1.Therapeutic Observation of Acupuncture at Phenomaxillary Ganglia for Allergic Rhinitis

Objective To observe the clinical efficacy of acupuncture at phenomaxillary ganglia in treating allergic rhinitis. Method Sixty patients with moderate-severe persistent allergic rhinitis were randomized into a treatment group and a control group, 30 cases in each group. The control group was intervened by oral administration of Loratadine plus Mometasone Furoate Nasal Spray, while the treatment group was by acupuncture at phenomaxillary ganglia in addition to the medicines given to the control group. Before treatment and after 4-week treatment, the peripheral blood immunoglobulin E (IgE) level and Rhinoconjunctivitis Quality of Life Questionnaire (RQLQ) were observed in the two groups, and the clinical efficacies were compared.Result The IgE levels and RQLQ scores were significantly changed after treatment in both groups (P<0.01). After treatment, the IgE level and RQLQ score of the treatment group were significantly different from that of the control group (P<0.01,P<0.05). The total effective rate was 96.7% in the treatment group versus 83.3% in the control group, and the difference was statistically significant (P<0.05). Conclusion Acupuncture at phenomaxillary ganglia is an effective method in treating allergic rhinitis.
2.Factors of postoperative intracranial infection after endoscopic repair of cerebrospinal fluid rhinorrhea

Objectives To investigate risk factors for postoperative intracranial infection after nasal endoscopic re-pair of cerebrospinal fluid rhinorrhea, in order to provide a reference for later treatment. Methods Selected 80 cases treated by nasal endoscopic repair of cerebrospinal fluid rhinorrhea from January 2010 to 2015 as study object, ret-rospective analyzed the clinical data such as the patient's age, gender, etiology, leak size, the leak location, prior in-tracranial infection, prophylactic antibiotics and postoperative application of antibacterial drug > 7 d, the number of operations, whether or not the operation using artificial materials, repair materials, repair of cerebrospinal fluid rhin-orrhea failure and by nasal endoscopic repair of cerebrospinal fluid rhinorrhea occurred between intracranial infec-tion. Results 4 of them (5.00 %) occurred intracranial infection. CSF bacterial culture show 8 strains of pathogenic bacteria were isolated, including 3 cases of leather of gram negative bacteria and 5 strains of leather of gram positive bacteria and gram positive bacteria were mainly Staphylococcus aureus and accounted for 40.00 %. Univariate anal-ysis showed that repair of cerebrospinal fluid rhinorrhea failure, improper surgical procedures, previous intracranial infection history, the choice of restorative materials and the size of the leak diameter > 1 cm and after endoscopic repair of cerebrospinal fluid rhinorrhea postoperative intracranial infection have some relevance ( < 0.05). Logistic multivariate analysis showed that repair of cerebrospinal fluid rhinorrhea failure, improper surgical procedures, previ-ous intracranial infection history, the choice of restorative materials and the size of the leak diameter > 1 cm of intranasal endoscopic repair of cerebrospinal fluid rhinorrhea independent risk factor for postoperative intracranial infection ( < 0.05). Conclusions Skull size of the leak diameter > 1 cm, previous history of intracranial infection, cerebrospinal fluid rhinorrhea repair a variety of factors fail, surgery failure factors are likely to increase patient after endoscopic repair of cerebrospinal fluid rhinorrhea intracranial infection the incidence.
3.Practice of strengthening students' identity as a‘trainee doctor’in pediatric practice teaching

Objective Through the practice of new practice teaching methods, to observe whether it can help students to strengthen their self-identity as ‘trainee doctors’. Methods By sim-ulating interrogation, building simulation medical environment and increasing operation project, the practice operation abilities and sense of identity were improved,The questionnaire were required to fill in at the beginning and at the end of the term, and the differences between two teaching methods in advantages, characteristics and the identity of ‘trainee doctors’by their own pairing were compared. Data were analyzed by software SPSS 18.0. Results The new teaching model, to a certain extent, alleviated the present situation of having relatively more students and insufficient source, which was superior to traditional teaching mode in improving students' initiative, enthusiasm, learning ability, practical ability, analysis of the problems, comprehensive ability and so on. It also enabled students to master some of doctor-patient communication skills(P<0.001), to realize the identity of‘trainee doctors’ (Z=-7.671, P<0.001). Conclusion The new practice teaching mode has solved the problems such as insufficient source in clinical teaching. It has also increased the initiative and enthusiasm of students' clinical learning, so that students can think, analyze and deal with the problems from the point of view of doctors, and implement the identity of a‘trainee doctor’.
4.Nosocomial Infections of Inpatients: A Clinical Investigation and Analysis with Large Sample Size from 2000 to 2004

OBJECTIVE To investigate the clinical feature of nosocomial infections in general hospital,based on which effective methods are taken.METHODS An analysis was made on the basis of the survey,the data came from 73 690 hospitalized cases from 2000 to 2004.Bacterium appraise adopted French VITEK-32 fully automatic Expression.RESULTS Their prevalence rate was 5.34%,the main infection site was lower respiratory tract,and followed by upper respiratory tract and urinary tract.Among bacteria isolated from clinical cases,49.24% were G~-,(25.30%) were G~+,and 25.46% were fungi.The proportion of conditional bacteria showed an increasing trend from 2000 to 2004.CONCLUSIONS The conditional bacteria show an increasing trend in the nosocomial(infections.) Strengthening the nosocomial infection management,treating underlying disease actively,lessing(invasive) process,supervising towards bacteria,and using antibacterial drug rationally are effective methods to(reducing) the rate of nosocomial infections.
5.The different effect of sleep quality on health-related quality of life among young and middle-aged people

Objective:To investigate the characteristics of the sleep quality and health-related quality of life (HRQoL), and analyze the different effects of sleep quality on HRQoL among young and middle-aged people.Methods:A cross-sectional study recruited 1 976 participants.All participants completed a self-designed questionnaire for the adults' general condition, the Pittsburgh sleep quality index (PSQI) and Short-Form health survey (SF-36). All participants were divided into 3 age groups: 18-29-year-old group( n=1 148), 30-44-year-old group( n=586) and 45-59-year-old group ( n=242). SPSS 23.0 software was used for statistical analysis.Chi-square test was used to analyze the general characteristics of the three age groups.Non-parametric test was used to analyze the scores of the three age groups in different dimensions of sleep quality. One-way ANOVA was used to analyze the mean scores of the three age groups in different dimensions of HRQoL. Stepwise regression analysis was used to analyze the effect of sleep quality on HRQoL among the three groups after control the confounding factors such as marital status, education, smoking, drinking and exercise habits and past medical history. Results:In terms of sleep quality, the total PSQI scores of 18-29-year-old, 30-44-year-old and 45-59-year-old groups(4(2, 6), 4(2, 6), 4(2, 6)) showed statistically significant differences ( Z=10.951, P=0.004). In terms of HRQoL, there were statistically significant differences in physical component summary scores (18-29-year-old: 82.51±12.62, 30-44-year-old: 80.72±13.63, 45-59-year-old: 82.04±13.07, F=3.667, P=0.026) and mental component summary scores(18-29-year-old: 76.09±15.46, 30-44-year-old: 77.20±16.14, 45-59-year-old: 81.82±14.14, F=13.649, P<0.001) among young and middle-aged people in different age groups.Regression analysis found that daytime dysfunction was an independent influencing factor for HRQoL in young and middle-aged population ( β=-0.308--0.425, all P<0.01). Sleep disorders significantly decreased Physical Component Summary of HRQoL in young-aged people ( β=-0.127--0.215, all P<0.01). The use of hypnotic drugs significantly reduced the scores in the physiological field in the young adults aged 30-44 ( β=-0.076, P<0.05). The duration of sleep significantly decreased the scores in the mental domain of young adults aged 30-44 ( β=-0.112, P<0.01). Subjective sleep quality was an independent factor that significantly decreased HRQoL in young adults aged 18-29 and 30-44 years ( β=-0.089--0.169, all P<0.01). Conclusion:Sleep quality and HRQoL of young and middle-aged people in different age groups show different characteristics.The effect of sleep quality on HRQoL is different among people in different ages.Taking targeted interventions for people of different ages to improve the sleep quality may be an effective way to improve their HRQoL.
6.Efficacy and safety of multiple-dose 5-HT3 receptor antagonists in preventing multi-day-based and highly emetogenic chemotherapy-induced nausea and vomiting

Objective:To evaluate efficacy and safety of multiple-dose tropisetron plus dexamethasone (DXM) versus palonosetron plus DXM for chemotherapy-induced nausea and vomiting. (CINV) in patients received multiple day-based highly emetogenic chemotherapy. Methods:Cancer patients who were receiving multiday-based highly emetogenic chemotherapy were randomly assigned to AB or BA groups. A randomized, cross self-control ed method was applied. Patients in AB group received palonosetron (0.25 mg) 30 min before chemotherapy on day 1 and 3 or additional day 5 in the first cycle;and with tropisetron (5 mg) 30 min before chemotherapy on day 1, 2, and 3, or sup-plementary days (day 4 and 5) in the second cycle. Patients in BA group were treated with tropisetron in the first cycle and with palonosetron in the second cycle. Tropisetron and palonosetron were administered with DXM (10 mg) on day 1, followed by additional doses (5 mg) on days 2 to 5. Palonosetron group comprised patients in the AB group in the first cycle and BA group in the second cycle, whereas tropisetron group included patients in the AB group in the second cycle and BA group in the first cycle. Efficacy and safety of tropisetron versus palonosetron in preventing CINV were evaluated. Results:Ninety-one patients were included in analyses. At day 3, 4, and 5, incidence rates of nausea in the palonosetron group reached 28.6%, 30.8%, and 24.2%, respectively, and those of the tropisetron group totaled 42.8%, 47.3%, and 39.6%, respectively (P<0.05). At day 4, 5, and 6, incidence rates of vomiting in the palonosetron group measured 28.6%, 18.7%, and 5.5%, respectively, and those of the tropisetron group reached 42.9%, 34.1%, and 14.3%, respectively (P<0.05). From day 4 to day 5, day 6 to day 7, and day 1 to day 7, the palonosetron group yielded significantly lower incidence rates of nausea and vomiting than tropisetron group (P<0.05). Rate of rescue treatment in the palonosetron group was lower than that in tropisetron group (13.2%vs. 24.2%, P=0.057). No statistical difference in toxicities was observed between the two groups. Conclusion:Palonosetron plus DXM features better efficacy than that of tropisetron plus DXM against delayed CINV induced by multiple day-based highly emetogenic chemotherapy, which was well tolerated in the two treatments.
7.Analysis of risk factors for cervical cancer in Xiangyuan County, Shanxi Province.

OBJECTIVESTo investigate the risk factors for cervical cancer in the areas of high incidence, and provide evidence for current intervention of cervical cancer.
METHODSIn the areas of Xiangyuan County, Shanxi Provicne with high incidence of cervical cancer, 1 997 women were interviewed using a questionnaire, including baseline information, menstrual, marital and pregnancy histories, sexual behavior, health habits, contraception, medical history and family history of cancer, etc., after its screening with six kinds of methods. All subjects, including 84 cases with pathological diagnosis of greater than cINI, and 1 784 cases with pathological diagnosis of normal, were tested for high-risk HPV.
RESULTSThe overall rates of HPV infection were 20.8% (415/1 997) in high-risk subjects, 97.7% and 14.2% in the cases and control groups, respectively. Univariate analysis showed that risk factors with statistical significance included high-risk HPV infection, age at first sexual intercourse, history of pregnancy and abortion, the number of sexual partners and family history of cancer. Analysis with non-conditional logistic regression model revealed high-risk HPV infection, multiple sexual partners and family history of cancer associated obviously with occurrence of cervical cancer. In addition, there was significantly positive relationship between HPV infection, which increased with the number of sexual partners, and extramarital sexual activity both in males or females.
CONCLUSIONSThe main risk factor for cervical cancer in this region was high-risk HPV infection, which related to sexual behavior, hygienic habits during menstruation and puerperium. It was particularly important to detect and treat precancerous lesions and to implement behavior modification. In addition, further research on genetic susceptibility was suggested.

8.Low-level Laser Therapy Improves Recovery after Exhausting Exercise induced Fatigue: A Randomized, Double-blind and Placebo Controlled Cross-over Study

Objective To explore the effect of low-level laser therapy (LLLT)applied to the quadriceps muscle on the recovery of exhausting-cycling-exercise-induced fatigue.Methods According to a randomised,double-blind and crossover design,16 healthy male students were randomly assigned to an LLLT-1,LLLT-3,LLLT-5 and a placebo group,and received LLLT for 300 s at the dosage of 0.06 J· cm-2,0.18 J·cm-2,0.3 J·cm-2 and 0 to the bilateral rectus femoris after the exhausting-cycling-exercise-induced fatigue.The blood lactate(BL),heart rate(HR),rated perceived exertion(RPE)and visual analogue scale(VAS)were assessed before the exercise,immediately after exercise,10 and 20 min after exercise,as well as immediately after the first Wingate(WG)test,5 and 30 min after the WG test.Meanwhile,the second WG test was performed 40 min after the first WG test.Results The average HR value of LLLT-1 group was significantly lower than the placebo group at 10 min after exercise(P< 0.05)and immediately after the WG test(P<0.01),while that of LLLT-3 and LLLT-5 groups was significantly lower than the placebo group immediately and 5 min after the WG test(P<0.01).Compared to the placebo group,the average BL of LLLT-1,LLLT-3 and LLLT-5 groups was significantly lower 10 min after exercise(P<0.05 for all)and that of LLLT-5 group was also significantly lower 30 min after the first WG test(P<0.05).However,the average blood glucose of LLLT-5 group was significantly higher than the placebo group right after the first WG test(P<0.05).Moreover,significant increase was observed in the mean(P=0.002)and peak power(P=0.006)at the second WG test and the mean(P=0.048) power at the first WG test of LLLT-3 group,compared to the placebo group.Conclusion LLLT applied to quadriceps muscles after exhausting exercise may enhance recovery,and LLLT at the dose of 0.18 J·cm-2 is of the best effect.
9.Research on the expression of aquaporin 9 gene and efficacy of mannitol in neonatal rats with hypoxia-ischemic brain edema

Objective To study the relationship between aquaporin 9 (AQP 9) gene and brain edema in neonatal rats of hypoxic-ischemic brain damage (HIBD) and the therapeutic mechanism of mannitol.Method Healthy and 7-day-old SD rats were randomly assigned into three groups:sham-operated group,HIBD group and mannitol group.Both HIBD and mannitol group were established on HIBD model.The mannitol group was given mannitol intraperitoneally at 0,24,48 h of HIBD.2 ml/kg of 2% Evans blue (EB) were injected intraperitoneally before sacrifice.0,6,12,24,48 and 72 h after HIBD,the outcomes were analyzed including the brain water content,the expression of AQP 9 mRNA measured using RT-PCR and immunofluorescence staining methods,and the permeability of blood-brain barrier (BBB) measured with EB.Result In HIBD group,the brain water content was higher comparing with sham-operated group at 0 h after HIBD(P < 0.05),and gradually increased over time,reaching peak at 48 h (89.3% ± 1.9%) and then decreased.In mannitol group,brain water content started to decrease from 1 h after mannitol administration to the bottom at 12 h (86.5% ±0.6%),then increased to peak at 72 h (87.2% ± 1.7%),and brain water content were decreased during 0 ~ 48 h comparing with HIBD group.HIBD group's EB were higher than sham-operated group (P < 0.05);Mannitol group's EB were decreased comparing with HIBD group (except 0 h,P < 0.05).AQP 9 mRNA expression in the HIBD group was decreased at 0 h,and reached the bottom at 48 h (0.09 ± 0.07).Comparing with sham-operated group,it was higher in the HIBD group at0,6,72 h,and lower (P< 0.05) at 12,24,48 h.Higher AQP 9 mRNA expression were detected in mannitol group than HIBD group and sham-operated group at each time point (with the exception of 48 h) (P < 0.05).AQP 9,which was closely related to water metabolism,were widely found in the pia mater and ependyma using immunofluorescence staining.After ischemia and hypoxia insult,an increasedecrease-increase pattern of AQP 9 expression was found.Conclusion AQP 9 is widely existed in various parts of the brain,influencing brain edema through a variety of pathways.AQP 9 also plays a role in alleviating brain edema in mannitol therapy.
10. Distribution and associated factors of high-risk HPV genotypes infection among HPV-positive women who participated cervical screening test in Shenzhen, 2014-2016, China

Objective:
To analyze the distribution and associated factors of high-risk genotypes of HPV in cervical infection among women in Shenzhen.
Methods:
The information on sociodemographic characteristics and HPV genotypes of HPV-positive women who participated cervical screening test from January 2014 to December 2016 was downloaded from Shenzhen Maternity and Child Healthcare Management Information System. According to the pathogenicity, the high-risk HPV genotypes were divided into 15 types including HPV 16, 18, 31, 33, 35, 39, 45, 51, 52, 53, 56, 58, 59, 66 and 68; and there were 6 low-risk genotypes including HPV 6, 11, 42, 43, 44, and 81. Chi-square tests were applied to compare the proportions of high-risk HPV infection among women who had different sociodemographic characteristics. A non-conditional logistic regression model was used to analyze the associated factors for high-risk HPV infection.
Results:
In total, all HIV positives received HPV genotyping, with an average age of (38.08±9.38) years old. There were 9 979 (93.9%) high-risk and 645 (6.1%) low-risk HPV infections.
