1.Effect of potato homogenate on the treatment of drug extravasation caused by intravenous injection

Objective To explore the treatment effect of potato homogenate on the treatment of drug extravasation during intravenous injection.Methods 320 cases of drug extravasation patients during the intravenous infusion were randomly divided into the potato homogenategroup,named group A; patato slice group named group B and magnesium sulfate group as group C.The group A,B and C were separately treated with the external application of potato homogenate,thin slice of fresh potato and 33% magnesium sulfate.The therapeutic effects of the 3 groups were compared.Results The therapeutic effect of group A was superior to that of group B and C,and the healing time in group A was much shorter than that in group B and C,and group B was batter than group C,there was significant difference between the above comparison groups.Conclusions The therapeutic effect of external application of fresh potato in the treatment of extravasation injury caused by the drugs is remarkable.Potato homogenate can improve the treatment effect and shorten healing time when compared with potato slice.
2.Clinical comparison of using sevoflurane and ketamine in infants' removing stitches after cleft lip surgery

Objective:To study the feasibility of using sevoflurane induction in infants' removing stitches after cleft lip surgery. Methods:60 infants after cleft lip surgery were randomly divided into three groups:group K (ketamine group,n=20),group S (sevoflurane group,n=20) and group SN (sevoflurane and nitrous oxide group,n=20). Group K were given intramuscular ketamine 5 mg/kg,midazolam 0.05 mg/kg and atropine 0.01 mg/kg. Group S were induced with inhalation of 8% sevoflurane under 6 L/min oxygen. Group SN were induced with inhalation of 8% sevoflurane under 4 L/min nitrous oxide and 2 L/min oxygen. After induction,anesthesia was continued with inhalation of sevoflurane under 3 L/min oxygen for 2 min before starting removing stitches. HR and SpO2 were monitored regularly during operation. The induction time,recovery time,occurrence of head moving,complication such as respiratory depression and increased secretion were recorded. Results:Induction and recover time in group S and group SN were similar,but faster than that of group K. Head moving in group S and group SN were less than that in group K. There happened glossoptosis and increased secretion in all the three groups,but no differences were found significantly. Conclusion:Inhaled induction of sevoflurane has more rapid induction and recover compared with intramuscular ketamine,and can be used safely in infants' removing stitches after cleft lip surgery. Additional inhalation of nitrous oxide can not shorten infants' induction and recovery time than sevoflurane inhalation alone.

4.Risk factors for postoperative delirium in patients after vascular free flap reconstruction performed under general anesthesia

Objective To investigate the risk factors for postoperative delirium in patients after vascular free flap reconstruction performed under general anesthesia.Methods Two hundred and sixteen ASA Ⅰ-Ⅲ patients aged 18-80 yr undergoing vascular free flap reconstruction surgery were enrolled in this study.Patient characteristics before and during operation were recorded.The patients were followed up for 5 days after operation.Their level of consciousness,severity of pain and sleep quality were evaluated daily.The patients were divided into 2 groups according to the occurrence of delirium during the 5 days after operation:delirium group and non-delirium group.The method of CAM-ICU was reed in the diagnosis of postoperative delirium.Multivariate logistic regression was used to analyze the risk factors for postoperative delirium.Results logistic regression analysis showed that old age,history of alcohol abuse and sleep diacrder after operation were risk factors for delirium developed after free flap surgery.Conclusion Old age,history of alcohol abuse and sleep disorder after operation were the risk factors for postoperative delirium in patients after vascular free flap reconstruction performed under general anesthesia.
5.Molecular mechanisms underlying function of hair bundle: study on genetic deafness in mouse models.

Although the basic principles for the function of peripheral auditory system have been known for many years, the molecular mechanisms which affect deafness are not clear. In recent years, the study of hereditary deafness associated mouse models has revealed the molecular basis which is related with the formation and function of the hair bundle and the mechanosensory organelle of hair cell. This review focused on the role of protein network, which is formed by the proteins encoded by the Usher syndrome type 1 genes, in hair-bundle development and mechanotransducer channel gating. And the review also showed how the stereocilia rootlets contribute to the hair bundle's mechanical properties and how the hair bundle produces suppressive masking. Finally, the review revealed multiple roles of the tectorial membrane and extracellular matrix in the hair bundles stimulating in the cochlea.

6.Effect on body surface thermograph in patients with myofascial pain syndrome treated with moxibustion on Yanglingquan (GB 34) .

OBJECTIVETo compare the immediate effect of moxibustion on Yanglingquan (GB 34) on body surface thermograph between the patients with myofascial pain syndrome (MPS) and healthy people.
METHODSThirty-five cases of MPS were selected in observation group and 15 cases of healthy people were in control group. ATIR-M301 medical infrared imaging device was used to observe the changes in body surface thermograph before and 15 min after moxibustion on Yanglingquan (GB 34) The qualitative analysis of picture pattern and the quantitative analysis of temperature were integrated to compare the differences between two groups.
RESULTSConcerning to the infrared thermograph expression on the back of MPS cases in observation group, the abnormal thermal zone presented in 88.6% of the cases, mostly distributed in the upper and secondly in the lower back. The spinal thermal line was broken or disappeared ine about 50% of cases. The location of pain complained by 71.4% of cases was coincident with the zone of infrared thermograph expression. There was statistical significant difference in the temperatures difference of the two sides in the upper abnormal thermal zone on the back in observation group as compared with control group (P < 0.001). Body surface thermograph on the back were impacted by moxibustion on Yanglingquan (GB 34) for either MPS patients or healthy people, especially in upper back and rhomboid fossa. Moxibustion on Yanglingquan (GB 34) increased much more apparently the temperature on the upper back in MPS patients as compared with healthy people (P < 0.05).
CONCLUSIONMoxibustion on Yanglingquan (GB34) brings the immediate improvements in the back thermograph for MPS patients.

7.Observation on the effects of different partitioned moxibustion in treating ulcerative colitis

Objective: To observe the clinical effect and syndrome scores improvements of herbal cake-partitioned moxibustion (HPM) and ginger-partitioned moxibustion (GPM) in treating ulcerative colitis (UC). Methods:A total of 65 eligible cases were randomly divided into a HPM group (n=32) and a GPM group (n=33) according to their visiting order. Bilateral Tianshu (ST 25) and Dachangshu (BL 25) were selected for the HPM or the GPM treatment once daily, 12 d as a treatment course with a 3-day interval, 6 courses in all. The clinical effect, syndrome scale and Mayo scale were evaluated and compared between the two groups. Results:Of the 65 cases enrolled, 2 cases dropped out in the HPM group and 3 cases dropped out in the GPM group, 30 cases of each group finished the treatment courses. The total effective rate is 93.3% in HPM group and 86.7% in the GPM group, there was no statistically significant difference in the total effective rate between the two groups (P>0.05); there were statistically significant differences between the two groups in score evaluation of lasting time of abdominal pain and frequency of diarrhea, HPM is prior to GPM (P=0.032,P=0.044). There are no statistical significant differences between the two groups in scores evaluation of general symptom, three main symptoms, quality of life (QOL), frequency and severity of abdominal pain, times, and pattern of diarrhea (allP>0.05). There was a statistical significant difference in the improvement of Mayo score between the two groups, and HPM was superior to GPM (P=0.048). Conclusion:HPM and GPM are both promising ways to treat UC, and the total effect is quite similar. HPM is superior to GPM in the improvement of lasting time of abdominal pain and frequency of diarrhea, and also the Mayo score.
8.Effects of immune modulation therapy on cardiac function in aged patients with chronic heart failure

Objective To investigate the effect of immune modulation therapy on cardiac function and lymphocyte subsets in aged patients with chronic heart failure(CHF). Methods CHF(NYHA classification: Ⅱ - Ⅳ)patients older than 60 years were randomly divided into two groups: CHF intervention group received regular therapy and thymopetidum(2 mg/day i.m. for 75 days, n = 48), CHF control group received regular therapy(n = 48), 45 healthy individuals older than 60 years served as normal control. Left ventricular ejection faction of(LVEF), inner diameter of left ventricular end-diastole(LVEDD), inner diameter of left ventricular end-systole(LVESD), lymphocyte subsets, plasma high sensitive C-reactive protein(hsCRP), plasma brain natrium peptide(BNP)and 6 minutes walking distance(6MWT)were measured at before therapy, after the ftrst course(15 days)of treatment and after the third course of treatment(75 days). Results(1)Before therapy, the levels of BNP, hsCRP, CD8 T cells, LVEDD and LVESD were significantly higher and the levels of CD3,CD4, CD19 T cells, NK, CD4/CD8 ratio, LVEF and 6MWT were significantly lower in CHF patients compared to compared normal controls(all P < 0.05).These parameters were similar between CHF intervention group and CHF control group.(2)At 15 days, the levels of CD3, CD4, CD19 T cells and NK weie signiftcantly increased(P < 0.05 or P <0.01)while the level of CD8, BNP and hsCRP were significantly decreased(P < 0.05 or P < 0.01)in CHF intervention group compared with CHF control group.(3)At 75 days, the levels of CD3, CD4, CD19 T cells, NK,CD4/CD8, LVEF and 6MWT were significantly increased(P < 0.05 or P < 0.01)while the levels of CD8,BNP, hsCRP and Minnesota Living with Heart Failure Questionnaire(MLHFQ)were significantly decreased (P < 0.05 or P < 0.01)in CHF intervention group compared with CHF control group. Conclusion Thymopetidum, an immunemodulating agent, might regulate the quantity and proportion of lymphocyte subsets and improve cardiac function in aged patients with CHF, indicating that immune modulation therapy might be a new treatment strategy for aged CHF patients.
9.Effects of huannao yicong recipe extract on the learning and memory and related factors of Abeta generation in the brain of APP transgenic mice.

OBJECTIVETo study the effects of Huannao Yicong Recipe (HNYCR)extract on the learning and memory ability, and the expressions of amyloid precursor protein (APP), beta-site APP-cleaving enzyme 1 (BACE1), presenilin-1 (PS-1), and beta amyloid protein (Abeta)in hippocampus CA1 area of APP transgenic mice, and to explore its mechanisms for treating Alzheimer's disease (AD).
METHODSTotally 3-month-old APP695V7171 transgenic mice were used to establish the AD model in this research. They were randomly divided into the model group, the Donepezil group, the large dose HNYCR extract group, the small dose HNYCR extract group, and the normal control group (C57BL/6J mice), 15 in each group. These animals were gavaged for 4 continuous months. Relevant indicators were detected: Morris water maze test was used to measure the spatial learning and memory ability. The immunohistochemical assay was used to detect the expressions of APP, BACE1, PS-1, and Abeta.
RESULTSThe times of crossing the original platform and the swimming time and distance in the fourth quadrant of the 7-month-old APP transgenic mice were significantly reduced in Morris water maze test, when compared with the normal control group (P < 0.01). The times of crossing original platform and the swimming time and distance in the fourth quadrant of all treatment groups significantly increased in Morris water maze test, when compared with the model group (P < 0.05). The expressions of APP, BACE1, PS-1, and Abeta in hippocampus CA1 area of 7-month-old model mice increased significantly (P < 0.01), when compared with the normal control group. The expressions of APP, BACE1, PS-1, and Abeta in each 7-month-old intervention groups were significantly reduced, when compared with the model group (P < 0.01).
CONCLUSIONEarly application of HNYCR extract can obviously improve the learning and memory ability of APP transgenic mice that has declined, reduce the expressions of APP, BACE1, PS-1, and Abeta in the hippocampal CA1 area, reduce the production of Abeta, and slow down the pathological process of brains in APP transgenic mice.

10.Two novel mutations in one pedigree with hereditary Factor VII deficiency.

OBJECTIVETo explore the mutations of coagulation factor VII (FVII) gene in one pedigree with hereditary FVII deficiency, and to investigate the molecular mechanisms of FVII deficiency.
METHODSFVII gene mutations were analysed in the pedigree by direct DNA sequencing. The mutated DNA fragments were cloned into pMD19-T simple TA vector, and sequenced to confirm their distribution on chromosome. The plasma activity of FVII of the probands and their family members was detected with coagulation assay. The antigen of FVII were identified with ELISA.
RESULTSThree gene mutations were detected in the pedigree: A/G to C at 15386 resulting in Arg353Pro/Gln353Pro, A to T at 15274 resulting in Lys316Stop, all three mutations were heterozygotes. Three kinds of polymorphisms were identified in his father: A to G transition at position 15386 resulting in Arg353Gln, heterozygotic deletion of 2050 - 2059 cctatatcct in promoter and G to A mutation in intron 1a, the same polymorphisms were found in his grandfather. The three polymorphisms were located in the same chromosome of his father.
CONCLUSIONTwo mutations were found in the pedigree with hereditary FVII deficiency. One is nonsense mutation (Lys316Stop), the other is missense one (Gln353Pro). Gln353Pro and Lys316Stop might be the molecular mechanisms of FVII deficiency. The two novel mutations were reported for the first time in the literature.
