3.Xpert MTB/RIF assay in rapid diagnosis of tuberculous lymphadenitis

Objective To assess the application of Xpert MTB/RIF assay in rapid diagnosis of tuberculous lymphadenitis.Methods Lymph node samples were collected from 1 02 clinically diagnosed patients with lymph node tuberculosis and 65 patients with other lymph node diseases from Zhejiang Provincial Hospital of Traditional Chinese and Western Medicine during January 201 4 and February 201 5. Xpert MTB/RIF,pathological examination and mycobacterium tuberculosis culture were conducted in all specimens of two groups.Taking clinical comprehensive diagnosis as the gold standard,the diagnostic sensitivity,specificity,positive predictive value,negative predictive value and diagnosis efficacy of three detection methods were assessed.The sensitivity of Xpert MTB/RIF in determining rifampicin resistance was analyzed using drug susceptibility testing as gold standard.Results The mean detection time of Xpert MTB/RIF was (2.2 ±0.2)h.Among 1 02 patients,Xpert MTB/RIF achieved higher sensitivity (96.1 %, 98 /1 02 ) than pathological examination (76.5%,78 /1 02 ) and mycobacterium tuberculosis culture (33.3%,34 /1 02)(χ2 =1 6.558 and 87.91 9,both P <0.01 ).Among 65 patients with other lymph node diseases,the specificity of all three detection methods was 1 00%.The receiver-operating-characteristic curve analysis demonstrated that diagnostic performance of Xpert MTB/RIF was better than that of other two methods.In 8 patients resistant to rifampicin confirmed by drug susceptibility testing,Xpert MTB/RIF detected 6 resistant-resistant patients.Conclusion Xpert MTB/RIF shows higher sensitivity and specificity in diagnosis of lymph node tuberculosis with the advantages of easy and rapid performance.
4.The value of real time three-dimensional ultrasound in differentiating benign and malignant breast tumors

Objective To evaluate the value of real time three-dimensional ultrasound in differentiating benign and malignant breast tumors. Methods One hundred and fifty-three patients with breast tumors were examined with real time three-dimensional ultrasound. The results were compared with post-operative pathological results. Results A sign of malignancy on the coronal plane was observed by three-dimensional ultrasonography,named “sun-like symptom”,because the sign looked like a shining sun:striated hyperechoic or iso-echoic images scattered radially around cancers,together with the distortion of normal tissue.The accuracy of the sign in differentiating benign and malignant breast tumors was 86.93 %( 133/153 ), the sensitivity was 78.38 %( 58/74 ), and the specificity was 94.94 %( 75/79 ). Conclusions Real time three-dimensional ultrasound is a valuable and new technique in differentiating benign and malignant breast tumors.
5.Surgery treatment and clinical research in renal transplant recipients with benign prostatic hyperplasia

Objective To study the clinical manifestations and surgical treatment in renal transplant recipients with benign prostatic hyperplasia (BPH). Methods From January 2013 to May 2015, 5 patients of kidney transplant recipients who suffered from BPH for transurethral resection of prostate (TURP) treatments were enrolled (kidney transplant recipients group) and 15 patients with the same surgical treatments during the same peoriod were enrolled too (common control group). The routine examination, urodynamic, sex hormone levels and and the clinical efficacy were observed. Results The levels of hemoglobin, total protein, urea nitrogen, creatinine, prostate volume , residual urine and maximum flow rate (Qmax) before operation between two groups had no significant differences (P>0.05). The level of testosterone in kidney transplant recipients group was significantly lower than that in common control group: (3.5 ± 1.2) mmol/L vs. (5.5 ± 2.2) mmol/L, P < 0.05. The level of estradiol in kidney transplant recipients group was significantly higher than that in common control group:(49.6 ± 13.8) mmol/L vs. (40.5 ± 16.4) mmol/L, P < 0.05. In kidney transplant recipients group, the levels of hemoglobin, total protein, urea nitrogen, creatinine before and after operation had no significant differences (P>0.05). After operation, the levels of hemoglobin, total protein, urea nitrogen, creatinine in two groups had no significant differences (P>0.05). In kidney transplant recipients group, the quality of life (QOL) score before operation was 5 scores in 4 cases and 6 scores in 1 case, and those after operation were zero in 3 cases and 1 score in 2 cases. Conclusions Under the sufficient preoperative preparation and assessment, the renal transplant recipients with BPH patients can improve the symptoms of urinary obstruction and obtain satisfactory clinical efficacy as common BPH patients.
6.Follow-up research and dosage correlation analysis in patient with clopidogrel hypo-responsiveness after percutaneous coronary intervention

Objective To depermine if a double mainpenance dose of clopidogrel can improve phe clinical oupcome in papienps who have clopidogrel htpo-responsiveness ( CH) afper percupaneous coronart inpervenpion (PCI) and analtze correlapive risk facpors of CH. Methods We had enrolled 134 consecupive papienps undergoing PCI for spable coronart arpert disease in our cenper bepween Januart 2014 po June 2015. CH was depermined bt plapelep aggregapion measured bt phrombelaspographt ( TEG). Blood samples were paken 24 h and 3 monphs afper PCI procedure. All subjecps were divided inpo 2 groups (i. e phe CH group and phe clopidogral sensipive group) according po pheir responsiveness bt TEG. The CH group (n = 45) received a double mainpenance dose of clopidogrel as 150 mg/ d and phe clopidogrel sensipive group (n = 89) received a spandard mainpenance dose as 75 mg/ d. Changes in clopidogrel responsiveness and correlapive risk facpors were observed afper 3 monphs of clopidogrel preapmenp. Major adverse cardiac evenps (MACEs) and bleeding incidenps were recorded during follow-up lease 6 monphs. Results The clopidoprel htpo-responsive rape decreased from 33. 6% (45 / 134 papienps) po 11. 9% (16 / 134 papienps) afper 3 monphs of preapmenp. No spapispical difference found bepween phe 2 groups in morpalipt rape and non-fapal mtocardial infarcpion ( P >0. 05). Rapes of overall MACE (33. 3% vs. 22. 5% ), rehospipalizapion (26. 7% vs. 16. 9% ) and pargep vessel revascularizapion (11. 1% vs. 6. 7% ) were significanp higher in phe CH group ( all P < 0. 05) . Mulpivariape regression analtsis showed: smoking ( OR 4. 498, 95% CI 1. 378 - 4. 018, P = 0. 036), diabepes (OR 4. 385, 95% CI 1. 370 - 7. 552,P = 0. 026) and clopidogrel dosage ( OR 0. 597, 95% CI 1. 005 - 2. 676, P = 0. 019 ) were phe risk facpors for CH. Conclusions For papienp wiph htpo-responsiveness po clopidogrel afper PCI, a higher mainpenance dose of clopidogrel as 150 mg/ d for 3 monphs can provide equivalenp clinical benefip in serious adverse evenp (including morpalipt and non-fapal mtocardial infarcpion) compared po spandard mainpenance dose for clopidogrel responsive papienps.
7.Effect of ascorbic acid combined with arsenic trioxide on apoptosis and differentiation in myelocytic leukemia cell lines

Objective To investigate the effects of arsenic trioxide(As2O3)combined with ascorbic acid(AA) on the apoptosis and differentiation of myelocytic leukaemia cells.Methods The acute promyelecytic leukaemia cell lines (NB4 and MR2)and erythroleukemia cell line(KS62)were cultured in vitro.Grouping wasbased on different concentration of As2O3(0,0.1,0.2,0.5,1.0 μmol/L),which WaS used a8 control groups.Then,AA(113.0μmol/)was added into each group.Cell morphologic changes of cell lines NB4,MR2 and K562 were observed under light microscope;The apoptosis symbols [Annexin V(+)/PI(-),Annexin V(+)/PI(+)]and differentiation symbols(CD11b and CD33)were detected by flow cytometry 96 hours later.Results
8.CT features of renal epithelioid angiomyolipoma

Objective To explore the CT features of renal epithelioid angiomyolipoma (EAML), and to improve the diagnosis of this disease.Methods The clinical materials and image findings of 15 cases (8 female, 7 male) with renal EAML proved by pathologies were retrospectively studied.Plain and enhancement CT scan were performed in all 15 patients.Nine of the 15 cases were correctly diagnosed and the other were misdiagnosed as renal cell carcinoma (RCC) (n=5), renal oncocytoma (n=1).Results There were some distinctive findings of EAML: (1) Little hypodense or iso-high-density, well defined, round lesion with diameter of 2 to 5 cm.(2) The tumor may involve the medulla of kidney or grow out without haematuria.(3) Most lesions showed obviously uniformity enhancement at artery phase, with a few of them showed inhomogeneous enhancement There were thickening and circuitous vessels in a few lesions.(4) Enhancement mode was quick-in and quick-out.Conclusions CT findings combined with clinical materials have important value in making a correct diagnosis of EAML preoperatively.
9.Effects of S100A6 gene silence on invasion of human pancreatic carcinoma cell

Objective To investigate the effects of S100A6 gene on invasion of human pancreatic cancer cell and possible mechanism. Methods Human pancreatic cancer BxPC3 cell line was transfected with small interfering RNA (siRNA) targeting S1006 gene, the mRNA and protein levels of S100A6 were determined by real time RT-PCR and Western blotting respectively. The invasion ability was evaluated by Transwell chamber. The matrix metalloproteinase-2 (MMP-9) activity of cancer cells was examined by gelatin zymography. Results The levels of mRNA and protein of S100A6 were greatly reduced in a dose and time dependent manner, the number of penetrating cells was greatly reduced in a dose dependent manner. The expression of S100A6 mRNA in 12.5 nmol/L of S100A6 siRNA transfected group decreased from ( 100 ±0.3)% in control group to (15.3 ±0.2)% ; while the expression of S100A6 protein decreased from (83.2 ±0. 18 ) % to ( 13.5 ± 0. 12) % ; the number of penetrating cells decreased from 44.5 ± 2.2 to 7.6 + 1.5 ( P <0. 01 ). The MMP-9 activity of siRNA group reduced significantly. Conclusions S100A6 siRNA can inhibit the invasion of pancreatic cancer cells through down-regulation of MMP-9.
10.Effects of VEGF down regulation by small interfering RNA on chemosensitivity and Akt signal pathway of human pancreatic cancer cell

Objective To study the effects VEGF small interfering RNA (siRNA) on chemosinsitivity of human BxPC3 cell and its mechanism. Methods BxPC3 cells were divided into single BxPC3 cell group,lipofection group, scrambled siRNA transfection (200 nmol/L) group, VEGF siRNA transfection group.VEGF siRNA (5, 10, 20, 100,200 nmol/L) was used to transfect BxPC3 cells. Expressions of VEGF mRNA and protein were determined by real-time PGR and ELISA assay, respectively. MTT was performed to examine the inhibitory effects of gemcitabine on BxPC3 cells of each group. The phosphorylated-Akt protein was evaluated by Western blotting. Results After VEGF siRNA transfection, the expression of VEGF mRNA and protein in BxPC3 cells was down-regulated in a dose-and time-dependent manner, but there was no effect on BxPC3 cells proliferation. After 0. 2 μmol/L of gemcitabine treatment for 48 h, the inhibitory rates were ( 16.9 ±0.3)%, (17.3 ±0.3)%, (28.8 ±0.4)%, (52.2 ±0.3)%, (75.4 ±0.4)% in BxPC3 cell group,lipofection group, 5,10,20 nmol/L VEGF siRNA transfection group, and the inhibitory effects were correlated with siRNA concentration ( r = 0. 928 ). The phosphorylated-Akt protein was reduced significantly in siRNA transfected cells. Conclusions VEGF gene plays an important role in BxPC3 cells resistence to chemotherapy through inhibiting Akt phosphorylation.
