1.Post-operative oral care procedure for patients with oral cavity cancer

Objective To establish an effective post-operative oral care procedure for patients with oral cavity cancer.Methods Eighty five post-operational patients with oral cavity cancer admitted to Sichuan Cancer Hospital during March 2011 and March 2013 were randomly divided into control group (42cases) and study group (43 cases).Patients in control group were given routine oral care,and those in study group received a special care procedure:normal saline washing + dioxogen cleaning + normal saline washing + normal saline gargle.Oral cleanliness,oral and lung infections were observed in both groups,and SPSS 17.0 was used for statistical analysis.Results Oral cleanliness,rates of oral infection and lung infection at d3 after operation,time of infections,the delayed wound healing and operating comfort were improved in the study group (Z/t/x2 =-2.48,4.57,4.64,2.36,2.25,-2.51,4.18,respectively; all P < 0.05).Conclusion The oral care procedure containing normal saline washing + dioxogen cleaning +normal saline washing + normal saline gargle can improve oral cleanliness early after operation,reduce complications and increase the operating comfort.
2.Analysis of Amino Acids and Acylcarnitines in the Blood of Patients First Diagnosed with Uremia Using Tandem Mass Spectrometry

Objective Analyze the amino acids and acylcarnitines in the blood of patients first diagnosed with uremia using tandem mass spectrometry in order to provide a basis for clinical diagnosis and treatment. Methods 30 patients with uremia were selected as the research objects,while 15 cases of healthy person were served as the control. The values of 11 kinds of amino acids and 2 kinds of acylcarnitines in the two groups were detected using tandem mass spectrometry,and the data were analyzed and compared. Results The levels of alanine(Ala)and tyrosine(Tyr)in uremia group were significantly lower than those in control group(P < 0.05),while the levels of citrulline(Cit),glycine(Gly)and proline(Pro)were significantly higher in uremia group than in control group (P < 0.05). The differences were mainly in non-essential amino acids between the two groups. The levels of free carnitines and total carnitines in uremia group were significantly higher than those in normal group(P < 0.05). Conclusion For the patients first diagnosed with uremia(not treated with replacement therapy),the disturbance of amino acid metabolism mainly in non-essential amino acids were found in their blood,and the carnitine levels were higher than normal levels. The results could provide the basis for clinical nutrition therapy for patients with uremia.
3.Incidence and risk factors of the causes of death among elderly hypertensive inpatients.

OBJECTIVETo retrospectively analyze the causes of death in elderly patients with hypertension in a hospital-based population from 1993 to 2012.
METHODSDuring the study period of over 19 years, a total of 2866 cases of death in 25238 hospitalized hypertensive patients with the age of 60 years or older were documented. Age, gender, complications, cause of death and other relevant variables were collected. All patients were divided into different subgroups according to gender, age or hypertension stage and risk stratification. The mortality of elderly hypertensive patients was analyzed using chi-square test.
RESULTS(1) Target organ damage (TOD) associated with hypertension was present in a substantial proportion of elderly patients. The complications related to death were heart disease (45.15%), stroke (34.37%), renal failure (11.88%), infective disease (4.58%), and cancer (4.06%). (2) Mortality in male elderly hypertension was higher than in women (53.31% vs 46.69%). The percentage of deaths from heart disease and stroke were higher in men than those in women (heart disease: 46.73% vs 43.35%; stroke: 37.04% vs 31.32%). (3) Age-specific constituent ratio of cause of death showed that deaths from stroke were significantly lower in very old patients (> or = 90 years) than in patients with 60-79 years of age (P < 0.01). In addition, deaths from heart disease, renal failure and infection disease were significantly lower in patients with more than 90 years than other patients. Deaths from cancer were highest in patients with 70-79 years of age (P < 0.01). (4) When compared with patients at stage 1 and 2 hypertension, subjects at stage 3 were more likely to die from stroke (P < 0.01) and renal failure (P < 0.05), while less likely to die from heart disease and cancer (P < 0.01). Patients in high and very high risk stratification of hypertension, compared with subjects in low and medium risk were likely to die from renal failure (P < 0.01) whereas less likely to die from heart disease (P < 0.05) and stroke (P < 0.01).
CONCLUSIONPrevalence of complication and TOD is high in elderly hypertensive inpatients, especially in deaths. The male patients and 60- 79-year-old patients have a higher percentage of causes of death. The stage and risk stratification of hypertension are associated with constituent ratios of the causes of death.

4.Fundus fluorescein angiographic features of patients with age-related macular degeneration

AIM: To explore the fundus fluorescein angiographic characteristics and relevant clinical significance of age-related macular degeneration(AMD).METHODS: Fundus fluorescein angiography (FFA) was performed on 149 eyes of 112 patients using Nikon NF-505 fundus camera.RESULTS: Out of 149 eyes, 90 eyes were atrophic AMD (60.4%), 59 eyes were exudative AMD (39.6%) which were further divided, according to the composition and location of lesion, into subfoveal choroidal neovasculari-zation (CNV)(7 eyes of classic type, 26 eyes of occult type, 9 eyes with disciform cicatrices, juxtafoveal CNV(2 eyes of classic type, 12 eyes of occult type), and extrafoveal CNV(3 eyes of occult type).CONCLUSION: FFA can show CNV of AMD patients and its quality and location, which is helpful to guide the treatment and evaluate the prognosis.
5.The experimental study of slow-release microcapsules of hepatocyte growth factor on angiogenesis in infracted rabbit myocardium

Objective To evaluate the effect of slow-release microcapsules of HCF( hepatocyte growth factor) on angiogenesis in infracted myocardium.Method Myocardial infarction was induced in 30 New Zealand rabbits by ligating the middle of left descending coronary artery. Group Ⅰ ( n = 10) was served as a control group, group Ⅱ ( n =10) as a blank microcapsule group, group Ⅲ ( n = 10) as experimental group with each microcapsule contains 1 μgHGF as HCF group. In group Ⅱ andⅢ, 5 blank microcapsules or FGF slow-release microcapsules were implanted into myocardium under epicedium between the left descending coronary artery and left circumflex branch. The heart function of each rabbit was evaluated with echocar-diography and cheterization, angiogenesis was evaluated by immunohistochemical technique 6 weeks later.Result As compared with group Ⅰ and Ⅱ , rabbits treated with HGF had higher microvessel counts ( P < 0. 01), and LVFS and EF were significantly increased [ (101. 28±19. 50,105. 28 ±18. 28,161. 28 ±15. 85, P <0.01 ]. Conclusion Subepicardial implantation of HGF slow release microcapsule in the infracted rabbit model can enhance effective angiogenesis and improve left ventricular function.
6.Study on pharmacokinetics of scutellarin in rabbits.

AIMTo establish a solid phase extraction-high performance liquid chromatographic (SPE-HPLC) method for determining plasma scutellarin concentration, and study its pharmacokinetics after i.v. breviscapine in rabbits.
METHODSMethanol-water-phosphoric acid (50:50:0.5) mixture was used as mobile phase, Nucleosil C18 column (250 mm x 4.6 mm ID) was selected. The wavelength of UV detection was 335 nm. Fifteen rabbits, randomized into 3 groups, were given breviscapine i.v. at the dose of 10, 20 and 40 mg.kg-1. Scutellarin in plasma was determined by SPE-HPLC method.
RESULTSLinearity was obtained over the range of 0.02-10.0 mg.L-1 of scutellarin. The method recovery was 96.15%-99.31%; the within-day and between-day RSDs were all below 10%. After i.v. 10, 20 and 40 mg.kg-1 of breviscapine to rabbits, the concentration-time curve of scutellarin fitted to a three compartment model. The main pharmacokinetic parameters showed no significant difference between low and medium doses, but the difference was significant between high dose and other doses.
CONCLUSIONThis assay method was accurate, sensitive, simple and suitable for the measurement of plasma scutellarin concentration. The pharmacokinetic characteristics were found to fit a three-compartment model following i.v. injection of breviscapine to rabbits. The changes of drug concentration in vivo exhibited linear kinetics ove the dosage range of 10-20 mg.kg-1, but when the dosage was 40 mg.kg-1, the linear kinetic properties disappeared.

7.Compliance guarantee in acupuncture clinical research.

Through review of the major databases in English and Chinese, it is found that there is an absence of articles on special coverage of compliance of research fellows and testees in acupuncture clinical trials. Therefore, the author proposed (1) The compliance of research fellows should be guaranteed through proper work division with reasonable, standard and strict research designation as well as comprehensive systems of selection, supervision and estimation. (2) The compliance of testees should be guaranteed through enhancing of the right to know, improving of the medical environment and constructing a better doctor-patient relationship. Therefore, the compliance of clinical trials can be improved, the quality of clinical research enhanced and the reliability of the research results can be guaranteed.

8.Application of vascularized free tissue transplantation in emergency repair of traumatized limbs:report of 86 cases

Objective To report the outcome of emergency repair traumatized limbs by vascularized free tissue transplantation.Methods From April 1988 to August 2004,86 patients,58 men and 28 women,had undergone emergency vascularized free tissue transplantation to have their injured limbs repaired in 54 cases and the missing thumbs reconstructed in 32.The patients aged from 5 to 55 (mean 27.9) years. The transplants included latissimus dorsi myocutaneons flap,anterolateral femoral skin flap,medial crural skin flaps,dorsal pedal flaps,medial plantar flap,composite tissue mass of the discarded limbs and big toe skin- nail flap.The operations were performed 1 to 5 days after injuries.Results Postoperative vascular crises occurred in 8 cases and were all followed by exploration with successes in 5 cases while failure in 3.The total survival rate of the transplants was 96.5% (83/86).In this series all the patients were followed up for 1 to 16 years with a mean of 7.5 years only to reveal satisfying functional recovery in all the repaired limbs and an ex- cellent and good rate of 87.5% in the reconstructed thumbs.Conclusion Emergency vascularized free tis- sue transplantation is an effective way to repair a traumatized limb and to reconstruct a traumatically missing thumb.
9.Application of forward recovery concept on swallowing training in patients with partial laryngectomy above cricoid

Objective To explore the application effects and safety of forward recovery concept on swallowing training in patients with partial laryngectomy above cricoid.Methods The study investigated 60 patients with partial laryngectomy above cricoid,and they were divided into intervention group (n =30) and control group (n =30) using block randomization.The patients of control group started the rehabilitation training after seven days of the operation as usual;the patients of intervention group received forward recovery during the whole perioperative period including psychological intervention,theoretical training,simulation training,and practical training.Swallowing function,compliance,incision pain,incidence of pneumonia,and extubation rate were compared between the two groups.Results The patients of intervention group had better swallowing function after eight weeks of the operation (Z =-2.202,P < 0.05),much more extubation case (x2 =9.017,P < 0.05),better compliance after 1,4,8 weeks of the operation (P < 0.05).The incision pain and incidence of pneumonia had no statistical significance (P > 0.05).Conclusions The forward recovery can significantly improve the compliance and the recovery of swallowing function,but has no effects on incision pain or incidence of pneumonia among the patients with partial laryngectomy above cricoid.
