1.Treatment of 1619 neonate-infants after open-heart surgery during perioperative period

Objective To study the treatment of neonate-infants after open-heart surgery during perioperafive period.Methods The clinical data of 1619 neonate-infants during perioperative period were analyzed.Results 68 cases died,mortality 4.2%.Postoperative complication 190 cases(11.7%).Conclusion The management of components and quantities of the infusion,admistration of cardiovascular drugs,the management of respiratory tracts and the protection of heart function,ale main points of perioperafive therapy of neonate-infants after open-heart surgery.
2.Influence of morning blood pressure on carotid atherosclerosis in elderly patients with essential hypertension

Objective To investigate the association between morniing blood pressure surge (MBPS) and carotid atherosclerosis in elder patients with essential hypertension. Methods According to the results of24h ambulatory blood pressure monitoring, 106 patients were classified as the morning BP surge group (MBPS group,n = 58) ,and nonsurge group (NMBPS group, n = 48). Patients underwent carotid ultrasound and the intima-medial thickness (CCA-IMT) and plaques were examined. Results The CCA-IMT of the MBPS group was significantly thicker than that the NMBPS group[(1.27 ± 0. 12)mm vs (0.92 ± 0.33 )mm], P < 0. 05 ) ;②Compared with the NMBPS group,the severity of carotid arteries plaque of the MBPS group was significantly higher (72. 15% vs 54.21% ), ( P <0. 01 ) ;③Pearson relation analysis showed CCA-IMT level positively correlated with age (r = 0.288, P < 0.001 ) ,the average of 24h SBP ( r = 0. 768 ,P < 0. 001 ), and MBPS ( r = 0. 768, P < 0.001 ). Conclusion The study showed that MBPS was closely related with carotid atherosclerosis in elder patients with essential hypertension and was an important risk factor in the process of atheresclerosis.

Objective To observe the efficiency of immunotherapy on allergic rhinitis and explore irs correla- tive factors.Methods 185 patients with AR were treated with immunotherapy in this department between 1993 and 2003.Specific allergens were selected through skin test.The patients were given subcutaneous injections with al- lergen extract produced by Xiehe Hospital.The doses were gradually increased to the maintenance dose.The clinical effectiveness evaluation based on the symptoms and signs before and after immunotherapy was judged as marked, moderated and poor response.The possihie factors which affected the efficacy of immunotherapy were selected.These factors included sex,age,illness duration,disease condition,chronic sinusitis,asthma,duration of immunotherapy and reaction condition of the skin test.The correlative factors with efficacy were sifted with multifactorial stepwise re- gression analysis.Results The marked,moderated and poor responses were 22.9 %,51.7 %,and 25.4 % respective- ly.The efficacy of immunotherapy correlated well with the duration of immunotherapy(P

Objective To compare the effect of high volume continuous blood purification (CBP) with routine CBP on multiple organ dysfunction syndrome(MODS).Methods 38 patients with MODS were randomly divided into high volume CBP group( treatment group,18 cases) and routine CBP group (control group,20 cases).And they were respectively detected on mean arterial pressure(MAP),dosage of Dopamine,PaO2/FiO2,APACHE [ score,lactic acid,length of stay and mortality in ICU post-treatment 24h and 48h.Results Compared with control group,MAP and PaO2/FiO2 on treatment group were more higher( P < 0.05 ),while dosage of Dopamine,APACHE Ⅱ score,lactic acid were significantly shorter than these of treatment group( P < 0.05 ),and length of stay and mortality in ICU were significantly lower than those of treatmen group [ ( 8.54 ± 4.15 ) d vs ( 11.82 ± 5.76) d,P < 0.05:22.2% vs 35.0%,P <0.05 ].Conclusion High volume CBP could reduce the mortality in ICU compared with control group routine CBP.
6.Gasless endoscopic surgery through chest for parathyroid tumor

Objective To investigate the possibility of chest approach gasless endoscopic surgery for the parathy-roid tumor. Methods Through a 2 cm incision on the surface of chest bone, a gasless procedure was developed to management of parathyroid tumor in our department. There were 2 cases of the parathyroid cyst-adenoma were dis-sected. Results 2 cases were successfully performed, and there were no paralyze of the revers laryngeal nerve and hypofunction of parathyroid could be observed. The cosmic and mini-invasive results were obtained. Conclusion Chest approach gasless endoscopic surgery for parathyroid tumor is a safe, mini-invasive, and cosmic procedure for selected patients.

Objective: To compare tea polysaccharides(TPS) characteristics and their role in scavenging free radicals and reducing blood glucose(BG) in diabetic mice(DM). Methods: TPS was extracted from green,Oolong and black tea which were made from the same fresh leaves from Hubei,Fujian and Yunnan. Then the recovery rate of TPS, contents of neutral sugar, uronic acid and protein were analysed, and scavenging rate of -2Oand 稯H in vitro and hypoglycemic effect were also determined. Results: 1. The yield and contents of neutral sugar, uronic acid and protein of green tea TPS were the highest, and those of black tea TPS were the lowest. Oolong tea TPS acted the best in scavenging-2O and 稯H . 2. The hypoglycemic effect of TPS from Hubei tea was the best . The effect of TPS extracted from semi-fermented Oolong tea and fermented black tea was better than that of non-fermented green tea. 3. There were obvious differences in yield, free radical scavenging rate and effect of reducing BG among TPS extracted from tea in different regions. TPS extracted from Fujian tea had the best effect in reducing BG,but that from Yunnan tea had not. Conclusion: There was remarkable effect of region and process on physico-chemical characteristics,effect of scavenging radical and reducing blood sugar TSP.

Objective To discuss the histology change,apoptosis state and Bcl-2,Bax expression after the bone cement leakage into the intervertebral disc in vertebroplasty.Methods Eight majority canis familiaris were studied.Three lumbar intervertebral discs(L2 to L5)in each dog were randomly classified into three groups(control group,PMMA group,and CPC group),the canine discs were stabbed by 18-gauge needle,and 0.1 ml cement was injected into them.Control discs were only stabbed and injected with nothing.Histology of all discs was studied 24 weeks after the operation.Terminal-deoxynucleotidyl transferase mediated nick end labeling(TUNEL)and immunohistochemisty were used to detect apoptosis and Bcl-2,Bax expression in the discs.The data were statistically analyzed by SPSS 12.0.Results Intervertebral disc degeneration was not found in control groups.In bone cement groups,however,ruptured or serpentine patterned fibers,decreased cellularity of the nucleus pulposus and condensed matrix of the nucleus pulposus were found in histologic results.The Bax protein decreased in the order of control group, CPC group,and PMMA group.However,the Bcl-2 protein increased in the order of control group,CPC group,and PMMA group.The histology grade was significantly different among the three groups under ANOVA analysis(P
9.Comparison of bone marrow smear, biopsy and flow cytometry in diagnosis of lymphoma patients with bone marrow involvement

Objective To compare the effectiveness,correlativity,and acceptability of the three methods:bone marrow smear,biopsy and flow cytometry in diagnosis of lymphoma patients with bone marrow involvement.Methods 68 cases of early stage lymphoma were studied by observing and comparing positive rates of the three methods:bone marrow smear,biopsy and flow cytometry.Results After confirming the bone marrow involvement,the positive rates for the 68 cases using bone marrow smear,biopsy and flow cytometry were 16.2 % (11/68),33.8 % (23/68) and 10.3 % (7/68) respectively.Bone marrow biopsy had the highest positive rate compared to the other methods.It was statistically significant when comparing the differences of the 3 methods (P < 0.05).According to the correlativity analysis of the 3 methods,bone marrow smear and biopsy correlated with each other (P =0.002),while flow cytometery did not (P =0.270).Conclusions Morphological examination of the bone marrow smear is a fundamental method to test bone marrow involvement in lymphoma.Bone marrow biopsy creates the highest positive rate and has clear advantage compared to the other methods,however,bone marrow smear must be used at the same time as a complement.
10.Role of heme oxygenase-1/carbon monoxide system in pulmonary ischemiareperfusion injury

AIM: To investigate the effect of heme oxygenase-1 (HO-1)/carbon monoxide (CO) system on pulmonary ischemia-reperfusion injury (PIRI) in rabbits. METHODS: Single lung ischemia and reperfusion animal model was used in vivo. The rabbits were randomly divided into three groups (n=10 in each), control group (C), PIR group (I-R), PIR+ hemin group (H) and PIR+zinc protoporphyrin IX (ZnPP) group (Z). Changes of several parameters which included plasma carboxyhemoglobin (COHb) at different time points, wet to dry ratio of lung tissue weight (W/D), the injured alveoli rate (IAR) and the HO-1 enzymatic activity were measured at 180 min after reperfusion in lung tissue. The tissue slides were also stained by immunohistochemistry (IHC) and in situ hybridization (ISH) for HO-1 to detect the expression of HO-1 in lung and to analyze the optical density. The lung tissue was prepared for electron microscopic observation at 180 min after reperfusion. RESULTS: The plasma content of COHb in I-R, H, and Z group increased in a time-dependent manner after I-R. But the increment of H group was higher than that of I-R group, while that of Z group was lower. The HO-1 activity in lung tissue was highest in H group, followed by IR group, Z group, and C group (P
