2.Effectiveness and safety of terbinafine for onychomycosis:A systematic review

Objective The standard continuous terbinafine regimen (SCTR) is a classical treatment for onychomycosis but has many deficiencies .Therefore, researchers have tried several other therapeutic options .This study systematically evaluated the ef-fectiveness and safety of SCTR versus other therapeutic options in the treatment of onychomycosis . Methods We searched the PubMed, Cochrane Library, Embase, CNKI, CBMdisc and Wanfang databases for the randomized controlled trials on the treatment of onychomycosis with terbinafine .According to the study design , two authors independently selected the literature , extracted data , con-ducted data checking , and then performed meta-analysis. Results Totally, 20 randomized controlled trials were included .The pooled analysis showed that SCTR had a higher long-term mycological cure rate than pulse therapy ( P=0.01, RR=0.81 [0.69, 0.96]), COMBO (P=0.05, RR=0.68 [0.46, 1.01]), and intermittent strategy (P=0.0001, RR=0.81 [0.73, 0.90]). However, SCTR exhibited a lower long-term mycological cure rate than continuous +topical treatment ( P =0.005, RR =1.19 [1.05, 1.35]) and >3 mo continuous therapy (P=0.02, RR=1.12 [1.02, 1.24]).For moderate-to-severe mycological infec-tions, SCTR +aggressive debridement had potential advantages over SCTR alone .Compared with SCTR , continuous +topical treat-ment induced fewer gastrointestinal reactions (P=0.03, RR=0.24 [0.07, 0.90]), and intermittent therapy had lower incidences of adverse events (P=0.03, RR=0.81 [0.67, 0.98]) and taste disorders (P=0.05, RR=0.44 [0.19, 1.00]).Bias risk assess-ment based the tool a domain-based evaluationrecommended by the Cochrane Library found that some high-risk biases in the includ-ed studies might impair the credibility of the results . Conclusion The best terbinafine regimen for onychomycosis remains to be fur-ther investigated with more high-quality randomized controlled trials .
3.Surgical effect of traumatic lens dislocation with secondary glaucoma

AIM: To retrospectively evaluate the effect of lens extraction combined with vitrectomy to treat traumatic lens dislocation with secondary glaucoma.
METHODS: Thirty - one eyes ( 31 cases ) of lens dislocation caused by blunt trauma with secondary glaucoma were treated respectively with cataract extraction combined with anterior vitrectomy, trabeculectomy and intraocular lens implantation. The visual acuity and pressure were observed 1wk, 1 and 3mo after operative.
RESULTS: Thirty - one eyes were all complete the operation successfully, and 6 eyes were given combined trabeculectomy, 9 eyes were implanted anterior chamber intraocular lens implantation ( IOL ) and 15 eyes were given posterior chamber suture fixation. Sixteen eyes were implanted in one-stage operation, while 8 eyes were implanted in two-stage operation. All intraocular pressure ( IOP ) were controlled to the normal level after operation and 23 eyes had visual acuity of more than 0. 3. CONCLUSION: Lens extraction combined with vitrectomy is an effective method for treatment of lens dislocation with secondary glaucoma. In order to control the IOP and get well visual function, we should choose IOL implantation or trabeculectomy according to the patient's condition.

5.Expression of Caspase-12 in cellular apoptosis of hippocampus in the rat model of posttraumatic stress disorder

Objective To observe changes of apoptosis and Caspase-12 expression in hippocampus of the rat model of posttraumatic stress disorder ( PTSD ) .Methods Single prolonged stress ( SPS ) was used to duplicate a rat model of PTSD.Male Wistar rats were randomly divided into 1, 4, 7 days groups after exposure to SPS and a normal control group.Transmission electron microscopy was used to observe morphologic changes of the hippocampus neurons .The expression of Caspase-12 was detected by immunohistochemistry , Western blotting and RT-PCR.Results After SPS, the hippocampus neurons had characteristic morphologic changes of apoptosis , and the expression of Caspase-12 reached the peak at SPS 4d.Conclusion PTSD activates the ERS-mediated apoptosis in hippocampus via Caspase-12, which may be a pathophysiology base of the atrophy of the hippocampus in PTSD .
6.Therapeutic effect observation of combined intra-arterial with intravenous versus single intravenous thrombolysis for patients with acute cerebral infarction

Objective To analyse the clinical efficacy of combined intra-arterial and intravenous(IA/IV) versus single intravenous(IV) thrombolysis using recombinated tissue plasminogen activator( rt-PA ) for acute cerebral infarction.Methods 20 patients with acute cerebral infarction were treated with IA/IV (10 patients) versus IV (10 patients) thrombolysis using rt-PA. Europe stroke scales (ESS) and Barthel Index (BI) scores were analyzed respectively both before and after the thrombolysis for the 20 patients. 7.CT diagnosis and surgical treatment of retroperitoneal paraganglioma

Objective To evaluate the surgical treatment and CT diagnosis of retroperitoneal paraganglioma.Methods Clinical data,CT image and surgical treatment of 17 cases diagnosed as retroperitoneal paraganglioma were analyzed retrospectively.Results In CT scan,tumors were singular and located around the abdominal aorta and inferior vena cava in 13 cases,while in the other 4 cases tumors were located in the lower abdomen and pelvic cavity.The masses were oval shaped,varied in size and the margins were clear.Most mass had integral capsule.The density were uneven and the low density area didn't have enhancement in contrast enhancement CT.All of the 17 patients received complete tumor resection in which 13 cases were pathologically diagnosed as benign,4 cases was boundary or low malignant and 3 cases received second operation for recurrence.Conclusion CT is helpful in diagnosis and locating retroperitoneal paraganglioma.Surgical resection is effective and satisfactory.
8.Influence of Aspirin on growth and NOS expression in astrocytoma cell line

Objective:To investigate the effect of Aspirin on the proliferation and NOS expression in astrocytoma cell line,and probe into ins mechanism.Methods:The effect of Aspirin on the growth of astrocytoma cells evaluated by MTT assay;NOS protein levels were determined by immunohistochemistry,NO and CEA concentration in the medium were determined by Griess assay and lepton catch immuning method respectively.Results:Aspirin can inhibit the growth of astrocytoma cells,induce the expression of iNOS,increase the concentration of NO in the medium. The effect of these were in a concentration dependent pattern. Moreover,Aspirin can reduce the concentration of CEA in the medium.Conclusion:Aspirin inhibit the growth of astrocytoma cell line.UP regulated iNOS expression resulting a increase of NO concentration are ascribed to mechanism of antrproliferation activity of Aspirin. CEA is a good indicator in monitoring curative effect of astrocytoma.
