2.Optimization of Automatic System in Outpatient Pharmacy Based on PDCA Cycle Management

OBJECTIVE:To provide reference for the optimization of automatic system in outpatient pharmacy of the hospital. METHODS:Based on PDCA(Plan,Do,Check and Action)cycle management,the modules for adding and dispensing drugs of the automatic system in the outpatient pharmacy were optimized and the work records and quality indexes before (July-September in 2013) and after (May-July in 2014) the optimization were compared. RESULTS:By the optimization of the software system, hardware equipment and staff training,the error rate of adding drugs was reduced by 95%,daily box number of added drugs was increased by 30.9%,monthly box number of drugs damaged by the machine was decreased by 80%,the error rate of dropping drugs was decreased by 96.6%,and the frequency of machine fault was decreased by 87.5%,compared with before. CONCLU-SIONS:By PDCA cycle management,the work efficiency of automatic outpatient pharmacy can be improved,and the errors of prescription dispensing are reduced.
3.APPLICATION OF LAPAROSCOPIC ULTRASONOGRAPHY IN ADNEXAL OPERATIONS

Objective:To explore the clinical value on application of laparoscopic ultrasonography (Lap US) in the adnexal operation. Methods:Eleven patients including 7 cases of tubal pregnancy, 3 cases of teratoma of ovary and 1 case of endometrial cyst of ovary were examined by LapUS, then operated with laparoscopy.Results:The results showed that the modality presented is a big progress over the traditional operative management for adnexal diseas. Conclusions:It is a good approach for micro-surgery in the adnexal operation by laparoscopy.
4.Therapeutic antibody: new opportunity for immunity and inflammatory diseases.

With the development of therapeutic antibodies over the past decade, they have become the treatment options for immunity and inflammation diseases. Major limitations of mouse antibodies as therapeutic agents - immunogenicity, lack of effectors' functions and short serum half-life -- were subsequently identified and largely overcome by the advent of humanized and fully human antibody technologies. The therapeutic antibodies for immunity and inflammatory diseases are primarily utilized in the treatment of allograft rejection, autoimmune disease, autoinflammatory syndromes, allergies and other chronic inflammation. The action mechanisms of therapeutic antibody include blocking ligands or receptors, regulating receptor activity, clearing the target cells or activating receptor. Strategies for generating the antibody drugs with high efficacy and low side effects can be realized by modulation of Fc-mediated activities and optimization of antigen-binding domains.
5.Optimizing the host bacteria to make a large naive phage antibody library in the recombination system.

To prepare large naive phage antibody library, the host bacteria with high transformation efficiency is used in the Cre-LoxP recombination system. The variable regions of immunoglobulin light and heavy genes were amplified from lymphocytes collected from adult peripheral blood and newborn cord blood. The genes were spliced to form the single-chain variable fragments (scFv) by overlap PCR, cloned into pDAN5a vector and then transformed into XL2-blue MRF' with the Hte gene. Compared with XL1-blue strain, the size of the primary library was increased by 3.9 times. The primary library infected Cre recombinase-expressing bacteria, and the genes between phagemids created many new VH/VL combinations. The library was calculated to have a diversity of 1.7 x 10(11) and validated by the selection of antibodies against six different protein antigens. This library provides the basis for further selection of antibody-based drugs. It is the first time to report that XL2-blue MRF' can be used to improve the diversity of the library in the recombination system.

7.Relative factors analysis on the stability of intraocular lens after cataract surgery

The stability of the intraocular lens ( IOL ) after cataract surgery is composed of decentration, tilt, rotation, and the change of anterior chamber depth. Its stability is an important factor affecting postoperative visual quality. By analyzing the related factors which influence the stability of intraocular lens, improvements can be identified for future cataract operations. The stability of intraocular lens is influenced by many factors: intraocular structure, the size and the symmetry of intraoperative capsulorhexis, the position of the intraocular lens, the material and design of the intraocular lens, etc. In order to improve the patient's vision, cataract surgeries have been experiencing an evolution. IOL material have also been contributing to such innovations.
8.Influence of Epstein-Barr virus infection on T cell subset in children with infectious mononucleosis

Objective To observe the influence of Epstein-Barr virus(EBV) infection on T cell subsets in children with infectious mononucleosis(IM).Methods Eighty children patients with IM in the Hanzhong Municipal People's Hospital from January 2013 to January 2016 were chosen as the study subjects and received the anti-virus therapy.The negative conversion rate of EBV-DNA after treatment was observed.The distributions of T cell subsets werecompared between the children patients with EBV-DNA positive and patients with EBV-DNA negative.The related factors of EBV-DNA non-negative conversion were analyzed.Results After treatment,67 children cases (83.75%) were EBV-DNA negative conversion.The proportions of CD3+ and CD8+ after treatment in the EBV-DNA negative group were lower than those in the EBV-DNA positive group,and the proportion of CD4+ and CD4+/CD8+ were higher than those in the EBV DNA positive group,the difference had statistical significance (P<0.05).In the analysis on the related factors in the children patients with EBV DNA non-negative conversion,the gender,WBC count and lymphocyte count had no significant correlation with EBV-DNA negative conversion(P> 0.05),while interferon use,age and initial Ct demonstrated significant correlation with EBV-DNA non-negative conversion(P<0.05).Conclusion EBV infection can lead to disturbance of T cell subsets in children with IM,so clinic should adopt an active anti-virus treatment.

10.Retrospective analysis of echocardiography on criss-cross heart

Objective To describe the characteristics of the criss-cross heart with comprehensively segmental echocardiographic scanning in order to explore the diagnostic value of the echocardiography.Methods Ten cases of criss-cross heart were retrospectively reviewed with 2-dimensional,color Doppler echocardiography to summarize the main diagnostic methods.Results The right ventricle was located superiorly and the left ventricle inferiorly in 9 patients and paralleled alignment in 1.The solitus situs was observed in 9 patients and dextrocardia in 1;the atrioventricular connections were concordant in 8 patients and discordant in 2.Echocardiography revealed that 5 patients presented with transposition of great artery and 5 with double outlet of right ventricle as well as all with ventricular septum defect.Conclusions Clear visualization is available with echocardiography to show the crossing inflow streams at the level of atrial-ventricular valve,abnormal alignment of the ventricular septum as well as the associated abnormalities.
