1.Reflections of the Subsidy Policy Regarding Assistive Devices Based on Needs Assessments on Persons with Disability

Objective To analyze the relationship between needs of assistive devices and the three other factors (age, disability categories, severity levels of disability) in order to scientifically formulate subsidy policy on assistive devices.MethodsThe assessment of rehabilitation needs for 928 Shenzhen citizens with new disability certificates in its six districts was performed by professionals, and the data was recorded into ACCESS database and analyzed.ResultsThe sequence of demand rates on assistive devices among disability categories was vision disability (78%), hearing disability (76%) and physical disability (74%); but speech, mental retardation and psychological disabilities had a low demand. The demand rates of assistive devices were dissimilar among four age groups for persons with vision, hearing and physical disabilities; for severity levels of three disability categories of vision, hearing and physical disabilities: the milder the vision disability was, the greater demand rates of assistive devices were; the demand rate of those with the second level of hearing disability was the highest; the demand rates of persons with the first and second levels of physical disability were relatively higher.ConclusionWhen drawing up subsidy policy on assistive devices for persons with disability, the disability categories, ages of persons with disability and severity levels of disability should be considered; the different types of assistive devices should be supplied according to rehabilitation goals at different age group; and necessary assistive devices should be provided according to severity levels of disability.
2.Effects of cripto siRNA on liver metastasis of colorectal cancer cell in nude mice

Objective To study the effects of cripto on migration, invasion, and liver metastasis of colorectal cancer cell. Methods After human colorectal cancer cell line SW480 was transfected by cripto small interfering RNA (siRNA), the mRNA and protein level were determined by Real-time RT-PCR and Western blot, respectively. The migration and invasion ability were evaluated by wound-healing assay and boyden chamber model, respectively. Thirty nude mice model of liver metastasis from colorectal cancer was established by splenectomy. Results The siRNA could down-regulate the level of mRNA and protein of cripto in a dose- and time-dependent manner. Suppression of cripto expression could inhibit migration and invasion ability of human colorectal cancer cell in vitro. The metastastic rate and tumor nodules were lower in transfection with cripto siRNA than in two control groups in vivo. Conclusions Cripto gene might play an important role in regulation of liver metastasis from colorectal carcinoma cell, and suppression of cripto gene by siRNA can inhibit liver metastasis of colorectal cancer.
3.Transaction log and its analysis applied to hospital information security

Objective To study transaction log and apply its analysis to hospital information security to prevent data loss and treat abnormal data.Methods The application of transaction log was described in log backup and database recovery,and the optration of log analysis was explored in HIS.Sqlplus and Ftp tools in Oracle were used to analyze the transaction log.Results The tracking and analysis of the log files contributed to making clear the incident,finding program bug,getting back lost data.Conclusion Log backup,database recovery and log analysis were of great importance for hospital information security,and log analysis has to be emphasized on in the future.

Adrenomedulin is an angiogenesis peptide that was found recently.It plays a critical role as an autocrine/paracrine tumor cell survival factor.Recently,adrenomedulin has been proved that it is overex- pressed in tumor,such as breast cancer.It may have a significant role in tumor progression and differentiation by stimulating mitogenic activities,inhibiting immune response,inhibiting apoptosis and enhancing angiogen- esis.We may realize targeted therapy tumor by intervening adrenomedulin,its receptor,and the signal con- duction pathway.

Biliary complication (BC) after liver transplantation has attracted increased attention from a clinical and research perspective.BC,which affects the long-term outcome of the liver transplant,includes diseases of biliary stricture,biliary obstruction,biliary fistula,and many more that need an operation or interventional therapy.The occurrence of these diseases is related to the surgical operation,biliary tract variation,vessel injury,and quality of the donor graft.This article reviews BC and its etiology,diagnosis,and therapy in order to help future clinical and experimental studies.
6.Donation after cardiac death versus donation after brain death for liver transplantation: a meta-analysis

Objective To compare the outcome of donation after cardiac death (DCD) versus donation after brain death (DBD) for liver transplantation.Method Such databases as PubMed,Cochrane Central Register of Controlled Trials (CENTRAL),EMbase,the ISI Web of Knowledge databases and CBMdisk were searched from Month 1990 to March 2011 for collecting the randomized controlled trials (RCTs),case control studies and cohort analysis about DCD versus DBD for liver transplantation,and the references of those trials were also searched by hand.After study selection,assessment and data extraction conducted by two reviewers independently,meta-analyses were performed by using the RevManS.1 software.The quality of evidence was assessed by using the GRADEpro software.Result DCD group had similar MELD of recipients with DBD group before operation [Z =1.37,95% CI(-2.25,0.26),P =0.17],and DCD group got shorter cold ischemia time than DBD group [Z=2.26,95%CI(-1.76,-0.12),P =0.02].DCD group had higher hiliary complication incidence [Z =6.37,95% CI(1.89,3.31),P<0.000 01],higher vascular complication incidence [Z =2.14,95% CI(1.03,2.17),P =0.03],higher liver primary non-function (PNF) incidence [Z =4.43,95% CI (2.02,6.17),P<0.000 01],lower 1-year graft survival rate [Z =3.78,95% CI(0.84,0.94),P =0.0002] and lower 3 year graft survival rate[Z=2.54,95% CI(0.73,0.96),P =0.01] than DBD group.The quality of the result was verified from low to moderate.Conclusion Liver transplantation using DCD had higher incidence of complications and lower 1-year and 3-year graft survival rate than DBD.For the poor quality of the original studies,a prudent choice is suggested.More randomized controlled trials are needed.
7.Different expressions of p53, bcl-2 and proliferative cell nucleus antigen in osteosarcoma tissues in patients of different nationalities in Xinjiang autonomous region

BACKGROUND: Osteosarcoma is the most common type of bone tumor and generally occurs between the age of 10 to 25; moreover, in clinical practice osteosarcoma is found to occur more often in minority nationalities in Xinjiang Autonomous Region. Do its incidence and prognosis vary between nationalities at gene levelOBJECTIVE: To observe the different expressions of p53, bcl-2 and proliferative cell nucleus antigen (PCNA) gene during the development of osteosarcoma between various Xinjiang nationalities.DESIGN: Non-randomized comparative experiment taking clinical pathological specimens as subjects.SETTING: Immunohistochemical Laboratory, the Pathological Center of Xinjiang Medical University.PARTICIPANTS: Totally 52 pieces of specimen were obtained from excised osteosarcoma tissues in the Pathological Department, the First Affiliated Hospital of Xinjiang Medical University, and the Pathological Department, the First Affiliated Hospital of Shihezi University, between January 1,1984 and December 31, 2001. The 52 cases of osteosarcoma included 29cases derived from male patients and 23 from female patients; among them there were 12 cases of Kazak minority, 17 cases of Uygur minority and 23cases of Han nationality. Other 32 specimens were obtained from 32 patients with tumor-like lesions (such as osteofibrous dysplasia or fibrous dysplasia), including 7 cases of Kazak minority, 11 cases of Uygur minority,and 14 cases of Han nationality. The informed consent was obtained from the patients.METHODS: This experiment was carried out at the Immunohistochemical Laboratory, the Pathological Center of Xinjiang Medical University. LSAB method was used to detect p53, bcl-2 and PCNA expression in the two groups. The first antibody was re placed by PBS as blank control, and the available positive expression was taken as positive control. P53 protein and PCNA were observed to express in cell nucleus, appearing obvious redbrown granules with positive expression, whereas Bcl-2 protein was expressed in cytoplasm.lationship between the expression of Bcl-2, p53 and PCNA in osteosarcoma.of different nationalities: The expression of p53, bcl-2 and PCNA in Kazak minority, Uygur minority and Han nationality was not remarkable p53, bcl-2 and PCNA in osteosarcoma and bone tumor-like lesions: The expression of P53, Bcl-2 protein and PCNA in osteosarcoma was remarkably higher than that in tumor-like lesions (42.31% vs. 3.13 %, 59.62%sion of bcl-2, p53 and PCNA in osteosarcoma: There was a close correlation between bcl-2 and p53, as well as between bcl-2 and PCNA in osteosarcoma tissues (X2 =5.818 2, 4.900 0, P<0.05).CONCLUSION: The results indicate that there was no statistical difference between various nationalities in the expression of p53, bcl-2 and PCNA, as well as osteosarcoma differentiation. Suggesting that these genes may share the common regulation during the development of osteosarcoma,which is less associated with their nationality-related hereditary background.

9.Effect of HL-60 cells stimulated by IFN-γ and IL-10 to express B cell activating factor via transcriptional pathway

Objective To explore the effect of IFN-γ, IL-10 and IL-4 on B cell activating factor (BAFF) expression in human HL-60 cells, a kind of myeloid tumor cell lines, and its possible regulation mechanism. Methods Cultured human HL-60 cells were treated with IFN-γ, IL-10 and IL-4 for 1-3 days. The expression of membrane-bound BAFF on HL-60 cells was examined by flow cytometry, the amount of soluble BAFF was detected by ELISA assay, and the level of BAFF mRNA was tested by real-time PCR method. A functional 1021 bp fragment of the 5'-tlanking region of the human BAFF gene (-1349 to -329 bp) was cloned and investigated with serial 5'-deletion. The 5'-deleted promoters were recombinated with chloramphenicol acetyltransferase (CAT) as reporter gene. These five recombinant plasmids were transiently transfected to HL-60 cells with liposomal transfectian method. Promoters activities were determined by CAT reporter gene assay(CAT-ELISA) in those transfected cells treated with different cytokines. Results The results showed that the expression of membrane-bound BAFF, soluble BAFF and BAFF mRNA in human HL-60 cells were significantly elevated (P < 0. 05) after incubated with IFN-γ and IL-10. In addition, IFN-γ and IL-10 showed significantly (P < 0. 05) increased effects on promoter activity in human BAFF gane. And the cytokines-responsive sequences were located between -929 and -719 bp of the BAFF promoter region. Conclusion The enhancement of IFN-γ and IL-10 on BAFF expression and synthesis were regnla-ted by promoter activation. Our in vitro studies also raise the possibility to investigate the mechanisms regula-ting BAFF expression in other tumor cells of myeloid origin under pathological circumstances.
10.The effect of cytokines on promoter activity of human BAFF gene

Objective To explore the effect of several cytokines, including interferon-γ, interleukin-10 and interlekin-4, on promoter activity of human BAFF (B-cell activating factor belonging to tumor necrosis factor family) gene. Methods A construct of phBAFF 1.02 containing sequence form -1349 bp to -329 bp of human BAFF gene, linking with chloramphenicol acetyltransferase (CAT) as reporter gene, was transiently transfected into human HL-60 cells, a kind of myeloid tumor cell lines. The cells were subsequently treated with IFN-γ, IL-10 and IL-4, and the CAT activity was assessed 24 hours after stimulation with each cytokines. Results IFN-γ of 5 ng/ml, IL-10 of 100 ng/ml could increase the CAT activity of phBAFF 1.02 to 4.18 and 2.13 folds respectively compared to the control. IL-4 at 100 ng/ml had no effect on promoter activity of human BAFF gene. Combination of IFN-γ, IL-10 and IL-4 could increase the CAT activity of phBAFF 1.02 to 3.41 and 1.58 folds respectively compared with controls. Conclusion IFN-γ and IL-10 can increase the promoter activity of human BAFF gene. IL-4 treatment can not affect the CAT activity driven by BAFF promoter. However, IL-4 can decrease the upregulating effect of IFN-γ and IL-10 on phBAFF1.02. These provide essential evidence for future study on the interaction mechanism of cytokines and BAFF in autoimmune diseases.
