1.Suitability study on techniques of cognitive behavioral therapy for obsessive-compulsive disorder

Objective To evaluate the suitability of cognitive behavioral therapy (CBT) techniques for obsessive-compulsive disorder (OCD),and analyze the domestic application status of CBT techniques for OCD and lay a foundation of standard CBT in China.Methods Delphi method was adopted to evaluate 30 CBT techniques on four dimensions,including operability,frequency,contribution of treatment and acceptability,and calculated the final scores of all techniques on different dimension and their comprehensive rank.Results After two rounds of experts consulting,Kendall coefficients of four dimensions were 0.249,0.269,0.234,0.250 (P<0.01),coefficient of variation ranged from 0.05 to 0.31 of each technology on different dimension.The operability score of thought stopping,cost-benefit analysis and pie chart belonged to general suitability.The frequency and treatment contribution score of thought stopping and pie chart belonged to occasional use and small.The acceptation score of socratic question,thought stopping,in vivo exposure and behavior prevention belonged to partial acceptation.Weight coefficients of four dimensions were 0.26,0.18,0.27,0.29 and techniques ranking in the top three were information collection and evaluation,therapeutic alliance foundation and psychological education.Conclusion Most CBT techniques for OCD are suitable in China but some techniques still need to be improved.
2.Efficacy of coenzyme Q10 in preventing propofol infusion syndrome in rats

Objective To evaluate the efficacy of coenzyme Q10 in preventing propofol infusion syndrome in rats.Methods Thirty pathogen-free healthy male Sprague-Dawley rats,aged 8-10 weeks,weighing 250-280 g,were randomly divided into 3 groups (n=10 each) using a random number table:control group (group C),propofol group (group P) and coenzyme Q10 group (group CoQ10).Normal saline was infused intravenously in group C.In group P,1% propofol in medium-and long-chain triglyceride emulsion injection was infused intravenously.In group CoQ10,CoQ10 100 mg/kg was administrated by intragastric gavage,and 1 h later propofol was infused intravenously.The infusion rate was 20mg·kg-1 ·h-1 within the first6hand40mg· kg-1 · h-1fortherest6h,and the total time was 12hin the three groups.Immediately after the start of administration (To),and at 6 and 12 h after the start of administration (T1,2),blood samples 2 ml were taken from the common carotid artery,with 0.5 ml for blood gas analysis and 1.5 ml for determination of the levels of serum aspartate aminotransferase (AST),alanine aminotransferase (ALT),creatine kinase (CK),creatine kinase isoenzyme-MB (CK-MB),cardiac troponin Ⅰ (cTnⅠ),blood urea nitrogen (BUN) and creatinine (Cr).After blood sampling,the rats were sacrificed,and myocardial tissues were obtained for pathological examination.Results Compared with group C,the serum AST,ALT,CK,CK-MB and cTnⅠ levels were significantly increased at T1,2 (P<0.05),no significant changes were found in serum BUN and Cr levels (P>0.05),the pathological changes of myocardium were aggravated in P and CoQ10 groups.Compared with group P,the serum AST,ALT,CK,CK-MB and cTnⅠ levels were significantly decreased at T1,2 (P<0.05),no significant changes were found in serum BUN and Cr levels (P>0.05),and the pathological changes of myocardium were significantly attenuated in group CoQ10.Conclusion Coenzyme Q10 can effectively prevent the development of propofol infusion syndrome in rats.

A real-time tele-monitoring system for physiological multi-parameter based on Internet is introduced,and physiological signals are transmitted by P2P network technology.The experiment results show that the speed of data transfer has been improved greatly using P2P technology,and physiological signals,such as ECG,can be monitored in real-time.
4.Roles of P53 and P21 in the differentiation of PC12 cells

Objective To investigate the roles of P53/P21 during neuronal differentiation with a differentiated model of PC12 cells. Methods A new cell line PC12(P53/m175) was created by stable transfection of a retrovirus plasmid pBabe-P53/m175, which contains a dominant-negative P53 gene mutant. After NGF treatment, observing with phase-contrast microscopy, flow cytometric analysis and western blotting of P53 and P21 were performed. Results Expression of P53 and P21 was obviously increased in NGF-induced PC12 cells. The appearance of cell cycle G1 phase arrest paralleled the increased expressions of P53 and P21. The level of P21 protein did not change after treatment with NGF in PC12(P53/m175) cells and the extent of G1 phase arrest markedly decreased. However, we did find the normal neurite outgrowth in NGF-treated PC12(P53/m175) cells. Conclusion NGF-induced an increased protein levels of P53 and its transcriptional element, P21 is essential for cell cycle G1 phase arrest, but does not necessarily correlate with the neurite outgrowth of PC12 cells.
5.Relationship between Tumor Necrosis Factor-α Gene Polymorphism and Mild Cognitive Impairment in Patients with Type 2 Diabetes Mellitus

Objective:To investigate the relationship between tumor necrosis factor-α (TNF-α)-308A/G gene polymorphism and mild cognitive impairment(MCI)in patients with type 2 diabetes mellitus(T2DM),and their correlative risk factors thereof.Methods:Forty cases of T2DM with MCI and 80 cases of T2DM without MCI were selected for this study.The polymorphism of the TNF-α-308A/G was detected by PCR-restriction fragment length polymorphism (PCR-RFLP).According to the clinical data,such as course of disease,plasma glucose,plasma fat and body mass index(BMI),the independent risk factors of T2DM and MCI were analyzed by non-conditional logistic regression.Results:The frequency of TNF-α2 allele was significantly higher in the group of T2DM with MCI than that without MCI (P<0.01).The indexes of the statistical significant difference between the two groups were the age,course of disease,postprandial blood glucose(P2BG),glycosylated hemoglobin,body mass index,family history of T2DM,hypertension,diabetic retinopathy,diabetic peripheral neuropathy and TNF-α.The independent risk factors included TNF-α,diabetic peripheral neuropathy,diabetic retinopathy,age and P2BG.Conclusion:There is a significant relationship between TNF-α2 allele and T2DM with MCI.There is a significant relationship between the age,control of plasma glucose and microvaseular complication of T2DM with the cognitive funotion.
6.Prognostic value of coronary artery calcium score in patients with stable an-gina pectoris after percutaneous coronary intervention

Objectives To evaluate the prognostic value of the coronary artery calcium (CAC) score in patients with stable angina pectoris (SAP) who underwent percutaneous coronary intervention (PCI). Methods A total of 334 consecutive patients with SAP who underwent first PCI following multi-slice computer tomography (MSCT) were enrolled from our institution between January 2007 and June 2012. The CAC score was calculated according to the standard Agatston calcium scoring algorithm. Complex PCI was defined as use of high pressure bal-loon, kissing balloon and/or rotablator. Procedure-related complications included dissection, occlusion, perforation, no/slow flow and emer-gency coronary artery bypass grafting. Main adverse cardiac events (MACE) were defined as a combined end point of death, non-fatal myo-cardial infarction, target lesion revascularization and rehospitalization for cardiac ischemic events. Results Patients with a CAC score>300 (n=145) had significantly higher PCI complexity (13.1%vs. 5.8%, P=0.017) and rate of procedure-related complications (17.2%vs. 7.4%, P=0.005) than patients with a CAC score≤300 (n=189). After a median follow-up of 22.5 months (4-72 months), patients with a CAC score≤300 differ greatly than those patients with CAC score>300 in cumulative non-events survival rates (88.9 vs. 79.0%, Log rank 4.577, P=0.032). After adjusted for other factors, the risk of MACE was significantly higher [hazard ratio (HR):4.3, 95%confidence inter-val (95%CI):2.4-8.2, P=0.038] in patients with a CAC score>300 compared to patients with a lower CAC score. Conclusions The CAC score is an independent predictor for MACE in SAP patients who underwent PCI and indicates complexity of PCI and proce-dure-related complications.
7.Effect of cytomegalovirus infection on infant weight growth and correlation between the weight growth and copies number of the virus

Objective To investigate the effect of cytomegalovirus(HCMV) infection on infant bodyquality growth,and analysis of the correlation between children of all ages body-quality growth rate and copy number of the virus.Methods One hundred and sixty-eight children with HCMV infection were enrolled in the current study who were hospitalized from Oct.2012 to Oct 2013 in the Affiliated Hospital of Xuzhou Medical College.Children's urine HCMV-DNA copy number was measured by TaqMan probe fluorescence quantitative PCR.The retrospective study was conducted to compare the HCMV infection in children with weight-for-age and normal children.The correlation between children with body-quality growth rate and copy number of the virus was analyzed.Results All 168 urine specimens were positive regarding HCMV-DNA.The age of all distribution was from 1 month to 3 years.There was a negative correlation between the increase body weigh and the copies of virus of children with 3-6 months (r =-0.774,P < 0.01).Conclusion The growth and development of children with HCMV at age before 6 months were slower than Children with the same age.The growth of cytomegalovirus infection increase gradually speeding when they were over 6 months and it was approaching to that same age when they are 2 years old.The growth rate is lower than that of normal children when HCMV children were before 1 years old.The correlation of body-quality growth rate and the number of copies of the virus was negative,especially from 3 months to 6 months.
8.Effects of dexmedetomidine on cellular immune function of immunosuppression juvenile rats

Objective To explore the effects of dexmedetomidine on peripheral blood T lymphocyte proliferation and T lymphocyte subsets of juvenile rats with splenectomy.Methods Twenty-four healthy male Sprague-Dawley rats,weighing 130-150 g,aged six weeks were enrolled in this study.Half of the rats received splenectomy to make an immunosuppressive model,then they were randomly divided into 2 groups (n=6 each): splenectomy+normal saline group (group SN) and splenectomy+dexmedetomidine group(group SD).The another half of the rats without splenectomy were randomly divided into 2 groups: normal saline group(group S) and dexmedetomidine group(group D).After one week of normal feeding,normal saline 10 ml/kg was injected intraperitoneally (ip) in groups S and SN,dexmedetomidine 50 μg/kg was injected ip in groups D and SD respectively.Two hours after the injection,blood samples were collected.MTT was utilized to examine the peripheral blood T lymphocyte proliferative capability.T lymphocyte subsets CD4+,CD8+ were determined by flow cytometry.CD4+/CD8+ was calculated.Results Compared with group S,T lymphocyte proliferative capability,the percentages CD4+,CD8+ and CD4+/CD8+ ratio were significantly decreased in group SN (P<0.05);T lymphocyte proliferative capability in group D was decreased (P<0.05),but no significant changes was found in the percentages CD4+,CD8+ and CD4+/CD8+ ratio.Compared with the group D,T lymphocyte proliferative capability,the percentages CD4+,CD8+ and CD4+/CD8+ ratio in group SD were significantly decreased (P<0.05).Compared with the group SN,T lymphocyte proliferative capability in group SD was significantly decreased (P<0.05).Conclusion Cellular immune function of juvenile rats with or without splenectomy is suppressed by dexmedetomidine,and the suppressive function is more severe in splenectomy rats than that in normal juvenile rats.
9.Effects of Icariin on Migration of Mesenchymal Stem Cells of Rat Bone Marrow

Objective To explore the mechanism of icariin mediating migration of mesenchymal stem cells (MSC) in rat bone marrow. Methods MSC proliferation was detected by CCK-8 test. Cell apoptosis was examined with Hoechst33342 staining after the establishment of cellular oxygen and glucose deprivation model. The protein expressions of CXCR4, the receptor of SDF-1, in the surface of MSC after stimulated by icariin were detected through Western blot. The migration of MSC was observed by Transwell chemotaxis assay. Results 0.01μmol/L, 0.1μmol/L and 1μmol/L of icariin could significantly promote the proliferation of MSC, while 10μmol/L of icariin inhibited the proliferation of MSC. After treatment of oxygen and glucose deprivation in vitro, 0.1μmol/L and 1μmol/L of icariin could inhibit the apoptosis of MSC. Icariin could not only improve the expression of CXCR4 in MSC, but also increase the number of transmembrane migrated MSC. After the addition of CXCR4 antagonist AMD3100, there was no significant difference in the number of cell migration among the different groups. Conclusion Icariin with appropriate concentration can promote the proliferation, survival and migration of MSC. SDF-1/CXCR4 signaling pathway is involved in the regulation of MSC migration by icariin.
10.Optimum ratio of medicine dosage for dexmedetomidine mixed with oxycodone used for PCIA after gastrointestinal surgery

Objective To investigate the optimum ratio of medicine dosage for dexmedetomidine mixed with oxycodone used for patient-controlled intravenous analgesia (PCIA) after gastrointestinal surgery.Methods Eighty patients of both sexes, aged 35-64 yr, weighing 55-75 kg, of American Society of Anesthesiologists physical status Ⅰ or Ⅱ , undergoing elective gastrointestinal surgery, were randomly divided into 4 groups (n=20 each) using a random number table: oxycodone group (group O), and different ratios of medicine dosage when dexmedetomidine was added to sufentanil groups (OD1-3 groups).At 15 min before the end of surgery, oxycodone 0.1 mg/kg was injected intravenously, and PCIA pump was connected simultaneously.In group O, the PCIA solution contained oxycodone 1.00 mg/kg in 100 ml of normal saline.In group OD1, the PCIA solution contained oxycodone 1.00 mg/kg and dexmedetomidine 2.5 μg/kg in 100 ml of normal saline.In group OD2 , the PCIA solution contained oxycodone 0.75 mg/kg and dexmedetomidine 2.5 μg/kg in 100 ml of normal saline.In group OD3, the PCIA solution contained oxycodone 0.50 mg/kg and dexmedetomidine 2.5 μg/kg in 100 ml of normal saline.The PCIA pump was set up to deliver a 0.5 ml bolus dose with a 15-min lockout interval and background infusion at 2 ml/h.Oxycodone 0.05 mg/kg was injected intravenously as a rescue analgesic, and visual analogue scale score was maintained ≤ 4.The requirement for rescue analgesics was recorded.The requirement for the rescue analgesic was recorded within 48 h after surgery.The number of successfully delivered doses, and occurrence of adverse reactions such as bradyeardia, hypotension, nausea, vomiting, over-sedation, somnolence, pruritus, and respiratory depression were recorded.Patient's satisfaction with analgesia was recorded at 72 h after surgery.Results No patients required the rescue analgesic or developed over-sedation, vomiting, respiratory depression and hypotension in the four groups.Compared with group O, the incidence of somnolence was significantly increased in group OD1, the incidence of nausea, somnolence, bradycardia and pruritus was decreased in OD2 and OD3 groups, and the degree of patient's satisfaction with analgesia was increased in OD1-3 groups (P<0.05).Compared with group OD1, the incidence of nausea, somnolence, bradycardia and pruritus was significantly decreased in OD2 and OD3 groups, the degree of patient's satisfaction with analgesia was increased in group OD3 (P<0.05) , and no significant was found in the degree of patient's satisfaction with analgesia in group OD2 (P>0.05).Compared with group OD2, no significant was found in the incidence of adverse reactions (P>0.05) , and the degree of patient's satisfaction with analgesia was significantly decreased in group OD3 (P<0.05).The number of successfully delivered doses was significantly larger in group OD3 than in O, OD1 and OD2 groups (P<0.05).Conclusion Dexmedetomidine 2.5 μg/kg added to oxycodone 0.75 mg/kg is the optimum ratio of medicine dosage when used for PCIA after gastrointestinal surgery.
