1.CXCL16 deficiency attenuates STZ-induced diabetic nephropathy in mice

AIM:To explore the effect of CXCL16 deficiency on streptozocin ( STZ)-induced diabetic nephrop-athy in mice.METHODS:CXCL16 knockout ( C16 KO) mice (8 years old) were used to build up diabetes model by treating with STZ.Age-and gender-matched wild-type ( WT) C57BL/6J mice treated with STZ were used as control.All mice were fed with chow diets for 12 weeks, and the development of diabetic nephropathy was evaluated.RESULTS:Compared with the WT mice treated with STZ, C16 KO mice treated with STZ presented lower fasting glucose levels and better glucose tolerance power.C16 KO mice treated with STZ also had lower urine protein levels and smaller areas of glo-merular injury as compared with WT mice treated with STZ.Furthermore, CXCL16 deficiency decreased the contents of re-nal reactive oxygen species ( ROS) , malondialdehyde ( MDA) and oxidized low-density lipoprotein ( ox-LDL) and the mR-NA expression of lectin-like oxidized low-density lipoprotein receptor 1 (Lox-1), and attenuated the expression of renal in-flammatory factors including tumor necrosis factor α( TNF-α) and interleukin 6 ( IL-6) , as well as chemokines including intercellular cell adhesion molecular 1 (ICAM-1) and chemokine C-X-C motif ligand 1 (CXCL1).CONCLUSION:CX-CL16 deficiency obviously inhibits the development of STZ-induced diabetic nephropathy in mice.

In the past decade, the role of the mevalonic acid pathway and its inhibitor statins in preventing tumorigenesis and improving tumor prognosis has attracted wide attention; however, so far, there are still controversies over the role of statins in liver cancer. Previous studies have shown that statins play an important role in cholesterol metabolism, antioxidation, and anti-inflammation, and cholesterol metabolism, oxidative stress, and inflammatory response are important factors to promote the development and progression of liver cancer. However, some recent studies have found that statins have an adverse effect on the treatment of some liver cancer patients. This article elaborates on the role of statins as a “double-edged sword” in the treatment of liver cancer and related molecular mechanisms.
3.Development of an fall prevention early warning bracelet based on red dot indication system and evaluation of its clinical application

Objective:to provide a new type of fall prevention early warning bracelet and share the feedback of clinical preliminary application data in order to reduce the incidence of falls in inpatients.Methods:The convenience sampling method was used to select the inpatients who were hospitalized in the Department of Geriatrics of Hunan Provincial People ′s Hospital from January to February 2021. The 30 patients who did not use bracelets in November 2020 were taken as the routine group and 30 patients who used bracelets in December 2020 were taken as the experimental group. The two groups were monitored during hospitalization and 2 weeks after discharge. The patients in the routine group were given routine anti-fall signs and health education, while the patients in the experimental group were given the use and health education of early warning bracelets on the basis of routine. The incidence of fall was recorded. The inpatients ′ nursing job satisfaction scale and the self-designed bracelet use feedback questionnaire (experimental group) were used to evaluate the application effect. Results:No fall occurred in the experimental group, but 4 patients fell in the routine group (13.3%). The incidence of fall in the experimental group was significantly lower than that in the routine group ( χ2=4.29, P=0.038). The scores of all dimensions of nursing satisfaction in the experimental group were also higher than those in the routine group ( t values were -8.20--4.05, all P<0.05). Another 26.7% (8/30) of patients said that wearing a bracelet would have a certain impact on hand movement or rest, but all the 30 patients surveyed indicated that they were willing to continue to wear the bracelet. Three nurses said it would not necessarily reduce the incidence of falls, but all the 10 nurses involved said the shift was more convenient. Conclusions:The preliminary clinical trial feedback of the bracelet is good, and the patients and their families have high recognition, which is helpful for the patients and their families to participate in the fall prevention activities and improve the self-management of the patients.

"Living the old age life with multimorbidity" has become the normal life of the elderly. How to realize the management of multimorbidity in the context of the combination of medical and health care is a hot topic of geriatrics, and also one of the important measures to cope with the national health strategy of "all-round and full-cycle protection of people's health" put forward in "Healthy China 2030". On the basis of literature search, this paper summarizes the research status and progress of the multimorbidity in elderly patients, so that it is expected to play a certain promoting role in the comorbidity management of chronic disease for the elderly in China.
5.Clinical observation of telbivudine's antiviral efficacy and protection against mother-to-infant transmission of chronic hepatitis B during the first trimester of pregnancy.

OBJECTIVETo explore the antiviral efficacy, safety and protective ability against mother-to-infant transmission of telbivudine in pregnant patients with chronic hepatitis B (CHB) during the first trimester.
METHODSEighty four gravid women who were diagnosed with CHB, in their first trimester of pregnancy, and had refused to terminate their pregnancies were enrolled; all study participants were clinically classified as active hepatitis cases with positivity for both hepatitis B surface antigen (HBsAg) and hepatitis B e antigen (HBeAg), HBV DNA more than or equal to 107 copies/mL and serum level of alanine aminotarnsferase (ALT) of more than or equal to 4 ULN.Patients with YMDD mutations were excluded from the study. The study participants were divided into a telbivudine treatment group (n=43; administered in the first trimester of pregnancy) and a control group (n=41, consisting of patients who refused to take antivirals). All babies bom to the women in both groups of the study received standard immune prevention (anti-hepatitis B immunoglobulin plus hepatitis B vaccine) and artificial feeding.Data recorded for the women during pregnancy included clinical findings for tests of hepatic and renal function, myocardial enzymes, blood and urine clinical parameters, hepatitis B virus makers and HBV DNA, as well as notation of any adverse reactions. The neonates were evaluated for presence of HBV infection, parameters of growth and development, presence of complications, and Apgar score. At 6 and 12 months old, all infants were evaluated for HBV DNA level and HBsAg presence.
RESULTSThe genetic variant rtM204I was detected in one of the women in the treatment group at 36 weeks of pregnancy. One woman in the control group developed severe hepatitis at 28 weeks of pregnancy and was put on the telbivudine treatment The treatment group showed greater recovery rates of ALT than the control group at 12 weeks of pregnancy (62.8% vs.29.3%, P=0.002), 24 weeks of pregnancy (76.7% vs.46.3%, P=0.000), and at ante partum (88.1% vs.60.0%, P=0.004). The treatment group also showed greater HBV DNA-negative conversion rates at 12 weeks of pregnancy (20.9% vs.0, P=0.006), at 24 weeks of pregnancy (37.2% vs.0, P=0.001) and at ante partum (78.6% vs.0, P=0.000), and greater HBeAg seroconversion rates at 12 weeks of pregnancy (2.3% vs.0, P=1.000), at 24 weeks of pregnancy (9.3% vs.0, P=0.116) and at ante partum (2 1.4% vs.0, P=0.002). The HBsAg-positive rates and HBV DNA-positive rates among the infants born to the mothers in the treatment and control groups, respectively, were 2.4% vs.17.5% (P=0.027) at birth, 0 vs.17.5% (P=0.005)at 6 months old and 0 vs.17.5% (P=0.005) at 12 months old. The Apgar scores were not significantly different for the children born to the mothers from the two groups, and all the children showed parameters of growth development within normal limits.
CONCLUSIONTelbivudine administration in the first trimester had a good antiviral curative effect and effectively blocked mother-to-infant transmission in women with CHB. The treatment was safe, causing no obvious adverse reaction in the gravid women or developmental effects on the infants.

6.Effects of a Modified Six-Sigma-Methodology-Based Training Program on Core Competencies in Rehabilitation Nurse Specialists

Purpose:
Nurses play an important role in ensuring patient rehabilitation and are involved in all aspects of multidimensional rehabilitation.Therefore, strengthening rehabilitation nursing education is vital to ascertain high-quality rehabilitation and optimum outcomes. This study examined the effectiveness of a new teaching reform—a modified Six-Sigma-based training program—against a conventional educational program on rehabilitation specialist nurses’ core competencies, post-training performance, and satisfaction.
Methods:
A quasi-randomized controlled trial was conducted to assess the effectiveness of the modified training program. We recruited 56 learners from the 2020 training course at the Hunan Rehabilitation Specialist Nurse Training Base as the control group. Sixty learners from the base’s 2021 training course were recruited as the intervention group. Data were collected in a consistent manner from both groups after the training program was implemented.
Results:
Those who underwent the modified training program showed better improvement in all core competencies than those who underwent the conventional training program (p < .05); the scores for theoretical knowledge, clinical nursing lectures, reviews, and nursing case management improved significantly following the teaching reform (p < 0.05). Further, overall satisfaction as well as base management and theoretical teaching satisfaction improved significantly (p < .05).
Conclusion
The modified training program strengthens rehabilitation nurses’ base management abilities; enhances their core competencies; expands their interest in and breadth, depth, and practicability of theoretical courses; and updates the teaching methods.
7.Application value verification of fecal SDC2 gene methylation detection in intestinal cancer screening

Objective To verify the diagnostic efficiency and application value of fecal syndecan 2(SDC2)gene methylation detection in intestinal tumor screening.Methods The clinical data of 1 456 patients with fecal SDC2 gene methylation detection in this hospital from November 2021 to December 2023 were ana-lyze retrospectively.The detection positive rate,colonoscopic compliance,sensitivity,specificity,positive pre-dictive rate and negative predictive rate were analyzed.The pathological diagnosis served as the gold standard.The receiver operating characteristic(ROC)curve and the area under the curve(AUC)were used to judge the diagnostic effect.Results In the results in 1 456 cases of fecal SDC2 gene methylation detection,90 cases were positive with a positive rate of 6.2%.The positive rate had no statistical difference between different sexes(P>0.05).The positive rate in the patients ≥50 years old was higher than that in the patients<50 year old(P<0.05).Among 90 cases of detection results positive,67 cases completed the enteroscopic examination and the enteroscopic compliance rate was 74.4%.The enteroscopic compliance rate had no statistical difference be-tween the different sexes and among different ages of patients(P>0.05).Among 67 cases of enteroscopic ex-amination completion,there were 6 cases(9.0%)of colorectal cancer,17 cases(25.4%)of progressive stage adenoma,15 cases(22.4%)of non-progressive stage adenoma,6 cases(9.0%)of non-adenomatous polyp and the lesion detection rate was 65.7%.Among 112 cases of fecal SDC2 gene methylation detection negative,there were 2 cases(1.8%)of progressive stage adenoma and 22 cases(19.6%)of non-progressive stage ade-noma.The sensitivity and specificity of this detection for colorectal cancer and progressive stage adenoma were 92.0%and 71.4%,respectively,which had obvious diagnostic significance for colorectal tumor(AUC=0.721,P<0.001).Conclusion The fecal SDC2 gene methylation detection has an important clinical value in the preliminary screening of colorectal cancer.
8.Application of MOOC teaching in new entry nurses standardized training of respiratory medicine

Objective To explore the effects of massive open online courses (MOOC) teaching in new entry nurses standardized training of respiratory medicine. Methods A total of 33 new entry nurses of respiratory medicine at a hospital were selected as control group from 2013 to 2014 with the routine training method. A total of 34 new entry nurses were chosen as intervention group from 2015 to 2017. Nurses of intervention group accepted standardized training with the MOOC teaching method by formulating systematic online and offline training course. The training effects of two groups were evaluated. Results The specialized theoretical achievements and skill achievements of nurses were (72.68±9.44) and (75.78±10.32) in intervention group, and (67.33±8.89) and (62.64±5.8) in control group with significant differences (P< 0.05). After intervention, the degree of satisfaction of patients in intervention group was higher than that in control group with a significant difference (χ2=10.823, P< 0.05). Conclusions MOOC teaching can effectively improve the level of specialized skill in new entry nurses, contribute to linking theory with practice, fast role switching and improve the occupational satisfaction.
9.Effects of early ambulation on rehabilitation of patients after hepatectomy: a Meta-analysis

Objective:To systematically evaluate the effect of early ambulation on the rehabilitation of patients after hepatectomy.Methods:Computer was used to search randomized controlled trial (RCT) literature on the effect of early ambulation on the rehabilitation of patients after hepatectomy in Chinese and English databases, such as China National Knowledge Infrastructure (CNKI) , Wanfang Data, VIP, China Biology Medicine, Ovid, PubMed, Web of Science, Embase, Cochrane Library. The retrieval time was from the establishment of database to May 10, 2020. RevMan 5.3 software was used to conduct Meta-analysis on literature that met the inclusion criteria.Results:A total of 8 RCT articles were included, with a total sample size of 936 cases, including 474 cases in intervention group (early ambulation) and 462 cases in control group (routine activity care) . The results of the systematic review showed that compared with control group, intervention group shorted the first exhaust time [weighted mean difference ( WMD) =-21.01, 95% confidence interval ( CI) (-25.98, -16.04) , P<0.05]and first defecation time [ WMD=-31.97, 95% CI (-54.69, -9.25) , P<0.05]after surgery, decreased gastrointestinal discomfort [relative risk ( RR) =0.44, 95% CI (0.34, 0.56) , P<0.05]and complications [ RR=0.76, 95% CI (0.56, 1.02) , P<0.05]after surgery, and had earlier time to early ambulation [ WMD=-20.64, 95% CI (-21.71, -19.56) , P<0.05], the differences were statistically significant. Conclusions:It is safe and effective to implement early ambulation in patients after hepatectomy, which is beneficial to the recovery of gastrointestinal function of patients, reduces the occurrence of gastrointestinal discomfort and complications, and promotes the recovery of patients.
10.Meta-synthesis of qualitative research on pulmonary rehabilitation experience in COPD patients

Objective:To integrate qualitative research on the experience of pulmonary rehabilitation in patients with chronic obstructive pulmonary disease (COPD) , to understand the experience of patients with pulmonary rehabilitation and its influencing factors, so as to provide evidence support for pulmonary rehabilitation.Methods:Qualitative research on pulmonary rehabilitation experience of COPD patients was retrieved by computer in 8 databases including the Cochrane Library, Web of Science, Embase, PubMed, CINAHL, China National Knowledge Infrastructure, WanFang and VIP. The retrieval time limit was from the establishment of the database to August 20, 2021. The quality of the articles those met the requirements were evaluated by the quality evaluation criteria for qualitative research of the Australian Joanna Briggs Institute Evidence-Based Health Care Center, and the results of each study were further integrated and analyzed.Results:A total of 12 articles were included, and 47 clear topics were extracted, and similar topics were summarized into 7 new categories. A total of 3 integrated results were synthesized, namely rehabilitation efficacy, pulmonary rehabilitation motivation, and pulmonary rehabilitation obstacles.Conclusions:COPD patients who can persist in training perceive the benefits of pulmonary rehabilitation, and both the pulmonary rehabilitation environment and interpersonal support are factors that promote their pulmonary rehabilitation. The patients' own condition and transportation and economic difficulties are the main obstacles. Therefore, the patients' physical and economic status should be comprehensively evaluated during pulmonary rehabilitation, and the most suitable pulmonary rehabilitation program should be selected.
