1.The role of intestinal endotoxemia in liver failure and its complications

Recent studies have found that endotoxin not only activates immune response,exacerbates liver injury,and induces liver failure, but also plays an important role in the development of hepatic encephalopathy,hepatorenal syndrome,spontaneous bacterial peritonitis,and other fatal complications.The recent progress in research on the pathogenesis of intestinal endotoxemia (IETM)and its impact on the devel-opment and progression of liver failure is reviewed and summarized.It is suggested that seeking effective methods to prevent and control IETM holds promise as the key to treating severe liver disease and liver failure successfully.
2.Effect of long non-coding RNA MEG3 on invasion and migration of color-ectal cancer cells

AIM:To investigate the expression of long non-coding RNA maternally expressed gene 3 (MEG3) in colorectal cancer ( CRC) cells, and to observe the effect of MEG 3 on the invasion and migration of CRC cells .METH-ODS:The levels of MEG3 in human normal colon cell NCM 460 and CRC cells SW48 and LoVo were detected by real-time PCR.MEG3 was over-expressed by plasmid transfection , and the effects of MEG 3 on the invasion and migration of SW 48 and LoVo cells were analyzed by Transwell assay and wound healing assay .The expression of matrix metalloproteinase ( MMP) family proteins was determined by Western blotting .RESULTS:The level of MEG3 was down-regulated in CRC cells compared with normal colon cell NCM 460.The invasion and migration of CRC cells were reduced after MEG 3 over-ex-pression.Transwell invasion and migration assays showed that the numbers of transmembrane SW 48 and LoVo cells were smaller in MEG3 over-expression group than control group (P<0.05).The cell spaces were broader after MEG3 over-ex-pression in the wound healing assay , indicating that MEG3 over-expression inhibited the mobility of CRC cells .Meanwhile, over-expression of MEG3 reduced the expression of MMP-2 and MMP-9, and elevated the expression of tissue inhibitor of metalloproteinase-2 (TIMP-2).CONCLUSION:The expression of MEG3 is down-regulated in CRC cells.Over-expres-sion of MEG3 inhibits the invasion and migration of CRC cells .TIMP-2, MMP-2 and MMP-9 might play an important role in this regulation .
3.MSCT findings of struma ovarii

Objective To evaluate computed tomographic findings of struma ovarii so as to improve the knowledge of the imaging diagnostics of this disease.Methods Computed tomography (CT)scans and clinical data of 12 pathologically proven struma ovarii were reviewed retrospectively.Scans were evaluated for the laterality,size,margins,configuration,internal architecture,presence of intracystic high attenuation lesions on pre-contrast scans,the presence of calcifications,solid components enhancement,cyst wall enhancement,and ascites.Results All tumors were unilateral and had smooth margins.Cystic (n = 3)or mainly cystic (n = 9) appearance was common to all the tumors.Eight tumors showed a high attenuation lesion in the cyst portion of the mass on pre-con-trast scans.Calcification was found in 5 cases.The solid components showed different degrees of enhancement after administration of contrast medium.The cyst wall showed mild to moderate enhancement after administration of contrast medium.Five cases of tumors accompany a small amount of ascites.On pathology,the solid portions of the tumor consisted of the thyroid tissue and stoma which contained abundant blood vessels and fibrous tissue,and the cystic portions were filled with high gelatinous fluid of eosinophil-ic colloid.Conclusion On CT scans,struma ovarii appeared most often as a smooth marginated multicystic mass with a high attenu-ation lesion and calcification on pre-contrast scans with marked enhancement of the solid components.
4.Glycolysis abnormality in neoplasm cells and the targeted therapy

Cancer cells display a decreased mitochondrial respiration along with an abnormal activation of glycolysis to satisfy the demand of the malignant growth even in the presence of oxygen.This phenomenon may be caused by mitochondrial dysfunction,hypoxia of microenvironment and the abnormal activation of upstream regulators of glycolysis pathway.Treatment targeted at the glycolysis pathway and its upstream regulators may provide a new therapeutic strategy of cancer.
5.Subtotal gastrectomy and gastroduodenal end-to-side anastomosis for treating duodenal bulb macroulcer

To find an ideal surgical therapy for duodenal bulb macroulcer which can replace the conventional Billroth Ⅱ procedure and reduce the rate of long term complication.Methods:Forty nine patients with duodental bulb macroulcer were operated by subtotal gastrectomy and gastroduodenal end to side anastomosis.Results:One patient bleeded on the seventh operative day,because of the blood vessel's ligature sheded at the submucous of the lesser curvature of stomach,the others recoveried satisfactoryly.Bilirubin assay in the gastric juice,was negative and pH scale rose from 2.7?0 5 to 5.2?0 4. Barium meal examination showed that the"Reproduced Pyloruses"had satisfactory function of evacuation control.Conclusion: This procedure accords with normal physiolagical channel.It has satisfactory function of evacuation control and anti reflux.There is no afferent influent loop syndrome or posterior anastomotic stoma space.It is easily operated,safy and reliably.
6.Clinical research on prophylactic pelviureteric instillation chemotherapy after endoscopic management of upper tract urothelial carcinoma

Objective To investigate the effect of endoscopic management of upper tract urothelial carcinoma (UTUC),and evaluate the indication and clinical value of postoperative prophylactic pelviureteric instillation chemotherapy.Methods Patients who met the inclusion criteria included elderly patients,patients who could not tolerate radical surgery,patients with renal insufficiency or needed dialysis after removal of the kidney,patients with bilateral UTUC,patients with tumor≥3 cm in diameter but could be completely resectted under ureteroscopy who required to preserve renal function.Surgical procedure was successful.The ureteral stents were placed,and prophylactic ureteral infusion chemotherapy via the retrograde transvesical ureteric catheterisation was conducted 1-2 times one week at 2 days after surgery.40 milligram of pirarubicin hydrochloride or epirubicin hydrochloride was dissolved in 40 milliliter sterile water for the prophylactic infusion chemotherapy.These instillations were completed within 40 minutes.Results Totally,9 patients (3 males and 6 females) aged 60-86 years,mean age of (69.7±6.4) years,were enrolled in this study.The carcinoma of the renal pelvis was found in 2 cases and ureteral tumors in 7 cases.In pathological results,ureteral polyps was found in 1 case,papillary ureteral neoplasm with low malignant potential in 2 cases,urinary tract epithelial carcinoma in 4 cases (1 case with poorlydifferentiation,1 case with well-differentiation,2 cases with urinary tract epithelial cancer),and 2 cases were lack of pathological reports because of too few pathologic specimens.4 patients received adjuvant chemotherapy by instillation successfully,and lumbago,fever or other complications were not found.Patients were followed up for a mean period of 15 moths (4-31 months).Only 1 patient had recurrence of bilateral ureteral tumor 15 months after surgery,and no tumor recurrence was found in other patients.Conclusions Endoscopic management is a safe and effective in treating UTUC,which can preserve renal function by avoiding nephrorectomy in some patients.The instillation chemotherapy after endoscopic management is safe,which has a satisfactory effect,but further validation in a large clinical sample is needed.Patients with tumor resection under ureteroscopy who cannot obtain the pathology results need to be closely followed up.
7.Effect of Probucol in combination with Enalapril-folic acid tablets on endothelial function in elderly patients with H-type hypertension

Objective To investigate the effect of Probucol combined with Enalapril-folic acid tablets on endothelial function in elderly patients with H-type hypertension.Methods A total of 180 elderly patients with H type hypertension were randomly divided into three groups (n =60,each):Group A (conventional treatment),Group B (conventional treatment + a tablet containing 10 mg enalapril and 0.8 mg Folic acid/day) and Group C (conventional treatment + 500 mg Protocol twice daily + a tablet containing 10 mg Enalapril and 0.8 mg folic acid/day).Plasma hemocyanin (Hcy) and asymmetric dimethylarginine (ADMA) levels and serum nitric oxide (NO) and endothelial NO synthase (eNOS) levels were measured before treatment,and 1 and 6 months after treatment.Results In Group A,plasma Hcy and ADMA levels and serum NO and eNOS levels had no significant differences before treatment versus 1 and 6 months after treatment (P>0.05).Plasma Hcy and ADMA levels decreased and serum NO and eNOS levels increased in Group B 6 months after treatment,compared with those at pretreatment or 1 month after treatment,which were either lower or higher than those in Group A 6 months after treatment (all P<0.05).Plasma Hcy and ADMA levels decreased and serum NO and eNOS levels increased in Group C 6 months after treatment,compared with those at pretreatment or 1 month after treatment,which were either lower or higher than those in Groups A and B 6 months after treatment (all P<0.05).After adjustment for other risk factors,partial correlation analysis showed that plasma ADMA was positively correlated with plasma Hcy,while plasma ADMA and Hcy were negatively correlated with serum eNOS and NO (all P<0.05).Conclusions Probucol in combination with Enalapril-folic acid tablets may be beneficial in improving endothelial function and mitigating the development of atherosclerosis.

9.Study on Extraction Process of Ferulic Acid from Angelicae Sinensis Radix with Technology of Three Can Group Dynamic Countercurrent

Objective To optimize technology of three can group dynamic countercurrent extraction process of ferulic acid from Angelicae Sinensis Radix.Methods The content of ferulic acid was determined by HPLC. With content of ferulic acid as index, comprehensive test was used to investigate effect of extraction solvent and extraction time on extraction efficiency.Results The optimum process parameters were as follows:extraction solvent with 10 times of water;20 minutes for each extraction time.Conclusion The process which uses method of three can group of dynamic countercurrent extraction of ferulic acid from Angelicae Sinensis Radix is reliable, highly efficient and energy saving.
10.Mechanism of HAG priming protocols inducing apoptosis of leukemia cell lines in vitro

Objective To investigate the effect and the mechanism of different G-CSF-priming protocols on leukemia cell lines (HL-60 and U937) in vitro and provide the clinical guidance to clinical treatment of acute leukemia.Methods The leukemia cell lines HL-60 and U937 were used as model to detect the effects of three drugs alone and combined two drugs (HA) or three drugs (HAG) respectively.Cell viability and cell growth inhibition were performed by cell count kit-8 (CCK-8) assay.Apoptotic marker AnnexinV/PI,cell membrane surface antigen CD11b,cell cycle,mitochondrial membrane potential (JC-1) and Caspase-3 were determined by flow cytometry.Results After using of HAG for 48 h,HL-60 and U937 cells counts were decreased significantly and the apoptotic marker Annexin V was significantly increased. To compare the single drug group with two drug combination group,the result was significantly different (P <0.05),and the apoptosis of U937 cells was higher than HL-60 cell line.CD11b expression among the three groups did not change (P > 0.05).Using of CAG and MAG,the mitochondrial nembrane potential of HL-60 and U937cells was increased,the three-drug combination group was significantly higher than single-drug group and control group (P <0.05); Caspase-3 was activated,the fluorescence intensities of Caspase-3 of the three-drug combination group and single drug group were significantly higher (P <0.05) comparing with the control group.Conclusion HAG regimen could induce leukemia cells to apoptosis through the reduction of mitochondrial membrane potential and the activation of Caspase-3 to induce apoptosis of leukemia cells.
