Anterior cruciate ligament is the most important ligament to maintain the anterior and rotation stability of knee joint. Rupture of anterior cruciate ligament is one of the most common injuries of knee joint, and thus leads to knee instability and traumatic osteoarthritis. Anterior cruciate ligament reconstruction is usually performed to restore the anterior stability of knee joint, and is considered to reduce the secondary injury of medial meniscus, lateral meniscus and cartilage. Thus anterior cruciate ligament reconstruction can improve the function of knee joint. Traditional single bundle technique to reconstruct anterior cruciate ligament has been performed for many years. This technique can restore the anterior stability of knee joint and has excellent clini?cal results. Nearly 61%patients showed obvious radiographic osteoarthritis 20 years after anterior cruciate ligament reconstruction using bone-patella-bone graft. But, there is no agreement regarding to reduce the development of osteoarthritis after reconstruc?tion of anterior cruciate ligament. However, it has been reported that osteoarthritis would develop after reconstruction of anterior cruciate ligament in long term follow up study. Recently, with the further understanding of anatomy of biomechanics of anterior cru?ciate ligament, new techniques for anterior cruciate ligament reconstruction are developed, such as double bundle reconstruction, anatomic reconstruction and individual reconstruction. It remains controversial that whether these new technique can prevent the development of osteoarthritis after rupture of anterior cruciate ligament. Currently, no reconstruction technique for anterior cruci?ate ligament is perfect, and every technique has advantages and disadvantages. In terms of reducing the prevalence of osteoarthri?tis after reconstruction of anterior cruciate ligament, which technique is the best still remains unclear. New treatment and evalua?tion methods should be developed. In the future, not only the restoration of stability of knee joint should be considered, but also the articular cartilage contact kinematics including tibiofemoral joint and patellofemoral joint after anterior cruciate ligament recon?struction. Reduction of the development of osteoarthritis is an important topic after reconstruction of anterior cruciate ligament.

Objective To investigate the effect of spleen-supplementing and lung-strengthening therapy on patients with chronic obstructive pulmonary disease(COPD)treated by mechanical ventilation and enteral-parenteral nutritional support. Methods Acute exacerbation COPD patients complicated with respiratory failure treated by mechanical ventilation were randomized into group A(n=32)and group B(n=30).The two groups received invasive or non-invasive mechanical ventilation, routine western medicine and enteral-parenteral nutritional support.Group A received oral use of Jianpi Yifei Granules(mainly composed of Radix Ginseng Destillata,Rhizoma Atractylodis Macrocephalae,Poria,Radix Ophiopogonis,Cortex Mori,Radix Astragali)and electroacupuncture(EA)on bilateral acupoints of Zusanli additionally. The treatment lasted 2 weeks. The changes of leg circumference and arm circumference, and serum levels of total protein(TP),albumin(Alb),prealbumin(PAlb)and hemoglobin(Hb)were observed. The outcomes of mortality rate during hospitalization, success rate of removing respiratory machine, time for parenteral nutritional support, time for invasive or non-invasive mechanical ventilation, fee for nutritional support and hospitalization fee in intensive care unit(ICU)were compared in the two groups. The incidences of complications of pneumonia, fungal infection, and upper gastrointestinal hemorrhage were also observed during mechanical ventilation. Results After treatment, serum levels of TP and PAlb were higher in group A than those in group B(P0.05);leg circumference and arm circumference in the two groups increased(P0.05).Time for parenteral nutritional support, time for invasive or non-invasive mechanical ventilation, and fee for nutritional support and hospitalization fee in ICU were lower in group A than those in group B(P
7.Three dimensional finite element simulation of humerus fracture caused by indirect force
Orthopedic Journal of China 2006;0(08):-
[Objective]To simulate the biomechanics mechanism and environment of humeral fracture caused by indirect impact force for the purpose of biomechanics understanding and treatment of such fracture.[Method]Based on the data source, which was high-resolution anatomic slice images from approximal clavicle to distal humerus, 1 mm thickness and totally 380 layers, the geometric model of total shoulder joint was established according to the order:point, line, area, and further meshed to set up the three dimension finite element model of shoulder, fracture sites and instantaneous stress and strain of humerus were simulated and analyzed under the condition which longitudinal impact force was loaded on the humerus based on the 12 functional positions of shoulder(abduction 30?、 45?、 60?、 90?, and simultaneous neutrality, internal rotation 45?,external rotation 45?).[Result]According to the humeral shaft load-strain curve in different functional positions of shoulder, linear relation was found when load changed from 0 N to 250 N, after which non-linear come out, and even load was removed , bone was deformed eternally. With the rise in load amount, the increase in stress was detected. When abduction degree changed from 90? to 30?, the strain of humerus, both the lateral and the medial increased gradually, and increase in internal rotation 45?and external rotation 45? was more significant than that in neutrality. Meanwhile, stress difference could be seen between the lateral and the medial , and medial was larger than the lateral. Increase in stress in rotation positions was quicker and more than that in other functional positions.[Conclusion]Based on 4 abduction degrees (30?, 45?, 60?, 90?) and 3 rotation degrees(neutrality, internal rotation 45?,external rotation 45?) ,the three dimensional finite element shoulder could simulate precisely stress, strain, general trend of fracture line, three dimension images of bone failure. Three dimension finite element simulation and analysis of shoulder is a valuable mechanical method for research on biomechanics theory related to humerus fracture.
