1.Effects of mycophenolate mofetil on anti-glomerular basement membrane nephritis SD rats

Objective To investigate the influence of mycophenolate mofetil(MMF) on blood and urine biochemical indica‐tors as well as the expressions of IL‐1 in anti‐glomerular basement membrane(GBM ) nephritis SD rats .Methods The SD rats were randomly divided into control group ,model group and MMF group .And then the models of SD rat anti‐GBM nephritis were estab‐lished by injecting rabbit anti‐SD rat GBM serum antibodies .The urinary protein and blood parameters were detected initially ,1st , 2nd ,3rd and 4th weeks after administration respectively .The expressions of IL‐1 and TGF‐β1 were detected by Elisa Kit and the re‐nal pathological changes were observed by HE and PAS staining .Results After treatment for 4 weeks by MMF ,the 24 h UP ,BUN and SCr levels were lower in MMF group than those of the model group(P<0 .05) .And the expressions of IL‐1 and TGF‐β1 were decreased in MMF group .The results of HE and PAS staining showed that in model group rats ,glomerular tuft morphology was destroyed and inflammatory cell infiltration was observed in the glomeruli .Some glomeruli had cellular crescent formation .Infiltra‐tion of inflammatory cells was also observed in the interstitium .MMF clearly improved these pathological changes overall ,with im‐provements noted in both glomerular and tubular degeneration as well as in the infiltration of inflammatory cells .Conclusion MMF shows certain renal protective effect on anti‐GBM nephritis SD rats .
2.Clinical value and classification of two dimensional ultrasonography combined with contrast-enhanced ultrasonography in cirrhotic paitients with intrahepatic nodules

Objective To evaluate the role of two-dimensional ultrasound combined with contrast-enhanced ultrasonography (CEUS) in the classification of liver nodules in cirrhotic patients.Methods Consecutively cirrhotic patients with intrahepatic nodules at Xixi Hospital of Hangzhou were included from November 2015 to December 2016.All (142 nodules in 109 patiens) presented as non-cancerous focal lesions on conventional magnetic resonance imaging and CT examination and had available information of liver biopsy.Each lesion was percutaneous biopsied under the guidance of two-dimensional ultrasound.Ultrasonographic parameters evaluated were as following:(1) sizes of nodules under US;(2) ultrasonographic characteristics of the nodular;(3) CEUS enhancement features of the nodules.Four types of hepatic nodule suggesting different histology were defined according to the ultrasonographicparameters.x2 test was used to compare the difference of hepatocellular carcinoma (HCC) incidence among liver nodules with varying sizes and nodules with different enhancement features under CEUS.As for the statistical differences of HCC and high-grade dysplastic nodule (HGDN) incidence between type Ⅲ & Ⅳ nodules and type Ⅰ & Ⅱ nodules,x2 test was also used for analysis.Results A total of 142 eligible nodules were detected in 109 patients with cirrhosis,including 16 HCCs,2 intrahepatic cholangiocellular carcinomas (ICC),41 HGDNs,40 low-grade dysplastic nodules (LGDN) and 43 regenerative nodules (RN).In terms of diameter,all (6/6) the nodules larger than 2.0 cm,20.0% (8/40) of middle size nodules (1.5-2.0 cm),were HCCs.The remained 2 lesions of HCC came from two subgroups with even small size nodules [1.0-1.4 cm (n=93),and < 1.0 cm (n=3),in diameter],respectively.Two lesions of ICC were attributed to nodules with a 1.0-1.4 cm diameter.About 28 nodules with a diameter of 1.5-2.0 cm,13 nodules with a diameter of 1.0-1.4 cm were HGDN.HCC incidences between these 4 groups were different significantly (x2=61.425,P < 0.001).Asfor the CEUS,14 nodules exhibited a rapid enhancement feature in arterial phase,12 of which were HCC.In56 nodules with a slow enhancement feature,4 nodules were HCC.HCC incidences between these 3 groups were different significantly (x2=75.752,P < 0.001).Under the combined ultrasonography,HCC incidences of type Ⅲ and type Ⅳ nodules were significantly higher than that of type Ⅰ and type Ⅱ lesions [21.9% (16/73)vs 0 (0/65),x2=15.222,P < 0.001],similar result was observed in the comparison of HGDN incidences between type Ⅲ & Ⅳ and type Ⅰ & Ⅱ nodules[53.4% (39/73) vs 3.1% (2/65),x2=38.842,P < 0.001].Conclusion The classification presented by this study,combining the three ultrasonographic parameters,which is nodule size,nodular echo characteristics and enhancement features of the nodules under CEUS,could be helpful for the diagnosis of HCC in cirrhotic patients with ill-defined nodule on routine image examination.
3.Effect of Yangyu Tuji on content of type Ⅰ, Ⅲ collagen and the expression of MMPs and TIMP-1 in wound caused by streptozotocin in rats

AIM: To study the effects of Yangyu Tuji (YYTJ) on delayed healing wound of diabetic rats caused by streptozotocin (STZ). METHODS: SD male rats were randomly divided into control group (control), model group (model); and 3 different dose groups of YYTJ. 55 mg/kg STZ were given by intraperitoneal injection except for control group. After 30 days, a round skin of 1.6 cm diametre was excised on all dorsal back of rats. The healing time and healing rate were observed according to re-epithelization. The content of collagenⅠ and Ⅲ was observed by Picric acid-Sirius red staining , Matrix metalloproteinase-1, 13 (MMP-1, -13), tissue inhibitor of metalloproteinases-1 (TIMP-1) by immuno-histochemistry assay. AIM: To investigate the effects of Astragalus polysacharin(APS) on human fibroblast and human umbilical vein endothelia cell (HUVEC) proliferation, as well as its acts on adhesion between white cells and HUVECs. METHODS: Human fibroblasts from distal and proximal skin away the ulcer were cultured as normal fibroblasts(NF) and wounded fibroblasts(WF). MTT assay was used for detecting cell proliferation, Rose Bengal staining and fluorescence immunohistology assay were used for examining the adhesion of human polymorpho-nuclear cell(PMN) and TPH-1 to HUVECs. RESULTS: 2 44-156 mg/L APS promoted WF proliferation, and 2 44-39 mg/L APS also promoted NF proliferation, but it did not show any proliferating effect on HUVECs. APS inhibited the adhesion of PMN or TPH-1 to HUVECs induced by tumor necrosis factor(TNF). At 25-100 mg/L, it also inhibited both VCAM-1 and ICAM-1 expression in HUVECs induced by TNF. Treatment with APS for 12 h also inhibited CD44 expression in HUVECs. CONCLUSION: APS shows mitogenic activity on both human normal and wounded fibroblasts. It also exerts anti-inflammation effects by inhibiting adhesion molecule expression and adhesion of white cells to HUVECs. [
6.An analysis of risk factors leading to complications in laparoscopic cholecystectomy

Objective To investigate the risk factors leading to complications in patients undergoing laparoscopic cholecystectomy (LC). Methods Clinical data of 11?974 patients undergoing LC from Mar. 1991 to June 2003 were collected and analyzed retrospectively. Fifteen clinical factors were recruited for the study in relation to surgical complications. Data were analyzed by ?2 test and Logistic regression. Results The overall operative complication rate was 1.896%. The procedure was shifted to open surgery in 2.389% of all cases, Logistic regression analysis revealed that Calot triangle adhesion, stage, expertise of the team, gallbladder wall thickness, gallbladder and vicinity adhesion were important risk factors for complications. Conclusion LC complications can be prevented by good training, strictly following protocol, and timely shifting to open surgery.
7.Cyclosporin A Combination with Angiotensin-Converting Enzyme Inhibitor in Treatment of Alport′s Syndrome

Objective To study the possibility of medication treatment in Alport′s syndrome(AS).Methods Two boys with AS,received cyclosporin A(CyA) in combination with angiotensin-converting enzyme inhibitor(ACEI) or only ACEI,which lasted 22 months and 20 months,respectively.Results After the treatments with the medication,there was obvious effectiveness in the 2 patients with relieved symptom and almost normal serum standard,especially the permanent and obvious recession of proteinuria.There was no side effects of medication during the treatment.Conclusion Patients with AS can be treated with CyA and ACEI,with symptomatic recession,as well as suspend the progression of pathology of kidney.
8.Assessment of the clinical efficacy of surgical treatment for cervical lymph node tuberculosis

Objective:To assess the effectiveness and practical value of surgery in clinical treatment of cervical lymph node tuberculosis.Methods:110 patients receiving surgical treatment for tuberculosis at Hunan Provincial Chest Hospital between January 2020 and December 2022 were selected, all of whom were diagnosed with cervical lymph node tuberculosis.According to patient preferences, they were divided into two groups, a drug treatment group consisted of 60 patients receiving conventional Western medicine treatment and a surgical treatment group of 50 patients receiving surgical treatment.Statistical analysis was made to compare the clinical efficacy, recurrence rate, complications, quality of life, and patient satisfaction between the two groups.Results:The overall clinical effectiveness rate was higher in the surgical treatment group than in the drug treatment group(94.00% vs.70.00%, χ2=10.170, P=0.001), with the rate higher in the surgical treatment group than in the drug treatment group for patients under 60 years of age(64.29% vs.40.00%, χ2=-6.996, P=0.035), but there was no difference in therapeutic efficacy between the two groups for patients aged 60 and above(54.55% vs.40.00%, Z=3.842, P=0.146). Compared with the drug treatment group, the surgical treatment group had a lower recurrence rate(8.00% vs.46.67%, χ2=19.767, P<0.001)and a lower rate of complications(8.00% vs.21.67%, χ2=3.899, P=0.048), but higher scores of quality of life in all four domains(physical function: 64.02±4.49 vs.76.11±4.47, t=14.090, P<0.001; material life: 66.50±2.39 vs.81.03±2.28, t=32.417, P<0.001; psychological function: 62.98±2.51 vs.79.24±2.50, t=33.892, P<0.001; social function: 63.12±3.39 vs.76.08±3.51, t=19.645, P<0.001)and a higher level overall patient satisfaction(90.00% vs.66.67%, χ2=8.455, P=0.004). Conclusions:Surgical treatment for cervical lymph node tuberculosis patients has better efficacy, fewer complications and better prognosis and is clinically practical and highly valuable.
9.Investigation of the status and related factors on tumor patients' PICC misplacement

Objective To explore the reason and remedy on tumor patients' PICC misplacement.Methods The reason of 32 tumor patients' PICC misplacement were analyzed. The patients whose PICC misplaced were observed by X-ray,and then regulated to precava before remove the wire.Results PICC misplacement was much more observed in patients who was sixty years old or above than that of their younger counterpart (x2 =4.733,P <0.05).87.5% of PICC misplacement were observed in the ipsilateral vein and less observed in right upper extremity ( x2 =7.552,P =0.006).The rate of PICC misplacement of cephalic vein,median cubital vein and basilic vein respectively were 15.7%,8.89% and 8.12%.And there was no significant difference was detected in the gender,method and the location of cancer ( P > 0.05 ).Finally,all the misplacement PICC were regulated to the right place.Conclusions Adept technology makes success,and more attention should pay to elderly patients.It is better to insert PICC in patient' s right upper extremity,especially basilic vein,and then median cubital vein.Utilize the X-ray to adjust patients position,and matching inspiration to adjust PICC' s location in order to avoid misplacement.
