Objective:To investigate the capability of corneal cells to stimulate lymphocyte proliferation and the expression of costimulatory molecules on primary human corneal epithelial cells.Methods:Human corneal epithelial cells were cocultivated with peripheral blood mononuclear cells(PBMC). Expression of HLA class Ⅱ antigens, CD40, CD154, CD80 and CD86 was measured by immunohistochemical staining,and the expression of such molecules above on rejected corneal grafts was contrastingly assessed. Activation of lymphocytes was determined by measuring upregulation of CD69 by FACS analysis.Results:HLAⅡ expression was detectable on human primary corneal epithelial cells,and furthermore, upregualtion of costimulatory molecules CD40 and CD80 were also found after induction with ?-interferon. Both of HLA and CD40 were expressed on rejected corneal grafts. T lymphocytes were activated by epithelial cells in the coculture system.Conclusion:Human corneal epithelial cells are involved in rejection process of the corneal transplantation. The immunological reaction is triggered by epithelial cells,and endothelial cells suffer as the targets.
3.Clone of S gene in occult hepatitis B virus infection and construction of prokaryotic expression vector

Objective To construct the clone vector of S gene in occult hepatitis B virus infection.To analyse its mutations and to construct its prokaryotic expression vector.Methods To amplify the HBV S gene with polymerase chain reaction(PCR),recombinate the S gene and vector PUC18,sub-clone the recombination and construct its prokaryotic expression vector with pThiohis B. Results There were two samples negative with positive HBV DNA from 64 samples with negative hepatitis B surface antigen.The clone vector and prokaryotic fusion protein expression vector of an occult HBV infection′s S gene were successfully constructed.The S gene was not changed by sequencing analysis.Conclusion The fluorescence quantitive polymerase chain reaction (FQ-PCR) can examine low-copies in occult hepatitis B virus infection.Determining quantitivly HBV DNA of recombination can identify the recombination and display multiplication state intuitirely.It is very successful in constructing the clone vector and prokaryotic fusion protein expression vector.
4.Human retinal pigment epithelial cells and T-lymphocyte activation

Objective To investigate the expression of Human leucocyte antigen(HLA)-DP,-DQ,-DR and CD40 in human retinal pigment epithelial(RPE)cells,to determine their molecule expression in immune response process,and their abilities to stimulate T lymphocyte activation. Methods Human RPE cells were cultured with or without(-IFN respectively.Expression of HLA-DP,-DQ,-DR and CD40 was measured by immunohistochemical staining.Meanwhile,peripheral blood mononuelear cells(PBMC)were co-cultured with RPE cells in vitro,and then the expression of activated lymphocytes CD69 was measured by fluorescence activated eell sorter(FACS). Results Expression of HLA-DP,-DQ,-DR and CD40 antigen were enhanced by γ-interferon inducement.Increasing amount of CD69 positive Iymphocytes were found in the co-culture system of RPE cells and PBMC. Conclusion T-lymphocytes in the peripheral blood were activated by human RPE cells which is antigen presenting cells with immunological characteristics potential.
5.Analysis of clinical curative effects of hip replacement in the treatment of femoral neck fracture and femoral head necrosis in elderly patients

Objective To explore the clinical curative effects of hip replacement in the treatment of femoral neck fracture and femoral head necrosis in elderly patients and the existing problems.Methods A total of seventy-five aged patients with femoral neck fracture or femoral head necrosis who were treated by hip replacement were selected as study objects.They were divided into fracture group and femoral head necrosis group according to disease type,all the patients were treated with hip replacement.The surgical time,intraoperative blood transfusion volume,length of hospital stay,in-bed time,perioperative complications and postoperative complications were observed and recorded.The Harris score was used to evaluate hip function of preoperation and postoperation.Results All patients were followed up for 6 ~ 24 months,no incision infection and severe perioperative complications.The volume of intraoperative blood loss,operation time,hospitalization time,in-bed time of the fracture group were (334.9 ± 90.4) mL,(91.7 ± 14.3) min,(19.5 ± 7.5) d,(5.9 ± 1.6) d,respectvely,which of the femoral head necrosis group were (362.9 ± 71.5) mL,(93.5 ± 7.8) min,(20.5 ± 4.2) d,(6.3 ± 1.4) d,respectvely,the differences between the two groups were not statistically significant (all P > 0.05).At the end of the follow-up,the Harris scores of the two groups were higher than before operation,and the score of the fracture group (89.0 ± 7.4) points was significantly higher than (84.0 ± 9.2) points of the femoral head necrosis group (t =19.639,P < 0.05).In the fracture group,1 year after surgery,1 patient appeared delayed fracture healing,2 cases with postoperative pain when walking the affected area.In the femoral head necrosis group,1 case with femoral nerve injury,1 case of death due to its original diseases,1 case of postoperative sore pain when walking,the two groups had no prosthesis loosening or dislocation phenomenon.Conclusion Hip replacement in the treatment of femoral neck fracture and femoral head necrosis in elderly patients can be used as a clinical treatment option with short in-bed time,less intraoperative blood loss,short operation time,less perioperative complication and good treatment effect,and it is worth to be promoted.
6.Treatment and Control Status of Patients with Diabetes in Urban Communities in China

Objective To study the treatment and control status of patients with diabetes in urban communities in our country. Method structured questionnaire survey with physical examination were conducted by well-trained investigator, the participants were patient with diabetes diagnosed at least one year ago in four communities from four cities of China. Result 1047 adults with diabetes were investigated, predominantly type 2 diabetes. 50.9% of participants had hypertension, 32.7% of participants had hyperlipidemia. 70.8% of patients were treated with oral medication alone, 10.2% with insulin alone, 6.3% with oral medication plus insulin, and 9.8% without any medication. So, total about 16.5% of patients were treated with insulin. The well-performed proportion of medication, diet, physical activity, self-management education and blood sugar monitoring were 81.2%, 69.4%, 80.6%, 69.8% and 66.9%, respectively. Participants who well-practised all the five treatment strategies accounted for 34.9% of all participants. Out of all participants, 63% participants had poor FBG control and 50% had poor BP control. Conclusion Most patients with diabetes in urban community receive nonstandard treatment and most of them had poor FBG control and BP control.

Treacher Collins syndrome (TCS, OMIM 154500), also known as Franceschetti-Klein syndrome, is a rare disorder that affects the first and second branchial arches. The estimated incidence is 1/50 000 live births. Mutations in TCOF1 (78%-93%) and POLR1C or POLR1D (8%) cause the disease. Most of TCS cases are inherited in a dominant pattern, while a small proportion are inherited in a recessive pattern. TCS has a variable phenotype with typical clinical characteristics including downward-slant of palpebral fissure, malar hypoplasia, mandibular hypoplasia and microtia. TCS management is a multidisciplinary affair, as interventions range from reconstructive to psychosocial. For hearing rehabilitation, TCS patients may have the choices of BAHA, ponto, vibrant soundbridge or bonebridge implantation. In this review, we summarize the TCS clinical malformations, diagnosis, genetics, management and auditory rehabilitation.

8.Respiratory Tract Infection by Stenotrophomonas maltophilia:Analysis of 32 cases

OBJECTIVE To analyze the risk factors of respiratory tract infection by Stenotrophomonas maltophilia and its resistance to common antibiotics so as to guide clinical prevention and treatment.METHODS Thirty two patients of respiratory tract infection by S.maltophilia were examined.RESULTS Among them,the ratio of susceptivity of S.maltophilia to compound sulfamethoxazole was 87.5%,to levofloxacin was 78.1%,and to ticarcillin/clavulanic acid was 81.2%,but the ratio of resistance of S.maltophilia to imipenem was 100.0%,and it was also high to aminoglycosides and cephalosporins.CONCLUSIONS The old age,serious underlying diseases,many kinds of antibiotics used,infection in hospital and the use of glucocorticoids are the main risk factors of the respiratory tract infection by multiresistant S.maltophilia.
9.The Effects of Recovery-Pulse Decoction of Colla corii asini (Ejiao) on Platelets, Blood Coagulation and Iipid Peroxidation in Rats with Endotoxemia

The endotoxemia model was established by intraabdominal injection of endotoxin Coli B. (E. Coli O55B5,8 mg/kg weight)in albino rats. Two hours before administration of endotoxin, the ejiao recovery-pulse decoction was perfused into the stomach tube of the rats in the experimental group (10mg/kg weight), and 12h later, the platelets were counted, kaolin activated partial thromboplastin time (KPTT), prothrombin time (PT) and thrombin time (TT) were measured, and malondialdehyed (MDA) levels in plasma and platelet were determined by thiobarituric acid (TBA) fluorometry. The results demonstrate that the ejiao recovery-pulse decoction could prevent from peripheral platelet dropping, KPTT and TT prolonging and MDA level rising which were caused by endotoxin.
10.Comparative study of isoflurane and sevoflurane on the effects of anesthesias in abdominal surgeries with the close-loop anesthesia system

Objectives: To study the differences between sevoflurane and isoflurane in abdomen surgery anesthetized with the target-controlled close-loop anesthesia system.Methods: Forty patients (ASA I-II) were divided into sevoflurane group (n=25) and isoflurane group (n= 15).BIS value of 55 was the control variable index.The target-controlled infusion system of sevoflurane was administered for the maintenance of anesthesia in both groups after intravenous nduction. The BIS values,blood pressures,heart rates and doses of assistant drugs were recorded and analyzed.Results: No statistical difference was observed in BIS、HR and pulse pressure in two groups.
