Acta Anatomica Sinica 1955;0(03):-
In this study, 90 fetuses aged from 3~10 months were collected, 48 of 90 ductus arteriosi were studied with histological method in order to observe the changes in the different fetal stages and to approach its closing mechanism. The gross morphology of the 90 ductus arteriosi and their abjacent portions were also observed. Ductus arteriosus belong to the muscular type artery. It was found that the histological structure of the ductus arteriosus after the 7th month was particulary different from that before the 7th month, the beginning of the perinatal period is the critical period of the histological changes of the ductus arteriosus, since the beginning of the perinatal period, the duct had grown rapidly, the internal elastic membrane was fragmented or split, the smooth muscular fibers of the inner layer in the tunica media remarkbly thickened and was disarranged, some oblique and spokelike smooth muscular fibers protruded into the intima through the gaps between the fragmental internal elastic membranes and formed typical intima cushions with the thickened tunica intima. From the beginning of the perinatal period, the lungs and the vessels of pulmonary circulation had been well developed, but the right and left pulmonary arteries were in the state of "the door being placed but usually closed". Blood of the pulmonary trunk flowed into the much smaller ductus arteriosus, hence the ductus arteriosus was an artery affected by local hyper-pressure, its histological changes were similar to the degeneration of the muscular artery affected by hypertension disease, but it had no lipid deposit. This local hyper-pressure may be the intrinstc cause of the histological changes of the ductus arteriosus. These histological changes were the morphological foundation of the functional closure and anatomical closure of the ductus arteriosus after birth.

Chinese Journal of Digestive Surgery 2011;10(4):241-246
Hepatocellular carcinoma(HCC)is nowadays one of the leading common causes of cancer death and mostly occurs in hepatitis B endemic areas.Typically,HCC patients presented late and only about 20% of the patients are amendable to surgical interventions.Surgical resection is the main curative treatment option.Nevertheless, the recurrence rate after potential curative liver resection is still very high.On the other hand, liver transplantation is the treatment of choice for early small HCC but its application is limited by the availability of liver grafts.Radiofrequency ablation is more commonly used in cirrhotic patients with small tumours who are not surgical candidates.Nowadays, transarterial chemoembolisation, drugeluting bead transarterial chemoembolisation and radioembolisation are the mainstay of treatments for the patients with intermediate HCC.Other systemic approaches, such as hormonal therapy, immunotherapy and chemotherapy have disappointing results. In particular,HCC is highly refractory to cytotoxic chemotherapy. This is no convincing evidence thus far that systemic chemotherapy improves overall survival in advanced HCC patients.Recently, encouraging results have been shown in using sorafenib in the treatment of advanced HCC patients.Similar to sorafenib, other anti-angiogenic multi-targeted tyrosine kinase inhibitors, such as pazopanib, brivanib and axitinib also show promising activity in various stages of clinical trials. There is an urgent need to identify biomarkers that may guide the rational use of sorafenib and other targeted agents in the era of personalised medicine.More importantly, the multidisciplinary model in managing HCC patients is the important key to the future success.

Objective To evaluate haematogenesis of complex Shenlu granule in treating myelodysplastic syndrome (RA/RCMD) due to kidney yang deficiency. Methods The qualified cases were randomly divided into the complex Shenlu granule group and the Andriol group. During treatment, indexes such as peripheral hemogram, T cell subgroup, NK cell and immunological phenotypic expression of BMC were tested. Results Compared with prior treatment, Rbc, Hb and Plt have significantly increased in Shenlu group [prior treatment: (2.39±0.99)×1012/L, (84.47±28.68) g/L, (81.13±96.85) ×109/L; post-treatment (2.80±0.98)×1012/L, (94.87±25.63) g/L, (98.67±107.9)×109/L, and the difference was statistically significant (t =4.0359, P =0.001, t =2.7009, P =0.0172, t =2.2573, P = 0.0405). Andriol group has only increased Hb [prior treatment: (71.93±27.53) g/L; post-treatment (80.07±26.03) g/L] was statistically significant (t =2.3125,P =0.0365). After treatment, CD+4, CD+8, CD+4/CD+8, and NK were at or near normal value in Shenlu group (37.9±5.9) %, (24.0±5.8) %, 1.75±0.83, (13.0±6.9) %, compared with prior treatment (29.3±11.7) %, (29.6±5.8) %,1.12±0.59, (8.8±5.7) %. The difference was statistically significant (t =2.6194, P =0.0202, t = 2.6595, P =0.0187,t =2.6581, P =0.0187, t =2.2288, P =0.0427, P <0.05), and CD+8, CD+4/CD+8 close to normal in Andriol group [(22.1 ±7.5) %, 1.50±0.74], prior treatment [(26.6±7.5) %, 1.18±0.55]. The difference was statistically significant (t =2.2377, P =0.0420, t =2.9352, P =0.0109, P <0.05). After treatment, CD+4 [Shenlu group:(37.9±5.9) %, Andriol group: (30.5±12.6) %] difference was statistically significant (t =2.1738, P =0.0474) in two groups; Shenlu group control abnormal expression of bone marrow cells, which were CD+13, CD+33, CD+34, CD+64,CD+117. The effect was better than Andriol group (the first three: u =2.76, u =3.39, u =2.85, P <0.01, the latter two: u =2.17, u =2.46, P <0.05). Conclusion clomplex Shenlu granule can effectively control normal hematopoietic function for MDS (RA/RCMD) due to kidney Yang deficiency.

Journal of Practical Obstetrics and Gynecology 2017;33(5):381-385
Objective:To determine the relationship between trimester-specific gestational weight gain and pregnancy outcome among women with twin pregnancies. Methods:A retrospective cohort study was conducted among Chinese women with twin pregnancies delivered from January 2013 to October 2015 in the International Peace Maternity & Child Health Hospital of China Welfare Institute.Gestational weight gain was compared between women with different progestational BMI.After the adjustment for age and progestational BMI,binary Logistic regression analysis was conducted to explore the relationship between gestational weight gain rate and gestational complications, premature birth, premature rupture of membrane and neonatal birth weight.Results:①A total of 472 subjects met the inclusion criteria.Rate of gestational weight gain were(0.41 ±0.15),(0.64 ±0.30) and (0.49 ±0.15) kg/week in Early,Mid-and Late-,and Whole of pregnancy respectively.②With regards to trimester-specific gestational weight gain, statistically significant associations were found between rate of weight gain and gestational hypertension and preterm birth (P < 0.05).Those with faster gestational weight gain had higher rate of premature rupture of fetal membrane(P < 0.05).And those with lower gestational weight gain had lower neonatal birth weight(P<0.05).③After adjusting maternal age, progestational BMI and gestational age at delivery, the sum of twins' birth weight increased 25.21g,30.89g and 21.46g respectively when the maternal body weight increased 1kg during the whole pregnancy,in Early and Mid-pregnancy (P < 0.001),and during in Late-pregnancy(P =0.001).Conclusions:Gestational weight gain among women with twin pregnancy are closely related with adverse pregnancy outcomes. The gestational weight gain rate in different gestation are of some predictive value for pregnancy outcomes.More research with long follow-up are needed to determine proper gestational weight gain for twin pregnancy suitable for Chinese women.
9.Advance in the study of CD4~+CD25~+FOXP3~+ regulatory T cell in tumor immune evasion
Qiang GAO ; Shuangjian QIU ; Jia FAN
China Oncology 2006;0(08):-
As a distinct T cell subset with acknowledged specific function and marker, CD4+CD25+FOXP3+ regulatory T cells are thought to dampen T-cell immunity and to be the main obstacle tempering antitumor immunotherapy. Accumulating evidence has confirmed an increased pool of regulatory T cells both in the peripheral blood and the tumor microenvironment of cancer patients, which are indicative of disease progression, response to therapy, invasive phenotype and prognosis. Therefore, manipulation of regulatory T cells—including depletion, blocking trafficking into tumors, or reducing their differentiation and suppressive mechanisms—and concomitant stimulation of effector T cells, systemically or locally in tumors, represent new strategies for cancer treatment.

ObjectiveTo explore the effect of patients with acute cerebral infarction (ACI) at different ages receiving intravenous injection of alteplase for thrombolysis on their prognosis. Methods Ninety-eight ACI patients admitted to Jinghai Hospital of Tianjin from January 2014 to December 2015 were conducted in this study, and they were divided into elderly group (over 75 years old) and younger age group (under 50 years old). The patients in two groups of were given intravenous thrombolytic therapy with alteplase (0.9 mg/kg), 10% of the dose was intravenously injected, and continuously the remaining dose was pumped into the vein for 1 hour. Before the thrombolysis and 1 day and 21 days after thrombolysis, the score of United States National Institutes of Health Stroke Scale (NIHSS) was used to evaluate neurological deficits, the incidence of bleeding adverse event and mortality were recorded. Results Compared to the before thrombolysis, the NIHSS scores after thrombolysis on the 1st day and 21st day were significantly decreased in the two groups (the younger age group 1 day: 6±4 vs. 10±5, 21 days 2±2 vs. 10±5, the elderly group 1 day: 7±5 vs. 10±7, 21 days: 6±4 vs. 10±7, allP < 0.05), and NIHSS score in the younger age group was significantly lower than that in the elderly group on the 21st day after thrombolysis (2±2 vs. 6±4,P < 0.05). After thrombolysis, the incidence of minor mucocutaneous bleeding in elderly group was obviously higher than that in younger age group, the difference being statistically significant [7.69% (4/52) vs. 2.17% (1/46),P < 0.05]; the elderly group had higher symptomatic intracerebral hemorrhage and death events than those in younger age group [1 (1.92%) vs. 0].Conclusion Different ages of patients with ACI can benefit from intravenous thrombolysis with alteplase, showing the younger the age of the patients, the better the benefit they can get and have relatively better prognosis.
