1.Experimental study on the protective effect of anisodamine on flaps with ischemia reperfusion injury.

OBJECTIVETo observe the effect of anisodamine (Ani) injection on the survival rate and histologic result of flaps with ischemia-reperfusion injury, so as to demonstrate the protective effect of Ani on the flap survival.
METHODSA total of 48 healthy male Wistar rats were randomly divided into model control, normal saline(NS) and anisodamine groups, with 16 rats in each group. An 3 cm x 6 cm axial flap was formed at the right lower abdomen with abdominal superficial blood vessel as the pedicle. 0.5 cm x 0.5 cm skin tissue was taken from the middle part of flaps in each group, immediately after operation, 12, 18, 24 h after operation. The superoxide dismutase (SOD), nitric oxide (NO), nuclear factor-kappaB contents in the specimens were detected. The histologic study was also performed. The flap survival rate was recorded 7 days after operation.
RESULTSFlap survival rate was (78.6 +/- 7.3) % in Anisodamine group. 12, 18, 24 h after reperfusion injury, the SOD was (103.3 +/- 3.9), (82.6 +/- 3.8), (67.5 +/- 4.6) U/mg; the NO was (5.33 +/- 2.05), (4.75 +/- 1.68), (4.15 +/-1.59) nmol/mg; the NF-kappaB was 0.211 +/- 0.039, 0.313 +/- 0.033, 0.096 +/- 0.028. The contents of SOD, NO and NF-kappaB had the statistical difference of at different time. The skin pathological changes in Anisodamine group was obviously better than those in NS group. Flap survival rate in Anisodamine group was significantly higher than that in NS group.
CONCLUSIONSIn the flap with ischemia-reperfusion injury, Anisodamine can reduce the damage of free radical, increase the blood flow, reduce the production of NF-KB, decrease inflammatory reaction. So Anisodamine can increase the survival rate of flaps with ischemia reperfusion injury.

2.Study on the promoter hypermethylation of TSHR, P16, and RAS genes in human papillary thyroid carcinoma

Objective To study the expressions of the tumor suppressor gene TSH receptor( TSHR),P16, and RAS in papillary thyroid carcinoma ( PTC ) , and the correlation between the occurrence of tumor and the aberrant promoter hypermethylation of three tumor suppressor genes. Methods RT-PCR was used to detect the mRNA expression of three tumor suppressor genes in tissues of 50 cases of PTC ,20 cases of nodular goiter,and 12 cases of thyroid adenoma. The promoter methylation status of three tumor suppressor genes was examined by methylation-specific PCR technique( MSP). Gene sequencing was used to test if the hypermethylation existed in the promoter of three tumor suppressor genes. Results In 68.0% (34/50) TSHR gene, 54.0% (27/50) P16 gene, and 60.0% ( 30/50 ) RAS gene in PTCs, hypermethylation in promoter region was detected, the respective results 21.9% (7/32) , 15. 6% (5/32) ,and 31. 3% (10/32) were found in control tissues. The rates of the three genes with promoter hypermethylation in PTC were significantly higher than those in control tissues ( all P<0. 05). The mRNA expressions of TSHR,P16,and RAS were significantly lower in PTC than those in control tissues (0. 41 ± 0.11 vs 0.63±0. 08,0. 51±0. 17 vs 0. 72±0. 22,0. 56±0. 10 vs 0. 67±0. 16, all P<0. 05). The sequencing confirmed that there was CC to TC transmission in the promoters of three tumor suppressor genes. Conclusions The methylation of three tumor suppressor genes in promoter region is a common molecule event and may be involved in the genesis and development of human PTC.
3.Effect of thoracic close drainage assisted by thin chest tube after video-assisted thoracic surgery lobecto-my

Objective To evaluate the clinical effects of thoracic close drainage with thin drainage tube assisted to thick drainage tube after video-assisted thoracic surgery(VATS)lobectomy. Methods We ret-rospectively reviewed 89 patients received VATS lobectomy in Chinese PLA General Hospital from January 2014 to September 2014. The patients with non-small cell lung cancer were divided into two groups:treatment group (50 patients)and control group(39 patients). Treatment group took thin tube assisted to thick tube of thoracic close drainage and control group took general thoracic closed drainage tube. We studied the operation time,the bleeding of operation,the number of lymph node dissection,time of first activity out of bed,the hospitalization time of post-operation,post-operative complications,the days of post-operative drainage,drainage volume,the effect of drainage,the VAS evaluation score of post-operative pain in the two groups. 4.Splenic thickness combined with routine clinical markers predicts esophageal varices in patients with liver cirrhosis.

OBJECTIVETo explore the noninvasive indicators for predicting the occurrence of esophageal varices (EV) in patients with liver cirrhosis.
METHODSA total of 202 patients with liver cirrhosis caused by hepatitis B or C or alcoholic hepatic disease were enrolled in this study. EV and high risk esophageal varices (HREV) were confirmed in these patients by gastroscopy. The hematological, serum biochemical and ultrasonic parameters of the patients were analyzed, and a model for predicting EV was established by stepwise logistic regression analysis.
RESULTSThe areas under receiver operating characteristics curve (AUROC) of splenic thickness (SPT) for detecting EV and HREV were 0.827 and 0.766, respectively. The combined index USWA (SPT, US, WBC and albumin [ALB]) showed an AUROC of 0.873 for detecting EV, and the index SPA (combining SPT and ALB) showed an AUROC of 0.777 for detecting HREV. The direct combination of SPT with USWA or with platelet/splenic thickness ratio (PSA) was capable of completely excluding a definite diagnosis of EV, while the sequential combination of SPT with USWA or with platelet was capable of a diagnosis of EV and clarifying the condition of EV in approximately half of the cirrhotic patients in the absence of gastroscopic findings. The combination of SPT and SPA allowed for a definite diagnosis of the condition of HREV in 10% of the cirrhotic patients.
CONCLUSIONSPT combined with SPT derived indexes or platelet status allows for a definite diagnosis of EV in patients with liver cirrhosis to offer a noninvasive option for diagnosis.
5.Case-control study on midterm follow-up effects of the extended release of posterior clearance in total knee arthroplasty.

OBJECTIVETo evaluate the midterm follow-up results of extended release of posterior clearance in total knee arthroplasty.
METHODSA total of 120 patients with knee osteoarthritis were equally randomly assigned to the experimental group and control group, and underwent unilateral TKA from March 2010 to March 2012. In experimental group, there were 21 males and 39 females with an average age of (62.2±10.9) years old. In the control group, there were 25 males and 35 females with an average age of (64.9±11.4) years old. All the patients were performed using the anterior knee approach. During operation, after osteotomy of the tibia and the femoral condyle, extended release of the posterior knee clearance were taken in experimental group, while only the clearance of osteophyte in the posterior condyle were performed in the control group. The KSS scores including knee functional score and knee clinical score,as well as the range of motion (ROM) of patients, were compared between the two groups at midterm follow-up.
RESULTSTotally 49 patients in the experimental group and 54 patients in the control group were followed up, and the median follow-up time was 46 months. The knee functional score of patients in the experimental group was 91.3±3.4, which was better than 86.4±3.9 of patients in the control group; initiative ROM of flexion of patients in the experimental group was (133.2±5.9)°, which was better than (126.9±7.4)° of patients in the control group. There were no significant difference of knee clinical score between 86.9±4.6 of patients in the experimental group and 85.7±5.1 of patients in the control group, and the initiative ROM of extension between (0.5±1.1)° and (0.3±1.2)°.
CONCLUSIONExtended release of the posterior knee clearance contributes to the knee function and initiative flexion ROM during a midterm follow-up and patients benefit.

6.Effects of Kouyanqing Granules on Prevention and Treatment of Radiation-induced Oral Mucositis Patients with Head and Neck Cancer after Radiotherapy

OBJECTIVE:To investigate the effects of Kouyanqing granules on prevention and treatment of radiation-induced oral mucositis patients with head and neck cancer after radiotherapy.METHODS:A total of 122 head and neck cancer patients receiving radiotherapy were randomly divided into kouyanqing group (61 cases) and chlorhexidine group (61 cases).Based on radiotherapy,kouyanqing group was given Kouyanqing granules 20 g dissolved in 50 mL lukewarm boiled water,twice a day,2-3 d before radiotherapy.Chlorhexidine group was given Compound chlorhexidine gargle 10-20 mL,twice a day,2-3 d before radiotherapy.Treatment courses of 2 groups lasted for 7 weeks.The occurrence of oral mucositis,VAS scores and ADR were observed in 2 groups.RESULTS:3rd week radiotherapy later,the incidence of oral mucositis at level 2-3 and 4th-7th week radiotherapy later,the incidence of oral mucositis at level 2-4 in kouyanqing group was significantly lower than chlorhexidine group,with statistical significance (P<0.01).2nd-7th week radiotherapy later,VAS score of kouyanqing group was significantly lower than that of chlorhexidine group,with statistical significance (P<0.01).There was no statistical significance in the incidence of ADR between 2 groups (P>0.05).CONCLUSIONS:Kouyanqing granules can relieve radiation-induced oral mucositis of head and neck cancer and the degree of pain,and improve the quality of life.Its safety is similar to that of chlorhexidine.
7.The neuro image study on the brain cortex in schizophrenic patients and their unaffected parents

Objective To investigate the morphological abnormalities of the brain cortex in schizophrenic patients and their parents. Methods We recruited 33 schizophrenic patients, 63 their unaffected biological parents. Thirty well matched healthy controls served as control group 1 (for patients), and 28 well matched healthy controls as control group 2 (for parents). The high-resolution magnetic resonance imaging (MRI) and Freesurfer software were used to measure the gray matter volume, cortical thickness, cortical surface area, curvature and volume of subcortical regions on all the sub-jects. Results The patients demonstrated significant reduction of cortical thickness in the left middle-anterior cingulate, middle occipital gyrus, inferior circular sulcus of the insula, lateral occipito-temporal sulcus, the right lateral occipito-temporal gyrus, middle temporal gyrus, superior circular sulcus of the insula, inferior temporal sulcus, the bilateral poste-rior-dorsalcingulated gyrus and superior temporal sulcus (P<6.0×10-4,corrected by Bonferroni method). Compared with control group 2, the unaffected parents exhibited slight reduction of cortical in the left subcentral, triangular part of the in-ferior frontal gyrus, middle frontal gyrus, superior circular sulcus of the insula, occipital pole, inferior frontal sulcus, mid-dle occipital sulcus and lunatus sulcus, lateral orbital sulcus, the right middle occipital gyrus, lingual gyrus, posterior lat- eral sulcus and inferior precentral sulcus, but the reduction was not significant after correction for multiple comparisons. Conclusions Our results indicate that cortical thickness can specifically reflect the brain morphological abnormalities of schizophrenia and the reduction of cortical thicknes may be a heritable phenotype of schizophrenia.
8.Relations between resistin and hepatic fibrosis in nonalcoholic fatty liver disease subjects

Objective To evaluate the relationship between resistin and hepatic fibrosis in nonalcoholic fatty liver disease (NAFLD) subjects.Methods Serum resitin and Interleukin (IL)-18's concentrations were measured in 144 subjects (70 NAFLD pa-tients and 74 healthy controls) with enzyme-linked immunosorbent assay (ELISA) and their serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), fasting blood glucose (FBG), HbA1c, triglyceride (TG), total-cholesterol (TC), low density lip-oprotein ( LDL) , high density lipoprotein ( HDL) , homeostasis model assessment of insulin resistance ( HOMA-IR) , hyaluronic acid , IVcollagen, height, body weight, waist circumference, and hip circumference were also measured .Their waist-to-hip ratio (WHR) and body mass indes (BMI) were calculated.Results There were higher levels of systolic blood pressure (SBP), WHR, BMI, TG, ALT, AST, HOMA-IR, hyaluronic acid, IV collagen, resistin, and IL-18 in the NAFLD patients than those of the healthy controls ( P <0.05 ) .The serum resistin level was significantly and positively correlated with WHR , BMI, ALT, HOMA-IR, TG, IV collagen , and IL-18 ( r =0.231 , 0.341 , 0.223 , 0.264 , 0.213 , 0.315 , and 0.669 , P <0.05 ) .After adjustment for body fat and IL-18 , the serum resistin level was still significantly and positively correlated with ALT and IV collagen ( r =0.222 , 0.0.326 , P <0.05 ) . Conclusions Resistin might take part in the development of NAFLD insulin resistance and its pro -inflammation.
9.Simultaneous determination of sivelestat and its metabolite XW-IMP-A in human plasma using HPLC-MS/MS.

A simple and rapid method was developed based on high performance liquid chromatography coupled to tandem mass spectrometry (LC-MS/MS) to determine sivelestat and its metabolite XW-IMP-A in human plasma. After a simple protein precipitation, the samples and internal standards were analyzed on a C18 column by a gradient elution program. The mobile phase consisted of 30% acetonitrile in methanol and 5 mmol · L(-1) ammonium acetate at a flow rate of 0.7 mL · min(-1). The mass spectrometric data was collected in multiple reaction monitoring mode (MRM) in the negative electrospray ionization. The standard curves were linear in the range of 10.0-15,000 ng · mL(-1) for sivelestat, and 2.50-1000 ng · mL(-1) for XW-IMP-A. The low limits of quantitation were identified at 10.0 and 2.50 ng · mL for sivelestat and XW-IMP-A, respectively. The intra- and inter-day precision were within 11.3% and 13.1% for sivelestat and XW-IMP-A, and accuracy was 0.3% and 0.6% for sivelestat and XW-IMP-A, within the acceptable limits across all concentrations. The method was successfully validated in the pharmacokinetic study of sivelestat in healthy Chinese volunteers.

10.Cellular toxicity and anti-tumor efficacy of iRGD modified doxorubixin loaded sterically stabilized liposomes.

iRGD-modified sterically stabilized liposomes loaded doxorubicin (iRGD-SSL-DOX) were prepared and their cellular toxicity and anti-tumor efficacy were evaluated, comparing to doxorubixin loaded sterically stabilized liposomes (SSL-DOX) and RGD modified doxorubixin loaded sterically stabilized liposomes (RGD-SSL-DOX). The iRGD peptide, with both tumor targeting and cell penetrating functions, was conjugated to DSPE-PEG-NHS and DSPE-PEG-iRGD was obtained. DSPE-PEG-RGD was gained in the same way. iRGD-SSL-DOX, RGD-SSL-DOX and SSL-DOX were prepared by ammonium sulfate gradient method. The size and zeta potential of the liposomes were characterized by dynamic laser light scattering. The cellular toxicity study was done on B16 melanoma cell line and the anti-tumor efficacy study was carried on B16 cell line bearing C57BL/6 mice. The results showed that the particle sizes of liposomes were all around 90-100 nm. DOX entrapment efficiency was above 95%. The formulations were with good preparation reproducibility. iRGD-SSL-DOX showed no significant difference in B16 cellular toxicity with SSL-DOX and RGD-SSL-DOX, but the anti-tumor efficacy on B16 melanoma bearing C57BL/6 mice was significantly better than that of SSL-DOX, similar as that of RGD-SSL-DOX. Therefore, iRGD modified liposomes loaded DOX would be a promising drug delivery system for tumor therapy.
