1.Disease onset regions and spreading patterns in sporadic amyotrophic lateral sclerosis and related influencing factors

Objective To evaluate the disease onset regions and spreading patterns in sporadic amyotrophic lateral sclerosis (ALS)patients and related influencing factors.Methods We performed a prospective analysis of 1 58 ALS patients.The disease-onset was confirmed according to the patients’self-reports,neurological examination results and electromyogram study.We followed up 1 5 1 patients with the second or other affected body regions during the disease progression.Data were analyzed according to the different groups of onset regions.Results 1.In 94.3% (149/1 58)of the patients,the early motor manifestations were focally in the bulbar,upper or lower limbs.2.The region of onset was associated with signs of lower motor neuron (LMN)and upper motor neuron (UMN)involvement (P = 0.000 ).The LMN involvement was more distinctive in patients with bulbar onset (65.4%,1 7/26 )group.Patients with cervical onset more frequently showed pure LMN (47.9%,45/94 )or concomitant UMN (52.1%,49/94)signs in the affected limbs.The highest proportion of UMN and LMN signs in the affected lower limb was found in the lumbar onset (83.8%,31/37 )group.3.Spreading patterns:Rostral to caudal spreading pattern was more frequent in bulbar onset patients.For patients with limb onset,there were significant differences between spreading patterns and disease-onset regions (P =0.04).Circular (31.5%,28/89),horizontal (30.3%,31/89)and vertical (21.3%,1 9/89)spreading patterns were more frequent in cervical onset patients whereas circular (47.2%,1 7/36)spreading patterns were more frequent in lumbar onset patients.4.There was a strong association between the rate of progression and age of disease onset (P =0.01 1).Patients aged over 60 had a faster progression.Conclusion ALS is a focal process at motor axis along the spinal cord and cerebral cortex.Different disease-onset can cause different distribution of UMN and LMN signs.Therefore,special attention should be paid to the signs of disease-onset clinically.ALS does start focally and spreads to adjacent regions.Elder patients have a faster disease progression.
2.Antioxidation of Picroside Ⅱ on Cerebral Ischemic Injury in Rats and Its Optimum Dosage and Time Window

Objective To optimize the therapeutic dose and time window of picroside Ⅱ in treating cerebral ischemic injury in rats by orthogonal test. Methods The forebrain ischemia models were established by bilateral common carotid artery occlusion (BCCAO) method. The successful models were randomly grouped according to orthogonal experimental design and treated by injecting picroside Ⅱintraperitoneally at different ischemic time with different doses. The concentrations of MDA, NO and H2O2 in serum and brain tissue were respectively determined by thiobarbituric acid assay, nitratase reductase assay and chemiluminescence immunoassay. Results The optimized composition of the therapeutic dose and time window of picroside Ⅱ in cerebral ischemic injury were ischemia 1.5 h with 10 mg/kg, 1.5 h with 20 mg/kg and 1.5 h with 10 mg/kg body weight according to the expressions of MDA, NO and H2O2 in serum, and ischemia 1.5 h with 10 mg/kg, 1.5 h with 20 mg/kg and 1.5 h with 20 mg/kg body weight according to the expressions of MDA, NO and H2O2 in brain tissue. Conclusion On the basis of the principle of lowest therapeutic dose with longest time window, the optimized composition of the therapeutic dose and time window in cerebral ischemic injury is injecting picroside Ⅱ intraperitoneally with 10-20 mg/kg body weight at ischemia 1.5 h.
3.Application of capillary electrophoresis in the diagnosis and differential diagnosis of benign and malignant monoclonal gammopathies

Objective:To evaluate the value of capillary electrophoresis in the diagnosis and differential diagnosis of benign and malignant monoclonal gammopathies (MGs).Methods:A retrospective analysis of the capillary electrophoresis test results of 2 445 newly diagnosed patients at the Affiliated Hospital of Qingdao University from January 2016 to June 2020 was carried out. Capillary zone electrophoresis and immunosubtractive assay were used to detect serum monoclonal protein (MP). The clinical diagnosis and other information of the patients were collected from the clinical database of the Affiliated Hospital of Qingdao University. Kruskal-Wallis rank sum test was used to compare the different amount of monoclonal protein among multiple groups. Receiver operator characteristic curve (ROC) was used to analyze the diagnostic sensitivity and specificity of the monoclonal protein of each type. Youden index was used to calculate the cut-off values.Results:Among the 2 445 patients, 1 183 were positive for monoclonal protein, of which 944 cases were diagnosed as malignant MG, 174 were monoclonal gammopathy of undetermined significance (MGUS), and 65 cases were monoclonal gammopathy of renal significance (MGRS). The percentages of M protein types from high to low is immunoglobulin G(IgG)-κ, IgG-λ, IgA-λ, IgA-κ, free λ light chain, free κ light chain, IgM-κ, double clone, and IgM-λ. The levels of MP of IgG, IgA, IgM and FLC in the malignant MG group were all higher than those in the MGUS group, with statistical significance( P<0.01). The MP levels of IgG and IgA types in malignant MG group were higher than that in MGRS group ( P<0.01). ROC curve analysis showed that the MP of IgG, IgA, IgM and FLC types had good diagnostic efficacy for malignant MG ( P<0.01), and their AUC values were 0.947 (95 %CI 0.926-0.968), 0.930 (95 %CI 0.895-0.966), 0.844 (95 %CI 0.722-0.967) and 0.865 (95 %CI 0.781-0.950), respectively. For IgG, the cut-off value was 14.24 g/L, and the diagnostic sensitivity and specificity were 88.5% and 90.1%, respectively. The cut-off value of IgA was 8.88 g/L, and the sensitivity and specificity of IgA were 87.9% and 81.4%, respectively. For IgM, the cut-off value was 26.93 g/L, and the sensitivity was 64.4% and the specificity was 90.9%. For FLC, the cut-off value, diagnostic sensitivity, and specificity was 7.08 g/L, 85.9%, and 77.8%, respectively. Conclusions:Capillary electrophoresis immunotyping technique can be used to diagnose malignant MG such as multiple myeloma (MM) and non-Hodgkin′s lymphoma (NHL), as well as to screen and track diseases like MGUS and MGRS. Serum MP level can be used to distinguish malignant MG from benign MG effectively.
4.The observation on the effect of Buyang Huanwu Decoction combined with suspension exercise training for the patients with cerebral infarction and lower limb hemiplegia

Objective:To explore the curative effect of Buyang Huanwu Decoction combined with suspension exercise training for the patients with cerebral infarction combined and lower limb hemiplegia.Methods:According to random number table method, 94 patients with cerebral infarction and lower limb hemiplegia meeting the inclusion criteria were divided into the control group and the observation group between January 2017 and February 2021, 47 in each group. The control group was treated with suspension exercise training, while observation group was additionally treated with Buyang Huanwu Decoction on the basis of the control group treatment. All were treated for 6 weeks and then followed up for 3 months. Before and after treatment, the Traditional Chinese Medicine (TCM) syndromes scores were recorded. The severity of nerve function injury was evaluated by National Institutes of Health Stroke Scale (NIHSS). The lower limb function was evaluated by Fugl-Meyer Assessment (FMA). The balance function was evaluated by Berg Balance Scale (BBS). The activities of daily life were assessed by Barthel Index (BI). The whole blood high shear viscosity, plasma viscosity and hematocrit were detected by full-automatic hemorheology analyzer. The levels of plasma total cholesterol (TC), triglyceride (TG) and low-density lipoprotein cholesterol (LDL-C) were detected by full-automatic analyzer. All were followed up for 3 months. The prognosis of patients was assessed by modified Rankin scale (mRS). The adverse reactions during treatment were recorded.Results:After treatment, scores of TCM syndromes and NIHSS in observation group were significantly lower than those in control group ( t=5.35, 4.54, P<0.01), while scores of FMA, BBS and BI were significantly higher than those in control group ( t=3.40, 3.10, 7.57, P<0.01). The whole blood high-shear viscosity, plasma viscosity and hematocrit in observation group were significantly lower than those in control group ( t=2.94, 3.81, 4.23, P<0.05 or P<0.01), and levels of TC, TG and LDL-C were significantly lower than those in control group ( t=4.10, 4.27, 3.61, P<0.01). The differences in good prognosis rate between observation group and control group were statistically significant [74.47% (35/47) vs. 51.06% (24/47); χ2=5.51, P=0.019]. Conclusion:The Buyang Huanwu Decoction combined with suspension exercise training can relieve clinical symptoms, recover lower limb function, improve activities of daily life and prognosis in cerebral infarction combined with lower limb hemiplegia.
5.Analysis of styryllactones from Goniothalamus cheliensis by UPLC-Q-TOF-MS.

OBJECTIVETo analyze the styryllactone components in Goniothalamus cheliensis Hu (Annonaceae).
METHODUPLC-Q-TOF-MS was used to identify the main styryllactone components in G. cheliensis. The chromatographic separation was performed on ACQU ITY UPLC BEH C18 column and eluted by actonitrile and 0.1% acetic acid in water gradiently. The mass spectrometer equipped with electrospray ionization souce was used as detector under the positive ion modes.
RESULTTwelve styryllactons were identified based on their MS data and published literatures, and the MS fragmentation regularity of the styryllactones was also proposed.
CONCLUSIONIt is an accurate and effective method to obtain the structural information of styryllactones.

6.Expression changes of miRNAs and EMT-related genes in human mesothelial cells induced by long-term exposure to asbestos

Objective:To investigate the effects of long-term exposure to chrysotile and crocidolite on miRNAs and epithelial mesenchymal transformation (EMT) -related gene expression in human pleural mesothelial cells.Methods:In November 2020, fluorescence quantitative polymerase chain reaction (RT-qPCR) was used to detect the expressions of EMT-related genes in human pleural mesothelioma cells (NCl-H2052 cells, NCl-H2452 cells) and human normal mesothelial cells (Met-5A cells). MiRNAs with abnormal expression in human pleural mesothelioma cells were screened out from the previous miRNA chip data of research group, and target genes of differentially expressed miRNAs were predicted using miRWalk database (http: //mirwalk.umm.uni-heidelberg.de). RT-qPCR was used to verify the abnormal expression of EMT-related miRNAs in cell lines. Met-5A cells were treated with 5μg/cm 2 chrysotile and crocidolite respectively for 48 h a time, once a week and a total of 10 times. Chrysotile group, crocidolite group and control group were set up. And the control group was added with the same volume of PBS. The expression changes of EMT-related genes and abnormal expression miRNAs in each group were detected by RT-qPCR. The differences among the groups were compared by one-way ANOVA, and the differences between the control group and the experimental group were compared by dunnet- t test. Results:Compared with Met-5A cells, the expression levels of Vimentin and Twist genes were increased, and the expression level of E-cadherin genes was decreased in NCl-H2052 cells and NCl-H2452 cells ( P<0.001). Target genes of miRNAs with abnormal expression in miRNA chip were predicted, and the results showed four abnormally expressed miRNAs associated with EMT and verified the expression of these four miRNAs in the cell lines. Compared with Met-5A cells, the expression level of hsa-miR-155-5p was increased in NCl-H2052 cells and NCl-H2452 cells, the expression levels of hsa-miR-34b-5p, hsa-miR-34c-5p and hsa-miR-28-5p were decreased in NCl-H2052 cells and NCl-H2452 cells ( P<0.001), which was consistent with the results of chip analysis. After exposure of Met-5A cells, it was found that compared with the control group, the expression levels of Vimentin and Twist genes, hsa-miR-155-5p, hsa-miR-34b-5p and hsa-miR-34c-5p in the crocidolite group were increased, while the expression level of E-cadherin gene was decreased ( P<0.05). Compared with the control group, the expression levels of Vimentin, Twist and E-cadherin genes in chrysotile group were increased, while the expression levels of hsa-miR-34b-5p, hsa-miR-34c-5p and hsa-miR-28-5p were decreased ( P<0.05) . Conclusion:Long-term exposure to chrysotile and crocidolite could cause Met-5A cells to produce miRNAs and EMT-related gene expression changes similar to mesothelioma cells.

8.Assessment and management of post stroke fatigue:a summary of best evidence

Objective To retrieve evidences on the assessment and management of post-stroke fatigue and summarise the best evidences.Method Evidence-based nursing was employed to systematically retrieve the literatures on assessment and management of post-stroke fatigue from both foreign and domestic databases,including BMJ Best Practice,UpToDate,Joanna Briggs Institute(JBI)Evidence-Based Health Care Centre Database,American Guide Network,Scottish Intercollegiate Guide Network,Medlive,Cochrane Library,PubMed,CNKI and Wanfang Data.Joanna Briggs Institute(JBI)critical appraisal tool was used to assess the evidences retrieved from the databases.Results A total of two evidence-related summaries,five clinical practice guidelines and twenty systematic reviews were included.After evidence extraction and aggregation,three evidence-related themes were identified:assessment,behavioral intervention and health education and they comprised 18 pieces of best evidence on post-stroke fatigue.Conclusion This study summarised the best evidences for the assessment and management of post-stroke fatigue in patients.The best evidences provide nursing managers with valuable references in conducting evidence-based post-stroke nursing hence to prevent post-stroke fatigue and improve the quality of nursing care.
9.Relationship between parenting rearing behaviors and adolescent depression: the mediating role of rumination

BackgroundSeveral studies have confirmed that there is a relationship among parenting rearing behaviors, adolescent depression and rumination, moreover, rumination has been found to mediate the relationship between overall parenting rearing behaviors and adolescent depression, while little is known about the mediating role of rumination in the relationship between each dimension of parenting rearing behaviors and adolescent depression. ObjectiveTo explore the mediating role of rumination in the relationship between each dimension of parenting rearing behaviors and adolescent depression, so as to provide reference for the prevention of adolescent depression. MethodsIn March and April 2022, a stratified random sampling technique was utilized to select 302 students in grades 7 to 12 at a middle school in Ningxian county, Gansu Province as the research subjects. The Beck Depression Inventory-II of Chinese Version (BDI-II-C), short-form Egna Minnen av Barndoms Uppfostran (S-EMBU) and Ruminative Response Scale (RRS) were administered to all subjects. Pearson correlation coefficients were calculated. Amos 24.0 was used to check the presence of mediation effect. ResultsA total of 302 (94.08%) completed valid questionnaires. Correlation analysis indicated that the scores of both father's and mother's emotional warmth dimension in the S-EMBU were negatively correlated with the BDI-II-C scores (r=-0.424, -0.297, P<0.01), RRS total score (r=-0.347~-0.175, P<0.01) and RRS each factor score (r=-0.179~-0.285, P<0.01); the scores of both father's and mother's rejection dimension in the S-EMBU were positively correlated with the BDI-II-C scores (r=0.355, 0.248, P<0.01), RRS total score (r=0.262~0.358, P<0.01) and RRS each factor score (r=0.274~0.339, P<0.01); the scores of both father's and mother's overprotection dimension in the S-EMBU were positively correlated with the BDI-II-C scores (r=0.286, 0.245, P<0.01), RRS total score (r=0.175~0.333, P<0.01) and RRS each factor score (r=0.150~0.255, P<0.01). Rumination might mediated the relationship of father's emotional warmth, father's overprotection, mother's overprotection, and mother's rejection with adolescent depression, the effect value were -0.110, 0.221, -0.121, 0.136, and the effect size were 36.07%, 100.00%, 100.00%, 100.00%. ConclusionRumination may play a mediation role in the relationship of father's emotional warmth, father's overprotection, mother's overprotection, and mother's rejection with adolescent depression. 10.Hospitalization rates for influenza?associated severe acute respiratory illness in children younger than five years old in Suzhou of China, 2016-2018

We analyzed the influenza surveillance data of Children′s Hospital of Suzhou University from 2016 to 2018 and estimated the hospitalization burden of children under 5 years old due to influenza infection in Suzhou. The results showed that the influenza virus positive rate of 1 451 severe acute respiratory infection (SARI) cases in Children′s Hospital of Suzhou University was 13.6% (95%CI :11.8%?15.3%;197 cases), among which the influenza pandemic intensity in 2017?2018 was relatively high, and A/H1N1 was the main pandemic virus. It was estimated that the hospitalization rate of influenza?related SARI in children under 5 years old in Suzhou was 6.9‰ (95%CI : 6.6‰?7.2‰), among which the hospitalization rate of children aged<6 months was higher, up to 11.4‰(95%CI: 9.9‰?12.8‰).
