Objective: To evaluate the rationality of the use of antibiotic prophylaxis in the perioperative period of herniorrhaphy. Methods: Seventy-five cases of herniorrhaphy discharged in March, June and September of 2009 were collected. Parameters including indications, the types of antibacterial drugs, time of administration, solvent choice, usage and dosage, combination, etc, were assessed for the rationality according to the "the guiding principles of clinical use of antibacterial drugs"and other relevant documents. Results: The antibacterial drugs application rate was 100%, while the rationali-ty of the drug selection, the drug administration route, the single dose and the number of dosing, was 70.7%, 100% , 100% and 93.3% respectively. Sixty-two patients(82.7%)were treated with antibacterial drugs at 0.5～2 hours before surgery, and the average time course of the prophylactic use of antibiotics after surgery was 2.3 days. Conclusions: Some problems existed for the use of antibiotic prophylaxis in the perioperative period of herniorrhaphy. Surgeon should ensure the correct use of the antibacterial drugs to make the safety, efficiency, rationality, and economy for the patients.

Objective:To get the prevalence of attention-deficit hyperactivity disorder and in Children. Methods:The subjects were 2188 children who were 4~16 year-old in Wuhan. We had their parents filled the investigation forms. Diagnosis was made according to criterion of attention-deficit hyperactivity disorder in DSM-IV. Results:of behavior problems were got through Achenbach Behavior Questionnaires. Those who were diagnosed as ADHD were classified into three subtypes: predominantly inattentive type; predominantly hyperactive-impulsive type and com-bined type. All suspicious patient and their parents were invited to have the diagnostic talk. Results: (1) The rate of ADHD was 13.58%. ADHD-C, 2.63%; ADHD-I, 8.27%; ADHD-HI, 2.68%. The male, 18.57%; female,8.78%. The ratio of male: female was 2.24:1。(2) There was significantly difference of rate of ADHD between age groups. The rate of 4~5 years-old was 15.88%, that for 6~11 years-old was 15.85%; and for 12~16 yeas-old, 9.98%. The rate of ADHD decreased by aging, especially the group of ADHD-HI. But the rate of ADHD-I was still increased, especially the male group of 6~11 years-old, 12.19%. (3) There was significantly difference of sociality ability between ADHD children and the normal. (4) No matter which age and sex, the scales of behavior problem of ADHD children were higher than the normal children. And the number of factors of behavior problem of ADHD children increased by ag-ing.Conclusion: The rate of ADHD-I was the highest in the three subtypes defined by DSM-Ⅳ, especially among boys 6~11-years-old. The rate of ADHD is significantly associated with sex and age.

Objective:To study the effect of recombinant human growth hormone(rhGH) and its combination with chemotherapy(5-FU) in tumor bearing mice.Methods:Hepatic metastases model were established in 615 mice by splenic injection of proventriculus squamous carcinoma cell(MFC).Among them,sixty mice were divided randomly into 6 groups(ten per group): control(NS),GH1,GH2,flurouracil(5-FU),flurouracil plus rhGH(GH1+5-FU) and flurouracil plus rhGH(GH2+5-FU).Body weights were measured every week.On the 28~(th) day,animals in each group were executed.The weight of body and liver was measured,and cell cycle of tumor DNA was determined by flow cytometry(FCM). Other 48 animals were divided randomly into 6 groups(eight per group) and treated with the same methods as above.They were fed till death to observe survival time. The microbiota of the human gastrointestinal tract inhabits a complex ecosystem.Intestinal normal flora is obtained by newborn after birth,and suffers influences on the type of delivery,the type of feeding and contamination from the environment.There is emerging evidence indicating that quantitative and qualitative changes on gut microbiota contribute to alterations in the mucosal activation of immune system leading to intra-or extra-intestinal diseases.A balance between pathogenic and beneficial microbiota throughout childhood is important to gastrointestinal health,including protection against pathogens,inhibition of pathogens,nutrient processing,and regulation of host fat storage.Probiotics can promote an intentional modulation of intestinal microbiota favoring the health of the host.This paper is a review about modulation of intestinal microbiota on prevention and adjuvant treatment of intra-and extra intestinal pediatric diseases.

Objective To investigate the expression levels of TAK1 and p38 genes among different subtypes of acute myeloid leukemia (AML) patients,and to analyze the clinical characteristics of patients with different expression levels of TAK1 and p38 genes.Methods GAPDH was made as an internal reference,14 healthy people as control group.The quantitative real-time PCR was used to detect the expression of TAK1 and p38 in bone marrow samples of 87 AML patients,and the results were analyzed statistically.Results The expression levels of TAK1 and p38 in experiment group were higher than those in control group (0.194± 0.125 vs 0.015±0.008,0.233±0.140 vs 0.010±0.005,P ＜ 0.001).TAK1 expression in M4 was higher than that in M2,M3 and M5 (P =0.005,0.000,0.002),TAK1 expression in M3 was lower than that in M2 (P =0.022).p38 expression in M4 was higher than that in M1,M2,M3 and M5 (P =0.013,0.035,0.000,0.045),as it was higher in M2 and M5 than that in M3 (P =0.001,0.012).The CD56 positive rate cells and the number of peripheral blood leukocytes of the TAK1 high expression group were higher than those of the TAK1 low expression group,the CD19 positive rate of the p38 low expression group was higher than that of the p38 high expression group.Conclusion The expression levels of TAK1 and p38 genes are elevated in AML patients,and the up-regulation may play an important role in the pathogenesis of AML.

Objective To assess the value of acoustic radiation force impulse (ARFI) imaging in the pathological damage of Henoch-Schonlein purpura nephritis on children by comparison with renal biopsy.Methods 50 cases of healthy control group as group A; 58 children with HSPN were divided into three groups according to pathological grading:Ⅰ ～ Ⅱ class (group B),Ⅲ class (group C) and Ⅳ ～ Ⅵ class (group D).ARFI was then used to measure the shear wave velocities(SWV) of renal cortex of each group,compared the differences SWV of each group.Results SWV values of children's renal cortex with HSPN were significantly higher (t =5.883,P =0.017) than those in the group A.Pairwise comparisons found that there were statistically significant differences between group D and the other three groups (P ＜0.05).According the ROC curve,the cut-off value of SWV was 2.59 m/s when the maximum area under the curve equal to 0.719,the sensitivity and specificity were 63％ and 67％.Conclusions ARFI technology can quantify the elastic properties of the kidney,which is expected as an important indicator to evaluate the pathological extent of damage of the HSPN.

Objective To evaluate the efficiency of levonorgestrel-releasing intrauterine system (LNG-IUS) in the treatment of recurrent endometriosis after conservative surgery or conservative surgery combined with medical treatment. Methods Twenty-three patients with recurrent endometriosis after conservative surgery or conservative surgery combined with medical treatment were treated by LNG-IUS. All patients rejected further operation and had no desire of fertility. The visual analogue scale (VAS) scores of pain, menstrual model, weight and serum CA125 level and the volume of ovarian endometriotic cysts before and after 3, 6, 12, 24 and 36 months of treatment were recorded and compared. Results ( 1 ) VAS score:after 12 months of using LNG-IUS, dysmenorrheal, chronic pelvic pain or dyspareunia were relieved significantly. VAS score were dropped from 5.9 ± 2. 3,4. 3 ± 2.0 to 1.0 ± 0. 7,1.4 ± 1. 1 ( P ＜ 0. 01 ). ( 2 )Volum of cysts :after 6 months of using LNG-IUS, the volume of recurrent ovarian endometriotic cysts in 11 patients were reduced from ( 11.4 ± 6. 1 ) em3 to ( 5. 5 ± 3.4 ) em3 significantly ( P ＜ 0. 01 ). At 12 months of follow-up, it suggested that 2 patients' ovarian endometriotic cysts disappeared. At 24 months follow-up,9 patients ovarian endometriotic cysts disappeared ( 3 ) CA125: serum CA125 decreased from ( 65.5 ± 19. 6 )kU/L to (42. 1 ± 13.6) kU/L at 6 months after treatment remarkably (P ＜ 0. 01 ). Continued to decrease after 12 months and then become steady. Irregular bleeding and spotting was the main side effects, weight gain was also observed in few patients. Conclusions LNG-IUS could be used in treatment of recurrent endometriosis after conservative surgery or conservative surgery combined with medical treatment effectively. It could relieve pain, reduce the level of CA125 and decrease the size of ovarian endometriotic cysts. LNG-IUS seems to be an effective, safe, and long term treatment for endometriosis with fewer side effects and better compliance.
