2.Effects of Xinnaoxin Pills on CAT Score,Cardiopulmonary Function and Hemorheology of Patients with Pulmonary Heart Disease Complicating with Coronary Heart Disease

OBJECTIVE:To explore the effects of Xinnaoxin pills on CAT score,cardiopulmonary function and hemorheology of patients with pulmonary heart disease complicating with coronary heart disease. METHODS:In retrospective study,80 cases of pulmonary heart disease complicating with coronary heart disease were randomly divided into control group and observation group with 40 cases in each group. Control group was given routine treatment as relieving asthma,eliminating phlegm and oxygen inhala-tion,anti-infective treatment,correcting acid-base balance,cardiotonic and diuretic treatment. Observation group was additionally given Xinnaoxin pills 1.0 g/time,bid after meal,on the basis of control group. CAT score,cardiopulmonary function and hemorhe-ology index were compared between 2 groups before and after treatment as well as ischemic ECG improvement effect and the occur-rence of ADR after treatment. RESULTS:After treatment,hematocrit hematocrit had changed slightly among hemorheology index-es,and CAT score,other hemorheology indexes and cardiopulmonary function indexes were improved significantly;the improve-ment of observation group was better than that of control group,with statistical significance(P<0.05). Total effective rate of isch-emic ECG was 90.0% in control group and 92.5% in observation group,there was no statistical significance(P>0.05). There was no statistical significance in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Xinnaoxin pills can effectively improve CAT score and cardiopulmonary function of patients with pulmonary heart disease complicating with coronary heart dis-ease. It also can improve hemorheology and shows good clinical efficacy,but great importance should be attached to the safety of drug use in the clinic.
3.Operative methods for cervical hyperextension injuries

Objective To explore rational operative methods to treat hyperextension injuries of cervical spine. Methods 47 cases of hyperextension injury of cervical spine were treated in our department from September, 2000 to October, 2004. They were followed up and analyzed retrospectively. According to the operative methods, there were 3 groups: anterior approach, posterior approach, combined approach. Their therapeutic effects were compared respectively. Results After follow- ups for 6 to 12 months, preoperative and postoperative Frankel classifications and ASIA (American Spinal Injury Association) grades were compared between the 3 groups. It was found that neural function improved significantly in the 3 groups after operation. The anterior approach and combined approach groups had significantly better outcome in neural function than the posterior approach group ( P0.05) . Conclusions Anterior decompression- graft- internal fixation is the preferred consideration to treat hyperextension injury of cervical spine, and the posterior approach is only the secondary choice. The combined approach should be carefully selected under appropriate conditions
4.Expression of type Ⅰ and type Ⅱ collagen in the healing of denervated tibial fracture in rats :A Western blot analysis

BACKGROUND:It has been recently indicated that nervous factors are able to adjust and dominate bone fracture healing. Type Ⅰ collagen is a major factor to promote the differentiation of osteoblasts and enhance the adhesion of osteoblasts; while, it is also a matrix protein for composing bone framework. Type Ⅱ collagen is derived from chondrocytes. OBJECTIVE: To study changing law of type Ⅰ and Ⅱ collagen expression during denervated bone fracture healing. DESIGN, TIME AND SETTING: Randomized controlled anima study was performed at the Animal Laboratory and Cell Biology Laboratory, the Second Military Medical University of Chinese PLA between May and December 2005. MATERIALS: Forty 3-month-year healthy male SD rats were randomly divided into fracture group (tibial fracture alone) and combination group (spinal cord injury combined with tibial fracture), with 20 rats at each group. METHODS: A φ 0.8 mm Kirschner wire was inserted into anterior border of left tibial plateau to establish tibial fracture models in the fracture group. A 0.3-cm spinal cord transection was cut at T10 segment to establish tibial fracture models with entire spinal cord injury. MAIN OUTCOME MEASRUES: Type Ⅰ and Ⅱ collagen protein expressions of callus were detected using Western blot technique in week 1, 2, 4, and 5 post-injury. RESULTS: One week after injury, type Ⅰ and Ⅱ collagen was represented in callus in the two groups, while the expressions in the combination group were significantly higher than fracture group (P<0.05); two weeks after injury, type Ⅱ collagen expression reached at the peak in the combination group, and the expression was significantly higher than the fracture group (P<0.05); four weeks after injury, type Ⅰ collagen expression reached at the peak in the fracture group, and the expression was significantly higher than the combination group (P<0.05), while type Ⅱ collagen still highly expressed in the combination group; five weeks after injury, type Ⅰ and Ⅱ collagen expressions were decreased in the two groups. CONCLUSION: Secretory law of type Ⅰ and Ⅱ collagen during denervated bone fracture healing is similar to normal bone fracture healing; however, the differences at time points, in particular expression at peak, are remarkable between them.
5.Analysis of the Utilization and Per Capita Burden of Antihypertensive Drugs Based on the Characteristics of the University Teaching Staff

OBJECTIVE:To understand the proportion of university faculty suffering from hypertension,and the regularity,ratio-nality and affordability of antihypertensive drugs under closed medical environment. METHODS:Empirical research and research methods were adopted to collect indicators during 2011-2014,such as average daily wage in Wuhan,the total number of staff in Wuhan University,the number of staff with hypertension,antihypertensive drugs amount/DDDs ranking ratio,affordability ratio,relative av-erage affordability, etc. The rationality and affordability of antihypertensive drug prices were evaluated. RESULTS:During 2011-2014,the prevalence rate of hypertension in the staff were about 23%,and the proportion increased year by year;the annual in-crease of antihypertensive drug amount was larger than that of the number of patients;the type of antihypertensive drugs with affordabil-ity ratio>1 in 4 years was ARB varieties,and affordability ratio of other drugs was less than 1. CONCLUSIONS:More than half of drugs is high in price,that result in heavy burden of patients receiving ARB;on the whole,antihypertensive drugs are affordable of the teaching staff of Wuhan University.
6.The therapeutic effect of full ablation and partial ablation of parathyroid for secondary hyperparathyroidism:a comparison study

Objective To investigate the curative effect of complete ablation and partial ablation of parathyroid for secondary hyperparathyroidism (SHPT) by using ultrasound-guided radiofrequency ablation (RFA) or microwave ablation (MWA). Methods A total of 26 patients with hyperparathyroidism secondary to uremia, who were admitted to authors’ hospital during the period from October 2012 to September 2013 to receive full ablation (n=11) or partial ablation (n=15) of parathyroid with MWA or RFA and were followed up for at least 9 months after the treatment, were included in this study. The clinical data were retrospectively analyzed. The therapeutic effect and the influence of MWA and RFA on the serum calcium levels were compared between the two ablation methods. Results Analysis of parathyroid hormone (PTH) levels indicated that in full ablation group the postoperative PTH level was gradually decreased , when compared with the preoperative level, until it became stable and maintained the normal level, and no recurrence was observed;in partial ablation group the postoperative PTH level was decreased first with a subsequent rebound about nine months after the treatment, and this PTH level was higher than the PTH level of full ablation group as well as higher than the normal level (P<0.05). On the 15 patients of partial ablation group, recurrence was seen in 5. Analysis of serum calcium levels showed that the postoperative serum calcium level was decreased in both groups, but in partial ablation group the serum calcium levels determined at 3 and 9 months after the treatment were higher than those in full ablation group (P<0.05). In 11 patients of full ablation group, hypocalcemia occurred in 4. Conclusion In treating SHPT, full ablation of parathyroid is not likely to have postoperative relapse, but it might carry the risk of hypocalcemia; while partial ablation of parathyroid is likely to have postoperative relapse, but the serum calcium level can be well controlled. Considering from the view that controlling of the serum calcium level is clinically very important, partial ablation of parathyroid may be more suitable for clinical purpose.
7.Effects of Yiangxuefukang Granules on The Contractility of Uterine Smooth Muscles

Objective: To observe effects of Yiangxuefukang Granules on the contractile function of uterine smooth muscles. Methods: Both the uteri of Sprague Dawley rats within 4~6 days postpartum and the uteri isolated from women of caesarean operation were used to study the effects of Yiangxuefukang Granules on the contractile amplitude, frequency and vitality of uterine smooth muscles. Results: The Granules significantly increased the contractile amplitude, frequency and vitality of both rat uterine smooth muscless in vivo and human uterine strips isolated.Conclusion: The Granules can be used in the treatment of the postpartum hemorrhage and the poor uterine involution caused mainly by lacking in the strength of uterine contraction.
8.Efficiency of two different operation ways for a complex kind of retinal detachment

Abstract? AIM: To evaluate the efficiency of two different operation ways, vitrectomy and vitrectomy combined with inferior scleral buckling, for a complex kind of retinal detachment.?METHODS:The complex kind of retinal detachment were diagnosed in 100 cases ( 100 eyes ) , with the common features:1)the course of more than 1mo;2) at least one retinal hole located in the inferior marginal retina;3) the detachment of retina was found proliferated, there was at least 1 retinal fold in retinal detachment area. The patients were randomly divided into two groups: the treatment group ( 50 eyes ) who received vitrectomy combined with inferior scleral buckling;the control group ( 50 eyes ) who received vitrectomy without scleral buckling. The retinal anatomic reattachment, visual function recovery, macular central fovea thickness and complications in two groups were observed.? RESULTS: There were 49 eyes ( 98%) with retinal anatomic reattachment in the treatment group while 42 eyes(84%)in control group(χ2=4.2605, P<0.05);the best corrected visual acuity ( BCVA) was 0.254 ±0.238 in the treatment group while it was 0.219±0.229 in control group (t=1.8879,P>0.05);the macular central fovea thickness was 272 ±32.21μm in the treat group while it was 316 ± 33.46μm(t=12.597,P<0.01).Intraocular pressure in 12 eyes ( 24%) was more than 30mmHg in the treatment group while 4 eyes ( 8%) in control group within 1wk postoperation (χ2=4.7619,P<0.05); intraocular pressure in 100 eyes were controlled below 21mmHg 1mo postoperatively.? CONCLUSION: It is a more effective method to vitrectomy combined with inferior scleral buckling than the single vitrectomy for the special kind of retinal detachment.It can increase the rate of retinal anatomic reattachment and reduce macular edema.
9.Analysis of 655 cases of mycoplasma pneumoniae infection in children

Objective To analyze the MP infection status in children with respiratory tract disease and the correlation with gen‐der ,age ,season and clinical conditions .Methods To investigate the clinical data retrospectively of 655 children with respiratory tract infection from January to December 2013 .Results The positive rate of MP antibody was 48 .09% with a higher incidence in girls than boys ,and those above 3 were more susceptible to it .Winter and spring were the peak seasons .The older group had a higher positive rate of MP antibody ,together with high morbidity of MPP and LP ,the differences were statistically significant (P<0 .05) ,while the younger group was inclined to get higher WBC count and percentage of increasing CRP and neutrophils ,the differ‐ences were statistically significant(P<0 .05) .Conclusion MP is a common pathogenic bacteria causes respiratory tract infection in children above 3 years ,especially in winter and spring .The antibody positive rate rise with age and the infected children are more likely to have pneumonia meanwhile the younger group has higher WBC count ,in which more cases get higher level of CRP and neutrophils .
10.Clinical status at first hospitalization and analysis of risk factors in 1242 patients with diabetic kidney diseases

Objective To investigate the clinical status of 1242 patients with diabetic kidney diseases (DKD) during their first hospitalization,and to analyze the risk factors of prognosis,so as to provide reference for clinical practice. Methods Retrospective case-control study was performed.Clinical data of 1242 patients diagnosed as DKD in first hospitalizaton from January 2003 to December 2008 were reviewed,and patients were followed up to realize the prognosis.Multiple regression analysis was carried out to screen the risk factors. Results Most of the patients were Mogensen stage Ⅳ or Ⅴ in their first hospitalization,accounting for 77.2%.24.8% of cases was complicated with cardiocerebrovascular diseases.Scr of 36.6% patients was higher than 176.8 μmol/L.One way ANOVA indicated that diabetes course,hemoglobin,serum albumin,Scr and Charlson index were significantly different among Mogensen stage Ⅲ, Ⅳ,Ⅴ patients.Logistic regression showed that age,albumin,Scr,cardiocerebrovascular diseases and Chalson index were risk factors for death in DKD patients (OR =1.057,0.908,1.002,2.006,1.371),but sex,diabetes course and hemoglobin were not risk factors,which was in accord with the resuh from 416 non-dialysis patients.Multiple linear regression analysis revealed serum albumin level was positively correlated with survival in non-dialysis DKD patients (P=0.003).The mean survival time was only 1.2145 year in 162 non-dialysis dead patients. Conclusions DKD patients in our hospital refer quite late,usually with poor conditions and complications.Most of DKD patients are Mogensen stage Ⅳ or Ⅴ in the first hospitalization.Age,serum albumin,Scr,cardiocerebrovascular diseases and Charlson index are risk factors of death,while gender,diabetes course and hemoglobin are not significantly correlated with death.In addition,serum albumin is positively correlated with survival time.Early diagnosis and management of risk factors are crucial for improving the prognosis of DKD patients.
