1.Clinical application of laparoscopic splenectomy combined with pericardial devascularization

Objective To explore the methods, safety and efficacy of laparoscopic splenectomy (LS) and pericardial devascularization. Methods Five patients underwent LS and pericardial devascularization from January to June 2003. Results All the operations were completed under laparoscope and no hand-assisted procedure was required. The operating time was 3.0~4.0 hours (mean, 3.5 hours) and the intraoperative blood loss was 250~450 ml (mean, 350 ml). No surgical complications occurred. Conclusions Laparoscopic splenectomy with pericardial devascularization is a feasible, effective, safe and minimally invasive procedure for patients with normal-sized to medium-enlarged spleen.
2.Clinical study on laparoscopic radical operation for rectal cancer after short-time preoperative radiotherapy

Objective To explore the application and efficacy of laparoscopic radical operation for rectal cancer after preoperative short-time radiotherapy.Methods The clinical data of 108 patients with Dukes B and C rectal cancer were analyzed retrospectively.Thirty-five patients underwent laparoscopic radical operation after preoperative short-time radiotherapy,meanwhile 30 patients underwent laparoscopic radical operation,and 43 patients underwent open operation,both later groups without preoperative radiotherapy.Results There were not significant differences in preoperative general condition,tumor size and stage,pathological type,site of operation,and mode of operation between the 3 groups.But there was a higher rate of radical resection and sphincter preservation in the laparoscopic operation plus radiotherapy group than in the other groups(P
3.Digital anatomy of nucleus accumbens in the human brain

Objective To explore the locating, parameter measurement and 3D display of nucleus accumbens in human brain in terms of digital anatomy .Methods The raw data of the head specimen of a 45-year-old male adult with 0.5mm as the section spacing was collected by using digital milling machine .Three hundreds images of continual cross sections containing brain were chosen and the segmentation of the caudate nucleus , putamen and nucleus accumbens was accomplished with Photoshop CS .The nucleus accumbens on the images of continual coronal section reconstruction were distinguished according to Harvard Medical School ’ s segment method to calculate the volume of nucleus accumbens and collect the correlative location information .The caudate nucleus , putamen and nucleus accumbens were 3D visualize with the software of Amira 3.1.1.Results The nucleus accumbens , the adjoining structure and the lesion target of nucleus accumbens were all clearly visible .The left nucleus accumbens volume was 972.5mm3 , and the right was 830.6mm3 .The 3D coordinate value was the left ( -11.0, 24.4, 1.3) and the right (9.3, 23.9, 1.7).Conclusion The digital anatomy of nucleus accumbens can distinctly display the nucleus accumbens , form and confirm it ’ s volume, location and adjoining area , which is useful to clinician .
4.Effect of MAPK/NF-kappaB signaling pathway on extracellular release of HMGB1 induced by hypoxia in laryngeal Hep-2 carcinoma cells.

OBJECTIVE:
To investigate the extracellular release of high mobility group box 1 (HMGB1) in laryngeal Hep-2 carcinoma cells induced by hypoxia and its possible mechanism.
METHOD:
The changes of HMGB1 concentration in the culture medium as well as HMGB1 protein and mRNA expression in Hep-2 cells were investigated after the cells were cultured with 1% O2 for different durations. Inhibitory effects of MAPK pathway inhibitors (PD98059. SP600125, and SB202190) and nuclear NF-kappaB pathway inhibitor (PDTC) with various concentrations on extracellular HMGB1 release were observed in hypoxia-induced Hep-2 cells. The HMGB1 concentration and HMGB1 protein expression were measured by enzyme-linked immunosorbent assay (ELISA) and western blot, respectively. The HMGB1 mRNA expression was determined by real-time quantitative PCR(RT-PCR).
RESULT:
The HMGB1 concentration in the culture medium and the HMGB1 protein expression in Hep-2 cells increased after the cells were subjected to hypoxia culture for 12 h in a time-dependent manner. The level of HMGB1 mRNA expression in Hep-2 cells increased after the cells were induced by hypoxia for 6h PD98059 and SP600125 with 20 micromol/ L and PDTC with 50 mg/L partly inhibited extracellular release of HMGB1 in hypoxia-cultured Hcp-2 cells.
CONCLUSION
Hypoxia induces laryngeal carcinoma cells to release HMGH1. which may be related to MAPK/NF-kappaB signaling pathway.
