Journal of Clinical Hepatology 2019;35(8):1723-1727
ObjectiveTo investigate the influence of dual positivity of HBsAg and anti-HBs on the development of hepatocellular carcinoma (HCC) in patients with hepatitis B virus (HBV) infection. MethodsPubMed, Embase, Cochrane Library, CNKI, and Wanfang Data were searched for articles on the influence of dual positivity of HBsAg and anti-HBs on the risk of HCC published from July 1, 1975 to March 27, 2019. RevMan5.3 and Stata11.2 were used for statistical analysis of data. A heterogeneity analysis was performed for the studies included; a random effects model was used in case of significant heterogeneity, and a fixed effects model was used in case of non-significant heterogeneity. Odds ratio (OR) and corresponding 95% confidence interval (CI) were used to investigate the association of dual positivity of HBsAg and anti-HBs with the development of HCC. Begg funnel plots were used to investigate publication bias. By removing one article each time, the sensitivity analysis was used to assess the quality and reliability of the Meta-analysis. ResultsA total of 4 articles were included, with 2 studies in the Korean population and 2 in the Chinese population, and there were 3042 patients in total. The meta-analysis showed that there was no significant association between dual positivity of HBsAg and anti-HBs and the development of HCC (OR=1.46, 95%CI: 0.76-2.80, P=0.25). A country-based subgroup analysis showed significant association between dual positivity of HBsAg and anti-HBs and the development of HCC in the Korean population (OR=2.67, 95%CI: 1.61-4.43, P=0.000 1), while no significant association was found in the Chinese population (OR=0.89, 95%CI: 0.48-1.64, P=0.70). ConclusionThere is no significant association between dual positivity of HBsAg and anti-HBs and the development of HCC, and further studies are needed in future.

We reported a case of preeclampsia complicated by acute stress cardiomyopathy following cesarean section at 37 weeks of gestation. The patient presented with chest tightness and suffocation two days after the operation and was considered as acute stress cardiomyopathy. The patient recovered and was discharged nine days after the surgery after a series of management, including non-invasive ventilation, vasodilation, lowing blood pressure, maintaining a negative fluid balance, anti-infection, and anticoagulation. Stress-induced cardiomyopathy during pregnancy and postpartum is rare, and cesarean section and preeclampsia may be the risk factors.

Objective To compare differences of clinical factors related to early pregnancy loss between invitro fertilization-embryo transfer (IVF-ET) treatment and natural pegnancy. Methods A retrospective analysis was performed on the 363 cases of early pregnancy loss between Dec. 2015 to May 2016 in Peking University Third Hospital, during which 173 cases were after IVF-ET treatment(IVF-ET group), and others were natural pregnancies(natural group). Results The average age in IVF-ET group was significantly higher than that in the natural group [(34.1±4.3)versus(31.8±4.1)years old, P<0.01]. The terminating time of pregnancy loss in IVF-ET group was short than that in the natural group [(59.8±9.2) versus(69.9 ± 11.1)days, P<0.01]. The incidence of embryo abnormal chromosome in IVF-ET group was significantly lower than that in the natural group [57.2%(99/173)versus 74.2%(141/190), P<0.01], during which abnormal chromosome numbers were the most common. Conclusions The pregnancy loss of early pregnancy is mainly caused by chromosome abnormality. The proportion of chromosome abnormality in early pregnancy loss after IVF-ET is not higher than that of natural pregnancy, indicating that there are relatively reliable gametes and embryo safety in IVF treatment.

Objective To analyze the relationship between the serum B-cell activating factor (BAFF) levels and clinical characters and pathological features in children with lupus nephritis (LN). Methods ELISA was used to detect the serum BAFF (sBAFF) levels of the 54 LN children diagnosed in the First Affiliated Hospital, Sun Yat-sen University during October 1, 2014 to December 31, 2016 and with complete clinical data. According to whether glucocorticoid or immunosuppressive agents has been used at their first admission, patients were divided into treated group (n=44) and non-therapy group (n=10). According to the renal response after induction treatment for 6 months, patients were divided into remission group (n=20) and non-response group (n=34). According to whether there was renal recurrence, they were divided into recurrence group (6 cases) and non-recurrence group (48 cases). According to renal biopsy, patients were divided into class-Ⅲ, class-Ⅳ and class-Ⅴ group. Another 15 healthy children were taken as a control group. The correlations between sBAFF and clinical manifestation, laboratory examination, renal biopsy and clinical outcome were analyzed. Results (1) Compared with the control group, the sBAFF was significantly increased in LN group (t=3.821, P<0.001). Compared with the non- neuropsychiatric systemic lupus erythematosus (NPSLE) group, sBAFF was significantly increased in NPSLE group (t=2.202, P=0.032). (2) Compared with that in treated group, sBAFF was significantly higher in untreated group (LSD - t=2.309, P=0.025). Compared with non-response group, sBAFF was significantly decreased in response group (LSD-t=2.035, P=0.046). (3) No significant difference was observed between class-Ⅲ, class-Ⅳ and class-Ⅴpathological classification group (F=1.080, P=0.459). sBAFF in LN children was not significantly correlated with the active index (AI) or chronic index (CI) of Austin index (r=-0.273, P=0.063; r=0.150, P=0.314). (4) In LN children, sBAFF has positive correlation with ESR and IgG level (r=0.289, P=0.036; r=0.340, P=0.017) and negative correlation with WBC (r=-0.337, P=0.013). Multiple linear regression model showed that serum IgG level (β'=0.517, P=0.001) and renal response (β'=-0.271, P=0.037) were independent influencing factors of sBAFF level. Conclusions Renal remission and serum IgG levels in LN children are influencing factors of sBAFF levels. sBAFF is helpful to clinical assessment on renal response of LN children.

OBJECTIVETo investigate the expression variation of RAR-β2, RASSF1A, and CDKN2A gene in the process of nickel-induced carcinogenesis.
METHODSNickel subsulfide (Ni(3)S(2)) at dose of 10 mg was given to Wistar rats by intramuscular injection. The mRNA expression of the three genes in induced tumors and their lung metastasis were examined by Real-time PCR. The methylation status of the 5' region of these genes were detected by Quantitative Real-time methylation specific PCR.
RESULTSThe mRNA expressions of the three genes both in muscle and lung tumor were decreased distinctly in comparison with normal tissue. But hypermethylation was found only in muscle tumor.
CONCLUSIONThese findings suggest that loss of function or decrease of RAR-β2, RASSF1A, and CDKN2A, as well as the hypermethylation of 5' region of these genes, are related with nickel exposure.

Objective To develop and validate the radiomics nomogram on the discrimination of lung invasive adenocarcinoma from'non-invasive'lesion manifesting as ground glass nodule(GGN)and compare it with morphological features and quantitative imaging. Methods One hundred and sixty pathologically confirmed lung adenocarcinomas from November 2011 to December 2014 were included as primary cohort. Seventy-six lung adenocarcinomas from November 2014 to December 2015 were set as an independent validation cohort. Lasso regression analysis was used for feature selection and radiomics signature building. Radiomics score was calculated by the linear fusion of selected features. Multivariable logistic regression analysis was performed to develop models. The prediction performances were evaluated with ROC analysis and AUC,and the different prediction performance between different models and mean CT value were compared with Delong test. The generalization ability was evaluated with the leave-one-out cross-validation method. The performance of the nomogram was evaluated in terms of its calibration. The Hosmer-Lemeshow test was used to evaluate the significance between the predictive and observe values. Results Four hundred and eighty-five 3D features were extracted and reduced to 2 features as the most important discriminators to build the radiomics signatures. The individualized prediction model was developed with age, radiomics signature, spiculation and pleural indentation, which had the best discrimination performance(AUC=0.934)in comparison with other models and mean CT value(P<0.05)and showed better performance compared with the clinical model(AUC=0.743,P<0.001).The radiomics-based nomogram demonstrated good calibration in the primary and validation cohort, and showed improved differential diagnosis performance with an AUC of 0.956 in the independent validation cohort. Conclusion Individualized prediction model incorporating with age, radiomics signature, spiculation and pleural indentation, presenting with radiomics nomogram, could differentiate IAC from'non-invasive'lesion manifesting as GGN with the best performance in comparison with morphological features and quantitative imaging.
