1.MR Diagnosis of Extrahepatic Biliary Obstructive Disease

Objective To study MR manifestations of biliary obstructive disease.Methods MR examination (Philips Gyroscan INTERA 1.0T)in 69 patients with biliary obstructive disease were performed.The conventional sequences included:T1WI/FFE,T2WI/TSE,T2WI/SPIR and contrast-enhanced at axial position,coronal B-FFE and MRCP.All images in all cases were compared with the results of operation and pathology.Results 43 cases were benign disorder,of them,42 cases were choledocholithiasis,one case was choledochal cyst.Choledocholithiasis presented as single or multiple round or ovoid with low signal.The direct sign of malignant lesions (26 cases) was the masses with slightly high signal at the obstructive part,the indirect sign of malignant lesions was obstruction or narrow of bile duct. The MR diagnostic accuracy of obstructive location was 100%, and the accuracy for evaluating the causes of obstruction was 89.9%.Conclusion MRCP combining with B-FFE and routine MRI, analysising the direct sign and indirect sign, can improve the diagnostic accuracy of obstructive jaundice.
2.Surgical treatment of ulnar lateral mass fractures of distal radius.

OBJECTIVETo investigate the type and corresponding operation methods of ulnar lateral mass in distal radial fractures.
METHODSFrom January 2004 to September 2010,32 cases of distal radial fractures with ulnar lateral mass were treated by surgical incision and relocate ulnar lateral mass,including 23 males and 9 females with an average age of (34 +/- 9) years old ranging from 28 to 65 years old. According to the classification of Melone-Doi, 2 cases with type I, 24 cases of type II, 6 cases with type III. The operative appoach involved palm-radial, palm-ulnar, radial-dorsal. Among them, 28 patients with closed fractures were fixed with plate, 4 cases (including 2 cases of open fractures) were fixed by Kirschner pin and braces.
RESULTSThirty-two patients were followed up for 8 to 18 months with an average of 12 months. All fractures were healing without complications. According to modified Gartland-Werley scoring system (GWSS), the total score was 1.12 +/- 0.45, the remain deformity was 0, subjective evaluation was 0.50 +/- 0.30, objective evaluation was 0.30 +/- 0.21, complications was 0.40 +/- 0.09; the result was excellent in 21 cases, good in 11 cases.
CONCLUSIONThe location of ulnar lateral mass in distal radial fractures is one of the important prognostic factors in wrist function and surgical treatment is an effective method of fixing ulnar lateral mass in distal radial fractures.

3.Changes of astrocytes and apoptosis of neurons in facial nerve nucleus after facial nerve transection in rats

Objective To observe the changes of astrocytes and expressions of bcl 2 and bax in neuron apoptosis in facial nerve nucleus after facial nerve transection in rats Methods A total of 72 rats were equally divided into normal control, and facial neurotmesis groups. Six rats were killed 3,7,15,21,30 and 60 d after transection respectively and same amount of rats were killed in same time interval in the control group. Brain samples were taken from the aboue mentioned rats and the facial neuron apoptosis was detected with TUNEL, and expressions of bcl 2 and bax and changes of GFAP in the astrocytes of facial nerve nucleus were studied with immunohistochemical methods and image analyses separately Results The apoptotic peak of facial neurons and remarkable expression of bcl 2 and bax appeared in 15 d after the facial neurotmesis, while the expression ratio of bcl 2/bax were the lowest Meantime, the reaction of the astrocytes(GFAP)was remarkable, too All the changes were statisfically significantly compared with the control groups ( P

5.Proteomic Analysis of Hypertrophied Left Ventricle in Spontaneously Hypertensive Rats

Objective To investigate the molecular mechanisms for the development of cardiac hypertrophy in hypertension,the present study provided the differential protein expression analysis of hypertrophied heart at differ- ent stages in spontaneously hypertensive rats (SHR).Methods The profiles of protein expression of left ventricu lar myocardium in SHR and its normotensive control Wistar-Kyoto (WKY) rats at the age of 4 and 20 weeks were analyzed with two-dimensional gel electrophoresis (2-DE) in combination with matrix assisted laser desorption ioniza- tion-time of flight (MALDI-TOF-TOF) mass spectrometry.Results Although the blood pressure of SHR was normal at 4 weeks age,hypertrophy of the left ventricle had already developed.The expression pattern in the hy- pertrophic myocardium was found 27 modulated proteins,20 of which were identified.These proteins are involved in reactions of energy metabolism,mitoehondrial oxidative phosphorylation and oxidative stress,etc.The expres- sion of 13 proteins was significantly changed in SHR rats at early stage prior to the development of sustained hyper- tension,while the expression changes of other 7 proteins occurred only at late stage in SHR rats when the blood pressure was significantly elevated.Conclusions lncrease in glycolysis and decrease in oxidation of fatty acid and glucose was shown in the hypertrophied myocardium from early stage in SHR prior to the development of hyperten sion.The significant changes in protein expression of the mitochondrial electron transport chain and antioxidative molecules support the hypothesis that oxidative stress promotes and accelerates the development of hypertensive car- diac hypertrophy.
6.Shuxuening injection combined with routine therapy in treating patients with active ulcerative colitis: an analysis of efficacy .

OBJECTIVETo observe the therapeutic effect and relevant mechanism of shuxuening Injection (SI) in treating patients with active ulcerative colitis (UC).
METHODSTotally 91 patients with active UC were randomly assigned to 2 groups, 44 in the control group and 47 in the treatment group. Patients in the control group received routine treatment, while patients in the treatment group additionally received intravenous injection of SI (15 mL), twice daily for 14 days in total. Colonoscopy was performed before and after treatment. The therapeutic effect was assessed by Mayo scoring system and the grading of activities evaluated by Baron endoscope. Serum levels of IL-6 and TNF-α were detected by ELISA. The activity of SOD was detected by xanthine oxidase method. The content of MDA was detected by thiobarbituricacid (TBA). Besides, 20 healthy subjects were recruited as the healthy control group.
RESULTSTotally 82 patients completed the study (40 in the control group and 42 in the treatment group). There was no statistical difference in serum levels of IL-6, TNF-α, SOD, MDA, the Mayo score and endoscope grading between the two groups before treatment (P >0. 05). Compared with the healthy control group, serum levels of IL-6, TNF-α, MDA significantly increased (P <0.01), and the serum SOD level decreased (P < 0. 05) in the treatment grup and the control group before treatment. Compared with before treatment in the same group, serum levels of IL-6, TNF-α, MDA, the Mayo score and endoscope grading all decreased in the treatment group and the control group after treatment (P <0. 01, P <0. 05). Compared with the control group after treatment, serum levels of IL-6, TNF-α, MDA, the Mayo score and endoscope grading all decreased (P <0.01, P <0.05), the serum SOD level increased (P <0.05) in the treatment group after treatment. The serum SOD level was obviously negative correlated with serum levels of IL-6, TNF-a, Mayo score, and endoscope score (r = -0. 621, -0.638, -0. 509, -0.787, P <0.01). The serum MDA level was obviously positive correlated with serum levels of IL-6, TNF-α, Mayo score, and endoscope score (r =0.711, 0. 882, 0. 525, 0. 639, P <0.01).
CONCLUSIONSI could improve inflammatory injury and clinical symptoms of patients with active UC, and its mechanism might be associated with antioxidant and scavenging oxygen free radicals.

7.Effect of Dexmedetomidine Combined Electrical Stimulation on Coanitive Function of Patients Receiving Extracerebral Intervention.

OBJECTIVETo explore the effect of dexmedetomidine combined electrical stimulation on cognitive function of neurosurgical diseases patients treated by extracerebral intervention.
METHODSTotally 122 patients with neurosurgical diseases who underwent selective intervention were randomly assigned to the observation group and the control group, 61 cases in each group. Patients in the control group recieved anesthesia by dexmedetomidine. Those in the observation group received electrical stimulation at Baihui (DU20), Yintang ( EX-HN3), and Neiguan (PC6) before dexmedetomidine anesthesia. The cognitive function of patients at preoperative day 1 and postoperative day 1 was respectively evaluated by Mini-Mental State Examinations (MMSE). Serum NSE, S-100β, IL-1β, IL-6, and TNF-α levels were detected in the two groups before intervention and immediately after intervention using ELISA.
RESULTSMMSE scores of two groups were significantly reduced at post-intervention day 1, as compared with one day before intervention. MMSE score of the observation group at post-intervention day 1 was (23.15 ± 1.87) points, significantly higher than that of the control group [ (19.34 ± 1.64) points , (P < 0.05)]. The postoperative cognitive dysfunction (POCD) incidence rate of the observation group was 16.4% (10/61), significantly lower than that of the control group [39.3% (24/61); P < 0.05]. Compared with before intervention, NSE and S-100β protein levels, IL-1β, IL-6 and α-TNF levels of the two groups increased (P < 0.05). Post-intervention NSE and S-100β protein levels, IL-1β, IL-6 and α-TNF levels were significantly lower in the observation group than in the control group (P < 0.05).
CONCLUSIONDexmedetomidine combied electrical stimulation could effectively prevent the occurrence of postoperative cognition, and reduce levels of NSA, S-100β, IL-1β, IL-6 and TNF-α.

8.Screening of Streptococcus Agalactiae of Genitourinary Tract in the Late Pregnant Women in Guizhou Province and the Genetic Sequencing Analysis

Objective To investigate the clonization of Streptococcus agalactiae (GBS) in the genitourinary tract of late pregnant women and perform the CAMP negative strains identification and analysis in Guizhou province.Methods Collected genital tract secretions from out-patient women in late pregnancy in the obstetric of Guizhou Provincial People's Hospital during the period from May 2016 to May 2017.Then the collected secretions conducted the bacteria culture and identification.The CAMP-negative GBS strain was furher identified by biochemical test and 16S rDNA sequence analysis,and cfb gene detection was conducted.Results Among the 3 794 samples,118 Streptococcus agalactiaes strains were isolated,and the positive rate was 3.11%.One of them was beta-hemolytic and the CFB gene was positive,but the CAMP test was negative,which was confirem to be Streptococcus agalactiae via 16S rDNA sequence analysis.The results showed that no GBS had been found to be resistant to Penicillin,Amoxicillin,Liniazolamine,Vancomycin,Cefotaxime.Cefepime and the strains with high drug resistence rate were followed by Erythromycin (81.3 %),Clindamycin (62.7 %),Levofloxacin (72.9 %).Conclusion CAMP-negative was extremly rare in Streptococcus agalactiae,and only one strains of the 3 794 samples was isolated.Therefore,the clinical laboratory should be alert to CAMP negatie Streptococcus agalactiae for avoiding omission and providing more accurate test results for clinic.Also,the monitoring of drug-resistant strains of GBS should be strengthen.
10.Comparison of two antibiotic prophylaxis schemes for the prevention of recurrent urinary tract infection in postmenopausal women

ObjectiveTo compare the efficacy and safety of intermittent patient-initiated single-dose antibiotic prophylaxis and continuous antibiotic prophylaxis for the prevention of recurrent urinary tract infection (UTI) in postmenopausal women. MethodsA randomized controlled clinical trial was conducted for the prevention of recurrent urinary tract infection. Single dose of antibiotic was given every night in continuous antibiotic prophylaxis group and every time after exposure to conditions predisposed to UTI in intermittent antibiotic prophylaxis group. The duration of prevention was 12 months in both groups. ResultsThe effective rates of intermittent antibiotic prophylaxis and continuous antibiotic prophylaxis were 71.0% and 81.8% respectively (P>0.05). The incidence of gastrointestinal adverse reaction in intermittent antibiotic prophylaxis group was significantly lower than that in continuous antibiotic prophylaxis group (7.7% vs 28.6%,P<0.05). ConclusionsCompared with continuous antibiotic prophylaxis, intermittent patient-initiated single-dose antibiotic prophylaxis is a better prophylaxis with less gastrointestinal adverse reactions for the prevention of recurrent urinary tract infection in postmenopausal women.
