2.Research and design of mobile medical service platform based on cloud computing

Objective:To design a mobile health service platform based on cloud computing so as to reform the current medical problems including serious waste of medical resources, low level of information sharing and poor feel of medical treatment and so on.Methods: On the basic of analyzing cloud computing and relative theories, a solution of mobile medical cloud based on Google App Engine(GAE) was proposed.Results: A mobile medical health service platform including overall business process and deployment of development environment based on GAE has been designed and the traditional medical service mode has been converted.Conclusion: The exploration and attempt that constructe mobile medical health service platform by combining cloud computing with medical services can promote the conversion of medical mode and provide a effective solution for meeting the requirements of medical service quality and service efficiency for the masses.
3.Changes of choroidal thickness and hemodynamic parameters and their influencing factors in patients with diabetic retinopathy

?AIM: To study the changes of choroidal thickness and hemodynamic parameters in patients with diabetic retinopathy and their influencing factors.?METHODS: From January 2013 to January 2015, 100 patients (100 eyes) with type 2 diabetes were divided into 3 groups:34 patients without diabetic retinopathy ( NDR) , 36 patients with non proliferative diabetic retinopathy ( NPDR) group, and 30 patients with proliferative diabetic retinopathy (PDR). According to the results of OCT, the patients with diabetic retinopathy were divided into 2 groups: diabetic macular edema ( DME ) group ( 28 cases) , and 38 cases without diabetic macular edema. During the same period in our hospital 35 subjects for physical examination were selected as the control group. The hemodynamic parameters of the posterior ciliary artery in different groups of patients at different distance from the center of the macular were compared, analyzing influencing factors.?RESULTS:With the aggravation of diabetic retinopathy, the choroidal thickness in different distance from the center of the macular decreased. The choroidal thickness of NPDR and PDR group were thinner than that of the control group (P<0. 05). The choroidal thickness of NDR group was not different from the control group (P>0. 05). There was no significant difference in the choroidal thickness between the DME patients and non DME patients (P>0. 05). Pearson correlation analysis showed that there was no significant correlation between choroidal thickness of patients with diabetic retinopathy and diabetic duration, fasting blood glucose, HbA1c, eye axis length, systolic blood pressure and diastolic blood pressure (P>0. 05), but there was a correlation with BCVA (logMAR) (P<0. 01). EDV and PSV in NDR group and NPDR group were significantly lower than those in control group, RI was higher than that in control group. PSV and EDV in PDR group were significantly lower than those in other three groups, RI was higher than the other three groups, and the difference was statistically significant ( P<0. 05).?CONCLUSION:With the severity of retinopathy in type 2 diabetic patients, the choroidal thickness decreased, and the thickness of the choroid is beneficial in the comprehensive analysis of 2 diabetic retinopathy.
4.Effects of Pulsed Radiofrequency on Hyperalgesia and Expression of NR2B Subunit in Spinal Dorsal Horn in Chronic Constriction Injury Rats

Objective To observe the behavioral and the histopathology changes and expression of NR2B subunit in spinal dorsal horn in chronic constriction injury (CCI) rats after pulsed radiofrequency (PRF). Methods Forty-eight adult Sprague-Dawley rats were randomly divided into Sham-Sham (SS), Sham-PRF (SP), CCI-Sham (CS) and CCI-PRF (CP) groups. The right sciatic nerves (SNs) of the CS and CP groups were ligated to create the CCI model. For the SS and SP groups, the right SNs were separated without ligation. The CP and SP groups accepted PRF at the ligation site 15 days after modeling, while the electrode was placed in rats in the SS and CS groups without elec-tricity. The hindpaw withdrawal threshold (HWT) was measured before and 3, 7, 11, 15 days after modeling, and 1, 3, 7, 11, 15 days after treatment. The right SNs at ligation sites were assessed with optical microscopic score 15 days after treatment, and the NR2B expression in the L4-6 spinal dorsal horn were determined with Western blotting. Results HWT was significantly shorter in the CS and CP groups than in the SS and SP groups after modeling, and was more in the CP group than in the CS group. Under the optical microscope, the axonal diame-ter and myelin sheath thickness increased in the CP group compared to those in the CS group (P<0.01), the NR2B expression was less in the CP group than in the CS group after treatment (F=10.769, P<0.05). Conclusion PRF may reduce hyperalgesia and repair damaged SNs in CCI-induced neuropathic pain, which may associate with inhibition of the NR2B subunit expression in spinal dorsal horn.
5. Sonic hedgehog signaling pathway in tumorigenesis of glioma

Sonic hedgehog(Shh) is a member of the hedgehog family in vertebrate. The Shh signaling pathway is mainly composed of a secreted glycoprotein, named Shh, two transmembrane proteins (Ptch and Smo) and the downstream transcription factor family Glis. It plays a vital role in the embryo development, especially in the neuronal system. Recent study have demonstrated that the Shh pathway is closely associated with the tumorigenesis of various tumors. Glioma, the most common malignant brain tumor of humans, is characterized by the rapid proliferation, infiltrative growth and high rate of relapse, and it is one of the brain tumors with poorest prognosis. Abnormal activation of multiple signaling pathways has been known to enhance the proliferation ability of glioma cells. Moreover, glioma is composed of various tumor cells and the glioma stem cells were endowed with the ability of self-renewal and unlimited proliferation, which plays a key role in the tumorigenesis, progress and relapse. Evidence has been found that Shh signaling pathway is closely assoicated with tumorigenesis of glioma. Herein we review the current knowledge on the components of Shh signaling pathway and its role in the tumorigenesis of glioma and glioma stem cells.
6.Neuroprotection of adenoassociated vira-mediated brain-derived neurotrophic factor on retinal ganglion cells in DBA/2J mice with glaucoma

Background Deficiency of neurotrophic factor is associated with the damage of optic nerve in glaucoma.Reaserches showed that ectopically applied neurotrophic factor has a transient neuroprotective effect in glaucoma model,and the viral expression of adeno-associated neurotrophie factor may provide long-term supplementation of neurotrophic factor and neuroprotection in tissues.Objective The present study Was to investigate the neuroproteetive effect of adeno-associated viral(AAV)-mediated brain-derived neurotrophic factor (BDNF)on retinal ganglion cells(RGCs)in DBA/2J mice with experimental glaucoma. Methods 10 clean DBA/2J mice were administered intravitreal injection with 1 microliter of AAV-BDNF-GFP in the left eyes at the age of 6 months,and the right eyes were injected with the same volume of saline solution as control.Intraocular pressure (IOP)was measured with Tonolab in the mice every month.Retinas were obtained after 3 months for the investigation of GFP expression in RGCs using fluorescence microscopy.Immunohistochemistry Was performed by applying TUJ1 and Cy3 antibodies to identify surviving RGCs. Results The IOP of DBA/2J mice were 11.90 mmHg and 11.40 mmHg in the right eyes and left eyes,respectively,at 4 months.The IOP of DBA/2J mice began to rise at 5 months and reached its peak in 8 month-old mice.There was no statistically significant difference in IOPs between the right eyes and the left eyes from 4 month-through 9 month-old mice(t=-1.78-0.61,P=0.11-0.90).Three months after intrlavitreal injection of AAV-BDNF-GFP,GFP was positively expressed in RGCs of retinas with the expression rate of 46.33%±8.08%.The over-expression of BDNF led to more RGCs survival than the control eyes (3168.13±1319.33/mm2 vs 2024.81±796.38/mm2,t=2.75,P=0.02). Conclusion These data suggest that BDNF can exert a protective effect in DBA/2J glaucoma mice.
7.Follow-up study on the etiology of acute hepatitis B in adults

Objective To understand the etiology of acute hepatitis B (AHB) in adults and investigate the mechanisms of hepatic injury and viral clearance in AHB. Methods One hundred and twenty adult AHB patients were enrolled. Epidemiological and clinical data were collected from the case history records or face-to-face inquiry, and serum samples were collected during hospitalization and follow-up. To observe dynamic patterns of AHB etiology, the markers of hepatitis B virus (HBV) were detected by enzyme-linked immunosorbent assay (ELISA); the level of HBV DNA and HBV genotype were determined by real-time polymerase chain reaction (PCR). Enumeration data were analyzed by non-parametric rank sum test. Comparison between groups was done by t test and that between rates of samples was done by Pearson χ2 test. Results Serum HBV DNA was positive in 48.33% of patients at the time of diagnosis with mean level of 9.84×04 copy/mL, and became undetectable after 12.5 days on average. The median levels of serum alanine aminotransferase (ALT) were 1600 U/L and 1490 U/L in HBV DNA positive and negative groups, respectively (z=-0. 678, P=0. 498). However, the mean levels of serum ALT were (2058±123) U/L and (1393±139) U/L in groups of HBV DNA<1×104 copy/mL and>1×104 copy/mL, respectively, which was significantly different (t=-2.17, P=0. 049). Genotype B accounted for 52.5%, genotype C 42.5 and genotype B and C mixed type 5.0% in 58 patients with HBV DNA positive. Eight patterns of serum HBV markers were presented at first visiting. HBsAg(+), HBeAg(+), anti-HBc(+), anti-HBc IgM(+) and HBsAg(+), anti-HBe(+), anti-HBc(+), anti-HBc IgM(+) were the most common patterns, which accounted for 38.3% and 30.0%, respectively. The dynamic patterns of serum HBV markers of 28 AFIB patients were prospectively followed up. The rate of serum FIBsAg loss was 100. 0% and the median time of negative-conversion was 3 weeks. The cumulative positive rate of anti-HBs was 85.7% after 52 weeks of follow-up. The rate of serum HBeAg loss was 100.0%. HBeAg was negative in 53.6% of patients at first visiting and the rest of patients achieved negative within 4 weeks after onset. The positive rate of anti-HBe was 82.1% during 52 weeks of follow-up. Total anti-HBc (including IgG and IgM) was keeping positive in all patients within 52 weeks, and the negative rate of anti-HBc IgM was 39. 3% after followed up for 52 weeks. Conclusions Rapid HBV clearance andserum HBV marker conversion are significantly different between AHB and chronic hepatitis B.
8.Experimental study on the diuretic effect and bacterial activity in vitro of compound Salvia plebeia granules and its different effective parts in rats

Objective To study on the diuretic effects and bacterial activity in vitro of compound Salvia plebeian granules and its different effective parts in rats.Methods The diuretic effects of the compound Salvia plebeian granules(CSPG)and every extracted part were studied by using rat metabolic cage and analyzing the contents of Na~+、K~+、Cl~- in rat's urine.MICs of CSPG and every extracted part on different bacteria strains were determined by the method of concentration gradient dilution.Results In diuretic experiment,doses of 40,20 g·kg~(-1) of both CSPG and extraction of acetic ether(AE,effective part of CSPG)can conspicuously increase the urine volume of water load rats and also increase the contents of Na~+、K~+、Cl~- in rats'urine.The MICs of CSPG on E.coli and Staphylococcus aureus were 0.25 g·mL~(-1) and 0.50 g·mL~(-1).The MICs of AE on E.coli and Staphylococcus aureus were 0.5 g·mL~(-1) and 1.0 g·mL~(-1).Conclusion CSPG and its effective parts have the diuretic effects in vitro and bacterial activity.
9.Traditional Chinese medicine phenomics research on glycolipid metabolism disorder: a review

Abstract
Traditional Chinese medicine (TCM) has demonstrated unique advantages in the prevention and treatment of chronic diseases such as glycolipid metabolism disorder. However, its widespread application has been hindered by the unclear biological essence of TCM syndromes and therapeutic mechanisms. As an emerging interdisciplinary field, phenomics integrates multi-dimensional data including genome, transcriptome, proteome, metabolome, and microbiome. When combined with TCM's holistic philosophy, it forms TCM phenomics, providing novel approaches to reveal the biological connotation of TCM syndromes and the mechanisms of herbal medicine. Taking glycolipid metabolism disorder as an example, this paper explores the application of TCM phenomics in glycolipid metabolism disorder. By analyzing molecular characteristics of related syndromes, TCM phenomics identifies differentially expressed genes, metabolites, and gut microbiota biomarkers to elucidate the dynamic evolution patterns of syndromes. Simultaneously, it deciphers the multi-target regulatory networks of herbal formulas, demonstrating their therapeutic effects through mechanisms including modulation of insulin signaling pathways, improvement of gut microbiota imbalance, and suppression of inflammatory responses. Current challenges include the subjective nature of syndrome diagnosis, insufficient standardization of animal models, and lack of integrated multi-omics analysis. Future research should employ machine learning, multimodal data integration, and cross-omics longitudinal studies to establish quantitative diagnostic systems for syndromes, promote the integration of precision medicine in TCM and western medicine, and accelerate the modernization of TCM.
10.Searching of teaching model absorbing some contents of pharmacokinetics into medical higher mathematics
