1.Application analysis of antibiotics in the pediatric outpatient and emergency treatment in our hospital

Objective To analyze the usage of antibiotics after renovation in the use of this present situation and existing problems of pediatrics.Methods By application of our self-developed analyzing system for the clinical use of antibacterial agents,the prescriptions of pediatric outpatient in 2012 was studied retrospectively and their rationality was evaluated.Results The antibiotic prescription rate was 37.7% in pediatric outpatient and emergency in our hospital,their were 26 antibacterial agents in clinic use which were involved in 6 pharmacological group.The defined daily dose(DDD) analysis was adopted and the results have shown that macrolides (42.20%),cephalosporins (35.90%) and penicillins (20.90%) were the most prescribed antibiotics and most of them were oral medicines (72.2%) and non restrictive drug (61.0%).The antibacterial agents' s combination therapy rate was 2.4%.Irrational use of drugs mainly include the wrong way of administration (78.0%),application of antibiotics with no indications (21.3%) and irrational combined use of antibiotics (0.5%).Conclusion The application of antibiotics in our hospital pediatric basic structure reasonable,but there are still improper usage and dosage,no indication of medicine,etc.It is necessary to further improve and perfect the use and management of antimicrobial agents.
2.Effect of core stabilization exercises on knee osteoarthritis

Objective To investigate the effect of core stabilization exercises( CSE) on knee osteoarthritis. Methods Sixty cases patients with knee osteoarthritis were randomly divided into treatment group (n = 30)and control group(n = 30). Patients in control group were accepted conventional therapy including conventional physiotherapy,acupuncture,and massage,while in treatment group were accepted CSE besides conventional therapy. All patients were assessed with Lysholm Knee Scoring Scale(LKSS)and Visual Analogous Scale(VAS)before and after treatment. Results VAS in treatment group before and after treatment were(8. 31 ± 1. 16)and(3. 37 ± 0. 97),and the difference was statistically significant( t = 16. 30,P = 0. 00). VAS in two groups after treatment was statistically significant (t = - 6. 98,P = 0. 00). LKSS in treatment group before and after treatment were(32. 92 ± 4. 21),(60. 19 ± 4. 42),and the difference was statistically significant(t = - 30. 44,P = 0. 00). LKSS in control group before and after treatment was(34. 82 ± 5. 58),(53. 49 ± 3. 66),and the difference was statistically significance (t = - 14. 82,P = 0. 00). After the treatment,LKSS in the two groups was significant difference(t = 7. 16,P= 0. 00). Conclusion The core stability training can improve the patients with knee osteoarthritis of knee joint function,relieve pain.
3.Clinical Observations on Superficial Surround Needling plus Quadriceps Femoris Training for the Treatment of Knee Osteoarthritis

ObjectiveTo investigate the efficacy of superficial surround needling plus quadriceps femoris training in treating knee osteoarthritis and observe electromyographic amplitude variations by knee osteoarthritis.MethodNinety patients diagnosed with knee osteoarthritis were randomly allocated to treatment and control groups, 45 cases each. The treatment group received superficial surround needling plus quadriceps femoris training and the control group, acupuncture alone. Both groups were treated for two weeks. Pre-treatment and post-treatment knee joint functions were scored and compared using the Visual Analogue Scale (VAS), the Lysholm Knee Score Scale and the electromyographic integral value. Meanwhile, electromyography was performed and electromyographic amplitude variations by knee osteoarthritis were compared between the two groups of patients.ResultThe total efficacy rate was 88.9% in the treatment group and 60.0% in the control group. It was higher in the treatment group than in the control group (P<0.05). After treatment, the VAS score, the Lysholm score and the electromyographic integral value were significantly better in the treatment group than in the control group (P<0.05).ConclusionSuperficial surround needling plus quadriceps femoris training has a marked therapeutic effect on knee osteoarthritis.
4. Fingerprint Study and Primary Quality Evaluation of Niuhuang Qingwei Pills Based on UPLC and Reference Drug

OBJECTIVE: To apply reference drug in the fingerprint study of Niuhuang qingwei pills, and perform primary evaluation of the quality grade of the samples. METHODS: Ultra performance liquid chromatography (UPLC) separation was performed on an AQUITY UPLC BEH C18 column with gradient elution using acetonitrile (containing 0.5% formic acid)-0.5% formic acid at a flow rate of 0.2 mL•min-1. The injection volume was 2 μL and the column temperature was maintained at 40 ℃. The detection wavelength was set at 254 nm. By comparing with reference substances and the reference herbal materials, characteristic peaks and their ascriptions were investigated. Using Niuhuang qingwei pills reference drug as the accompanying physical control, the similarities of the fingerprints of 49 batches of samples from 18 manufacturers were calculated, and the quality grades were evaluated. RESULTS: The similarities of all samples fell within the range of 0.76-0.98. The similarities of 49 samples were above 0.75 and met the second-grade limit. The similarity of 24 samples were above 0.90 and met the first-grade limit. CONCLUSION: The method is simple, accurate and rapid. It can be used for the quality control and grade evaluation of Niuhuang qingwei pills, which provides reference for the quality grade research of Chinese patent medicines.
5.Full length genome sequencing and phylogenetic analysis of two strains of Getah virus

Abstract: Objective The main aim of the study is to sequence the complete genome of two Getah virus strains (GS11-155 and HNDZ1712-1) isolated in Gansu Province and Hainan Province in 2011 and 2017 respectively and analyze the molecular and genetic evolution of the two strains compared with M1, which was first isolated in 1964 in Hainan Province, China. Methods Genome of two newly isolated Getah viruses were sequenced by virus gene amplification technique, and the genomic database of Getah viruses was established. The molecular characteristics and genetic evolution of the viruses were analyzed by bioinformatics software. Results The genome length of two new isolated Getah virus strains (GS11-155 and HNDZ1712-1) was 11 690 nt and 11 621 nt, respectively. Both strains had the structural characteristics of Alphavirus genome. Although the nucleotide sequence lengths of structural genes, non-structural genes and non-coding junction regions of the two strains were identical, the nucleotide sequence lengths of the 5' and 3' non-coding regions of the viral genomes were a few different. The 3'UTR repeats elements in the genomes of the two virus strains did not change. It was 97.7% and 98.1% different of nucleotide and amino acid homology between both strains of Getah virus, HNDZ1712-1 isolated in 2017 and M1 isolated in 1964 in Hainan Province. Interesting, Gansu 2011 cluster and Hainan 2017 cluster were emerged leading by both strains GS11-155 and HNDZ1712-1 respectively, those two clusters totally independent with M1 virus isolated from Hainan in 1964 in whole genome phylogenetic analysis first. Conclusions Although the HNDZ1712-1 was also isolated from mosquito samples in Hainan Province, it was in a completely different evolutionary branch from the M1 isolated from Hainan Island in 1964, and was closely related to the strain isolated from Gansu Province (GS11-155) thousands of kilometers away. It is suggested that the two new strains of Getah virus are different from the Getah virus isolated in 1964.
6.Influence of human epithelial ovarian cancer HO-8910 cells with modified survivin gene on the cell cycle distribution and chemosensitivity

Objective To study the influence of survivin mutant-T34A ( survivinT34A) and survivin deletant-N-terminal 8 amino acids residues ( survivinN-8AA ) on the cell cycle distribution and chemosensitivity in human ovarian cancer HO-8910 cells for explorating the roles of modified survivin-mediated apoptosis induced by chemotherapeutic agents and possible signaling pathways involved. Methods pcDNA3.1 plasmid contained wild-type, survivinT34A and survivinN-8AA genes were transfected into HO-8910 cells,respectively, the control groups were HO-8910 cells transfected with pcDNA3. 1 plasmids. The expression of mRNA was examined by reverse transcription(RT) PCR and identified by DNA sequencing; the cell cycles were determined by flow cytometer analysis ( FCM ); the growth inhibitions rate of cisplatin ( DDP),paclitaxel (PTX) and LY294002 on the transfected cells were determined using methyl thiazolyl tetrazolium (MTT) assay. Results (1) The RT-PCR procedures and genome sequences showed that the survivin mRNA were expressed stable in the transfected HO-8910 cells. (2) There was lower percent of G0/G1 phase cells in SN-HO-8910 cells than that in PC-HO-8910 cells (44. 72% vs. 49.64%, P <0. 05) ;while higher percentage of G2/M phase and S phase cells( 1.06% and 54. 22% vs. 0. 56% and 49. 80%, P < 0. 05 ).There was lower the G2/M phase and S phase cells in M-HO-8910 cells 0. 16% and 36. 33%, than that in PC-HO-8910 cells( P < 0. 05 ); while higher percentage of G0/G1 phase cells(63. 51% ,P < 0. 05 ). G0/G1 ,G2/M and S phase cells in Sur-HO-8910 cells were 54. 46%, 0. 62% and 44. 92%, and there were not significantly difference ( P > 0. 05 ), compared to those in PC-HO-8910 cells. ( 3 ) The inhibitory concentration ( IC50 ) of DDP and PTX were higher in Sur-HO-8910 cells than those in control cells [(20. 4 ±6. 1)vs. (14.4 ±3.9)μmol/L,(36.7 ±4.0) vs. (28.6 ±3.6) μmol/L;all P<0.05]. The IC50 of DDP and LY294002 in SN-HO-8910 cells were lower than those in control cells[(7. 6 ± 1.0) vs. ( 14. 4 ± 3.9)μmol/L, ( 13.2 ± 4. 0) vs. (41.0 ± 7. 9 ) μmol/L; all P < 0. 01]. The IC50 of PTX [( 37. 9 ± 4. 8 ) μmol/L]in SN-HO-8910 cells were higher than that in control cells(P <0. 05). The IC50 of DDP in M-HO-8910 cells [(9.9 ± 1.2) μmol/L] were lower than that in control cells(P <0. 05) ,and the IC50 of LY294002 in M-HO-8910 cells [(66. 9 ± 4. 8) μ mol/L] higher than that in control cells ( P < 0. 01 ). Conclusions The changes of cells cycle distribution caused by survivinT34A or survivinN-8AA enhanced the G2/M cell cycle-dependent chemosensitivity of PTX. Compared to survivinT34A, survivinN-8AA preferentially to mediate the cytotoxicity of DDP and LY294002, suggesting that it may be related to the cell cycle-dependence of survivin function and to blockage of the formation of its active dimer.
7.Experimental study of the inhibitory effect of octreotide on transforming growth factor-alpha-induced proliferation of human hepatocellular carcinoma cells

Objective To investigate the inhibitory effect of octreotide on transforming growth factor-alpha (TGF-?)-induced cell proliferation of human hepatocellular carcinoma cells and its possible mechanism. Methods The effect of octreotide on TGF-?-induced cell proliferation of the liver cancer cells (LCC) was evaluated by immunohistochemistry method. The effect of octreotide on TGF-? secretion and epidermal growth factor receptor(EGFR) expression in the cells was determined by reverse-transcriptase polymerase chain reaction(RT-PCR). The effect of octreotide on extracellular signal-regulated protein kinase (ERK) expression in the cells was measured by Western-blot and immunohistochemistry method. Results The TGF-?-induced expressions of proliferating cell nuclear antigen (PCNA) in nucleus were obviously increased by TGF-?. TGF-?mRNA index of hepatocellular carcinoma cells was decreased by octreotide. Octreotide inhibited significantly the expressions of EGFR mRNA induced by TGF-?. Octreotide inhibited significantly the expressions of ERK protein induced by TGF-?. There was intense staining in the nucleus of the cells by TGF-? and weak staining in the nucleus of the cells treated simultaneously by octreotide and TGF-?.Conclusions Octreotide can inhibit the secretion of TGF-?, the expression of EGFR, and the signal transduction of EGFR of LCC, and consequently exerts an inhibitory effect on TGF-?-induced hepatocellular carcinoma cells proliferation.
8.Abnormal imaging manifestations of delayed graft function on susceptibility weighted imaging

Objective To explore the value of abnormal imaging findings on susceptibility weighted imaging (SWI) in delayed graft function (DGF). Methods The conventional MRI and SWI images of 26 cases with DGF and 20 cases with normal renal function of transplanted kidneys were retrospectively analyzed. Patients with cysts and angiomyolipomas were excluded. Normal structures of transplanted kidney were identified. If lesions of abnormal signal intensity were found in the transplanted kidney, the location, border and signal intensity compared to renal cortex would be analyzed. The differences in signal intensity between the abnormal signal lesions and normal renal cortex in the same SWI layer of DGF were compared by using independent-sample t test. The differences in positive detection rate of discovering the abnormal signal lesions in DGF between conventional MRI and SWI were compared by using McNemar test. Results Of the 26 cases with DGF, one case was found to exhibit abnormally low signal lesions with fuzzy boundary located at junctional zone between cortex and medulla on both conventional MRI and SWI images. Ten cases were found to exhibit abnormally low signal lesions with fuzzy boundary located at junctional zone between cortex and medulla on SWI images only. Fifteen cases exhibited no abnormal signal lesions on both conventional MRI and SWI images. Twenty cases with normal renal function of transplanted kidney, no abnormal signal lesions were found on both conventional MRI and SWI images. The differences in signal intensity between the abnormally low signal lesions (130±20) and normal renal cortex (177±25) in the same SWI layer of 11 cases with DGF were statistically significant (t=-4.582,P<0.01). The differences in positive detection rate of discovering the abnormally low signal lesions in DGF between conventional MRI [3.8%(1/26)] and SWI [42.3% (11/26)] were statistically significant (χ2=8.100,P=0.002). Conclusions Abnormally low signal lesions with fuzzy boundary located at junctional zone between cortex and medulla on SWI images suggest the presence of DGF. Compared with conventional MRI, SWI appears to be superior in detecting the abnormally low signal lesions.
9.Expression of Monocyte Chemoattractant Protein-1 and Vascular Endothelial Growth Factor mRNA in Rats with Acute Methanol Poisoning

Objective This study was aimed to investigate the expression of monocyte chemoattractant protein-1 (MCP-1) and vascular endothelial growth factor (VEGF) mRNA in rats with acute methanol poisoning. Methods Animal models of acute methanol poisoning in rats were duplicated using a plexiglas chamber exposed to a mixture of N2O/O2. Right atrium venous blood of rats were taken at each time point (2 h, 12 h, 24 h,3 d,1 w), and gas chromatography was used to determine the methanol concentration of the rat blood (n=5) . Then got brain tissue to extract total RNA and reverse transcription (n=3) . SYBRGreen real-time PCR was used to monitor the expression of MCP-1 and VEGF mRNA. Results (1) Results of methanol concentration determination:The blood methanol concentration of the low-dose group was significantly increased in comparison with that of the saline control group at 2 h and 12 h time points ( < 0.05) . In the high-dose group, the blood methanol concentration was increased significantly compared with the low-dose group as well as the saline group at 2 h, 12 h and 24 h time points ( < 0.05); (2) The expression of MCP-1 mRNA: The expression level of MCP-1 were significantly enhanced along with the time lapse after acute poisoning,and became most severely at 24 h. The expression levels of MCP-1 have significant differences in groups,of which high-dose group was higher than low-dose group at 2 h, 3 d and 1 w ( <0.05);(3) The expression of VEGF mRNA:The expression level of VEGF was significantly enhanced along with the time lapse after acute poisoning,and became most severely at 24 h. The expression levels of MCP-1 have significant differences in groups,of which high-dose group was higher than low-dose group at 2 h and 12 h ( <0.05) . Conclusion The expression levels of MCP-1 and VEGF mRNA were significantly enhanced, and the degree of poisoning was apparently related with the dose administered. MCP-1 and VEGF might play the important roles in the pathogenesis and progression of brain injured.
10.Human brucellosis:seroepidemiology of Tangshan City from 2003 to 2006

Objective To study the human brucellosis infection status and its epidemic feature during 2003-2006.Methods A total of 485 blood samples of serum underwent rose bengal plate test(RBPT)and serum agglutination test(SAT),which include 422 cases having a history of contacting with sheep/goats or cattle,22 suspected eases,and 41 cases having contacted with infected persons.All cases were diagnosed according serology test and epidemiologic survey.Results The positive rates of Serum antibodies(SAT titer)were 8.8%(12/137),12.7%(8/63),13.1%(22/171)and 16.2%(19/114),respectively,during 2003 to 2006 in Tangshan,in an increasing Rend and peaked in the second and the third seasons 77.0%(47/61).The mostly infected population were those who contacted with sheep/goats or cattle,accounting for 98.4%(60/61).Livestock farmers 86.9%(53/61),and veterinarians 8.2%(5/61)were mostly likely to be infected.Assay of serum antibodies is negative in the persons contacting with infected cases.Conclusions The prevalence of human brucellsis had presented a rising tendency in highly dangerous crowd of Tangshan.Contacting with infected livestock(such as sheep/goats or cattle),especially their aborted materials and placenta were mainly transmitted modes of human brucellsis.However,cases of person-to-person transmission has not been discovered in this study.
