1.Discussion of Effect on Nitrite Test Results of Reagent Strips Placed Time on COMBI-SCAN XL Automatic Urine Analyzer

Objective To explore the division XL automatic urine analyzer reagent strips placed time impact on nitrite (NIT) test results,to ensure that provide accurate and reliable experimental data for clinical,and improve clinical diagnostic rate. Methods Used a new load reagent storehouse article in the new reagents respectively in <5,30 min;1,2,3,3.5,4,4.5,5, 5.5,6,6.5,7 and 8 h,in different period,16 0.9 g/dl saline NIT in the false positive rate,and carried on the comparison to different times NIT false-positive rate analysis.Results <3.5 h NIT false positive rate was 0% (0/16),4~4.5 h false pos-itive rate was 18.8% (3/16),5~5.5 h false positive rate was 43.8% (7/16),6~6.5 h was 62.5% (10/16),7~8 h was 87.5% (14/16).More than 4 h each time comparison between NIT false positive rate,differences were statistically signifi-cant (χ2 =11.7~59.2,all P <0.01).Conclusion Division XL automatic urine analyzer reagent strips placed after 4 h,NIT false-positive rate started to rise,NIT false-positive rate was significantly increased after 5 h.Therefore,strengthen the divi-sion XL automatic urine analyzer reagent strips placed reasonable time management,should be in the < 3.5 h advisable,it helps reduce the NIT false positive rate.
2.Present Situation Investigation of Red Blood Cells Glucose-6-Phosphate Dehydrogenase Dificiency of Pregnancy Couple in Baoan District of Shenzhen

Objective To understand red blood cells glucose-6-phosphate dehydrogenase (G6PD)dificiency of pregnancy cou-ple in baoan district of Shenzhen,in order to draw the attention of society.Methods Collected 6 574 cases of couples of child-bearing age (3 192 males and 3 382 females),in Baoan District People’s Hospital,from February to November 2013. Venous blood was collected using EDTA anticoagulant blending respectively,the improvement of red bloodcells and glucose-6-phosphate dehydrogenase quantitative ratio method to determine anticoagulant G6PD/6 PGD ratio in the whole blood,with ratio <1.0 convicted of G6PD deficiency.Results 3 192 cases of male subjects,G6PD deficient 176 cases,3 382 cases of pregnant women client detection of 135 cases in the detection rate were 5.51% and 3.99%,respectively,the total detection rate was 4.73% (311/6 574),G6PD lack of rate was higher than that of pregnant women,men G6PD lack of male∶female was 1.30∶1 (176∶135),the difference was statistically significant between (χ2 =7.16,P <0.05).Conclusion The baoan district couple G6PD lack had a higher incidence of pregnancy,men were significantly higher than women,should arouse at-tention.Couples pregnancy test during pregnancy should be advocated the G6PD activity screening,the eugenics and mater-nal and child health care has important significance.
3.Research on electrochemical luminescence analyzer test anemia indicators of performance verification methodology

Objective:To detect anaemia parameter methodology performance for validation of Roche Cobas E601 automatic electrochemical luminescence immunity analyzer.Methods:Recommended by the American association of clinical laboratory standardization (CLSI) method was developed for the determination of folic acid, iron, protein, and this precision, accuracy, linear range, sensitivity, biological reference range and carry pollution index, and validated.Results: Cohas E601 determination of folic acid, iron, protein and precision, the daytime in this batch variation coefficient were 3.03%~4.27% and 3.51%~4.68%. Relative bias must lean on(%) between -3.54%~4.46%. The scope of determination of linear range and the manufacturer to provide similar. Folic acid, iron, protein and numerical value with the determination of this instrument manufacturers provide reference interval coincidence rate were 90.0%, 85.0% and 90.0% respectivel. Instrument to detect carry pollution rate is 0.04%~0.16%. CohasE601 detection sensitivity were 0.23 ng/ml, 0.21 ng/ml and 0.19 pg/ml.Conclusion: Cobas E601 detect folic acid, iron, protein and good performance of this methodology, but manufacturers provide biological reference range is not suitable for the local crowd, should establish the corresponding normal reference range.

5.The effect of goal-directed therapy on postoperative infections in high risk surgical patients: a Meta analysis

ObjectiveTo determine the effects of perioperative hemodynamic goal directed therapy (GDT) on postoperative infection rates. MethodsWe conducted a systematic review and Meta-analysis.MEDLINE,the Cochrane Library,EMBASE,CNKI and other sources were searched until March 2011.All randomized controlled trials (RCTs) on adult major surgical patients managed with perioperative GDT or according to routine haemodynamic practice were included.Primary outcome measure was specific for type of infection. Two authors independently extracted data and assessed study quality using standardized instruments; Consensus was reached by conference.The Cochrance Collaboration's software RevMan 5.0 was used for data analysis. ResultsSixteen studies were included in the final analysis,providing a sample of 3309 patients. Perioperative GDT significantly reduced surgical site infections (SSI)(OR =0.60; 95%CI0.46-0.74; P <0.0001),pneumonia (OR =0.69; 95%CI 0.53 -0.90;P =0.007),and urinary tract infections (UTIs) (OR =0.44; 95% CI 0.22 -0.88; P =0.02),and there was no heterogeneity between studies (P > 0.1,I2 < 50% ).A significant decrease in total infection episodes was observed (OR=0.37; 95%CI0.22-0.61; P<0.00001). ConclusionsA flow-directed hemodynamic therapy protects the high-risk surgical patients against postoperative hospital-acquired infections.
6.Results of open surgery or endoscopic choledocholithotomy in the treatment of hepatolithiasis and risk factors

Objective To investigate the short and long-term outcomes and risk factors after open surgery or and endoscopic hepatolithotomy for hepatolithiasis. Methods A retrospective study was made on cases of hepatolithiasis who underwent hepatolithotomy by open surgery or endoscopically from Jan 2001 to Dec 2008.Of 254 patients,189 were followed-up including 127 after open surgery and 62 endoscopically.The univariate and multivariate analyses were performed to determine the risk factors. Results Complete stone clearance was achieved in 85.0 % ( 108/127 ) of open surgery including hepatecomy,61.3 % ( 38/62 )of endoscopic treatment.After a median follow-up period of 6.0 years (2.5 to 10.5 years),stone recurred in 32.8% (62/189) of patients,biliary cirrhosis in 7.4% ( 14/189),cholangiocarcinoma in 7(3.7% ),all cancer cases were dead with a mortality rate of 7(3.7% ).Bile duct stricture (OR:7.522,95% CI:2.642 -21.415),stones in both lobes (OR:11.630,95% CI:3.989 -33.912),and endoscopic treatment ( OR:21.374,95% CI:6.713 - 68.056 ) were independent risk factors ( P < 0.05 ) for incomplete stone clearance by unconditional Logistic regression analysis.In addition,recurrent stones and/or cholangitis were associated with residual stones ( OR:3.059,95% CI:1.307 - 7.159),stricture ( OR:3.702,95% CI:1.567-8.745) and endoscopic therapy (OR:4.841,95% CI:1.946 - 12.043) (P < 0.05).Conclusions Stricture,stone in both lobes and endoscope therapy were independent risk factors for residual stones; Residual stones,bile duct stricture and endoscope therapy were independent risk factors for recurrent stones and/or cholangitis.
7.Construction of Common TCM Academic Patent Database

There is rich scientific information in the traditional Chinese medicine ( TCM ) patents . However , few of them have been efficiently utilized by the academic researchers for a long time . In this paper , TCM patent literatures from 1992 to 2011 were collected by retrieving with SooPat patent searching engine . A total of 638 TCM entries have been indexed, which are consistent with the index in the Modern Chinese Materia Medica. The patents with special academic values were manually screened , and were finally stored in an Access database . A survey on the filed patents was outlined . The work provided a new information resource for the TCM researchers. And it was important to promote the effective use of TCM patents.
9.Risk factors of vascular cognitive impairment among Chinese population:Meta-analysis

Objective To explore the risk factors of vascular cognitive impairment (VCI)among Chinese population, and to clarify the scientific evidences for further prevention and treatment.Methods PubMed,CNKI,CBM,VIP and Wangfang databases (from 2002.1 to 2013.1)were searched to collect case-control studies or cohort studies studying risk factors of VCI among Chinese population. Meta-analysis was performed to calculate combined odds ratio (OR)or mean difference (MD)and its 95% confidence interval (95%CI).Results A total of 42 proper papers involving 3 282 cases and 7 815 controls were included in the review.For categorical variables,pooled OR and its 95% CI were as follows:hypertension 2.56 (2.03 - 3.21 ), hyperlipidemia1.79 (1.39 - 2.30 ), hyperglycemia 2.46 (1.90-3.19),Leukoaraiosis 5.46 (2.60-11.46),cerebral infraction multiple foci 4.39 (2.61-7.38),stroke history3.79(2.35-6.11),left hemisphere lesions 2.13(1.42-3.20),smoking 1.51 (1.08-2.11),drinking 0.99(0.73-1.36),basal ganglia lesions 2.15(1.55-2.99),thalamus lesions 2.34(1.57-3.47);for continuous variables,MD and its 95%CI were as follows:level of TG 0.35(0.15-0.55),level of TC 0.44(-0.16-1.04),level of folic acid -4.10(-5.50- -2.69),vitamin B12 -130.44(-225.46--35.41).Conclusion Except for drinking and level of TC, hypertension, hyperlipidemia, hyperglycemia, leukoaraiosis,cerebral infraction multiple foci,stroke history,left hemisphere lesions,smoking,basal ganglia lesions,thalamus lesions,high level of TG,low level of folic acid and vitamin B12 might be the risk factors of VCI among Chinese population.
10.The diagnostic value of metagenomic next-generation sequencing in pulmonary infectious diseases

Abstract: Objective To compare the application value of metagenomic next generation sequencing (mNGS) with traditional culture in diagnosis of pulmonary infection pathogens. Methods The clinical documents of 310 patients with suspected pulmonary infection admitted to the General Hospital of Center Theater Command from February 2021 to September 2022 were retrospectively analyzed. The results of mNGS and traditional culture were analyzed, followed by comparison on the positive rate, sensitivity, specificity, accuracy (ACC), positive predictive value (PPV) and negative predictive value (NPV) between the two methods. Results The study revealed that mNGS can simultaneously detect multiple pathogens, with the highest efficiency of detection for bacteria and the lowest for fungi. And the sequencing numbers of bacteria, fungi and viruses shown by mNGS were significantly different (H=70.361, P<0.001). In comparison, mNGS displayed a higher positive detection rate (88.40%) than traditional culture (29.70%) (χ2=162.373, P<0.001), but the consistency between the two methods was not significant (Kappa = -0.003, P=0.902). The sensitivity, specificity, ACC, PPV and NPV of mNGS were 91.29%, 28.26%, 81.94%, 87.96%, and 36.11% respectively, compared to corresponding 30.30%, 73.91%, 36.77%, 86.96% and 15.60% of traditional culture respectively. Through analysis, it is confirmed that the sensitivity and specificity between the two methods were statistically significant (91.29% vs 30.30%, χ2=148.120, P<0.001 and 28.26% vs 73.91, χ2=13.793, P<0.001). Conclusions mNGS can significantly improve the detection rate of pathogens in pulmonary infections and provide a complementary tool besides to traditional culture method for accurate anti-infection therapy. Furthermore, both traditional culture and mNGS pathogen detection methods are highly dependent on sample quality and detection quality control. mNGS requires the correct interpretation of comprehensive, non-destructive pathogenic genetic information to accurately identify pathogens.
