1.Value of 1H-MRS on SCA3/MJD diagnosis and clinical course

Objective:To investigate the value of proton magnetic resonance spectroscopy (1H-MRS) on the diagnosis of SCA3/MJD,and to calculate the correlation between 1H-MRS ratio and the clinical score.Methods:Sixteen patients with SCA3/MJD and 19 healthy volunteers were scanned with 1H-MRS.The data of N-acetyl aspartate,creatine,choline-containing compounds,myoinositol,NAA/Cr,Cho/Cr,and mI/Cr ratio were collected,which were grouped for comparative study.The onset patients with SCA3/MJD were evaluated with the International Cooperative Ataxia Rating Scale and Scale for the Assessment and Rating of Ataxia,the correlation between NAA/Cr,Cho/Cr or mI/Cr ratio and the clinical score was calculated.Results:The NAA/Cr in the pons and cerebellar dentate nucleus from the onset patients with SCA3/MJD was significantly reduced compared to that in the normal control group.The NAA/Cr in the cerebellar dentate nucleus of onset patients with SCA3/MJD was obviously correlated with ICARS.Conclusion:SCA3/MJD lesions are mainly located in the cerebellum and brainstem,where gray and white mater are also involved.The cerebellar dentate nucleus may be the earliest involved area.There is a correlation between the ICARS and the cerebellar lesion degree.The ICARS reflects the severity of clinical manifestations.1H-MRS is useful in the diagnosis of SCA3/MJD.
2.Studies on immune-enhancing activity of polysaccharides from Borojoa sorbilis cuter

Objective:Studies on the immune-enhancing activity of Polysaccharides from the fruits of Borojoa sorbilis cuter ( polysaccharides BSCP) in vivo.Methods:54 male BALB/c mice were randomly divided into six groups.The immune suppression mice in the three experimental groups,which were induced by cyclophosphamide ( CY) at 80 mg/kg/d via intraperitoneal injection, were perfused with 100,200,400 mg/kg/d BSCP for 30 d.The effect of BSCP on cellular immune function,humoral immune function and mononuclear macrophage function were measured.Results:CY-treated mice showed a significant decrease in immune function( P<0.05),humoral immune function(P<0.01) and mononuclear macrophage function(P<0.01).The administration of BSCP was able to overcome the CY-induced immunosuppression,treatment with BSCP-L,BSCP-M and BSCP-H groups significantly enhanced the cellular immune function( each was P<0.05 ) and mononuclear macrophage function ( each was P<0.05 ) , and treatment with BSCP-M and BSCP-H significantly enhanced humoral immune function(each was P<0.01).Conclusion:BSCP could significantly increase immune responses and reduce the side effects of CY in immune system.Therefore,the BSCP could be an immunomodulatory agent.
3.Application and Practice of Intelligent Management and Solution for Clinical Rational Drug Use in Our Hos-pital

OBJECTIVE:To improve the level of rational drug use,and to reduce the workload of clinical pharmacists. METH-ODS:Established“Intelligent Management and Solution for Clinical Rational Drug Use”of our hospital was introduced,including post-prescription review(all prescriptions review)system, self-maintenance system for rational drug use knowledge base,pre-pre-scription review (rational drug use real-time review) early warning system. The effects of intelligent system were evaluated. RE-SULTS & CONCLUSIONS:After the application of the system,the following aspects could be achieved,including“all prescrip-tions review”,“multi-prescriptions/longitudinal review”,system knowledge base real-time update and autonomous definition of re-view rules,effective communication and intervention between physicians and pharmacists before printing prescription by Yaozhidao instant messaging system. Through related trial evaluation,the proportion of false positive prescriptions decreased by 17.65% after 4 months of the application of the system;drug consultation prescriptions review average score of outpatient pharmacists increased from 81 to 94.85;the time of prescription review decreased by 4.72-5.46 times. Through mandatory interception of unqualified pre-scriptions,severe warning,timely prompt and pharmacists’intervention,the rate of unqualified prescriptions pre-reviewed by the system in respiration department decreased from 32% in Jan. 2015 and 21% in May to 4.62% in Dec. So,the application of the system promotes rational drug use in outpatient department,reduces the workload of pharmacists and ensures the safety of drug use comprehensively.
4.Continuous Improvement Practice and Effectiveness of Electronic Pharmaceutical System on Tollying in Outpatient Pharmacy of Our Hospital

OBJECTIVE:To optimize the workflow of electronic pharmaceutical system(ePS),and improve the work efficiency of outpatient pharmacy. METHODS:The problems about the application of ePS on tollying in outpatient pharmacy of our hospital were analyzed. The improvement measures were put forward and implemented. The effectiveness was summarized by evaluating relevant work indicators 6 months before and after optimization. RESULTS:Referring to main problems during the application of ePS on tollying as repetition of tallying drugs,complexity of stocking checking,low efficiency in the management of drug expiry date,deviation in the tracking of"batch/expiry",a series of measures were carried out in outpatient pharmacy of our hospital,including optimizing tallying process,updating tracking process,adding the function of alerting the near-expiry drug, etc.The system optimization shortened the time of replenishment(26.32 s vs. 7.78 s,improving by 3.38 times)and stock checking [(165.50±5.75)s vs.(146.70±4.54)s,shortening by 11.36%],realized accurate tracing of drug information"batch/expiry"and improved drug management [(58.00±1.88)s vs.(18.00±1.23)s,shortening by 68.97%]. CONCLUSIONS:The optimization of ePS on tallying in our hospital improves work efficiency of pharmacists and guarantees the accuracy of tracing drug"batch/expiry".
5.Drug repurposing against coronavirus disease 2019 (COVID-19):A review

Since December 2019,severe acute respiratory syndrome coronavirus 2 has been found to be the culprit in the coronavirus disease 2019 (COVID-19),causing a global pandemic.Despite the existence of many vaccine programs,the number of confirmed cases and fatalities due to COVID-19 is still increasing.Furthermore,a number of variants have been reported.Because of the absence of approved anti-coronavirus drugs,the treatment and management of COVID-19 has become a global challenge.Under these circumstances,drug repurposing is an effective method to identify candidate drugs with a shorter cycle of clinical trials.Here,we summarize the current status of the application of drug repurposing in COVID-19,including drug repurposing based on virtual computer screening,network pharmacology,and bioactivity,which may be a beneficial COVID-19 treatment.
6.The study of characteristic of serum Lp(a)level in in patients with chronic kidney disease

Objective To investigate the characteristic of lipoprotein(a)[Lp(a)]in different phases of chronic kidney disease (CKD ),to provide the basis for clinical prevention and treatment of CKD.Methods 200 patients with CKD in the Republic Hospital of Shifang were collected as study group,including 5 phases (every phase had 40 cases),and 100 healthy people were selected as control group.Measured the serum Lp(a)of both study and control group,analyzed the correlations between Lp(a)and different phase of CKD.All data were analyzed by SPSS version 17.0.The significant level was established at 0.05.Results CKD1 [(146.0 ±95.5)mg/L]and all CKD group [(231.5 ±133.2)mg/L]had higher level of serum Lp(a)than the control group [(115.5 ±70.2)mg/L] (Z=-2.800,P<0.05 and Z=-7.922,P<0.05).CKD3 had higher Lp(a)level than CKD2(Z=-2.069,P<0.05 ),while there were no significant differences between each of the other two groups.CKD4 -5 [(325 .0 ± 194.7)mg/L]also had higher Lp(a)level than CKD1 -3 [(182.0 ±110.5)mg/L](Z=-4.439,P<0.05). Conclusion Patients with CKD always have high level of serum Lp(a),which have been slowly increased since CKD1 ,meanwhile the level of Lp(a)may have a certain correlation with the stage of CKD development,since Lp(a) is an important promoting factor in the progress of CKD.
7.Investigation and Influencing Factors of Medication Literacy for Urban Elderly Patients with Chronic Diseases in Anhui Province

Objective To explore the current status of medication literacy among urban elderly patients with chronic diseases in Anhui Province,aiming to reveal the factors influencing their medication literacy,and to propose targeted measures for improvement.Methods This research involved 381 participants aged 60 and above.It was conducted in Anhui province between December,2022 and January,2023,with data collected through face-to-face interviews by pharmacists.Single-factor analysis and ordinal multi-class logistic regression analysis were conducted to determine factors affecting medication literacy.Results Medication literacy cognition and medication literacy behavior were rated as good among urban older adults in Anhui province of the 294 valid questionnaires.Those who did not understood package insert exhibited significantly lower medication literacy behavior than those who fully understood[estimate=-1.224,95%CI=(-2.130,-0.317),P<0.01].Elderly patients with chronic diseases faced issues such as an inability to read or understand drug instructions in the investigation.90.48%of elderly patients with chronic diseases never heard or seldom heard of medication guidance services.Conclusion Medication literacy among urban elderly patients with chronic diseases is generally good in Anhui province.The ability to understood drug instructions significantly influenced the medication literacy of urban elderly patients with chronic diseases.Modifying the drug instructions to meet the reading needs of the elderly patients with chronic diseases and developing pharmaceutical care could effectively enhance rational drug use among this demographic.
8.Mutations of G38R and D40G cause amyotrophic lateral sclerosis by reducing Annexin A11 protein stability.

To explore the role of the mutations G38R and D40G of Annexin A11 (ANXA11) in the onset of amyotrophic lateral sclerosis (ALS).
Methods: The plasmids expressing ANXA11 wild type protein, ANXA11 G38R protein and ANXA11 D40G protein were constructed, respectively. The recombinant plasmids were then transfected into HEK293 cells respectively followed by cycloheximide (CHX) treatment for 0, 2, 4 and 8 h. The protein expressions of ANXA11 wild type, ANXA11 G38R and ANXA11 D40G mutations were determined by Western blot. Gray analysis by Image J was performed to compare the half-life of each protein. The NSC-34 cell lines constantly expressing ANXA11 wild type protein, ANXA11 G38R protein and ANXA11 D40G protein were established. The cells were treated with NP-40 lysis buffer to examine the protein solubility by Western blot.
Results: Both ANXA11 G38R protein and ANXA11 D40G protein showed a shorter half-life than ANXA11 wild type protein (P<0.05), while there was no difference between ANXA11 G38R protein and ANXA11 D40G protein (P>0.05). There was no visible insoluble substance in the NP-40 lysates for ANXA11 wild type protein, ANXA11 G38R protein and ANXA11 D40G protein.
Conclusion: G38R and D40G mutations reduce the stability of ANXA11 protein. G38R and D40G mutations do not alter ANXA11 solubility.

9.Advances in genetics research in the pathogenesis of amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is a rare neurodegenerative disease affecting the upper and lower motor neurons. It is characterized by progressive muscle weakness, atrophy and ultimate death due to dysphagia and dyspnea. There are many causes of ALS, among which the genetic factors show great relevance. Imbalance of protein homeostasis in neurons, prion-like proliferation and propagation of abnormal proteins, mitochondrial dysfunction, glutamate mediated excitotoxicity, and intraneuronal substance transport disorders are recognized as the pathogenesis.The study on gene mutation related to pathogenesis will bridge the molecular and cellular research of ALS, which can deepen the understanding of the occurrence and development of ALS and the role of gene mutation in ALS, and provide new ideas and enlightenment for the treatment of ALS.

10.Practice of clinical pharmacist developing in medication reconciliation for the inpatients in cardiovascular medi- cine department

OBJECTIVE To analyze the medication reconciliation for the inpatients in cardiovascular medicine department ,to provide reference for the establishment of working mode of clinical pharmacists in the department of cardiovascular medicine and to provide a basis for clinical pharmacists and community pharmacists developing pharmaceutical care for patients after transfering to community health center. METHODS From October 2020 to September 2021,newly admitted or newly transferred inpatients with chronic disease were selected from Shiyan People ’s Hospital of Shenzhen Bao ’an District. Medication reconciliation was conducted by clinical pharmacists after pharmaceutical consultation. According to the Pharmaceutical Care Network Europe (PCNE) classification system V 9.1,the existing drug-related problems (DRPs)were classified and summarized. The effectiveness and safety evaluation,medication education and other measures were provided ,and the acceptance of intervention was analyzed at the same time. RESULTS A total of 100 patients were included ,including 54 males and 46 females. The average age was (60.21±9.69) years,the average number of chronic diseases was (2.84±0.83),and the median number of drugs was 5.00. Among them ,110 treatment drug deviations were found in 74 patients,involving 10 categories and 61 drugs. Top three drugs in the list of accumulative drug deviation were cardiovascular system drugs (35 deviations),digestive medicine drugs (16 deviations)and endocrine system drugs (15 deviations). The above treatment drug deviation may cause 122 DRPs, mainly “treatment effectiveness”problems(74 DRPs),and the causes were “inappropriate medication time ormedication interval ”(32 DRPs), followed by “inappropriate drug combination ”(10 DRPs). Interventions to DRPs mainly concentrated on patient level ,drug level (58)and doctor level (58),155 of which (84.70%)were fully accepted and implemented. CONCLUSIONS Some patients have a weak awareness of medications according to doctor ’s advice;drug reconciliation led by clinical pharma- cists at admission can fully understand the potential drug problems of patients ,and help doctors improve the drug compliance of patients and ensure their medication safety .
