1.Development strategy of cancer hospitals under normalized epidemic prevention and control based on a PEST analysis

Objective:To explore the external factors of the development of cancer hospitals under normalized epidemic prevention and control, and to propose targeted strategies in accordance with existing practice.Methods:PEST model was used to analyze the political, economic, social and technological environment of specialized cancer hospitals, with an overall strategy proposed based on the specific analysis of a cancer hospital.Results:In the era of normalized epidemic prevention and control, cancer hospitals were facing a new environment that was common or unique to other types of medical institutions. In response, the case hospital had adopted such strategies as integrating prevention and control with medical services, integrating offline with online services, and integrating internal and external resources to promote its recovery and development.Conclusions:Cancer hospitals and government should fully leverage the environmental changes incurred by the normalization of prevention and control, and formulate a more integrated development strategy covering short, medium up to long term conditions, based on the two major objectives of epidemic prevention and control and cancer prevention and medical services.
2.Influence of three-dimensional teaching mode based on "Internet plus" on the assessment results, disease grading ability and active learning ability of junior nurses in emergency department

Objective:To explore the influences of three-dimensional teaching mode based on "Internet plus" on the assessment results, disease grading ability and active learning ability of junior nurses in the emergency department.Methods:This study selected 43 junior nurses in the emergency department of our hospital in February 2018-2019, and they were randomized into observation group ( n=22) and control group ( n=21). The control group were given conventional teaching, and the observation group adopted three-dimensional teaching mode based on "Internet plus". The results of examination, disease grading ability, active learning ability and clinical practice ability of the two groups were compared. SPSS 23.0 was used for chi-square test, Fisher exact probability and t test. Results:The case analysis score, daily performance score and total score of the observation group were higher than those of the control group, with statistical significance ( P<0.05). After training, the ability of disease grading of the two groups was both improved, and that of the observation group was significantly higher than that of the control group, with statistical difference ( P<0.05). After training, the scores of self-motivation belief, task analysis, self-monitoring and regulation, self-evaluation, and total score of the two groups were higher than before training, and those of the observation group were higher than those of the control group ( P<0.05). The emergency nursing ability, emergency response ability, critical thinking ability, team cooperation ability and humanistic care of the observation group were higher than those of the control group, with statistical significance ( P<0.05). Conclusion:The three-dimensional teaching mode based on "Internet plus" can improve the teaching effect on the junior nurses in the emergency department, enhance their disease grading ability and active learning ability, and improve their clinical practice ability.
3.Practice and reflection of traumatic orthopedics nursing skills homogenization training

Objective To explore the clinical effects of traumatic orthopedics nursing skills homogenization training in our partner hospitals. Methods Totally 97 orthopedics nurses from 8 partner hospitals of the Sixth People's Hospital Affiliated to Shanghai Jiaotong University were selected to attend the traumatic orthopedics skills homogenization training in four batches between March and June 2018. Training contents included theories and performance standards, clinical practice and examination of nursing skills. Training methods included intensive classroom teaching, group discussion and operation practice. The managers and trainees were investigated with the self-designed questionnaire after the training, which concerned training models, training contents, curriculum, teaching quality and teaching ability of the trainers. Results The satisfaction with the nursing skills homogenization training in the nursing mangers and 97 trainees from the 8 hospitals was 100%;and the 97 trainees' satisfaction with curriculum, training contents and the trainers' teaching ability was (4.88±0.361), (4.93±0.260) and (4.94±0.282), respectively. The nurses' satisfaction with the training scored higher than 4.70 in all dimensions;and the total score of training management was (4.88±0.331). Conclusions The intensive training of traumatic orthopedics nursing skills improves the specialized skills and service abilities of the orthopedics nurses in the hospitals, enable patients to receive homogeneous nursing, and accelerate patients' healing, which is worth promoting in clinical practice.
4.Construction of an early postoperative frailty prevention nursing program for elderly patients with enterostomy

Objective:To construct an early postoperative frailty prevention nursing program for elderly patients with enterostomy, so as to provide a basis for formulating targeted nursing programs.Methods:The overall research period was from July to October 2022. Guided by the frailty integration model and cross-cultural nursing theory, the content framework indicators of the early postoperative frailty prevention nursing program for elderly patients with enterostomy were initially constructed through literature research, semi-structured interviews, and combined with the results of the previous cross-sectional survey. A total of 20 gastroenterostomy and nursing experts from 10 ClassⅢ hospitals in 7 provinces and municipalities were selected through the purposive sampling. Delphi method was used to carry out 2 rounds of consultation with 20 experts to determine the early postoperative frailty prevention nursing program for elderly patients with enterostomy.Results:In the 2 rounds of expert consultations, the effective response rates of the questionnaire were all 100% (20/20), the expert authority coefficients were all 0.95, and the Kendall's W were 0.23 and 0.32 respectively ( P<0.05). The coefficient of variation of indicators at all levels ranged from 0.050 to 0.167. The final early postoperative frailty prevention nursing program for elderly patients with enterostomy included 5 first-level indicators, 23 second-level indicators, and 63 third-level indicators. Conclusions:The early postoperative frailty prevention nursing program for elderly patients with enterostomy is scientific and necessary, and can provide a theoretical basis for nurses to implement frailty prevention and nursing for elderly patients.
5.Summary of best evidence for perioperative nutritional management in elderly patients with hip fracture

Objective:To summarize the relevant evidence of perioperative nutritional management for elderly patients with hip fractures, so as to provide scientific guidance for clinical nursing staff to implement nutritional management.Methods:Literature related to perioperative nutrition management of elderly patients with hip fracture were systematically searched from domestic and foreign guide websites, professional association websites, as well as UpToDate, PubMed, BMJ, Web of Science, Cochrane Library, Embase, CINAHL, Chinese Medical Journal Network, Wanfang Database, CNKI and other databases. The search period was from the establishment of the databases to February 15, 2023. Three researchers conducted quality evaluation and data extraction on the literature.Results:A total of 20 articles were included, including one clinical decision, six guidelines, seven expert consensus and six systematic reviews. After extracting and integrating the evidence included in the literature, a total of 29 best pieces of evidence were finally summarized from eight aspects, such as establishing a multidisciplinary nutrition support team, nutrition risk screening, nutrition assessment and content, nutrition support goals, nutrition support methods, preoperative and postoperative nutrition support, health education and follow-up.Conclusions:This study summarizes 29 best evidence for perioperative nutrition management in elderly patients with hip fractures. Nursing staff can develop scientific and standardized perioperative nutrition management plans for elderly patients with hip fracture based on clinical conditions, improving their nutritional status and clinical prognosis.

Objective To evaluate the 15 years changing trends of prevalence of high risk HPV (HR?HPV) infection and the risks of cervical cancer and precancerous lesions (CIN2+) among a Chinese rural population. Methods The screening cohort with 1 997 women aged 35 to 45 years old was built in 1999 in Xiangyuan County, Shanxi province ( SPOCCS?I) and followed up by cytology and HR?HPV testing in the years of 2005, 2010, and 2014. The changes of HR?HPV prevalence and the risks of cervical precancerous lesions with CIN2+ as the endpoints were analyzed during the past 15 years. Results The detection rates of HPV infection and CIN2+ were 15.7%?22.3% and 1.1%?4.3% for the baseline visit and the other 3 follow?ups, respectively. The cumulative risk of CIN2+ in HR?HPV positive women at baseline was significantly higher than HR?HPV negative women ( P<0.01) during the 15?year follow?up. The risk of CIN2+ in the four?times HPV positive group was 40. 0%, while the group with four?times negative HPV results was 0.6% (Adjusted RR = 55.0, 95% CI: 11.3 to 268.4). Conclusions The prevalence of HR?HPV infection and CIN2+ lesions were high in Xiangyuan county during the 15 years. HR?HPV positivity elevated the risk of CIN2+ compared to women whose HR?HPV test was negative. The risks of CIN2+incidence in 6 years were low among women with negative HR?HPV test. The risk of CIN2+ increased with the numbers of HPV infection events. The screening interval could be extended to 5?6 years.

8.Genotype distribution and behavioral risk factor analysis of human papillomavirus infection among Kazak women in Xinjiang Uygur Autonomous Region

Objective:To investigate the prevalence and genotype distribution of high-risk human papillomavirus (HR-HPV) and the correlation between cervical lesions and analyze the risk factors for HR-HPV infection.Methods:In June 2018, a population-based study for cervical cancer screening in Tuoli county of Xinjiang Uygur Autonomous Region was conducted. A total of 2 000 Kazak women aged 25-64 years were included in the study. Three cervical exfoliated cells samples were collected from them for careHPV, PCR HPV, p16 INK4a, and liquid-based cytology (LBC) tests. Women with any positive test were referred for colposcopy with biopsies taken at abnormal sites. Histo-pathological diagnoses were used as the gold standard. Results:The overall prevalence of HPV was 14.55%, among which the infection rate of HR-HPV was 12.90%, which was even higher in the 50-54 years age group. The most prevalent genotypes of HR-HPV were HPV16 (2.80%), HPV51(2.35%), HPV52 (1.70%), HPV56 (1.50%), and HPV39 (1.20%). The most common HPV infection was a single infection (71.48%). In the age group of 50-54 years, the multiple infection rates were higher, with the majority of double infection (69.88%), and HPV42 and 56 were the most common co-infection types. HPV16 (31.82%), HPV51 (27.27%) and HPV18 (13.64%) were higher in cervical intraepithelial neplasia grade 1, HPV16 (57.14%) was higher in cervical intraepithelial neplasia grade 2, and HPV16 (55.56%) and HPV18 (33.33%) were higher in cervical intraepithelial neplasia grade 3 or worse. Results from the multivariate logistic regression analysis showed that higher education, menopause, and syphilis infection increased the HPV infection.Conclusions:The most common prevalence genotypes of HR-HPV among Kazak women were HPV16, HPV51, and HPV52. The infection rate of HR-HPV among Kazak women was correlated with education level, menopausal status, and syphilis infection. Measures should be taken targeting high-risk factors. This result suggests that STD patients and women aged 50 and above should be encouraged for screening.

Hip fracture in the elderly is characterized by high incidence, high disability rate, and high mortality and has been recognized as a public health issue threatening their health. Surgery is the preferred choice for the treatment of elderly patients with hip fracture. However, lower extremity deep venous thrombosis (DVT) has an extremely high incidence rate during the perioperative period, and may significantly increase the risk of patients′ death once it progresses to pulmonary embolism. In response to this issue, the clinical guidelines and expert consensuses all emphasize active application of comprehensive preventive measures, including basic prevention, physical prevention, and pharmacological prevention. In this prevention system, basic prevention is the basis of physical and pharmacological prevention. However,there is a lack of unified and definite recommendations for basic preventive measures in clinical practice. To this end, the Orthopedic Nursing Professional Committee of the Chinese Nursing Association and Nursing Department of the Orthopedic Branch of the China International Exchange and Promotive Association for Medical and Health Care organized relevant nursing experts to formulate Expert consensus on perioperative basic prevention for lower extremity deep venous thrombosis in elderly patients with hip fracture ( version 2024) . A total of 10 recommendations were proposed, aiming to standardize the basic preventive measures for lower extremity DVT in elderly patients with hip fractures during the perioperative period and promote their subsequent rehabilitation.
