1.The effect of ultrasound in the diagnosis and treatment of rheumatoid arthritis

Objective To explore the effect of ultrasound in the diagnosis and treatment of rheumatoid arthritis .Methods though early diagnose of RA by ultrasound ,DAS28 and MRI′s result to measure the accuracy of ultrasound test .then though the ul‐trasound index at different time point in the treatment of RA patients ,we knew the changes of the joint ,and provide treatment plan and prognosis .Results ultrasound had high accuracy rate in the diagnoses of RA(P< 0 .05) ,and there were correlation among ul‐trasound diagnoses accuracy and DAS28 and MRI score(r= 0 .859 ,P< 0 .05) ;at the same time ,it provided accurate changes of the joint during the treatment ,and provided basis for treatment .Conclusion Ultrasound has manifest advantageous in diagnose RA .It could be used as a method in early diagnose RA and evaluate the effect on RA′s treatment .
2.The diversity of endophytic fungi in endangered medicinal material Daphnes Cortex as determined by high-throughput sequencing technology

In order to provide a scientific basis for the establishment of a Daphnes Cortex medicinal material fungus library and the screening of endophytic fungi that promote the growth of Daphnes Cortex and increase the content of daphnetin, we used Illumina high-throughput testing technology to analyze 9 Daphnes Cortex samples from Gansu and Shanxi provinces. A total of 632 766 valid sequences were obtained, including 348 OTUs, 4 phyla, 20 classes, 48 orders, 108 families, 154 genera, and 208 species. The sum of the first 3 fungal genera account for more than 65% of the total abundance, with the highest reaching 98.4%.
3.Construction of a recombinant adenoviral vector expressing human endostatin.

OBJECTIVETo construct the recombinant adenovirus vector expressing human endostatin.
METHODSHuman endostatin gene extracted from pGEM-T Easy vector containing the target gene fragment was successfully amplified using PCR and cloned into pShuttle2 vector. The target gene was subcloned into an adenovirus vector and the resulted recombinant adenovirus (Ad-hEndo) was linearized before transfected into HEK 293 packaging cells. The Ad-hEndo recombinant adenovirus was efficiently amplified in 293T cells and purified by CsCl density centrifugation, and the titer of the virus was determined.
RESULTSThe amplified hEndostatin cDNA was verified by PCR and sequencing, and the resulted virus titer reached 5.2 x 10(9) pfu/ml.
CONCLUSIONThe recombinant adenovirus containing human endostatin gene has been successfully constructed, which may provide important basis for gene therapy research for angiogenesis-dependent diseases.

4.The vascular endothelial function damage as well as early atherosclerosis in hypertensive youth.

OBJECTIVETo explore young hypertensive people's vascular endothelial dysfunction and the early atherosclerosis changes.
METHODSRural youth subjects aged 20 to 35 in 6 villages were randomly selected from 20 villages of 3 townships, including Qili, Laojun and Shayan, in Hanzhong, Shanxi, China, from March to April of 2005, to survey their blood pressure and other related factors. 97 subjects with high blood pressure (hypertensive group) were detected while 400 with normal blood pressure (normal group). We measured their brachial artery flow-mediated vascular endothelial-dependent diastolic function (FMD) and the carotid intima-media thickness (IMT) by high resolution vascular ultrasound method, and other biochemical indexes.
RESULTSThe body mass index (BMI) ((24.7 ± 4.3) kg/m²), waistline ((84.1 ± 11.7) cm), hipline ((92.5 ± 7.4) cm), TC ((4.6 ± 0.8) mmol/L), LDL-C ((2.8 ± 0.5) mmol/L), TG ((1.7 ± 1.1) mmol/L) and the blood insulin (INS) ((8.6 ± 4.4) mIu/L) were all significantly increased in young hypertensive group than those in normotensive group (respectively were (22.1 ± 3.0) kg/m(2), (76.7 ± 8.9) cm, (88.9 ± 5.6) cm, (4.3 ± 0.7) mmol/L, (2.6 ± 0.4) mmol/L, (1.4 ± 0.8) mmol/L and (6.9 ± 2.1) mIu/L) (P < 0.05). The FMD of the brachial artery in hypertensive group was markedly lower than normal group ((0.103 ± 0.04)% and (0.117 ± 0.05)%, P < 0.05). The IMT of two groups were (0.49 ± 0.07) mm and (0.48 ± 0.07) mm, but there was no statistically significant differences (P > 0.05).
CONCLUSIONYoung hypertensive people with a few cardiovascular risk factors earlier appear dysfunction of the flow-mediated vascular endothelial-dependent dilation, which is prior to the increase in carotid IMT.

5.The correlation between the DNA content in human thyroid follicular epithelial cells and the postmortem interval.

OBJECTIVE:
To study the relationship between the DNA content in follicular epithelial cells of the human thyroid and postmortem interval (PMI).
METHODS:
Changes of the DNA content in thyroid follicular epithelial cells at different PMI were determined by Methyl Green-Pyronin (MGP) stain combined with an image analysis technique.
RESULTS:
The average DNA content in the thyroid follicular epithelial continued to decrease with increased PMI. The coefficient of determination for DNA content with average gray, aimed area, aimed area ratio and positive index in follicular epithelial cells were 0.960, 0.987, 0.988 and 0.990, respectively.
CONCLUSION
There is an apparent correlation between the average DNA content of follicular epithelial cells and the PMI. MGP stain combined with an image analysis technique seems to be a useful means to study the degradation of DNA in thyroid follicular epithelial cells.

OBJECTIVETo observe the effect of Schistosoma japonicum cysteine protease inhibitor (rSjCystatin) for treatment of lipopolysaccharide (LPS)-induced sepsis in mice.
METHODSAfter a week of adaptive feeding, 54 BALB/c mice were randomly divided into normal control group (group A), sepsis group (group B), and rSjCystatin intervention group (group C). The mice in group A received an intraperitoneal injection of PBS (100 µL), and those in groups B and C were injected with PBS (100 µL) containing LPS (10 mg/kg); the mice in group C were also intraperitoneally injected with 25 µg sjCystatin in 100 µL PBS 30 min after LPS injection. From each group, 10 mice were randomly selected 24 h after PBS or LPS injection for detecting serum levels of TNF-α, IL-6, and IL-10 using ELISA and the levels of ALT, AST, BUN, and Cr using automatic biochemical analyzer; the pathological changes in the liver, lung and kidney were observed with HE staining. The remaining 8 mice in each group were used for observing the changes in the general condition and the 72-h survival.
RESULTSThe 72-h survival rates of the mice was 100% in group A, 0 in group B, and 36% in group C, showing a significant difference among the 3 groups (P<0.05). Compared with those in group A, the mice in group B exhibited obvious liver, lung, and renal pathologies with increased levels of ALT, AST, BUN, Cr, IL-6, and TNF-α (P<0.05). Treatment with sjCystatin significantly lessened LPS-induced organ pathologies, lowered the levels of liver and renal functional indexes and the pro-inflammatory cytokines, and increased the serum level of IL-10 in the mice (P<0.05).
CONCLUSIONSjCystatin can produce a significant therapeutic effect on sepsis induced by LPS in mice.
7.Expression of novel cloned genes HC56, HC71 and HC90 mapped on human chromosome 17p13.3 in leukemic cells.

In order to investigate expression of novel genes HC56, HC70 and HC90 mapped on chromosome 17p13.3 in human leukemic cells, HC56, HC71 and HC90 genes expression was examined in cells from 35 patients with acute leukemias and 4 leukemic cell lines by using semi-quantitative RT-PCR with incorporation of alpha(32)P dATP, betaM2 gene as endogenous control. The results showed that HC90 gene expression level was lower in patients with acute lymphocytic leukemia and T lymphocytic leukemic cell line Jurkat than that in normal control. Low-level HC71 gene expression was detected in acute myeloid leukemia. There was no expression difference in HC56 between leukemic cells and normal blood cells. It was concluded that both HC70 and HC90 genes were aberrantly expressed in human leukemic cells.

8.Serosa-penetration in human T4aN0M0 gastric carcinoma correlates with worse prognosis after D2 gastrectomy.

BACKGROUNDIn both the seventh edition of the International Union Against Cancer (UICC) staging system for gastric cancer and the 14th edition of the Japanese Gastric Cancer Association (JGCA) system, T(4a) is defined as "Tumor perforates serosa (visceral peritoneum) without invasion of adjacent structures." The aim of this study was to investigate the differences in prognosis between patients with serosa-penetrating and serosa-invading T(4a)N(0)M(0) gastric carcinomas.
METHODSData were collected from 221 patients with T(4a)N(0)M(0) gastric carcinoma who underwent D2 resection at our cancer center between January 1990 and December 2008. The cohort included 42 patients with serosa-penetrating tumors and 179 patients with serosa-invading tumors. The average follow-up time was 85.5 months. Spearman's rank correlation, Kaplan-Meier plots, and Cox proportional hazards regression models were used to analyze the data.
RESULTSThe 5-year survival rate of patients with serosa-penetrating and serosa-invading gastric cancers were 31% and 62% respectively (P < 0.05). The relapse rates after D2 radical surgery were 16.2% in patients with serosa-invading gastric cancer versus 59.5% in those with serosa-penetrating tumors (P < 0.05). Peritoneal dissemination and distant organ/lymph node metastatic rates were 76.0% and 24.0% respectively in patients with penetrating tumors, versus 44.8% and 55.2% respectively in patients with invading tumors (P < 0.05). Multivariate analysis showed that penetration of the serosa was an independent prognostic indicator of overall survival.
CONCLUSIONSOur findings provide a basis for the concept that serosa-penetrating T(4a)N(0)M(0) gastric carcinoma represents a more aggressive cancer than serosa-invading T(4a)N(0)M(0) gastric carcinoma. Serosa penetration is an independent factor for poor prognosis of patients with gastric carcinoma.

9.Metabolic syndrome is an independent risk factor for cardiovascular disease events in patients with ischemic stroke.

OBJECTIVETo explore the association between metabolic syndrome (MS) and risk of cardiovascular disease events (CVD) in patients with ischemic stroke.
METHODA total of 1087 patients with ischemic stroke were enrolled from 5 community-based medical centres and underwent baseline evaluation on risk factors of stroke during the period of Jan. 2003 to Dec. 2006. After baseline survey, all patients were followed up until Dec 31, 2008 and new CVD events were recorded. MS was defined using CDS criteria. Proportional hazard models were used to assess the HRs and 95% CI of CVD events associated with MS and other components.
RESULTSThe prevalence of MS was 40.4% at baseline. During an average follow-up of 3.5 years, 178 patients developed new CVD events. After adjusted for age, gender, smoking, drinking, marriage status, education level, hospitalization, recurrence of stroke, stroke duration, depression, cognition impairment and ADL, MS remains the independent predictor for the risk of CVD events. Compared with patients with non-MS, the risk of CVD events increased by 44% (HR: 1.44, 95%CI: 1.06 - 1.95). The risk of CVD also increased with the number of MS components. Compared with patients with 1 or less than 1 components of MS, the risk of CVD events increased by 30% (HR: 1.30, 95% CI: 0.83 - 2.04) in those with 2 components and by 69% (HR: 1.69, 95%CI: 1.11 - 2.56) in those with 3 or more components of MS. Hypertension and hyperglycemia and impaired fasting glucose also served as independent risk factors for CVD event (all P < 0.001).
CONCLUSIONSMS was independently associated with increased risk of CVD events in patients with ischemic stroke. There was a dose-response relationship between the numbers of MS components and the risk of CVD event.

10.Comparison of 18F-FLT and 18F-FDG PET/CT for lymph node staging in thoracic esophageal carcinoma

Objective To compare the diagnostic value 18F-fluorothymidine (FLT) and 18F-fluorodeoxyglucose (FDG) PET/CT in detecting lymph node metastases of untreated thoracic esophageal carcinoma. Methods Twenty-two patients with thoracic esophageal squamous cell carcinoma underwent both 18F-FLT and 18F-FDG PET/CT before surgery. The imaging results of the two modalities in detecting regional lymph node metastases were compared prospectively with the pathologic findings. The X2-test was used with SPS S 13.0. Results All patients underwent esophagectomy and lymphadenectomy. The metastatic lymph nodes were found in 16 patients, from which 47 of 424 excised nodes were positive by pathologic examination. False positive results were 14 while false negative 8 on 18F-FDG PET/CT. In contrast, false positive results were only 3 but false negative were 12 on 18 F-FLT PET/CT. The sensitivity, specificity, accuracy,negative predictive value, and positive predictive value were 74.47% ( 35/47 ), 99.20% ( 374/377 ),96.46% (409/424), 96.89% ( 374/386 ) and 92.11% ( 35/38 ) respectively for 18 F-FLT PET/CT, whereas the corresponding values were 82.98% (39/47), 96.29% (363/377), 94.81% (402/424), 97.84%(363/371 ) and 73.58% (39/53) respectively for 18 F-FDG PET/CT (X2 = 0.572, 6.018, 1.017, 0.348,3.852, P>0. 05, <0.05, >0.05, >0.05 and >0.05). Conclusions Compared with 18F-FDG PET/CT, 18F-FLT PET/CT may be less sensitive but more specific for the detection of lymph node metastases of thoracic esophageal carcinoma.
