3.The Therapeutic and Nursing Care Comparison of Vacuum Sealing Drainage to Treat Skin Avulsion Injury

[Objective] To summarize the therapeutic and nursing care of vacuum sealing drainage to treat skin avulsion injury. [Methods]We used the method of vacuum sealing drainage combined with split thickness skin graft and pre-and post-operation nursing care to treatment of 21 patients suffering with skin avulsion injury, and compared therapeutic effect with routine therapy group. [Result]Skin graft survival rate in VSD group was higher than in routine therapy group. The dressing change, wound healing time, hospitalization time in VSD group were lower in routine therapy group. [Conclusion] The method of vacuum sealing drainage is good in treatment of intervention skin avulsion injury.
4.Variation of serum ghrelin and leptin in patients with functional dyspepsia

Objective To investigate the alteration of serum ghrelin and leptin in patients with functional dyspepsia (FD) and their clinical significance. Methods Sixty patients with FD [-30 patients with postprandial distress syndrome(PDS) and 30 patinets with epigastric pain syndrome(EPS)] were enrolled. Thirty healthy subjects were severed as controls. The serum ghrelin and leptin were measured by enzyme linked immunosorbent assay and radioimmunosorbent assay, respectively. Results The concentrations of serum ghrelin and leptin were both significantly lower in patients with FD than in controls (P<0.01). Meanwhile, the concentrations of serum ghrelin and leptin in PDS patients were significantly lower compared with controls (P<0.01) and EPS patients (P<0.01). There was no significant difference in levels of serum ghrelin and leptin between controls and EPS patients(P>0.05). Conclusions The alterations of serum ghrelin and leptin in PDS patients are responsible for their reduction in FD patients. The PDS may he associated with abnormalities in gastrointestinal motility. There is an interaction between serum ghrelin and leptin in development of FD. The detection of serum ghrelin or leptin is helpful in classification and treatment of FD.
5.Establishment of the Guidelines for Assigned Xingnaojing Injection of Medical Insurance in Beijing

OBJECTIVE:To promote rational use of Xingnaojing (XNJ) injection in medical insurance patients.METHODS:Clinical trials on indications of XNJ were searched systematically,and clinical evidence that XNJ was applied for different indications were collected and classified.A questionnaire survey was carried out in 20 medical institutions to find out the doctors' attitude and prescribing practice relating to indications,application indications,usage and dosage,timing of application and drug combination of XNJ.The results of above 2 kinds of studies were provided to a group of experts to reach consensus about guideline of XNJ.RESULTS:Guidelines of XNJ,which contained the application indications,timing of medication,dosage,treatment course and drug combination,were developed.The strength and degree of evidence were labeled for each items of recommendation.CONCLUSION:For drugs with low-quality clinical evidence,guideline should be developed according to experts' consensus which is based on evidence-based medicine.
6.Clinical and Imaging Analysis of Acute Intussusception in Infant

Objective To discuss the imaging representations and its clinical significance of acute indigitation in infants.Methods The imaging representations including CT,ultrasonography stand abdominal plain film and gas clysters of indigitation comfirmed by operation in 58 infants were analysed.Results (1)Of 58 cases,33 were ileum-colon and ileum-caecal type,25 were ileum-ileum-colon,ilem-cecal-colon and ileum-colon-colon type;(2)The main imaging representations were as following:gas decreased in colon of small intestine (12 cases),inflation in intestine and incomplete ileus (18 cases),ileus of small intestine(17 cases),no abnormity on stand abdominal plain film (8 cases).Gas clyster showed the "goblet sign" and masses (28 cases).The sign of targed rings was seen on ultrasonography(3 cases).The layered homocentric round tumors showed by CT (1 case).Conclusion The imaging representations of acute indigitation in infants are typical,which are significant for the diagnosis.
7.A Study of Evaluation on Life Quality among Tuberculosis Patients

Objective To evaluate the quality of life (QOL)among patients with anti -TB treatment in Zhoushan city and to analyze the influence factors.Methods Our self -designed questionnaire and WHOQOL -BREF scale were used to investigate the general information and the quality of life among 187 tuberculosis patients.Scores of life quality under different conditions were compared.Results The scores of physiological domain,psychological domain,social relations and environmental domains were 61.13 ±13.12,53.24 ±16.59,59.91 ± 12.08 and 60.12 ± 15.68 respectively. The scores of all domains excluding the environmental relations were significantly lower than the normal level (P <0.05). The scores of males were significantly higher than females in psychological domain (P <0.05),and the score differences among age groups were of statistical significance both in physiological domain and psychological domain (both P <0.05). Those with high education levels scored higher than those with lower education level in psychological and social domains (both P <0.05).The scores of different economic income in 4 domains showed significant differences (all P <0.05). The first time treated,hospitalization time less than 1 month and effectively treated patients scored higher than those not (all P <0.05).Conclusion The quality of life among tuberculosis patients could be influenced by sex,age,income, education,and treatment factors,so appropriate health care such as psychological support and health education should be provided.
8.miR-548a-3p targeting MMP-2 inhibits the metastasis and invasion of gastric cancer cells

Objective To study the mechanism of miR-548a-3p targeting MMP-2 inhibits the metastasis and invasion of gastric cancer cells.Methods Bioinformatics were used to search those miRNAs targeting MMP-2.The level of miR-548a-3p in gastric cancer tissues were detected by qRT-PCR and the expression of MMP-2 were detected by Western blot.The level of miR-548a-3p in three reconstructed gastric cancer cell lines were detected by qRT-PCR and the expression of MMP-2 were detected by Western blot.miR-548a-3p targeting the 3'-UTR of MMP-2 were detected by Luciferase reporter assay detected.And the changes of ability of metastasis and invasion were examined by Transwell experiment before and after transfection.Results There were high expression of miR217 and low expression of MMP-2 in human poorly differentiated gastric cancer cell line BGC-823.There were low expression of miR-548a-3p and high expression of MMP-2 in human middle and high differentiated gastric cancer cell line MKN-28 and SGC-7901 (P < 0.05).The result of Luciferase reporter assay showed that miR-548a-3p can targeting the 3’-UTR of MMP-2.There were high expression of miR-548a-3p and low expression of MMP-2 in MKN28-miR-548a-3p mimics.There were low expression of miR-548a-3p and high expression of MMP-2 in others reconstructed three cell lines.There were smaller invaded cells in SGC-7901-miR-217 mimics than in others three cell lines (P < 0.05).Conclusions miR-548a-3p can target the 3'-UTR of MMP-2 and down regulate its expression and inhibit the ability of invasion of gastric cancer cells.
9.Strengthening Clinical Application and Management of Antibacterials

OBJECTIVE To control abuse use of antibiotics.METHODS Various methods were used to strengthen the management of antibacterials,for example,fully exert the rational use antibiotics,enforce the training of the doctors and nurses for the rational use of the antibiotics,and stipulate the rules and principles for the use of antibiotics.RESULTS The management of antibacterials was strengthened,and the rational use of antibiotics was improved.CONCLUSIONS To enhance surveillance of antibacterial use and prevent antibacterial unrational use are propitious to control occurrence of nosocomial infections.
10.Strong IgA-mediated mucosal immune responses elicited in BALB/c mice primed with Vaccinia Tian-tan-based HIV vaccine intramuscularly and boosted with protein intranasally

Objective To analyze the mucosal immune responses induced in BALB/c mice after immunization with Vaccinia Tiantan-based HIV vaccine in combination with protein and to evaluate the effi-cacy of different immune strategies and adjuvants.Methods The BALB/c mice were intramuscularly or in-tranasally immunized with the recombinant Vaccinia virus Tiantan strain ( rTV) carrying CN54 Gag-Pol-Env gene and boosted with the gp140 protein and MF59 adjuvant by intramuscular, intranasal or subcutaneous in-jection.Serum, saliva and vaginal lavage samples were collected from the BALB/c mice.The titers of anti-gen specific IgG and IgA were detected by ELISA.Results Significantly enhanced mucosal immune respon-ses were induced by immunization with gp140 protein used in combination with MF59 adjuvant.The IgA an-tibodies elicited in mice mucosal tissues by gp140 protein and MF59 adjuvant were as high as those induced by gp140 protein and cholera toxin subunit B or recombinant flagellin adjuvant.High tiers of gp140-specific IgA antibodies were observed in serum, saliva and vaginal lavage samples from the mice intramuscularly primed with rTV and intranasally boosted with gp140 protein and MF59 adjuvant.The tiers of IgA antibodies in serum and mucosal tissue samples were 1 ∶15 000 and 1 ∶600, respectively.Conclusion High titers of mucosal IgA antibodies were elicited in BALB/c mice intramuscularly primed with Vaccinia Tiantan-based HIV vaccine and intranasally boosted with gp140 protein and MF59 adjuvant, especially in vaginal and oral tissues.This immunization strategy might be able to block the heterosexual transmission of HIV-1 through va-ginal and oral mucosa.
