Objective To provide a description of clinical manifestations, special imaging features and treatment of diffuse axonal injury(DAI).Methods Retrospective analysis of 32 cases of diffuse axonal injury.Result 87.5% patients were involved in traffic accident. All patients came into persistent comatose state immediately. 75% patients had abnormal CT scan findings. MRI showed more sensitive. It could reveal injury appearance of corpus callosum and brain stem clearly. Hypothermic therapy and calcium ion antagonist were used. Conclusions combination of clinical manifestations and special imaging features helped to diagnosis of DAI in early stage. Intensive care and preventing secondary brain injury were the important treatment points.

AIM: To determine the suitable condition and the optimal parameters for the extraction and purifiction process of total flavonoid compounds in Scutellaria baicalensis. METHODS: It was investigated by orthogonal experiment design of form L_9(3~4) with the extraction percentage of the total flavonoid compounds and baicalin from Scutellaria baicalensis as the standard. RESULTS: The optimal condition of extraction was with water as much as ten times of the medicine amount and was boiled for 2 h and three times,and the optimal condition of purifiction was as follows: solution concentration 0.25 g(crude medicinal materials) /mL,adjusting pH to 2,80 ℃ heat preservation up to 60 min, 50% alcohol as precipitant and the precipitated adjusted pH to 4.0. CONCLUSIONS: The optimal process is rational,practical and adapted to industry production.

Objective To explore the clinical value of total laparoscopic hysterectomy(TLH).Methods Retrospective analysis was conducted on clinical data of 38 cases of TLH(Laparoscopic Group)and 40 cases of total abdominal hysterectomy(Open Group)from November 2005 to October 2006 in our hospital,to compare operative time,intraoperative blood loss,the incidence of postoperative pyrexia,recovery time of bowel movement,and hospital stay in both groups.Results The operative time in the Laparoscopic Group(85.8?13.8)min was significantly shorter than that in the Open Group(99.3?9.3)min(t=-5.923,P=0.000).The blood loss was significantly less in the Laparoscopic Group(105.8?32.4)ml than that in the Open Group(166.0?67.4)ml(t=-4.930,P=0.000).The incidence of postoperative pyrexia was distinctively lower in the Laparoscopic Group(3 cases)than that in the Open Group(15 cases)(?2=9.270,P=0.002).The time to first flatus was significantly shorter in the Laparoscopic Group(26.2?4.2)h than that in the Open Group(40.5?4.9)h(t=-13.601,P=0.000).The postoperative hospital stay in the Laparoscopic Group(6.8?0.9)d was distinctively shorter than that in the Open Group(10.6?0.8)d(t=-19.612,P=0.000).Follow-up examination for six months showed that two cases of poor healing of incision and two cases of vaginal residual granulation in the Open Group and one case of vaginal residual granulation in Laparoscopic Group,and no other complications were found.Conclusions TLH gets the advantage over total abdominal hysterectomy.If surgeons are familiar with the laparoscopic operative skills,TLH can be an ideal procedure for hysterectomy.

Objective To investigate the clinical application of the emergency triage criteria in the Hospital Emergency De‐partment Standardization Process by comparing it with the 5 level national triage system (5L NTS) .Methods Totally 984 patients visiting the emergency department of the Wushan County People′s Hospital from January 1 to January 31 ,2015 were chosen to con‐duct the evaluation of 4‐grade classification standard triage with 5L NTS as the control evaluation criteria .The error rate ,evaluation time and degree of difficulty or ease were compared between the two criteria .Results The error rate of 4‐grade triage standard was 4 .7% ,which of 5L NTS was 3 .6% ,the difference was not statistically significant (χ2 =1 .220 ,P=0 .269) .The triage time had sta‐tistical difference between the two criteria (t= -2 .291 ,P=0 .04) ,while the degree of difficulty or ease in using criteria had no sta‐tistical difference(χ2 =1 .143 ,P=0 .285) .Conclusion The 4‐grade triage criteria recommended by the Hospital Emergency Depart‐ment Standardization Process has no obvious differences in the aspects of triage error rate ,degree of difficulty or ease in use com‐pared with the widely recognized 5L NTS in the world ,and has the clinical promotion value .

Ninty-eight patients of type 2 diabetes mellitus with mild cognitive impairment were collected according to the Montreal Cognitive Assessment( MoCA )and mismatch negativity( MMN ).Forty-eight patients were assigned to conventional treatment with nimodipine (20 mg/d).The MoCA and MMN were evaluated and compared with the remaining 50 patients who served as a control group.There was no significant difference between two patient groups in MoCA( P＞0.05 ) ;the latency and amplitude of MMN were significantly improved in the treated group( P＜0.05 ) ; the amplitude of MMN was reduced in control group ( P＜0.05 ),but the latency of MMN showed no significant change( P＞0.05 ).

Aim To study the effect of PLC on rabbit and human platelet actin polymerization, and then to explore the mechanism of PLC anti-aggregation to platelet. Methods Platelets of rabbit and human were treated with PSS, ASA and different doses of PLC respectively and then were extracted by Triton abstraction. The relative concentration of actin of differently treated platelets induced by ADP was determined by SDS-PAGE and spectrophotometre. Results For rabbit platelets were treated with PSS, the relative concentration of actin determined at static state was 1.682?0.319; when the platelets were treated with ASA 668 ?mol?L -1,PLC 5,10,15,20 and 25 U?ml -1, the relative concentration of actin determined at activated state induced by ADP was 2.450?0.562,1.089?0.322,1.727?0.442,1.450?0.324,1.161?0.306, 0.857?0.242 and 0.692?0.187 respectively. The endothelial-to-mesenchymal transition (EndMT) in endothelial cells contributes to the development of cardiac fibrosis,ultimately leading to cardiac remodeling.In this study,the effects and molecular mechanisms of celastrol (CEL) on transforming growth factor-β1 (TGF-β1)-induced EndMT in human umbilical vein endothelial (HUVEC-12) cells were investigated.The presented data demonstrated.that CEL significantly blocked the morphology change of HUVEC-12 cells induced by TGF-β1 without cell cytotoxicity.In accordance with these findings,CEL blocked TGF-β1-induced EndMT as evidenced by the inhibition of the mesenchymal markers,including collagen Ⅰ,Ⅲ,α-SMA,fibronectin mRNA expression,and the increase in the mRNA expression of endothelial cell marker CD31.These changes were also confirmed by double immunofluorescence staining of CD31 and vimentin.The in vitro scratch assay showed that CEL inhibited the migration capacity of the transitioned endothelial cells induced by TGF-β1.Further experiments showed that the beneficial effect of CEL on blocking the EndMT in HUVEC-12 cells was associated with the suppression of the TGF-β1/Smads signalling pathway,which was also confirmed by the inhibition of its downstream transcription factor snail1,twistl,twist2,ZEB1 and ZEB2.These results indicate that CEL blocks TGF-β1-induced EndMT through TGF-β1/Smads signalling pathway and suggest that it may be a feasible therapy for cardiac fibrosis diseases.
