1.Application of one-part form mini-implant in immediately and delayed repair of edentulous space

Objective To analyze the effect of one-part form mini-implant in treatment of immediately and delayed repair of edentulous space. Methods Retrospectively analyzed 80 cases of edentulous space patients in our hospital from Jan. 2009 to Jan. 2011, and divided them into the immediately repair group ( observation group) and the delayed repair group ( control group) according to time of edentulous space repair. Patients of the two groups were given permanent PFM 3 months later, and regular clinical and X-ray examinations were made to evaluated clinical efficacy and complications of the two groups. Results The score of ISQ of the observation group was higher than that in the control group with significant difference (P <0. 05); and the postoperative complications of the observation group was significantly decreased compared to the control group with a significant difference (P <0. 05). Conclusion Mini-implant used in immediately repair in edentulous space can remove pain in a short time with less postoperative complications, it has more advantages than delayed repair of edentulous space.
2.The changes of the glycolipid and coronary artery lesions in coronary artery disease patients with isolated post-challenge hyperglycaemia

Objective To observe the changes of the glycolipid and coronary artery lesions in coronary artery disease patients with isolated post-challenge hyperglycaemia (IPH). Methods Hospitalized coronary artery disease patients diagnosed by coronary angiography were received oral glucose tolerance test. The labarotary test datas, including blood hematochrome, fasting and post-challenge plasma glucose levels, plasma lipid and insulin levels, were recorded. The number of diseased coronary artery and coronary lesion scores were calculated according to coronary angiogram for each patient. Continuous 77 patients with normal glucose metabolism(normal glucose metabolism group) and 104 patients with IPH (IPH group) entered the study. Results The occurrence of hypertension, acute coronary syndrome, multivessel coronary disease, revascularization were 84.6%(88/104), 82.7%(86/104), 69.2%(72/104), 70.2%(73/104) in IPH group and 63.6%(49/77), 70.1%(54/77), 50.6%(39/77), 54.5%(42/77) in normal glucose metabolism group, and there were significant differences(χ2=3.985-10.583, all P<0.05). The body mass index, triglyceride, total cholesterol, fasting glucose, post-challenge glucose, hemoglobin A1c, fasting insulin, insulin resistance index, coronary lesion scores were (27.42±3.55) kg/m2,(2.18±1.53) mmol/L,(4.73 ± 1.00) mmol/L,(5.98 ± 0.92) mmol/L, (14.11 ± 1.94) mmol/L, (5.90 ± 0.78)%, (10.07 ± 6.13) mU/L, 2.617 ± 1.535, (41.35 ± 27.21) points in IPH group and (25.11 ± 3.40) kg/m2, (1.52 ± 0.85) mmol/L, (4.22 ± 1.02) mmol/L, (5.24±0.62) mmol/L, (6.49±1.07) mmol/L, (4.97±0.39)%, (7.72±4.28) mU/L,1.785±0.974, (33.36±23.36) points in normal glucose metabolism group, and there were significant differences(t=3.035-33.698, P<0.05 or 0.01). Conclusions Coronary artery disease patients with IPH have much more seriously abnormal glycolipid metabolism and complex coronary artery lesions, and need specialized nursing management.
3.Effects of Astragalus Polysaccharides on Damage of Keratinocytes in UVB Radiation Skin

Objective To investigate the effects of Astragalus polysaccharide (APS) on damage of keratinocytes in ultraviolet B (UVB) radiation skin in HaCaT cells; To preliminarily explore its mechanism of action.Methods HaCaT cells were treated by UVB irradiated for modeling. Blank control group, model group, UVB irradiation group and low-, medium- and high-dose APS interventional groups were set up. Each medication group was given relevant medicine. MTT assay was used to determine the cell activity; GSH-Px, CAT, SOD activity and MDA content were detected by biochemical colorimetric method; contents of TNF-α and IL-6 were detected by ELISA.Results Compared with the control group, the contents of SOD, CAT and GSH-Px in the model group decreased significantly, while the contents of MDA, TNF-α and IL-6 increased significantly (P<0.05); compared with the mode group, the contents of SOD, CAT and GSH-Px in the medication groups increased significantly, while the contents of MDA, TNF-α and IL-6 were reduced significantly (P<0.01).Conclusion APS can reduce the oxidative stress injury of UVB to HaCaT cells to some extent.
4.Endoscopic mucosal resection for the treatment of colorectal tumors

Objective To further study the effectiveness of endoscopic mucosal resection (EMR) for the treatment of early cancer and precancerous lesions on colon and rectum. Methods A total of 58 patients with 70 lesions were included in this study. EMR were performed by means of " injection and cut " method. Results All the lesions were resected completely by EMR. All the diagnoses were proved by pathology as follows: mucosal cancer 3 cases, adenoma with mild to severe dysplasia 53 cases, and inflammatory polyps 14 cases. No recurrence was found during the follow up survey for 1 23 months. Except 3 cases of treatable hemorrhage, no other complications were found. Conclusion The results of this study further confirm that EMR is effective and safe for the treatment of early cancer and precancerous lesions on colon and rectum

Objective To evaluate the effects of angiostatin on vascular endothelial growth factor expression in human tongue squamous cell carcinoma cells.Methods Subcutaneous xenografts of human tongue squamous cell carcinoma(Tca8113)cell line in BALB/c nude mice were employed and eighteen mice were randomly divided into three groups(6 mice each):the PBS group injected with PBS,the AS group injected with 250 ng purified angiostatin,and the AS group injected with 30 ?g purified angiostatin.Reagents were injected intraperitoneally twice daily and tumor volume was calculated twice weekly for 21 d,then animals were sacrificed and each tumor specimen was divided into two parts after surgical resection.The first part was submitted to both immunocytochemistry for VEGF,VEGFR1,VEGFR2 and CD34,and TUNEL apoptosis assay.The second part was prepared for semi-quantitative RT-PCR analysis of VEGF,VEGFR1 and VEGFR2 mRNA abundance.Results There was an increase of apoptotic index(AI)(P
6.Protective effect of PPAR-αagonist on PAN-induced podocyte injury

Aim To investigate the function of fenofi-brate on PAN ( puromycin aminonucleoside )-induced podocyte injury. Methods SD female rats of 18-week-old were randomly assigned into 3 groups ( n =6 ) . Mice in PAN group and fenofibrate treated group received a single intravenous injection of PAN ( 65 mg ·kg-1 ) , while those in control group received equal volume of saline. Mice in fenofibrate treated group re-ceived 40 mg · kg-1 · d-1 of fenofibrate ( intragastric administration ) on day 1 after PAN injection , while those in PAN group and control group received equal volume of vehicle. 24 hours urine samples from all group were collected on day 0(1 day before PAN injec-tion), day 6, day 10. The 24 hours urine protein was detected by Bradford assay. All the rats were sacrificed 10 days after the induction of podocyte injury, and glo-merulus sample were collected. The expression of podocyte injury marker and transcription level in apop-tosis, podocyte cytoskeleton protein, slit diaphragm protein were evaluated by Western blot and real-time PCR. Results Compared with the control group, 10 days after injection of PAN, 24 hours urine protein was obviously increased, and the expression and transcrip-tion level of podocyte injury marker desmin, apoptosis, podocyte cytoskeleton protein, slit diaphragm protein were upregulated greatly, however, those were signifi-cantly lower in fenofibrate treated group as compared with those in PAN group. Conclusions PPAR-α ago-nist fenofibrate can ameliorate PAN-induced glomerulus podocyte injury, and the mechanism involved may be associated with inhibition of the mitochondria apopto-sis, TGF-β/Smad pathway and p38 pathway.
7.Endoscopic measurement by virtual internal standard:Feasibility and accuracy

Objective: Endoscopic measurement mainly depends on visual estimation or open biopsy forceps,which have been proved lacking in accuracy due to the image barrel distortion caused by wide-angle lens in endoscope.This study aimed to validate a new concept of "virtual internal standard" proposed by ourselves and develop a new method for endoscopic measurement based on the concept.Methods: We photographed planar grid graphs printed with standard calibrations with an electronic endoscope at different object distances,obtained the information on the virtual internal standard from each photo at the corresponding object distance,and analyzed the digital characteristics of the barrel distortion photos,including the size of the visual field,the image compression ratio,and the credible area for measurement with the virtual internal standard.Based on the analyses,the method of endoscopic measurement by the virtual internal standard was established and experiments both in vivo and in vitro were conducted.Results: The ratio of the object distance to the visual field was 1 ∶ 2 when the former was between 10 mm and 40 mm.Calculation of the compression ratio showed that the credible area of measurement in the central area was 10 mm at the object distance of 10-15 mm,20 mm at 20-30 mm,and 30 mm at 30-50 mm.The credible area of measurement on various planes accounted for 2/5 to 3/5 of the total image width.There was a tendency of double-curve change in the arrangement of pixel values of the central grids on different planes.The differences between the pixel values per centimeter became slight at the object distance of 35-50 mm.Conclusion: Endoscopic measurement by the virtual internal standard is a objective,accurate,simple and practical method.
8.The application of a new type of laparoscopic training method in the standardized training for res-idents

For a long time, limited by the factors such as laparoscopic technology, and limited medical resources , the residents accepting standardized training are lack of mastery of the technology . Meanwhile, it is the key to the training of personnel training and reserve in the field for residents to contact the laparoscope as soon as possible and carry out scientific and effective training. Therefore, based on the traditional method, we have developed a new type of laparoscopic teaching system for the standardized training residents and increased and integrated the LAP GAME R operations training system and the real-time multimedia teaching platform. The preliminary practice effect is good.
9.Expression of FHL1 and the significance in pancreatic fibrosis of chronic pancreatitis

Objective To explore the expression of transcriptional regulation factor FHL 1 in pancreatic fibrosis of chronic pancreatitis (CP) and clinical significance.Methods Serum samples from 20 CP patients treated in the Department of Gastroenterology , Nanjing General Hospital and 20 healthy controls in the corresponding period were collected .The level of FHL1 in the serum was detected by ELISA .Twenty male Wistar rats were randomly divided into control group and CP group according to the body weight .The CP rat model was established by injecting dibutyltin dichloride ( DBTC) through tail vein .All the rats were killed at 28 days after injection and the pancreas was collected , which was observed using hematoxylin and eosin ( HE) staining to investigate pathological changes . The expression of FHL1 in pancreas was measured by immunohistochemistry (IHC) and Western blotting (WB).Results The average of serum FHL1 level in two groups were (0.86 ±0.22)g/L and (1.18 ±0.22)g/L, respectively.The level of FHL1 in serum of CP patients was significantly lower than that of healthy cases , and the difference was statistically significant (t=10.54, P=0.001).Damaged pancreatic lobular structure , acinar atrophy and the obvious fibrous hyperplasia were observed in CP rats .The pathological score of abnormal structure , acinar atrophy, fibrosis, and inflammatory cell infiltration in the pancreas of the CP group was (2.2 ±0.4),(2.0 ±0.7),(2.4 ±0.3) and (2.8 ±0.6), respectively, but was all 0 in control group.The difference between two group was statistically significant ( t value =17.24, 9.01, 25.22 and 14.75, respectively, all P<0.001).The expression of FHL1 in the pancreatic tissue of CP group were less than that of normal group , and the difference was statistically significant (IHC:15 798 ±3 247 vs 37 570 ±9 254, t=2.438,P=0.016;WB:0.44 ±0.11 vs 0.89 ±0.03, t=4.289, P=0.002).Conclusions The protein of FHL1 was in low expression in the pancreas of CPs , indicating that FHL1 may participate in the formation of pancreatic fibrosis .
10.A Retrospective Analysis of Capsule Endoscopy in 573 Cases

Background:Capsule endoscopy has been demonstrated to be an effective diagnostic tool for small bowel diseases in a series of small sample researches. However,the results need to be verified by large sample studies. Aims:To investigate the diagnostic performance and safety of capsule endoscopy for small bowel diseases. Methods:Consecutive patients undergoing capsule endoscopy for suspected small bowel diseases from May 2008 to Apr. 2013 in Nanjing General Hospital of Nanjing Military Command of PLA were collected and analyzed retrospectively. Results:A total of 573 patients were enrolled,the overall success rate of the examination was 99. 13%(568 / 573)and the overall completion rate was 88. 38%(502 / 568). The mean time of capsule passing the pylorus was 43. 45 min,and that of passing the small bowel was 287. 21 min. The overall detection rate of small bowel lesions was 53. 52%(304 / 568)and the overall diagnostic rate was 51. 06%(290 / 568). Both the detection rate and diagnostic rate in patients with obscure gastrointestinal bleeding(OGIB) were significantly higher than those in patients with obscure abdominal pain and chronic diarrhea(64. 26% and 62. 46%vs. 41. 72% and 39. 07% ,and 32. 14% and 27. 38% ,P all < 0. 05). Small bowel lesions detected by capsule endoscopy included angiopathy(21. 38% ),ulceration(20. 72% ),neoplasms(14. 47% ),erosion(11. 84% ),and Crohn’s disease(11. 18%),etc. Capsule retention occurred in 2. 29%(13/568)of the patients,and one acute intestinal obstruction and 2 perforations were observed. Conclusions:Capsule endoscopy is a safe and effective diagnostic modality for small bowel diseases. OGIB is the most common indication for capsule endoscopy,and capsule endoscopy is also helpful for evaluation of established or suspected Crohn’s disease.
