In recent years, remote ischemic preconditioning is found as a novel way to protect the brain, which may be achieved through nerve pathways, humoral factors or nerve-humoral interaction. The molecular mechanisms for the protection might be related to mitochondria ATP-sensitive potassium channel, mitogen-activated protection kinase, mammalian target of rapamycin, including nitric oxide synthase and cannabinoid receptors. The aim of this review is to explain the mechanism of action of remote ischemic preconditioning on brain protection, as well as the possible direction of clinical application.

OBJECTIVE: To analyze the use situation and trend of cardiovascular and cerebrovascular drugs during 2007～2009 in a military sanatorium, and provide reference for the rational use of cardiovascular and cerebrovascular drugs and its essential medicines. METHODS: Cardiovascular and cerebrovascular drugs in the rehabilitation center during 2007～2009 were analyzed statistically in the respect of category, consumption sum and DDDs. RESULTS: The consumption sum and DDDs of cardiovascular and cerebrovascular drugs were increasing in the past three years. And its essential drugs account for 94.8%.The most widely used drugs were cerebrovascular drug, brain metabolism drug, nootropics and antihypertensive drugs. Oxiracetam and Xueshuantong were widely used cerebrovascular drug, brain metabolism drug and nootropics. Amlodipine were widely used antihypertensive drugs in the clinic. CONCLUSION: Cardiovascular and cerebrovascular drugs and its essential drugs were characterized with complex classification, numerous categories, extensive application and potential market demand, which require rational use of drug should be strengthened.

Diabetic peripheral neuropathy is a common complication of diabetes mellitus,can occur in some patients with painful diabetic peripheral neuropathy.The occurrence of PDPN is related to many factors,such as long term severe hyperglycemia and metabolic disorders,deficiency of neurotrophic factors, disorder of microcirculation,increase of oxygen free radicals in oxidative stress and disorder of immune func-tion.Mitogen activated protein kinase (MAPK)is a kind of serine /tyrosine protein kinase ,which can widely expressed in cell,and can be involved in the pathophysiological process of stress,inflammation,cell cycle and apoptosis through increased nuclear transcription factors.P38MAPK signaling pathway is an im-portant branch of the MAPK,this overview focuses on the research progress of p38MAPK signaling pathway in the pathogenesis of PDPN.

Objective To evaluate the effect of oral health instructions in nursing of marsupialized jaw cysts. Methods 46 patients with cysts of jaw were divided randomly into two groups (education group and control), 23 in each. In addition of normal nursing processes in both groups, patients in education group were offered more oral health instructions, such as pathogenesis of jaw cysts, mechanism of marsupialization, wear and clean of a cyst plug, and oral hygiene maintenance. 12 months later, compliance, satisfaction, treatment effect and oral hygiene condition of the patients in the two groups were studied and compared. Results 100%of patients in the education group could return to clinical visits on time and 96%of patients did cyst rinsing after every meal, which were significantly higher (χ2=6.9, P<0.05) than in the control (74% and 70% respectively). Patients′satisfaction in the two groups were also significantly different (χ2=9.109,P<0.05). Patients in the education group were much more satisfied with the treatment procedures than control. The treatment effect of the cyst in the education group [good (83%), moderate (17%), not so good (0%)] was significantly (χ2=7.793, P<0.05) better than control (good (43%), moderate (52%), not so good (5%)]. The secondary infection rates in the two groups were similar (χ2=1.022, P>0.05). Oral hygiene condition of patients in the education group was better than control in Debris Index (χ2=9.576, P < 0.05) and Plaque Index (χ2=8.212, P < 0.05). Conclusions Oral health instructions played positive role in improving patients′ compliance, degree of satisfaction, treatment effect and oral hygiene condition in patients with jaw cysts.

Objective:To get the prevalence of attention-deficit hyperactivity disorder and in Children. Methods:The subjects were 2188 children who were 4~16 year-old in Wuhan. We had their parents filled the investigation forms. Diagnosis was made according to criterion of attention-deficit hyperactivity disorder in DSM-IV. Results:of behavior problems were got through Achenbach Behavior Questionnaires. Those who were diagnosed as ADHD were classified into three subtypes: predominantly inattentive type; predominantly hyperactive-impulsive type and com-bined type. All suspicious patient and their parents were invited to have the diagnostic talk. Results: (1) The rate of ADHD was 13.58%. ADHD-C, 2.63%; ADHD-I, 8.27%; ADHD-HI, 2.68%. The male, 18.57%; female,8.78%. The ratio of male: female was 2.24:1。(2) There was significantly difference of rate of ADHD between age groups. The rate of 4~5 years-old was 15.88%, that for 6~11 years-old was 15.85%; and for 12~16 yeas-old, 9.98%. The rate of ADHD decreased by aging, especially the group of ADHD-HI. But the rate of ADHD-I was still increased, especially the male group of 6~11 years-old, 12.19%. (3) There was significantly difference of sociality ability between ADHD children and the normal. (4) No matter which age and sex, the scales of behavior problem of ADHD children were higher than the normal children. And the number of factors of behavior problem of ADHD children increased by ag-ing.Conclusion: The rate of ADHD-I was the highest in the three subtypes defined by DSM-Ⅳ, especially among boys 6~11-years-old. The rate of ADHD is significantly associated with sex and age.

Objective To evaluate the distribution and drug resistance of bacteria causing neonatal infectious pneumonia in Jiaxing,and to provide a therapy for clinical doctor to make a correct diagnosis,choose reasonable anti-biotics and avoid abuse of antibiotics.Methods Took expectoration from trachea in condition of asepsis to conduct culture and perform drug-sensitive test from 3025 cases.Results Totally 1 156 strains of aerobic bacteria were iso-lated.875 strains were gram negative bacilli(75.7%),269 strains were gram positive cocci(23.3%),and 12 strains were fungi(1.0%).Klebsiella pneumoiae,Escherichia coil,Acinetobacter baumanni,Enterobacter cloacae were com-mon in gram negative bacilli( respectively 178 cases,151 cases,87 cases,113 cases) .The proportion of the Staphylo-coccus aureus was the largest in gram positive cocci(245 cases) .The results showed that gram-negative bacilli were resistant to cefazolin, ampicillin, piperacillin and sensitive to meropenem, imipenem, piperacillin-tazuobatanna and cefoperazone-sulbactam.The drug resistance was severe of ESBL-positive.Staphylococcus aureus was resistant to penicillin, ampicillin, erythrocin, clindamycin and sensitive to linezolid, vancomycin, nitrofurantoin.Conclusion Gram-negative bacilli are the main bacteria in neonatals with infectious pneumonia.The drug resistance is severe.It is important to make a standard management and isolation.

Objective Previous study found TGF-βpathway might be the molecular pathway influencing the prognosis of colo-rectal cancer, while it was uncertain whether Chinese population is associated with the disease.The article was to evaluate the genetic factors associated with prognosis in colorectal cancer. Methods 52 cases patients with colorectal cancer were followed-up for 36 months in our hospitals from January 2013 to August 2014.Their DNAs were extracted and stored and gene typing were carried out in 5 candidate genes to detect the association between SNPs and the prognosis in colorectal cancer. Results The results showed that within the TGF-βsignaling pathway, after adjusting for Bonferroni multiple testing, allele A of SNP rs10749971 located in gene POU2AF1 was associated with the recurrence of patients with stage III disease under additive and recessive genetic models ( HR =1.968, P=0.004;HR=2.174, P=0.010).Allele C of SNP rs961253 in the gene BMP2 could increase the recurrence risk (HR=1.992, P=0.005) and the death risk (HR=3.161, P=0.007) of patients with stage III disease under recessive genetic models.Allele A of SNP rs4464148 in SMAD7 gene could significantly decrease the death risk of patients with stage II and III colorectal cancer under dominant genetic model (HR=0.382, P=0.017;HR=0.230, P=0.006).In addition, accumulated effects of several adverse genes showed gene high risk group could increase the risk of death for patients with stage III colorectal cancer significantly ( HR=15.512, P=0.036;95%CI:1.611-149.360). Conclusion In different genetic models, SNP locus mutation within gene POU2AF1, BMP2 and SMAD7 on TGF-βpathway was associated with the prognosis of patients with colorectal cancer.With the increase of the number of unfavorable genes, the death risk increases accordingly.

OBJECTIVE:To analyze the utilization of drug included in the army of rational medicine directory in a military sanatorium in 2009.METHODS:To extrac convalescent medication data from a military sanatorium "military sanatorium information system",and analyze the drug use situation.RESULTS:The first 6-bit key role of drug consumption was the circulatory system drugs,anti-microbial drugs,a major role in the central nervous system drugs,clinical subjects medication,a major role in the respiratory system drugs,a major role in the digestive system drugs.CONCLUSION:The drug use of the military sanatorium for the convalecents basically matches the spectrum of convalecent disease.

OBJECTIVE:To analyze the utilization of different dosage forms of essential drugs in the clinical practice in our sanatorium and the tendency of drug use of patients.METHODS:The utilization of essential drug in our sanatorium were analyzed statistically in respect of drug category,constitution ratio of drug use.A questionnaire survey was carried out to survey the tendency of drug use of patients.RESULTS:Tablet was the most common dosage form in the clinic.Antibiotics and drugs for clinical therapy had complete dosage forms.The common dosage forms used patients were tablets and capsule,etc.CONCLUSIONS:Patients of our sanatorium select drugs tendentiously.The tendentious selection of drug is no difference in each age group.

Objective To explore the causes of nosocomial infections in ICU as to provide scientific evidence for the corresponding intervention measures.Methods The retrospective survey was used to investigate and analyze the incidence of nosocomial infections in ICU from 2013 to 2014.Results The total of 1225 patients investigated,182 patients suffered from nosocomial infections and the nosocomial infections rate was 14.86％.The etiology of ICU specimen was 95.22％.Gram negative bacilli,Gram positive bacteria and fungi,which accounted for 69.16％,18.22％ and 12.62％ respectively.The main infection site were respiratory tract,blood and urinary tract,which accounted for 70.43％,11.30％ and 4.78％ respectively.Conclusion The hospital infection rate of ICU in our hospital is kept the same level of the hospital in the same area.Etiology specimens rate comply with national standards.Main pathogens are gram negative bacilli,and the main site of infection is respiratory.
