1.Postoperative functional restoration of the elbow for adults with supracondylar or intercondylar fractures of the humerus

Objective To study the factors which affect the functional restoration of the elbow for adults with supracondylar or intercondylar fractures of the humerus treated with open reduction and internal fixation. Methods 22 patients with supracondylar or intercondylar fractures of the humerus were treated with open reduction and internal fixation from January, 1999 to December, 2004. They were followed up for an average of 30 months and complete clinical data were kept. The function of the elbow was evaluated with the Mayo elbow performance index. The relationship between the results and the factors —initial time of postoperative rehabilitation, fracture classification, operative approach and interval from injury to operation —were analyzed with cumulative logistic regression model of SAS software V8.2 (SAS institute, Cary, North Carolina). Results Of the 22 cases, 7 were excellent, 9 good, 5 fair and 1 poor. The excellent and good rates were 72.7%. And the odds ratio estimates of the 4 factors were 0.786, 0.299(B/A)0.221(C/A), 1.038, 0.197 respectively. Conclusions The initial time of postoperative rehabilitation, the fracture classification and the interval from injury to operation affect the functional restoration whereas the operative approach does not. Early operation, anatomical reduction, stable fixation and early rehabilitation will help improve the functional restoration.

4.Visual cortex activation range of color vision with BOLD-fMRI in anisometropia amblyopia children

AIM:To analyze the recovery of visual cortex activation range of color vision in anisometropia amblyopia children after treatment by using blood oxygen level dependence-functional magnetic resonance imaging ( BOLD- fMRI ) and SPM8 software.
METHODS:Self-control study. This fMRI study directly compared activity in visual cortex produced by color monocular stimulation in 13 monocular anisometropia amblyopia children. The project of study was blocked design. The data of functions and anatomical MRI was stimulated by three-primary colors and was preprocessed and analyzed by SPM8 that based on MATLAB software. According to the data, we compared the change of the central area of color vision when first visit and 1, 2 ,4wk after treatment.
RESULTS: The BA17, BA18, BA19 and BA37 of amblyopia children were different activated after they accepted the three-primary colors stimulation (P<0. 01). After 4wk treatment, amblyopia children accepted red stimulation, the visual cortex activation range without expanding; after accepting green stimulus, on the left side of the visual cortex BA37, BA19, BA20, the average t value were 3. 5210, 3. 2716, 3. 1534 ( P< 0. 01 ); after accepting blue stimulation, the scope of the right side of visual cortex at BA19, BA18 were expanded to different extent, the average t value were 3. 7345, 3. 2701 (P<0. 01).
CONCLUSION: After 4wk treatment, visual cortex activation of color vision of anisometropia amblyopic children is expanded, but with low extent, after the short-term treatment of amblyopia, visual cortex activation of color vision has a certain degree of recovery but not obvious.
5.Protection of Oxymatrine on Myocardium of Mice with Chronic Viral Myocarditis

Objective To observe the expression of anti-cardiac myosin antibody(AMA) and the collagen volume fraction(CVF) in serum in mice with chronic viral myocarditis(CVMC),and to explore the preventive and protective function of oxymatrine on myocardium.Methods BALB/c mice(n=60) were infected with coxsackievirus B3(CVB3) of increased dose biweekly to establish CVMC model.Mice in normal control group(n=8) received equal-volume 9 g?L-1 saline without CVB3 at the same time.The surviving mice in CVMC model group were randomly divided into CVMC control group(n=8) and oxymatrine therapy group(n=8) at the 42th day.From then on,mice in oxymatrine therapy group were treated with oxymatrine at the dose of 100 mg?kg-1?d-1 by gastric perfusion once a day for 28 days,and meanwhile mice in CVMC control group and the normal control group received equal-volume 9 g?L-1 saline by gastric perfusion every day.Then all mice were sacrificed at the end of the experiment.The ratio between heart weight to body weight(HW/BW) was calculated.Myocardium slides were stained with collagen specific Picric acid-Sirius red staining,and the CVF was calculated with image analysis software.The serum level of AMA(optical density value,A value) was detected by enzyme-linked immunosorbent assay(ELISA).Results HW/BW(0.007 9?0.000 3),CVF [(15.30?1.08)%] and the A value of AMA(0.286?0.053) in mice of CVMC control group were increased significantly compared with those in normal control group HW/BW(0.005 5?0.000 2),CVF[(6.84?1.11)%],the A value of AMA(0.160?0.050)(Pa
6.Influence of Human Chorionic Gonadotropin on Testicular Germ Cell Apoptosis of Experimental Unilateral Cryptorchidism in Rats

Objective To explore the influence of human chorionic gonadotropin(HCG)on testicular germ cell apoptosis of experimental unilateral cryptorchidism in rats.Methods Forty immature male Sprague-Dawley rats were randomly divided into unilateral cryptorchi-dism group(n=20) and sham operation group(n=20).The 2 groups were divided into group treated with HCG and group without HCG.At age 21 days,unilateral cryptorchidism was produced.Half of the rats were injected with 20 U HCG from day 22 to 34 every other day.At age of 35 d and 60 d,rats were sacrificed for detection germ cell apoptosis by terminal deoxynucleotidyl transferase mediated dUTP nick end labeling(TUNEL).Results Apoptosis index(AI) of cryptorchidism testis were higher compared with the scrotal testis in sham operation group(P0.05).AI of the scrotal testis of sham operation group and unilateral cryptorchidism group with HCG was higher compared with the correspond groups without HCG,and there were significant difference of AI between sham operation group and the correspond group without HCG at age 35 days(P0.05).Conclusions AI of testicular is increased both in cryptorchidism testis and scrotal testis in experimental unilateral cryptorchidism;HCG adds the number of apoptotic germ cells,and histology damage of testis is not completely recovery after stop using HCG.So clinical application of HCG must be cautious and operation ought to be done as early as possible in cryptorchidism.
7.Selection of short- or long-segment posterior fixation for treatment of unstable thoracolumbar fractures

Objective To discuss the selection of short- or long-segment posterior fixation for treatment of unstable thoracolumbar fractures. Methods A total 134 patients with thoracolumbar fractures treated with pedicle instrument fixation from January 2005 to December 2008 were studied retrospectively. According to AO fracture classification, there were 70 patients with type A fractures, 37 with type B and 27 with type C. The patients were divided into two groups according to the number of instrumented levels; short-segment posterior fixation (SSPF) group (four screws; one vertebral body above or below the fractured vertebrae) and long-segment posterior fixation (LSPF) group (eight screws; two vertebral bodies above or below the fractured vertebrae). Clinical outcomes and radiological parameters (superior-inferior endplate angle, vertebral body angle, displacement of vertebral body) were compared according to AO fracture classification. Results All type A fractures were treated with SSPF, mean superior-inferior endplate angle changed from preoperative 21.3° to postoperative 8.5° and 11.1° at final follow up. There was no statistical difference in the correction of Cobb angle for type B fractures in SSPF group (26 patients) and LSPF group (11 patients), while the correction loss of vertebral body angle was 3. 64° in SSPF group and 1.09° in LSPF group, with statistical difference (P < 0. 05). There was no statistical difference in the correction of Cobb angle for type C fractures in SSPF group (7 patients) and in LSPF group (20 patients), but the correction loss of vertebral body angle was 3.6° in SSPF group and 0. 8° in LSPF group, with statistical difference (P < 0. 05). There was no statistical difference in vertebral displacement correction. Conclusions Most types A and Bl fractures should be treated with SSPF; most types B2, B3 and C fractures should be treated with LSPF.
