Periventricular leukomalacia in premature infants is the most common brain injury.It often results in spastic cerebral palsy,cognitive and behavioral abnormalities and other sequelae.The microglia is the primary immune response cell in brain,and it plays an important role in balancing mechanism for the defense response and brain injury.The suppression of its activated reaction can reduce the cytotoxic effect and pathological damage in the early phase.

Objective To explore the clinical value of colposcopy directed biopsy in diagnosis of vaginal intraepitheal neoplasia (VaIN).Methods The clinical data of 106 VaIN patients diagnosed by colposcopy directed biopsy were reviewed and analyzed retrospectively.In the past medical history of 106 patients,46 patients had cervical cancer history,43 patients had cervical intraepithehal neoplasia history,13 patients had previous hysterectomy because of benign disease and 4 patients had uncertain past medical history.Results Among 106 patients,97 patients(91.51％,97/106) had abnormal pap smears of vagina,9 patients (8.49％,9/106) had abnormal pap smears of cervix.Histological analysis revealed that 69 patients were VaIN Ⅰ,24 patients were VaIN Ⅱ,13 patients were VaIN Ⅲ.The high-risk human papilloma virus (HPV) test at diagnosis were positive in 98 patients (92.45％,98/106),negative in 8 patients (7.55％,8/106).Eighty-seven patients VaIN were localized in the upper one third of the vagina,19 patients VaIN were localized in other parts of vaginal wall.Ninety-eight patients VaIN affected to the multifocal vaginal wall,8 patients VaIN affected to the focal vaginal wall.Conclusions It has highly therapeutic value of colposcopy directed biopsy in diagnosis of VaIN.But should closely correlated to past medical history,colposcopy should performed on the whole vagina wall and multiple punch biopsy to elevated positive rate.

Salt formation is the most important method for increasing druggability of active pharmaceutical ingredients ( API) . Nowadays, API containing different bases are emerging continuously. In this paper, the influence of various bases on the pharmacokinetics, pharmacodynamics and side effects of drugs was reviewed, which can provide reference for the selection of clinical therapeutic drugs and the base choice for new drugs.

Objective To explore the synchronous cosmetic surgical procedures for the beauty-seekers who have both single eyelid and commonly horizontal cockeye in order to acheive either time saving or beautifying. Methods Firstly, the distance of moving backwards and shortening length of the eye muscle were calculated and designed according to the degree of cockeye; secondly, the height of the double eyelids line and positioning were re-designed; then the moving backwards and shortening of the horizontal macula were put up to rectify the cockeye; finally, the incisal double eyelid operation was performed. Results All 25 patients with inner cockeyes were corrected at the right position; 4 of 11 patients with outer cockeyes were corrected 5～10 degrees over the right position, and others were at the right position. The single eyelid with thin-eyelid and middle-thick-eyelid restored within 3 months; but the patient with thick-eyelid restored slowly, about half a year in average. 2 patients with redundant skin at inner corner of eye were also corrected at the right position. Conclusions Although there is somewhat difficulty in the plastic surgery for patients with cockeye and single eyelids, synchronous operation can save time and outlay, and also acheive the aim of beautifying. Therefore, it is a good approach to deal with cockeye.

A series of pathophysiologic changes in brain tissues will occur after intracerebral hemorrhage, including the enlargement of hematoma, metabolism abnormality in perihematoma tissues, and formation of cerebral edema. Recent researches have demonstrated that iron ions play an important role in the secondary brain injury after intracerebral hemorrhage. Iron chelator can block iron-induced injury process by specifically binding iron ions. This article reviews the changes of iron metabolism, iron-related mechanisms of brain injury, and the neuroprotective effect of iron chelator.

Objective To study the clinical characteristics and therapeutic effect of the combination of intensive immunosuppression therapy with umbilical cord blood infusion in severe aplastic anemia (SAA) patients with infections.Methods A retrospective analysis of bacterial spectrum and treatment effect was performed for infections occurred in 27 SAA-Ⅰ patients who received antithymocyte globulin (ATG),cyclophosphamide (Cy) and cyclosporine A (CsA) followed by umbilical cord blood infusion.Results The prevalence of infections in 27 SAA-Ⅰ patients was 48.1％,among which 70 ％ were infected with bacteria (mostly gram-negative bacilli) and 30 ％ with fungal infection.Upper respiratory tract was the most common site,followed by the blood and lungs.The primary infections occurred at the median time of 9.5 d (-2-10 d)after the immunosuppressive therapy,and the initial infections of ANC were ＜ 0.2 ×109/L.Conclusion Combination of intensive immunosuppression and umbilical cord blood infusion is proven effective in treating SAA-Ⅰ.As a result,infection rate is low and can be controlled with sensitive antibiotics.

Objective To explore whether health examination save medical insurance fund or not. Methods To observe the sparing medical cost which 371 health examination people avoid repeated checking and shorten the length of stay during hospitalization from March 2006 to March 2007. To observe the effect of sparing medical cost because of regular health examination, early discovery diseases, pretreatment promptly and avoidance transforming to advanced diseases. Results Because of avoiding repeated checking and shortening the length of stay,everybody save the medical cost about 570 yuan,371 people can save about 21 ten thousands yuan. To statistics the average hospital cost of different courses of eight common diseases and malignant tumors from January to November at 2007, we discovered that the medical cost is large distinction between the earlier period and advanced stage of disease. For example: the medical cost of one stage of hypertension is 6608 yuan ,the two stage is 8838 yuan ,the three stage is 13 869 yuan, and the stage of hypertensive heart disease is 13 033 yuan. Parts of health examination crowd who stay at my hospital from 1996 were visited for five years ,we conformed many chronic diseases by prompt intervention can relieve and even reverse, avoiding the transformation to incurable disease which will consume a large number of strength and money. Conclusion Health examination resource is a medical treatment resource which isn't restricted by social security fund, the utilization can not only save medical cost but also discover and treat diseases earlier, thus we can save the strength and money before the advanced diseases.

Objective To analyze the clinical characteristics and prognostic factors of endometrial carcinoma in senile women, explore the optimized therapeutic modality. Method The clinic pathological characteristics and survival outcome of 268 patients of endometrial carcinoma in senile women were reviewed retrospectively, and prognostic factors affected the survival of endometrial carcinoma were evaluated. Results According to univariate analysis, therapeutic modality, surgical-pathological classification and pathological grade were closely correlated with prognosis of endometrial carcinoma in senile women(P < 0.01 ). According to COX multivariate analysis,surgical-pathological and pathological grade were independent prognostic associated-factor (P = 0.002,0.000). Conclusions There are closely correlations between prognosis and surgical-pathological classification,therapeutic modality and pathological grade,surgical-pathological classification and pathological grade are independent prognostic associated-factor. The therapy on endometrial carcinoma in senile women should connect with high risk factors and adopt integrating and individual therapeutic modality.

Objective To explore the alteration in ethology and change of muscle fibers of gastronomies of rats following spinal cord injury(SCI). Methods 108 Wistar rats were divided into sham operation group (n=38) and SCI group (n=38) and rehabilitation group (n=32).The ethology was evaluated by Basso, Beattie, and Bresnahan Locomotor Rating Scale (BBB Scale), and the alteration of muscle tissue's reactivitywas observed by immunohistochemistry staining. Results The BBB scores of rehabilitation group were higher than SCI group 3weeks after injury, but all was lower than 10. The cross section areas of gastrocnemius in both groups were getting smaller, but the rehabilitationgroup was slighter. Conclusion Spontaneous recovery occurred in some degree for rats' hind limb after spinal cord injury. Rehabilitationtraining is beneficial for the recovery of rats' motor function, and alleviates the atrophy of hind limb after spinal cord injury.
