoms of anxiety and depression and improve the coping style.
