Chinese Journal of Postgraduates of Medicine 2011;34(12):18-19
Objective To study the changes of interferon ( IFN )- γ ,interleukin(IL)-4 and IL-6 in endometriosis. Method The levels of peripheral blood IFN-γ ,IL-4 and IL-6 in 25 patients with endometriosis (EM group) and 20 normal adults (control group) were detected by enzyme linked immunosorbent assay (ELISA). Results The level of peripheral blood IFN- γ in EM group [(109.64±26.79 ) ng/L] was lower than that in control group [(155.17±30.36) ng/L] (P < 0.05 ), IL-4[(33.52 ± 8.69)ng/L]and IL-6 [(44.12 ± 3.57) ng/L] were higher than those in control group [(22.52 ± 6.54), ( 15.12 ±2.63 )ng/L](P<0.05 ).The ratio of IFN-γ/II-4(3.27±1.45 ) and IFN- γ/IL-6 ( 1.15 ± 0.56 ) in EM group were significantly lower than those in control group (6.89 ±1.68,2.54 ±1.03)(P <0.01 or <0.05).Conclusions Th1 cells shift toward Th2 cells in patients with endometriosis. Th1/Th2 ratio may be correlated with the pathogenesis of endometriosis.

A discussion of the connotation and key issues of the new normal in the healthcare sector, with an introduction to the new normal. The furthering reform is unveiling roadblocks in the healthcare system. For example, human resources available can hardly support development of the sector, and revenue growth of tertiary hospitals in the cities will decline, while county hospitals will be further challenged by debt crisis. In the meantime of steady elevation of medical capabilities in both urban and rural areas, the gap between the two will be maintained for a while. The authors predict that the sector will be faced with restructuring essential medicine system, grading diagnosis and treatment practice, rising risks for health insurance, and growing market shares of commercial health insurance and private capital, as well as fiercer competitions to challenge public hospitals.

Objective To observe the effect of wound specialist group on preventing and managing pressure ulcer in operation. Methods The knowledge about pressure ulcer and ability of risk assessment for pressure ulcer of operating room nurses were tested and the incidenc-es of pressure ulcer were compared before and after the intervention of the wound specialist group. Results After the intervention of the wound specialist group, the passing rate of nurses in operating room increased from 58.23%to 94.11%in pressure ulcer theory (χ2=29.63, P<0.001) and from 56.96%to 95.29%in new type of dressings paste (χ2=33.80, P<0.001), and the rate of pressure ulcer risk factor assessment increased from 56.38%to 93.35%(χ2=5828.07, P<0.001), accuracy of assessment increased from 56.23%to 96.78%(χ2=4674.89, P<0.001). The incidence of intraoperative acute pressure ulcer decreased from 1.5‰to 0.22‰(χ2=17.59, P<0.001). Conclusion The intervention of wound specialist group may improve the awareness of assessing risk factors of pressure ulcer in the operation room and standardize the oper-ation to prevent pressure ulcer, and reduce the incidence of pressure ulcer in operation.

Chinese Journal of Experimental Ophthalmology 2012;30(4):353-357
BackgroundCycloplegia is well accepted for the first refraction estimate in childhood.Yet no good evidence is offered in terms of which cyclopegia is preferable for the different ages and refractive status in children. ObjectiveThis study aimed to compare the effectiveness of cycloplegia between 1% cyclopentolate and 1% atropine sulphate before optometry in ametropia children. Methods This was a prospective clinical trail. The self matched-pairs control randomly observation was designed. One hundred and sixty eyes of 80 children of 4-9 years old with refractive error were recruited in this study.1% cyclopentolate eye drops were topically administered once per 5 minutes for 3 times and 1%optometry was performed 45 minutes after eye dropping.Three days after that,1% atropine then was used 3 times per day for consecutive 3 days and again the refractive diopter was obtained. The differences of the results in autorefraction, retinoscope and residual accommodation were compared between 1% cyclopentolate and 1%atropine eye drops. This trail was approved by the Ethic Committee and written informed consent was obtained from each custodian. Results The autorefraction values were ( 0.55 ±3.52 ) D and ( 2.22 ±3.52) D before and after the administration of 1% atropine with the difference value( 1.66± 1.62) D (t =13.02,P =0.00 ).The autorefraction value was( 1.74±3.46 ) D after dropping of 1% cyclopentolate and the difference value from that of 1% atropine was (0.48 ± 0.46) D ( t =13.08,P =0.00 ).The cy(e)lplegic autorefractions of atropine and cyclopentolate have strong correlation ( R2 =0.98,P =0.000 ).The residual accommodation values were ( 0.32± 0.44 )D and(0.05±0.41 ) D after dropping of 1% cyclopentolate and 1% atropine with the difference( 0.27±0.55 ) D ( t =4.56,P =0.00 ).The difference value of refractive diopter was (0.31 ± 0.37 )D in myopic group,(0.56±0.48 )D in moderate hypermetropic group and(0.59±0.50)D in high myopic group, and that of myopic group was significantly lower than the moderate hypermetropic group ( t =- 3.14,P =0.00 ).No significant difference was found in the autorefraction difference between 4-6 years group and 7 -9 years group [ ( 0.61 ±0.53 ) D vs ( 0.49 ±0.39 ) D ] ( t =1.21,P=0.23 ).The hidden value because of accommodation had weak correlation with the difference value between atopine and cyclopentolate(r=0.43,P=0.00). ConclusionsBoth 1% atropine and 1% cyclopentolate have the cycloplegic effects. This study suggestes that 1% atropine should be used for the optometry of hypermetropia children.

OBJECTIVETo investigate the effectiveness of anterior debridement, bone graft, and posterior pedicle screw-rod internal fixation in the treatment of lumbosacral tuberculosis.
METHODSFrom January 2005 to June 2012,18 patients with lumbosacral tuberculosis undergoing the surgical treatment by anterior debridement, bone graft, and posterior pedicle screw-rod internal fixation were reviewed retrospectively. There were 12 males and 6 females with an average age of 44 years old ranging from 35 to 67. Among them, 2 cases were simple low back pain,3 cases were low back pain combined with radiating pain of lower extremity and 13 cases were muscle strength hypesthesia. According to Fankle grading, the nerve function was grade C in 3 cases, grade D in 10 cases, grade E in 5 cases. Of these cases,L4.5 was involved in 8 cases,L5 in 4 cases and L5S1 in 6 cases. The lumbosacral angle was 150 to 270 and the erythrocyte sedimentation rate (ESR) was 45 to 93 mm/h before treatment. The clinical indexes including the lumbosacral incidence, Frankel grade and ESR were reviewed at follow-up.
RESULTSAll 18 cases were followed up for 14 to 22 months. The mean operation time was 180 min. The amount of bleeding was 400 to 800 ml. Except 1 case with iliac vein injuried and 4 cases with abdominal distension,no spinal injuries and severe relative complication occurred, and neurologic function improved in various degrees. Pain in lumbosacral area and radicular pain in lower extremities disappeared. The X-ray and CT films demonstrated that bony fusion was obtained in all patients during 9 to 13 months postoperatively. The lumbosacral angle and ESR were improved significantly.
CONCLUSIONAnterior debridement, bone graft, and posterior pedicle screw-rod internal fixation is an effective method in dealing with lumbosacral spine tuberculosis.
