1.Influence of different nutritional support on the perioperative nutritional status for gastric cancer patients undergoing total gastrectomy

Objective To investigate the early enteral nutrition (EN) and total parenteral nutrition (TPN) on the periopera‐tive nutritional status of gastric cancer patients underwent total gastrectomy .Methods 96 cases of total gastrectomy for gastric cancer patients treated in our hospital from January 2011 to March 2013 were randomly assigned ratio EN group and the TPN group ,with 48 cases in each group;nutritional status were compared before and after treatment .Results EN group was superior in nutritional status than TPN group ,it had shorter recover time and less complications (P<0 .05) .Conclusion Early enteral nutri‐tion support can quickly restore the nutritional status of patients and improve the immune status .
2.Determination of Pharmacokinetics Differences of Ammuxetine Isomers in Rat Plasma Using On-Line Solid Phase Extraction with Liquid Chrommatography-Tandem Mass Spectrometry

An on-line solid phase extraction ( SPE ) coupled with HPLC-MS/MS method was developed to determine S-ammuxetine and R-ammuxetine in rat plasma. The sample preparation consisted of the following steps:A protein precipitation extraction used methanol and acetonitrile ( 50:50 , V/V ); an on-line SPE treatment to remove most matrixes in plasma;an enrichment and separation step used a C18 analytical column. S-and R-ammuxetine were determined by tandem mass spectrometry. The SPE column was a Retain PEP Javelin (10 mm × 2. 1 mm × 5 μm), while the chromatographic separation was achieved using a ZORBAX SB-C18 (50 mm × 2. 1 mm × 3. 5 μm) analytical column with an isocratic mobile phase composed of acetonitrile-water-formic acid (40:60:0. 1, V/V/V, 0. 3 mL/min). The selected reaction monitoring mode of the positive ion was performed and the precursor to the product ion transitions of m/z 292 . 1/154 . 0 and m/z 260. 4/116. 2 were used to measure S-ammuxetine, R-ammuxetine and internal standard (propranolol). The method was linear over a concentration range from 0 . 2 to 1000 μg/L with the correlation coefficients of 0 . 9903 and 0 . 9951 . The average intra-day precision values were 1 . 2% -12 . 0% for S-ammuxetine and 0. 4%-11. 2% for R-ammuxetine, respectively. The average recoveries were 94. 2%-101. 6% for S-ammuxetine and 94. 3% -109. 4% for R-ammuxetine. Compared to the literature, the sensitivity of this method increased dramatically. The present method has been successfully applied to the preclinical rat research of ammuxetine isomers following intragastric administration.

Using the technique of fluorescent-labled mRNA differential display, new apoptosis related gene 2ass-bnip3 of type 2 diabetic cardiomyopathy was found, the sequence of 1594 bp with coding 187 amino acids was obtained by the full-length clone, and its structural and functional predictions were performed. 2ass-bnip3 may play an important role in the development of diabetic cardiomyopathy via a regulatory pathway of calcium regulation and apoptosis.
4.High intensity focused ultrasound in the treatment of primary malignant bone tumor.

OBJECTIVETo develop a new noninvasive limb-salvaging method in the treatment of primary malignant bone tumor in the extremities.
METHODSForty-four patients with primary malignant bone tumor were treated by extracorporal high intensity focused ultrasound (HIFU) with or without chemotherapy, with a mean follow-up of 17.6 months.
RESULTSThe overall survival and complication rates were 84.1% and 18.2%. In 34 patients with stage II b disease, 30 were disease-free, with 2 died of tumor metastasis to brain and lung and 3 developed local recurrence. In 10 patients with stage III b disease, 5 survived with tumor, with 1 developed local recurrence and 5 died of lung metastasis. Thirty-six of 44 patients were > or = 15 points by Enneking system.
CONCLUSIONHigh intensity focused ultrasound combined with chemotherapy for primary malignant bone tumor in the extremity is proved to be effective and safe, preserving good function in the limbs. HIFU may possibly become a new limb-salvaging treatment for this tumor.

5.High intensity focused ultrasound alone for malignant solid tumors.

OBJECTIVETo investigate the efficacy and side-effects of high intensity focused ultrasound (HIFU) in the treatment of malignant solid tumors.
METHODSThirty such patients who refused surgery and or refractory to chemotherapy, were treated by HIFU alone, with the efficacy and side effects monitored in terms of vital organ signs observation, functional assay of important organs, imaging examinations: digital subtraction angiography (DSA), CT, MRI, single photoemission computed tomography (SPECT), large core needle biopsy, complications or metastasis.
RESULTSAfter HIFU therapy, the vital signs remained stable and the function of heart, lung, kidney and liver was also normal. DSA images showed that small or larger arteries were not damaged. After a follow-up of 10 - 38 months (23.1 months), 26 patients (87%) have been alive. The volume of the tumor underwent complete regression in 10 patients. Shrinkage of tumor volume >/= 50% was observed in 13. Eight of 13 patients were examined by large core needle biopsy, all showing necrosis and/or fibrosis though 3 patients (10%) had local recurrence. Two of these were retreated again by HIFU and the locally recurrent tumor became well controlled. New metastases developed in 5 patients after HIFU therapy. Two patients suffered from peripheral nerve injuries, most of which have recovered during the follow-up. However, one patient developed skin injury.
CONCLUSIONHigh intensity focused ultrasound alone is effective and safe in the treatment of malignant solid tumors.

6. Integration of clinical competence evaluation index system for master of nursing specialist

Objective:
Integrate the evaluation indexes of clinical competence of master of nursing specialist.
Methods:
Search the Cochrane Library, Joana Briggs Institute Library, Embase, Web of Science, PubMed, CNKI, VIP and Wanfang database, from the establishment of the database to December 10th,2018, and to collect researches about master of nursing specialist clinical competence evaluation index and then analyzed by content analysis method and thematic analysis.
Results:
A total of 9 articles were included, 26 items and 9 categories including comprehensive practical ability, professional development ability, teaching guidance ability, clinical scientific research ability, organization and management ability, professional accomplishment, professional values, professional knowledge level, relevant knowledge level were selected. 3 dimensions including skill structure, attitude structure and knowledge structure.
Conclusion
Evaluating the clinical competence of master of nursing specialist we should not only consider their comprehensive practical ability, but pay attention to their professional development, teaching and scientific research, organization and management, professional accomplishment, knowledge and professional values assessment.
