1.Radiologic analysis and bone mineral density measurement of patients with lumbar degenerative scoliosis

Objective To investigate the radiographic characteristics of degenerative lumbar scoliosis (DLS) and its relationship to osteoporosis. Methods 229 cases (83 males, 146 females) of DLS from January 1998 to June 2005 were reviewed. The mean age was 56.8 years (ranged from 40 to 74 years). The Cobb angle and vertebral stability in coronal plane were measured in anteroposterior radiographs and the changes of lordosis in sagittal plane were observed in lateral radiographs. The bone mineral density(BMD) and T-Score of lumbar spine (L2-L4) were measured using Dual Energy X-ray Absorptiometry. Results The mean Cobb angle is 9.45??4.79?, 151 cases(66%) with the angle less than 10?, 60 cases (26%) with 10?-20?, and 18 cases (8%) with more than 20?. Right side scoliosis were found in 52% (120 cases), left side in 48% (109 cases). 159 cases (69%) companied with gradeⅠ(Nash-Moe) vertebra rotation, 54 cases (24%) with grade Ⅱ. And the vertebra rotation was most evident on scoliosis apex. There were 20 cases (9%) with more than 4 mm lateral translation between the lumbar vertebrae which were usually the apex vertebrae with the most degenerative changes. The physical lordosis decreased in most cases in which 16 cases (7%) developed lumbar or thoracolumbar kyphosis. And the kyphosis degree was not relevant to scoliosis Cobb angle. The mean T-Score of BMD measurement was -1.88?0.17, which was -1.49?0.14, -2.56?0.24, -2.89?0.50 for the groups of with Cobb angle 20? respectively. BACKGROUND: The pain and the unstability of the knee joint are the main clinical manifestation of knee osteoarthritis.OBJECTIVE: To verify the efficacy of ultrasonic drug penetration combiued with quadriceps exercise in treating senile knee osteoarthritis.DESIGN: Randomized controlled study.SETTING: Rehabilitation Department of the Affiliated Hospital of Luzhou Medical College.PARTICIPANTS: Seventy-six in-patient and out-patient cases of the knee osteoarthritis, 28 males and 48 females and total 109 knees, were chosen in the Orthopaedic Department and Rehabilitation Department in the Affiliated Hospital of Luzhou Medical College in theperiod between August,2001 and August, 2004. The cases were randomly divided into the experimental group and the control group with 38 cases each.therapy apparatus made in Mianyang was adopted and the diameter of the handpiece was 3 cm. 10 g/L dichloride sodium emulsion (made by Beijing Nuohua Pharmaceutical co.,Ltd) was taken as the coupling agent to smear around the knee joint about 0.75 -1.25 W/cm2, once a day, and 8-10 minutes per time. Meanwhile the static isometric contraction exercise of the knee joint in the extension position was done. Using the quardriceps isometric exercise apparatus, the Delorme progressive resisThe same ultrasonic therapy was done as the therapy group. The nonthermal and microthermal ultrasonic wave was adopted on the suffered location 10 -15 minutes per time, and the tolerance dose of the mid-frequency current was given 20 minutes per time and once a day using the CM-efficacy was assessed in the two groups 4 weeks after the therapy. During the therapy period the anti-inflammatory and analgesic drugs were not patient's knee joint activities of daily life (0 score signifies no pain, 10 scores severe pain, the pain relief better than 40% that before the therapy means the effectiveness). The lower limb activity of daily life was chosen to evaluate the patient's knee joint activities of daily life (5 items,total 10 scores, high score means good, low score means poor). The maximal loading dose in the extension position of the knee joint was measured on the quadriceps isometric exercise apparatus using the suspension weight method.The maximal loading dose in the extension position of the knee joint in the two groups.result of the walking pain scale in the two groups before and after the therapy: The score of pain evaluation after therapy was significantly lower than that before the therapy (P < 0.01), and there was no significant differs knee joint activities of daily life in the two groups before and after the therapy: the result after the therapy was siguifieantly better than that before the therapy, and the efficacy after the therapy was significantly higher than that before the therapy, the efficacy of the experiment group was better than that of the control group (9.61±1.82, 8.77±2.28), while there was tion results of the maximal loading dose in the extension position of the knee joint in the two groups: the result of the experimental group after the therapy was significantly better than that of the experimental group before the therapy and the control group [ (10.05±2.41) kg vs (7.86±1.98) kg and (8.76±1.65) kg, P < 0.01 ].CONCLUSION: The ultrasonic wave using the dichloride sodium emulsion as the ultrasonic coupling agent in combination with quadriceps strengthening exercise in treating the senile knee osteoarthritis is efficacious in relieving pain and improving the ADL ability of the knee joint, increasing the maximal loading dose in the extension position of the knee joint and the stability of the knee joint. The efficacy of the combination therapy is better than the single ultrasonic therapy.
3.Effect of Transcutaneous Electrical Acupoint Stimulation on Emergence Agitation in Children Undergoing Cochlear Implantation

Objective To study the effect of transcutaneous electrical acupoint stimulation (TEAS) on emergence agitation in children undergoing cochlear implantation.Methods Sixty patients (age range,1-4 years) scheduled to undergo pediatric cochlear implantation were enrolled and randomized into the TEAS group (group T,n =30) that received continuous TEAS and the control group (group C,n =30) that did not receive TEAS.Anesthesia time,operation time,change in blood flow dynamics during the perioperative period,extubation time,and PACU stay time were recorded.The incidence of emergence agitation and postoperative nausea and vomiting (PONV) was monitored.Results No significant intergroup differences were observed in age,sex,weight,operation time,and PACU stay time (P > 0.05).The MAP and HR in group T were lower than those in group C at different time points such as 30 min after the start of surgery,the end of the surgery,the extubation and 5 min after extubation,and the difference was statistically significant (P < 0.05).The FPS-R,FLACC,and PAED scores in group T were lower than those in group C at different time points within 30 min after exmbation (including the time of extubation),and the difference was statistically significant (P < 0.05).Extubation time,anesthesia time,and incidence of PONV were lower in group T than in group C,and the difference was statistically significant (P < 0.05).Conclusion TEAS combined with general anesthesia significantly reduces the incidence of emergence agitation in children undergoing surgery for congenital hearing loss;it also helps control hypotension,reheve dramatic hemodynamic changes during the waking period,and reduce the incidence of PONV.
4.Feixin mixture for vascular remodeling in rats with monocrotaline-induced pulmonary hypertension

BACKGROUND: Pulmonary hypertension induced by alternation of pulmonary function and structure is the prerequisite of pulmonary-cardiac disorder. Chinese herb, fenxin mixture(FXM) reduces effectively pulmonary hypertension, but the mechanism on which has not been completely explained yet.OBJECTIVE: To comp are the reversal effects between feixin mixture and nifedipine on vascular remodeling in rats with pulmonary hypertension.DESIGN: Randomized controlled experimental study in which experimental animals are taken as the objects.SETTING: Nursing department of affiliated hospital of a medical university;a medical college affiliated hospital and department of traditional Chinese medicine of a provincial hospital.METHODS: Monocrotaline(MCT) (50 mg/kg) was used to duplicate pulmonary-cardiac model in rats. Totally 60 Wister male rats were randomized into 6 groups, namely normal control group, model group, FXM groups of low, middle and high dosages successively and nifedipine group, 10 rats in each group. Since the 14th day of modeling, the corresponding medications were administrated with gastric infusion, continuously for 8 days. After the rats sacrificed, the special staining collaborating with pathological image analysis was used to determine the measurement alternations on path-morphology of pulmonary small artery.wall square(VWS)/total vessel square(TVS), cavity square(CS)/TVS of pulmonary small artery in rats of every group.RESULTS: Both feixin mixture and nifedipine alleviate pulmonary vascular remodeling remarkably( P < 0.01 ), of which, the effect was the best in feixin mixture group of high dosage. But, the every index in experimental groups had not recovered to the level in normal control group completely.CONCLUSION: Feixin mixture reverses partially the remodeling of pulmonary vascular structure and reduces effectively pulmonary hypertension.
5.Chronic salt-loading downregulates large-conductance Ca2+-activated potassium channel in mesenteric arterial smooth muscle cells from SD rats

Objective Large-conductance culcium-activated potassium (BKCa) channel modulates vascular smooth muscle tone. In the present study, we tested the hypothesis that salt, one of the factors which significantly influence bleed pressure (BP), can regulate BKCa activity and then elevate blood pressure. Methods Male Spragne-Dawley rats aged 6 weeks were randomized into high salt diet group (HS) and control group, fed with high salt diet (containing 5% NaCi) and standard rat chow (containing 0.4% NaCl) respectively for 16 weeks. Tail systolic blood pressure (SBP), body weight (BW) and 24-hour urinary output were tested every 4 weeks. Content of urinary Na+ was detected using flame spectrophotometrical method. At the end of 16 weeks, all the rats were killed, the mesenteric arteries were obtained, and single mesenteric smooth muscle cells were isolated at once. The resting membrane potential (Em), the total potassium currents and the currents after perfusion with TEA solution of the cells were all recorded by whole cell patch clamp. The transcriptions of BKCa channel α and β1 sobunits in mesenteric arterial vascular smooth muscle cells (VSMC) of each group were calculated by real-time RT-PCR. Results There was no difference in SBP and BW at each stage between control group and HS group; the urinary Na+ level in HS animals was elevated significantly after 4 weeks.The negative values of Em in HS group VSMCs were reduced compared with these in the control group. Transcriptions of β1 subanit of BKCa channels were decreased in HS group, but α subunit transcriptions did not differ between the two groups. Whole cell potassium currents did not differ hetween HS and control groups, but BKCa currents of HS group VSMCs were lower than these of control group ones. Conclusion Even without elevating SBP, salt-loading can still modulate the expression and activity of BKCa channel in the mesenteric arterial VSMC and elevate vascular tone.
6.Double locking mini-plates for fixation of comminuted fracture of the first metacarpal base

Objective To investigate the clinical efficacy of double locking mini-plates for fixation of the comminuted fracture of the first metacarpal base.Methods Twenty-four patients with the comminuted fracture of the first metacarpal base were treated by double locking mini-plates in our hospital from January 2013 to January 2016.They were 17 males and 7 females,from 27 to 65 years of age (average,33.5 years).By the Green classification,6 cases were type Ⅰ,13 cases type Ⅱ and 5 cases type Ⅲ.All the fractures were closed.The average time from injury to surgery was 2.3 days (from 8 hours to 7 days).After open reduction via the palmar-radial incision,the fracture was fixated with 2 mini-plates,one locking T-plate on the radial side and one straight locking plate on the dorsal side.Fracture healing time,pain and finger function were followed up postoperatively.Visual analogue scale (VAS) was used to evaluate the pain at 12 months and scoring for digital total range of motion to assess the function of the affected finger at the final follow-up.Results The 24 patients obtained follow-up for 8 to 28 months (average,18 months).All the Fractures healed after 9 to 12 weeks (average,10.5 weeks).The VAS scores at 12 months ranged from 0 to 3 (average,1.5).The function of the affected finger at the final follow-up was excellent in 20 cases and good in 4,giving an excellent to good rate of 100%.Two patients complained of pain after frequent motion of the finger.No complications like skin problems,dislocation of the first metacarpal base,implant failure,necrosis or irritation of the soft tissue were observed during follow-up.No angulation or rotational deformity occurred after fracture union.Conclusion Fixation with double locking mini-plates can effectively treat comminuted fractures of the first metacarpal base,because it can provide rigid stabilization which promotes early functional exercise of the finger,and prevents joint stiffness as well.
7.Auto recognizing critical values from medical image examination reports by using regular expressions

Objective To introduce a method of automatically identifying critical values from medical image examination reports and prompt the physician to report it,to prevent the omission of the critical value reporting and improve the medical quality.Methods According to the requirement of critical value reporting system,regular expressions were made for each emergency situation of medical image examination,in order to form a critical value feature library.And an algorithm was designed to find critical value and prompt doctors automatically.Results According to this method,the critical value auto recognize software was developed and had been tested in Nanfang Hospital for 6 months.The software ran well.Conclusion Using regular expressions to define a criteria value feature library and design an algorithm of identifying criteria values,can recognize critical values and prompt physician automatically.
8.Establishment of a method for the determination of four volatile components in Compound shexiang xuelian liuzhi plaster by GC

OBJECTIVE To establish a method for simultaneously determining the contents of camphor ,menthol,borneol and methyl salicylate in Compound shexiang xuelian liuzhi plaster . METHODS The test solution was prepared by reflux extraction with ethyl acetate ,and was determined by gas chromatography (GC). A Shimadzu SH -Rtx-Wax capillary column was used as the chromatographic column ,and a flame ionization detector was used as the detector . The detector temperature and the injector temperature were both set at 200 ℃. The flow rate of carrier gas (nitrogen)was 2.0 mL/min,the separation ratio was 20∶1,and the sample size was 1.0 μL. RESULTS The linear ranges of camphor ,menthol,borneol(calculated by the sum of isoborneol and borneol)and methyl salicylate were 11.5-230.4,10.6-211.6,11.3-225.5,11.0-219.1 μg/mL(r>0.999). RSDs of the precision , repeatability and stability (48 h)tests were all less than 4%. The average recoveries of the four components were 100.7%,99.7%, 98.9% and 100.7%(RSDs were 4.3%,2.9%,2.2%,3.7%,n=9). The contents of camphor ,menthol,borneol and methyl salicylate in two specifications of Compound shexiang xuelian liuzhi plaster were 16.8-19.5,4.6-6.0,9.8-11.9,6.9-8.2 mg/piece(7 cm×10 cm/piece),and 8.3-8.6,2.2-2.4,4.7-4.8,3.2-3.6 mg/piece (5 cm×7 cm/piece). CONCLUSIONS The method is successfully established for simultaneous determination of four volatile components in Compound shexiang xuelian liuzhi plaster .
9.Effects of Moxibustion on Colonic JNK Signaling Pathway in Crohn's Disease Model Rats

Objective: To explore the anti-inflammatory immune mechanism in moxibustion treatment of Crohn.s disease (CD) from the perspective of c-Jun N-terminal kinase (JNK) signaling pathway, through observing the regulatory effect ofmoxibustion on colonic JNK, c-Jun, monocyte chemoattractant protein 1 (MCP-1) and cyclooxygenase 2 (COX2) in CDmodel rats. Method: Male Sprague-Dawley rats of clean grade were randomized into a normal group, a model group, amoxibustion group and a sham moxibustion group. CD model was developed by the mixture of 2, 4, 6 Trinitro-benzene-sulfonic acid (TNBS) and ethanol via enema. Hematoxylin-eosin (HE) staining was used to observe the morphologicalchanges in rat.s colon tissues for pathological scoring; enzyme-linked immunosorbent assay (ELISA) was used to detectthe contents of MCP-1, COX2, JNK, and c-Jun in colon tissues; real-time fluorescence quantitative PCR was adopted toexamine the mRNA expressions of JNK and c-Jun in rat.s colon. Result: Compared with the normal group, the modelgroup showed more significant colonic damage and thus had a higher colonic damage score (P < 0.01), manifested astopical inflammation which involved the submucosa, fissuring ulcers and granuloma; the model group also showedincreased contents of protein MCP-1 and COX2, and elevated contents of JNK protein and mRNA in colon (all P < 0.05), while the change in the content of c-Jun was insignificant (all P> 0.05) . Compared with the model group and shammoxibustion group, the colonic damage score was lower in the moxibustion group (P < 0.01, P < 0.05), with improvementin colonic structure and inflammation; the contents of MCP-1 and COX2 in colon tissues declined, so did the proteincontent and mRNA expression of JNK (all P < 0.05), while the change in the content of c-Jun was insignificant (all P>0.05) . There were no significant differences between the model group and sham moxibustion group comparing all theindexes (all P> 0.05) . Conclusion: Moxibustion down-regulates the expressions of JNK protein and mRNA in CD rat.scolon, as well as the contents of MCP-1 and COX2 in colon tissues, which is possibly one significant mechanism formoxibustion to ease intestinal inflammation and promote the repair of colon tissues in CD.
10.Genetic characteristics of norovirus in children with acute gastroenteritis in a single center in Shanghai

Objective:To investigate the genetic characteristics of norovirus (NoV) in children with acute gastroenteritis in Shanghai.Methods:A total of 709 stool specimens were collected from outpatients with acute gastroenteritis in Children′s Hospital of Shanghai from October 2018 to September 2019. Real-time RT-PCR was used for qualitative detection of NoV, and RT-PCR was used to identify the genotypes of NoV with the primers of VP1 gene, RdRp region and RdRp-VP1 region. SPSS20.0 statistical software was used for data processing and bioinformatics software was used for homology, phylogenetic and recombination analysis of NoV gene sequences.Results:NoV was detected in 265 out of the 709 stool specimens with a positive rate of 37.4%. Sequence analysis of RdRp region and VP1 gene showed that seven different genotypes including GⅡ.P16-GⅡ.2, GⅡ.P12-GⅡ.3, GⅡ.Pe-GⅡ.4_Sydney 2012, GⅡ.P7-GⅡ.6, GⅡ.P8-GⅡ.8, GⅡ.P21-GⅡ.13 and GⅡ.P17-GⅡ.17 were detected from 111 NoV-positive specimens. The predominated genotype was GⅡ.P16-GⅡ.2 (30.6%, 34/111), followed by GⅡ.Pe-GⅡ.4_Sydney 2012 (27.0%, 30/111) and GⅡ.P12-GⅡ.3 (24.3%, 27/111). Two new NoV recombinant strains belonging to GⅡ.P21-GⅡ.13 genotype were identified and the recombination site was in the junction region of ORF1 and ORF2. NoV infection occurred every month, but the predominant genotype was different. No significant difference in the positive rates of NoV was found between male and female patients ( P=0.329). However, there were significant differences between different age groups ( P=0.011) and the children in the age groups of >11-12 years old and >2-3 years old had higher rates of NoV infection. Conclusions:The predominated recombinant NoV strains belonged to GⅡ.P16-GⅡ.2, GⅡ.Pe-GⅡ.4_Sydney 2012 and GⅡ.P12-GⅡ.3 genotypes, and two new recombinant NoV strains (GⅡ.P21-GⅡ.13) were found in Shanghai during October 2018 to September 2019. Gene sequencing across ORF1 and ORF2 was conducive to better understanding the NoV genotypes and recombination.
