1.Diagnosis and treatment of extranodal NK/T-cell lymphoma, nasal type
Chinese Journal of Clinical Oncology 2016;43(14):603-606
Extranodal natural killer (NK)/T-cell lymphoma, nasal type (ENKL) is an uncommon subtype of non-Hodgkin's lymphoma, which is aggressive and has poor prognosis. ENKL occurs predominantly in the nasal cavity and less frequently in the skin and in the gastrointestinal tract. The pathology of this disease is characterized by vascular invasion and destruction. ENKL is strongly associated with the Epstein–Barr virus, which aids in the diagnosis of ENKL. The characteristic markers on the surface of an NK/T cell and the spe-cific genetic variations are also useful in the diagnosis. At present, the treatment of ENKL is still under discussion. Although radiothera-py combined with chemotherapy on patients with early-stage ENKL and L-ASP-based chemotherapy and hematopoietic stem cell trans-plant on patients with advance-stage ENKL have achieved favorable effects, further studies are still necessary to develop the principles and methods of a standardized treatment.

G protein-coupled bile acid receptor 1(TGR5) is a specific membrane receptor of bile acids , playing an important role in the bile acid signaling network .Its activation has been proved to increase the glycemic control , regulate of blood lipid balance , enhance energy expenditure , exert anti-inflammatory actions and so on .It suggests that TGR5 may play an important role in metabolic diseases .

Journal of Practical Stomatology 1995;0(04):-
Objective: To intimately assess the internal stress distribution in ceramic veneer bonding layer. Methods:Six buccolingual cross section models of the incisor restored with IPS-Empress ceramic veneer were built with the MSC/NASTRAN software. 2D-finite element method was used to analyze the stress distribution regularity of the bonding layer on the models under different functional load. Results: Stress concentration areas of ceramic veneer bonding layer were regularly located on the incisal margin and cervical margin. Under the same condition, incisal load caused highest stresse on the adhesive layer. Following the load shift from incisal to gingival area, the stress was gradually decreased. When the functional load acted on the palate margin of the restoration, the compressive stress on the bonding layer was significantly increased. Different load angle generated stress change in the bonding layer. The shear stress on adhesive layer increased following the horizontal load angle increasing close to the debonding level. Among the different incisal reduction designs there was no significant Von Mises stress variation under the same load condition. However, when the incisal edge was reduced by more than 4 mm, the tensile stresses in the bonding layer greatly increased. Conclusion: Restoration margin should be kept away from the tooth contact area, be exposed to minimal occlusion contact. The functional load should be induced to follow the tooth long axis. Edge to edge occlusion should be avoided. When incisal areas need to be altered, the incisal overlap or butt margin design will not cause any stress problem in the bonding layer. When restoring the incisal defect for more than 4 mm, the bonding agents must be selected carefully.

Chinese Medical Equipment Journal 1989;0(03):-
The exposure of lead in environment has caused a major public health problem all over the globe.Because of rapid industrialization and constant existence of lead in environment,it is necessary to screen, monitor and intervene in the content of lead of human body during a long time. The toxical and harmful influence of lead on human body, especially on children are expounded. The content index of lead in the body set by international and national standards as well as its evaluation basis are included. The most frequently used testing method of blood lead is also described: atomic absorption spectrometry and its operative process.

Objective To investigate experience in nursing the patients with emergency traumatic injury combined with postoperative atelectasis. Method The clinical data of 11 patients with emergency traumatic injury combined with postoperative atelectasis were reviewed for summarizing the nursing experience. Result The clinical symptoms of all the 11 patients disappeared and the lungs reexpanded. Conclusion Careful observation of the disease conditions in order to prevent and treat atelectasis by airway humidification, sputum drainage and early exercises are effective for the cure of postoperative atelectasis.

Objective To investigate the effects of rhubarb and mirabilite and compound danshen injection on gastrointestinal dysmotility of severe acute pancreatitis (SAP). Methods Forty-one SAP patients were randomly divided into two groups: Integration group (n=21) were treated with rhubarb and mirabilite and compound danshen injection. Control group (n=20) were treated with conventional therapy. Recovery of gastrointestinal motility of two groups was observed, and the complication rate and hospital stay was recorded. Results The recovery of gastrointestinal motility, first defecation and hospital stay in integration group was (3.1±0.8)d, (3.1±0.8)d, (21.5±2.8 )d, which were significantly shorter than those in control group [(5.2±1.4) d, (4.7±1.3 ) d, (32.1±3.60) d, P < 0. 05 or P < 0.01. Five patients in integration group developed complication, which was significantly lower than 8 patients in control group ( P < 0.01 )]. One patient died and one patient received operation in integration group, and one patient died in control group, without significant difference between the two groups (P > 0.05 ). Conclusions The combination of rhubarb and mirabilite and compound danshen injection in severe acute pancreatitis can promote the recovery of gastrointestinal motility.

Objective To report the treatment outcomes and repair methods for soft tissue defects at thumb using miniature flaps. Methods Six different kinds of surgical flaps were used for the repair on different thumb injuries. They were radial lateral thumb reverse island flap, dorsal ulnar thumb revers flap, first metacarpal dorsal reverse flap, first digital dorsal island flap, neurovessel pedicled V-Y advanced island flap, free nail flap first foot digital, and so on. The emergency repair using miniature flaps were studied on 132 different cases, all 137 flaps. Results The flaps survived expect 5 flaps had partial epidermis necorsis in the 137 flaps. But the 5 flaps survived too after disinfecting and changing dressing. The appearances and functions of thumbs were satisfactory for the 1 to 2 years post-operation follow-up. Conclusion It will gain better results if choosing appropriate emergency repair methods using miniature flaps on different cases.

Chinese Journal of Emergency Medicine 2016;25(5):605-609
Objective To systematically review the efficacy of hypotensive resuscitation for traumatic-hemorrhagic shock. Methods Randomized controlled trails (RCTs) or quasi-Randomized controlled trails (qRCTs) were searched in Pubmed,Embase and the Corchrane Library from inception to August 2015.Two reviewers respectively picked out the useful data and performed quality evaluation.Metaanalysis was carried out with RevMan 5.3 software, risk ratio (RR) and its 95% confidence interval (CI) were pooled to estimate the enumeration data, and GRADE 3.6.1 software was used to rate the level of evidence. Results The results of meta-analysis and GRADE rating system which included 4 studies showed that:compared with conventional resuscitation, hypotensive resuscitation was associated with lower total mortality [RR =0.77,95% CI:0.62-0.95,P =0.01;n =984,GRADE rating:moderate],and 24-hour mortality [RR =0.47,95% CI:0.24-0.91,P =0.03;n =281,GRADE rating:moderate],but the subgroup analysis of total mortality showed that there were no significant differences in mortality between the subgroup of blunt or penetrating trauma and the subgroup of penetrating trauma. Conclusions Hypotensive resuscitation reduced total mortality and 24-hour mortality, and the quality of the evidence was moderate. The future studies should do further research to explore the efficacy of hypotensive resuscitation for different types of trauma.
