1.Clinical efficacy of Yangxueqingnao granule improve cerebral small vessel disease cognitive impairment

Objective To explore the effect of Yangxueqingnao granule in treatment of cerebral small vessel disease (SVD)patients with mild cognitive dysfunction (MCI).Methods 52 patients with SVD related MCI were selected.They were treated with Yangxueqingnao granule,each 4.0 g,3 times a day,3 months for a course of treat-ment.Montreal cognitive scale (MoCA),simple mental state scale (MMSE)and event related potential (P300)were detected before and after treatment,and the clinical effect was observed.Results After a course of treatment by Yan-gxueqingnao granule,the cognitive function and memory of 52 patients were improved to some extent.In addition to the attention of an MoCA meter to a foreign project[Visual space:before taking the medicine (2.92 ±0.26)points,after taking the medicine (3.25 ±0.66)points;Named:before taking the medicine (2.26 ±0.70)points,after taking the medicine(2.92 ±0.49)points;Language:before taking the medicine (1.47 ±0.70)points,after taking the medicine (2.17 ±0.98)points;Abstraction:before taking the medicine (0.45 ±0.38)points,after taking the medicine (0.68 ±0.63)points;Delayed recall:before taking the medicine (1.67 ±0.74)points,after taking the medicine (2.52 ±1.50)points;Directional:before taking the medicine (4.73 ±0.35 )points,after taking the medicine (5.52 ±0.57)points ]and total score[Before taking the medicine (18.75 ±0.66)points,after taking the medicine (19.12 ±1.45)points],the differences were statistically significant (the total score:t =7.56,P =0.000;Visual space:t =5.86,P =0.002;Named:t =5.42,P =0.000;Be careful:t =1.23,P =0.121;Language:t =4.52,P =0.000;Abstraction:t =2.65,P =0.001;Delayed memory:t =7.96,P =0.000).While the total score of MMSE scale [before taking the medicine (25.36 ±1.89)points,after taking the medicine (28.53 ±2.91 )points],memory [before taking the medicine (2.64 ±0.42)points,after taking the medicine (2.75 ±0.53)points]and recall [before taking the medicine (1.52 ±0.48)points,after taking the medicine (1.98 ±0.78)points ],the differences were statistically significant(the total score:t =2.78,Memory:t =1.95,Recall:t =3.43,all P <0.05).P300 before and after treatment[before taking the medicine P300 latency of (389 ±21)ms,after taking the medicine P300 latency of (341 ±18)ms)],the difference was significant (t =12.514,P <0.001).Conclusion Yangxueqingnao granule had therapeutic effect in patients with SVD -MCI.

The research of esophageal target is a difficult spot in the precise radiotherapy and the target mobility has an important effect for the radiotherapy of esophageal cancer.By different breathing control techniques,controlling the patient's respiratory may narrow target mobility so as to improve the accuracy of radiotherapy.But the present studies of the movement range of esophageal cancer with the respiratory control technologies have not come to a consistent standard.The area shape and position change of esophageal cancer target is a key problem that needs to be solved in the esophageal precise radiotherapy.

As a transcriptional repressor,Bmi-1 is a member of polyeomb group gene family.It may play an important role in self-renewal ability and proliferation of stem cell.At present,studies show that Bmi-1 is highly expressed in a variety of solid tumors,which can be regarded as one of the markers of cancer stem cells and has a close relation to the cancer development and progression and radiation resistance.It will provide an important target gene for the treatment of malignant tumors.
4.Study on quality standard for Guangxiao Liniao Capsules

Objective: To establish the quality standard for Guangxiao Liniao Capsules (Herba Verbenae, Herba Leonuri, Rhizoma Alismatis, Radix Linderae, etc.). Methods: Herba Verbenae, Herba Leonuri, Rhizoma Alismatis, Radix Linderae were identified by TLC, and the content of stachydrine hydrochloride was determined by TLC scanning. Results: Herba Verbenae, Herba Leonuri, Rhizoma Alismatis, Radix Linderae could be identified by TLC. Stachydrine hydrochloride showed a good linear relationship at a range of 4?g~20?g, r = 0.9975. The average recovery was 97.9%, and RSD was 1.20%. Conclusion: The methods are accurate and can be used for the quality control of Guangxiao Liniao Capsules
5.Design of Voltage-clamp-controlled Current-clamp

Previous research rcvealed that distortion is detected in transient voltage signal recorded with traditional patch clamp amplifier under current clamp mode, which is essentially resulted by electronic design of the headstage of the patch clamp. A new kind of headstage is designed to modify the defect, the circuit of which not only measures the transient potentials as the classical voltage follower does but also is quite suitable for the standard voltage-clamp mode. Furthermore, the technique of voltage-clamp-controlled current-clamp is applied for modifying the conventional patch-clamp amplifier, the variable low-pass filter is added into the circuit to reduce the response speed of voltage-clamp module, thus the transient potentials changes can be measured while membrane potential is kept at a constant value. Bridge balance circuitry is designed to eliminate the voltage drop while the variable current injected into the electrode. And fast capacitance compensation stage of conventional PCA is modified to nentralize the capacitance and accelerate system response speed for current-clamp mode. The experirnental results on cell model demonstrate that modified PCA meets the rcquirement of monitoring transient potential changes in electrophysiology research.
6.Value of exercise stress test guiding aerobic exercise rehabilitation in patients with myocardial infarction

Objective: To explore value of exercise stress test guiding aerobic exercise rehabilitation in patients with myocardial infarction.Methods: A total of 102 patients with myocardial infarction in recovery were selected.According to random number table, they were randomly and equally divided into routine rehabilitation group (received routine rehabilitation guidance) and aerobic exercise rehabilitation group (received aerobic rehabilitation exercise in guidance of exercise stress test).Amount of exercise, hemodynamic parameters and psychological state were compared between two groups.Results: After treatment, amount of exercise: compared with routine rehabilitation group, there were significant rise in amount of exercise[(6.79±0.58)METs vs.(7.42±0.69)METs], maximum heart rate[(126.27±5.26) beats/min vs.(138.18±5.81) beats/min], heart rate blood pressure product[(221.87±21.46) vs.(244.85±23.09)]and exercise duration[(10.36±1.36) min vs.(11.18±1.45) min]in aerobic rehabilitation group, P<0.05 or <0.01;hemodynamic parameters: compared with routine rehabilitation group, there were significant rise in cardiac output[(5.36±0.57) L/min vs.(5.72±0.68) L/min], cardiac index[(2.96±0.53) L min-1 m-2 vs.(3.25±0.52) L min-1 m-2], stroke volume[(73.16±8.47) ml vs.(78.12±9.14) ml]in aerobic exercise rehabilitation group, P<0.05 all;psychological state: compared with routine rehabilitation group, there were significant reductions in scores of Hamilton rating scale for anxiety[HAMA, (9.74±3.26) scores vs.(6.35±1.18) scores]and Hamilton rating scale for depression[HAMD, (14.83±4.58) scores vs.(10.56±3.27) scores]in aerobic exercise rehabilitation group, P<0.05 both.Conclusion: Exercise stress test guiding aerobic exercise rehabilitation can help to improve exercise function and hemodynamic indexes, alleviate the negative mood, then promote patients recover.

8.Risk Factors Analysis on Acute Respiratory Dysfunction Caused Death in Patients After Type A Aortic Dissection Surgery

Objective: To analyze the risk factors on acute respiratory dysfunction caused death in patients after type A aortic dissection surgery.
Methods: A total of 223 patients who received aorta replacement surgery in our hospital from 2010-01 to 2012-12 were retrospectively studied. 80 patients suffered from post-operative acute respiratory dysfunction including 61 male and 19 female with the mean age of (49.2 ± 11.6) years. Those patients were divided into 2 groups as Death group, n=18 and Survival group, n=62. We analyzed the most relevant risk factors for death, such as gender, age, histories of smoking, diabetes, hypertension, Marfan syndrome;pre-operative acute or chronic dissection, hypoxemia, mal-perfusion, LVEDD and LVEF;CPB time, aortic-clamping time;post-operative ICU retention time, mechanical ventilation time, permanent neurologic dysfunction, pulmonary infection, MACE, renal failure, hypohepatia, septicemia and wound mal-healing, et al.
Results: The early post-operative (< 3 days) respiratory dysfunction rate was 35.8% and the mortality was 22.5%(18/80). The relevant risk factors of death included female gender (P=0.019), haemorrhage (P<0.01), mechanical ventilation time (P=0.011), permanent neurologic dysfunction (P=0.013), pulmonary infection (P=0.001), MACE (P=0.022), renal failure (P<0.01), hypohepatia (P<0.01) and septicemia (P=0.001). Female gender and renal failure were the independent risk factors for respiratory dysfunction caused death in patients after type A aortic dissection surgery.
Conclusion: The occurrence and mortality were high in patients after type A aortic dissection surgery especially in those with female gender and post-operative renal failure.
9.Clinical significance of anatomic observation of the nasal alar cartilage

BACKGROUND: Nasal alar cartilage constitutes the main component of the lower 1/3 of the nose, that is, nose tip, nose wing, and nasal columella, its structure has a decisive role on the nose shape, especially the tip of the nose shape. The intensive study on nasal alar cartilage will help deepen our understandings of nasal alar cartilage morphology, structure and function, and help clinicians to correctly handle the lesions of nose and the lower part and to carry out medical beauty. OBJECTIVE: By observing external nasal anatomy, to clarify the histological role of nasal alar cartilage on nose shape, especially the nasal tip shape.DESIGN, TIME AND SETTING: Experimental observation of repeated measurement was conducted at the Laboratory of Anatomy, Second Military Medical University of Chinese PLA from September 1~(st) to 26~(th), 2006.MATERIALS: Well-preserved bodies of 15 fresh adult, containing 10 males and 5 females were used in this study.METHODS: To fully observe the fine structure of external nose, 30 sides of 15 external noses were dissected, and autopsy started from the medium dorsum of nose, layered anatomy, to observe various layers and the characteristics of the layers with blood vessels, focusing on observation of in vitro pre - and free post-nasal alar cartilage morphology, and measurement and recording were performed.MAIN OUTCOME MEASURES: Big foot medial alar cartilage, lateral feet and the angle of inside and outside legs were measured.RESULTS: Alar cartilage was open for a pair of backward "u"-shaped thin cartilage plate, and lateral nasal cartilage was located below and anteriomedialis the nose, was composed of medial and lateral crus and fornix, with thin body shape, unfixed structure. The shape of fornix was difficult to accurately describe; most presented wavy or folded. Lateral crus presented diamond-shaped or long strip, (16.21±2.71) mm in length, (8.45±1.72) mm in width, (1.09±0.18) mm in thickness. Cephalic rim intersected lower edge of lateral nasal cartilage, and slightly covered the lower edge of the lateral nasal cartilage, so that the two were overlapped, but also only the intersection without overlapping. Lateral crus constituted the base of nasal wings. Narrow medial crus formed nasal tip and the frame of front nasal columeila, showing posteroinferior curve or S shape, (13.06±2.16) mm in length, (3.79±0.58) mm in width, (1.02±0.18) mm in thickness. The left and right medial crus in the middle were connected by connective tissue, and in the same way connected to the anterior margin of the lateral nasal cartilage. Medial and lateral crus in the nasal tip showed an acute angle intersection, its angle (75.25±11.17)°. The medial and lateral crus intersected in the nasal tip and formed the fomix of the greater alar cartilage. The bilateral cornix constituted the frame of the nasal tip. CONCLUSION: Meager nasal alar cartilage is composed of the medial crus, lateral crus and fornix, which determined the nose shape, especially the nasal tip shape. External nose plastic surgery should pay attention to the protection of nasal alar cartilage.
10.The relationship between serum HBeAg、HBV DNA and liver pathological change in chronic hepatitis B virus carriers with normal liver function

Objective To explore the relationship between serum HBeAg、the qualification of HBV DNA and the liver pathologic change in chronic hepatitis B virus (HBV)carriers with normal liver function.Methods Two hundred and forty-four chronic hepatitis B virus carriers with normal liver function were performed pathology examination by Liver biopsy.Meanwhile liver function,HBV DNA level and serological serum markers of B-hepatitis examination were detected.Results Pathology results showed that,of all 244 cases,7 cases was cirrhosis(2.9%),143 for slight CHB (58.6%),32 for moderate CHB (13.1%) and 9 for severe CHB (3.6 %).And 53 (21.7%)chronic hepatitis B virus carriers were the normal histology morphology.Fony-eight cases (19.7%) were in inflammation stage G≥2 and 54(22.1%) were with fibrosis stage S≥2.For cases with HBV DNA positive,The inflammation and ftbrosis stages in HBeAg negative group were more severe than that in HBeAg positive group (P < 0.05).The fibrosis stages in patients with low HBV DNA level were severe than that in high HBV DNA levels (P < 0.05).No significant differences were observed in the inflammation stage between subjects with high or low HBV DNA level.Conclusion Most chronic HBV carriers with normal liver function were with different degrees of liver inflammation and fibrosis.HBeAg and HBV DNA associated with liver pathological change.
