1.The role of intestinal endotoxemia in liver failure and its complications

Recent studies have found that endotoxin not only activates immune response,exacerbates liver injury,and induces liver failure, but also plays an important role in the development of hepatic encephalopathy,hepatorenal syndrome,spontaneous bacterial peritonitis,and other fatal complications.The recent progress in research on the pathogenesis of intestinal endotoxemia (IETM)and its impact on the devel-opment and progression of liver failure is reviewed and summarized.It is suggested that seeking effective methods to prevent and control IETM holds promise as the key to treating severe liver disease and liver failure successfully.
2.Glycolysis abnormality in neoplasm cells and the targeted therapy

Cancer cells display a decreased mitochondrial respiration along with an abnormal activation of glycolysis to satisfy the demand of the malignant growth even in the presence of oxygen.This phenomenon may be caused by mitochondrial dysfunction,hypoxia of microenvironment and the abnormal activation of upstream regulators of glycolysis pathway.Treatment targeted at the glycolysis pathway and its upstream regulators may provide a new therapeutic strategy of cancer.
3.MSCT findings of struma ovarii

Objective To evaluate computed tomographic findings of struma ovarii so as to improve the knowledge of the imaging diagnostics of this disease.Methods Computed tomography (CT)scans and clinical data of 12 pathologically proven struma ovarii were reviewed retrospectively.Scans were evaluated for the laterality,size,margins,configuration,internal architecture,presence of intracystic high attenuation lesions on pre-contrast scans,the presence of calcifications,solid components enhancement,cyst wall enhancement,and ascites.Results All tumors were unilateral and had smooth margins.Cystic (n = 3)or mainly cystic (n = 9) appearance was common to all the tumors.Eight tumors showed a high attenuation lesion in the cyst portion of the mass on pre-con-trast scans.Calcification was found in 5 cases.The solid components showed different degrees of enhancement after administration of contrast medium.The cyst wall showed mild to moderate enhancement after administration of contrast medium.Five cases of tumors accompany a small amount of ascites.On pathology,the solid portions of the tumor consisted of the thyroid tissue and stoma which contained abundant blood vessels and fibrous tissue,and the cystic portions were filled with high gelatinous fluid of eosinophil-ic colloid.Conclusion On CT scans,struma ovarii appeared most often as a smooth marginated multicystic mass with a high attenu-ation lesion and calcification on pre-contrast scans with marked enhancement of the solid components.
4.Effect of long non-coding RNA MEG3 on invasion and migration of color-ectal cancer cells

AIM:To investigate the expression of long non-coding RNA maternally expressed gene 3 (MEG3) in colorectal cancer ( CRC) cells, and to observe the effect of MEG 3 on the invasion and migration of CRC cells .METH-ODS:The levels of MEG3 in human normal colon cell NCM 460 and CRC cells SW48 and LoVo were detected by real-time PCR.MEG3 was over-expressed by plasmid transfection , and the effects of MEG 3 on the invasion and migration of SW 48 and LoVo cells were analyzed by Transwell assay and wound healing assay .The expression of matrix metalloproteinase ( MMP) family proteins was determined by Western blotting .RESULTS:The level of MEG3 was down-regulated in CRC cells compared with normal colon cell NCM 460.The invasion and migration of CRC cells were reduced after MEG 3 over-ex-pression.Transwell invasion and migration assays showed that the numbers of transmembrane SW 48 and LoVo cells were smaller in MEG3 over-expression group than control group (P<0.05).The cell spaces were broader after MEG3 over-ex-pression in the wound healing assay , indicating that MEG3 over-expression inhibited the mobility of CRC cells .Meanwhile, over-expression of MEG3 reduced the expression of MMP-2 and MMP-9, and elevated the expression of tissue inhibitor of metalloproteinase-2 (TIMP-2).CONCLUSION:The expression of MEG3 is down-regulated in CRC cells.Over-expres-sion of MEG3 inhibits the invasion and migration of CRC cells .TIMP-2, MMP-2 and MMP-9 might play an important role in this regulation .
5.Subtotal gastrectomy and gastroduodenal end-to-side anastomosis for treating duodenal bulb macroulcer

To find an ideal surgical therapy for duodenal bulb macroulcer which can replace the conventional Billroth Ⅱ procedure and reduce the rate of long term complication.Methods:Forty nine patients with duodental bulb macroulcer were operated by subtotal gastrectomy and gastroduodenal end to side anastomosis.Results:One patient bleeded on the seventh operative day,because of the blood vessel's ligature sheded at the submucous of the lesser curvature of stomach,the others recoveried satisfactoryly.Bilirubin assay in the gastric juice,was negative and pH scale rose from 2.7?0 5 to 5.2?0 4. Barium meal examination showed that the"Reproduced Pyloruses"had satisfactory function of evacuation control.Conclusion: This procedure accords with normal physiolagical channel.It has satisfactory function of evacuation control and anti reflux.There is no afferent influent loop syndrome or posterior anastomotic stoma space.It is easily operated,safy and reliably.
6.Pregnancy outcomes of 131 twin pregnancies complicated with severe pre-eclampsia

Objective To explore the clinical characteristics and pregnancy outcomes of twin pregnancies complicated with severe preeclampsia.Methods The pregnant outcomes of 131 twin pregnancies (twin group) and 572 singleton pregnancies (singleton group),all complicated with severe preeclampsia,were analyzed retrospectively.All patients were treated in the Department of Obstetrics and Gynecology,West China Second University Hospital,Sichuan University from June 2007 to June 2011.The patients' age,onset of disease,gestational weeks at delivery,mean duration of expectant treatment,blood pressure,laboratory parameters and incidence of pregnancy complications,including placental abruption,heart failure,pulmonary edema,postpartum hemorrhage,uteroplacental apoplexy,eclampsia,HELLP syndrome (hemolysis,elevated liver enzymes and low platelets syndrome),hypoproteinemia,retinopathy,intracranial hemorrhage and renal insufficiency,were compared between the two groups.Perinatal outcomes such as premature delivery,perinatal mortality,neonatal intensive care unit (NICU) hospitalization,fetal distress,hypoxic-ischemic encephalopathy (HIE),asphyxia neonatorum,neonatal pneumonia,hyperbilirubinemia and neonatal hypoglycemia of two groups were also compared.Chi-square test,Fisher's exact test or t-test were used for statistical analysis.Results The severe preeclampsia incidence of twin pregnancies (5.03%,131/2604) was higher than that (1.94%,572/29 452) of singleton pregnancy (x2=106.40,P<0.001).The onset time [(33.6±1.8) weeks] and gestations at delivery [(34.6±2.9) weeks] in twin group were earlier than those in singleton group [(34.4± 2.0) weeks,t=2.364,P<0.05; (35.6±3.2) weeks,t=3.902,P<0.05].The duration of expectant treatment of twin group [(6.4±0.3) d] were shorter than that of singleton group [(7.4± 0.5) d,t=5.314,P<0.01].The incidence of placental abruption [9.9% (13/131) vs 4.2% (24/572),x2=7.013,P<0.01],heart failure [11.5% (15/131) vs 3.2% (18/572),x2=16.430,P<0.01],pulmonary edema [4.6% (6/131) vs 0.9% (5/572),x2=9.505,P<0.01],postpartum hemorrhage [16.0% (21/131) vs 7.0% (40/572),x2 =10.990,P < 0.01] and uteroplacental apoplexy [5.3% (7/131) vs 0.5% (3/572),x2 =17.650,P<0.01] of twin group were higher than those of singleton group,respectively.The incidence of premature delivery [77.1% (202/262) vs 29.9% (171/572),x2=162.000,P<0.05],NICU hospitalization [76.2% (205/262) vs 58.4%(332/572),x2 =31.980,P<0.05] and HIE [8.4% (22/262) vs 4.7% (27/572),x2 =4.392,P<0.05] of twin group were higher than those of singleton group,respectively.Conclusions Twin pregnancy women are more likely to be complicated with severe preeclampsia and more vulnerable to suffer from severe complications,resulting in poor perinatal outcomes.More attentions should be paid on the management of twin pregnancy.Once hypertensive disorders complicating pregnancy is diagnosed,active management should be provided and timing of termination should be considered.

9.SACC-83-derived exosomes induce fibroblast activation protein expression in normal salivary gland stromal fibroblasts

Objective:To study the effects of exosomes (EXO) released by adenoid cystic carcinoma SACC-83 cells on the expression of fibroblast activation protein (FAP) in normal human salivary gland stromal fibroblasts (hSGSFs).Methods:ACC exosomes were isolated from SACC-83 cell culture supernatant by using Total Exosome Isolation Reagent.The whole-mount EXO were characterized and assessed by transmission electron microscope and Western Blot.The exosomes were labeled with green fluorescent dye PKH67 and co-cultured with hSGSFs for 48h,followed by staining with Alexa Fluor 594 Phalloidin and DAPI.Mterwards,exsosomes uptake was observed under a laser scanning confocal microscope.After a 48-hour co-culture of SACC-83 exosomes with hSGSFs,the expression of FAP in SACC-83-EXO-treated hSGSFs was investigated by qRT-PCR and Western Blot.Results:The vesicles isolated from SACC-83 cell culture supernatant had the reported size range of 30-100 nm,expressed the exosomal marker CD63 and TSG101.Mter co-culture of hSGSFs with PKH67 labeled SACC-83 exosomes,exosomes were taken up by hSGSFs and FAP expression was elevated in hSGSFs.Conclusion:Exosomes derived from SACC-83 cells can be taken up by hSGSFs and can induce the expression of FAP in hSGSFs.These results suggest that exosomes derived from SACC-83 cells might induce the transformation of normal salivary gland strormal fibroblasts to cancer associated fibroblasts.
10.Applying contact-mode argon plasma coagulation for retroflex colonoscopic treatment of ultra-lower rectal polyps

Objective To explore the clinical values of applying contact-mode argon plasma coagulation (APC) for retroflex colonoscopic treatment of ultra-lower rectal polyps. Methods 46 wide outsole and applanate polyps smaller than 1.0 cm located at ultra-lower rectum in 17 cases of patients were treated by contact-mode APC under U-type retroflex colonoscopy after failure in regular colonoscopic treatment. Then observe the contacting rate of APC probe with polyps, success rate of curing polyps, rate of probe being adhered and blocked by the solidification structures, incidence of submucosal emphysemas, and incidence of colonoscope ambustion. Results Under U-type retroflex colonoscopy, the probe could contact with polyps in 17 patients. The polyps in every patient were cured by APC in the first time of colonoscopic treatment, whereas all 46 polyps were cured by 97 times of APC spurt. Mild adhesions occurred between the probe and solidification structures at 5 times (5.15 %) among 97 times of APC spurt, without injuries to the coagulation surface from which when the probe separated. The solidification structures blocked the probe only twice (2.06 %). When the solidification structures were cleared, efficiency of the probe restored. No sub-mucosal emphysemas and colonoscope ambustion happened. Conclusion Applying contact-mode APC for retroflex colonoscopic treatment of ultra-lower rectal polyps is safe and effective, it can prevent the damage of colonoscope from the argon knife.
